1.Evaluation of Foodservice Managers' Perception on Safety Management in Childcare Centers and Kindergartens.

The purpose of this study was to investigate the perception of foodservice managers in childcare centers and kindergartens for identifying the vulnerable areas of safety management. The foodservice managers in 1,144 of nationwide childcare centers and kindergartens were surveyed from May to October 2007 to answer a total 72 questions in areas of general characteristics, actual practice of foodservice management and safety management perception. Meals were served in 97.7% of surveyed facilities and 91.0% among them were self-operated. The hiring rate (27.8%) of dietitians in childcare centers was significantly lower than the rate (82.0%) of kindergartens. The needs of dietitian employment between childcare centers and kindergartens differed as 44.9% and 87.5%, respectively. In knowledge on food-borne illnesses, they have the wrong information in the field of season for frequent outbreak, major pathogens and foods causing food-borne illnesses. The food-borne illnesses (36.2%) were indicated as the major problem in foodservice facilities and the degree of risk exposure was considered as safe (70.1%). More than 70% of both facilities answered as sanitary in the status of personal hygiene. Only 38.0% recognized the Facility and Equipment Standard in the Childcare Law and Early Childhood Education Law. In preparation of meals and management of cooking processes, kindergarten scored significantly high compared to childcare centers (p < 0.05 or p < 0.01) while the necessity of standards was not different in both facilities. Among managers surveyed, 28.8% had taken food safety education from professional institutions and more than 80% in both facilities indicated the need of professional educations. These results suggest that it is necessary for the systematic sanitary education of foodservice managers by developing the sanitary management standard as well as the amendment of laws related foodservice in childcare centers and kindergartens.

2.Using Medical Error Cases for Patient Safety Education.

PURPOSE: To draw attention to patient safety and increase its awareness among medical students, we developed a program that teaches patient safety based on common medical error cases. The aim of this study is to introduce this program and improve student receptivity to it. METHODS: As part of the "Patient, Doctor, and Society" course, third-year medical students participated in 8 hours of a medical error education program. Students discussed recent, typical medical lawsuits that were generated from internal medicine, surgery, pediatrics, obstetrics and gynecology, neurosurgery, medication, anesthesia, and blood transfusion cases. Students weighed these issues in small groups, using various discussion methods. After finishing the program, students completed a course evaluation questionnaire. RESULTS: The students rated this program as satisfactory, highly motivating, and helpful in preparing their future practices. They responded that although the cases were interesting, some were difficult. They stated that the small group discussion techniques encouraged them to take active part in the discussion and to consider the cases more deeply. CONCLUSION: Small group discussion of medical error cases is an effective method for students to study patient safety.

3.A Study on Occupational Stress and Coping, Turnover, Knowledge and Practice of Infection Control in Dental Hygienists of COVID-19

Background:
The importance of infection with COVID-19 is being emphasized in dentistry with high risks such as aerosols. The purpose of this study is to investigate the knowledge and practice of infection control, stress and coping, and turnover of dental hygienists.
Methods:
Questionnaire was conducted knowledge and practice of infection control, occupational stress and coping, turnover. Survey data was investigated about 149 dental hygienists from February to March 2021 Data were analyzed t-test, ANOVA, Pearson’s correlation using statistical programs of PASW Statistics ver. 21.0.
Results:
Regarding occupational stress, relationship conflict was higher in the group with less than 2 years of experience (p<0.05). Job anxiety, organizational system, inadequate compensation, and workplace culture were highly surveyed in the 3 to 5 year of experience. The group with more than 6 years of experience had the highest perception of lack of job autonomy (p<0.05). The group with higher knowledge of infection control had lower mean inappropriate rewards and stress (p<0.05). The group with high infection control performance had a lower average in items such as job instability, organizational system, inadequate compensation, workplace culture, and stress. And problem-focused coping ability was found to be high (p<0.05). Infection control knowledge and performance were positively correlated (r=0.251, p<0.01), infection control practice and stress were negatively correlated (r=−0.264, p<0.01), and stress and emotional coping were positively correlated (r=0.367, p<0.01). Stress was positively correlated with turnover rate (r=0.549, p<0.01).
Conclusion
Infection control training was required to reduce occupational stress. Occupational stress was highly correlated with turnover, a holistic and systemic organizational operation and improvement of the quality of medical care were required to reduce stress.
4.Application of Noninvasive Positive Pressure Ventilation in Patients with Respiratory Failure.

BACKGROUND: Noninvasive positive pressure ventilation(NPPV) has been increasingly used over the past decade in the management of acute or chronic respiratory failure and weaning of mechanical ventilation. We performed this clinical study to evaluate the usefulness of NPPV in patients who developed acute respiratory failure or post-extubation respiratory failure. METHODS: We analysed thirty four patients(sixteen males and eighteen females, mean ages 58 years) who applied NPPV(BIPAP S/T, Respironics co., USA) for respiratory failure or weaning difficulty at medical intensive care unit(MICU), emergency room and general ward of a tertiary hospital. We evaluated the underlying causes of respiratory failure, duration of treatment, the degree of adaptation, complication and predictive parameters of successful outcome. RESULTS: The overall success rate of NPPV was seventy-one percent. The duration of NPPV applying time, baseline blood pressure, pulse rate, respiration rate, PaO2, PaCO2, SaO2 were not different between success group and failure group. But, the baseline pH was higher in the success group. Predictors of success were higher baseline pH, patients with underlying disease of COPD, improvement of vital sign and arterial blood gas value after NPPV application. The success rate in patients with post-extubation respiratory failure was eighty percent. There were no serious complication on applying NPPV except minor complications such as facial skin erythema, abdominal distension & dry mouth. CONCLUSION: NPPV may be effective treatment in patients with acute respiratory failure or post-extubation respiratory failure in selected cases.
