OBJECTIVE: Some growth factors may promote tumor growth by affecting tumor angiogenesis. Midkine(MK) are polypeptides that belong to a new family of heparin-binding growth/differentiation factor and has also been reported to be angiogenic. In various tumor tissues, MK was highly expressed between tumor and normal tissues; however, the pattern of MK expression in normal cervix and cervical cancer has not been established. The aim of this study was to determine the MK mRNA expression in cervical cancer. And we questioned whether its expression is related to cancer stages and prognostic factors. METHODS: The cervical and cervical cancer tissues were taken from patients; healthy women(n=15), and the patients with cervical cancer(n=29). The MK mRNA expression was examined by quantative competitive PCR after polymerase chain reaction amplification of reverse transcriptase copies of RNA transcripts(RT-PCR). RESULTS: The cervical cancer expressed higher levels of MK mRNA than normal cervix(p<0.05). The MK mRNA expression was not correlated with the cervical cancer stage and histopathologic type(p>0.05). CONCLUSION: These results suggested that increased MK mRNA expression is associated with the development of cervical cancer.

Angiogenesis is an essential requirement for development, progression, and metastasis of malignant tumors. Vascular endothelial growth factor (VEGF) is one of the important angiogenic factors. Recently the role of angiogenesis has been known in premalignant lesions. This study was performed to determine whether the angiogenesis and VEGF expression were increased in association with histological grade of cervical intraepithelial neoplasia (CIN) and to see the relationship between the angiogenesis and VEGF. Immunostainings for factor VIII and VEGF were performed on 52 cases of cervical neoplasia (12 cases of CIN I, 11 cases of CIN II, 15 cases of CIN III, 7 cases of microinvasive squamous cell carcinoma, and 7 cases of invasive carcinoma) and 5 cases of normal cervix. The results showed a significant increase of microvessel count from normal cervix through CIN grades to invasive squamous cell cacinoma. VEGF expression was increased in proportion to the CIN grades. There was no significant correlation between microvessel count and VEGF expression. In conclusion, the tumor angiogenesis is an early event in tumorigenesis of uterine cervix. In addition, no significant relationship between the microvessel count and VEGF expression in CIN suggests the possibility of other growth factors affecting mainly angiogenesis of premalignant lesion of uterine cervix.

Purpose: The diagnostic usefulness of fetal echocardiography in congenital heart disease is well known. One of the indications for fetal echocardiography is presence of extracardiac anomalies and such coexistent congenital heart disease may have important implications for obstetric and neonatal management. MATERIALS AND METHODS: 496 women with singleton pegnancies with amgenital anomalies that underwent penatal care and follow-up in Severance Hospital for 5 years from 1991 to 1995 were evaluated for the incidence of congenital heart disease. RESULTS: The incidence of congenital heart disease in fetuses with the associated extracardiac anomalies were as follows; central nervous system 6.9%(11/160), gastro-intestinal system 10.7%(8/75), genito-urinary sysem 4.7%(4/85), others 5.6%(4/72). Thirty-one of eighty-ane fetuses with congenital heart disease were found to have the associated extracardidac anomalies and ten of them chenoscenal abnormalities. CONCLUSION: We recommend that fetuses with congenital extracardiac anomalies sbould be checked for the presence of congenital heart disease and chmmosomal abnormalities, if needed.

PUPPOSE: This study was to investigate the knowledge, attitudes, and educational needs for sexuality of the elderly. METHOD: This was a descriptive study, in which data were collected from February to April 2002 from 354 elderly who were residing in Chung-Chung province. RESULT: 1. The level of knowledge regarding sexuality was below average. 2. The level of the attitudes toward sexuality of the elderly was neither positive nor negative. 3. The educational needs of the elderly were above average. 4. Higher level of knowledge on sexuality was significantly correlated with more positive attitudes and education needs for sexuality, and more positive attitudes toward sexuality was significantly correlated with more educational needs for sexuality. CONCLUSION: Particularly men, with good health, and those who were in intimate relationships with their spouses and still engaged with active sexual life showed somewhat higher level of knowledge and positive attitudes as well as higher educational needs. Therefore, further development and applying of sexuality promotion programs for healthy elder couples should be considered in the future to enhance the quality of life of elder population. In addition, it is also suggested to develop an alternative intervention program on sexuality for elderly who are women, those with meager health, and the bereaved.

Most ectopic pregnancies occur in the fallopian tubes. There have been numerous theories to explain the occurrence of ectopic pregnancy in fallopian tubes. The most commonly held view is that the passage of the fertilized ovum through the fallopian tube is delayed or hindered by chronic inflammation and its sequelae. We designed a study to evaluate the details of histopathologic changes and the location of implantation and how they relate to the clinical history. 182 fallopian tube specimens from patients who had undergone total or partial salpingectomy were examined. A high incidence of non-specific inflammation of plicae and wall of tube (31.9%) and salpingitis isthmica nodosa (12.6%) were observed. Other associated findings included acute salpingitis, complex plicae or complex hyperplasia of tubal epithelium, fibrous adhesion with ovary, endometriosis, and calcification. History of previous ectopic tubal pregnancy was found in 8 cases. The cases with serum beta-HCG value above 2,500 I.U./L (group I, n=97) were more frequently noted in those exhibiting myosalpingeal invasion of trophoblast (67 cases) than in those without invasion (30 cases). Of the 182 tubal pregnancies, 117 (64.3%) cases were found in the ampulla and 47 (25.8%) cases in isthmic location. In 117 ampullary pregnancies, the products of conception were found intraluminally in 71 cases (60.7%), and extraluminally in 34 (29.1%) cases, of which the products of conception were found entirely extraluminal. The products of conception, found both within and outside the tubal lumen, were found in 12 cases (10.2%). Of 47 tubes with isthmic pregnancies, 33 cases were intraluminal (70.2%), 12 cases were extraluminal (25.5%), and two cases were mixed (4.3%). In conclusion, significant histopathologic abnormalities accompany a majority of ectopic tubal pregnancy, and myosalpingeal invasion of trophoblast is correlated with high serum beta-HCG. Thus, it is necessary to confirm not only the ectopic placental tissue but also the accompanying details of the other histopathologic findings or the pathologic evaluation of ectopic tubal pregnancy.

Acquired tufted angioma is a benign, slowly progressive angioma with a typical histological pattern that was first described by Wilson-Jones in 1976. We report a case of acquired tufted angioma in a 19 year old female who had erythematous papules and plaques on the right thigh. Histopathological findings showed multiple capillary lobules in a cannonball arrangement scattered throughout dermis, which was diagnostic of acquired tufted angioma.

The present study was intended to define the relationship of radiological findings and pathological types ofprimary lung cancer. The 85 cases were selected after confirmation of the cell types by bronchoscopic biopsy,cervical lymph node or thoracotomy biopsy and lung resection. Results of the study were presented below. 1.Primary lung cancer is frequently developed after 4th decade and males were affected more frequently than femaleswith ratio of 2 to 1. 2. The frequencies of pathologic cell types of lung cancer were presented as follows.Squamous cell carcinoma 40% Adenocarcinoma 25% Undifferentiated cell carcinoma 30% Alveolar cell carcinoma 5% 3.The findings of plain chest radiograph were presented as follows. In squamous cell carcinoma, hilar enlargement orhilar mass is the most frequent finding (53%) with atelectasis(26%) or obstructive pneumonitis(26%). Inadenocarcinoma, pleural effusion is accompained about half of cases (53%). In undifferentiated cell carcinoma,hilar mass with mediastinal widening and pleural effusion is frequent finding.
