2.A Study on the Type A Behavior Pattern and Health Locus of Control in the Coronary Heart Disease Patients.

This study is a descriptive survey conducted for the purpose of providing basic data to develop the effective nursing in interventions for Coronary Heart Disease patients. It was focused the relations between the type A behavior pattern and Health Locus of Control of them datd were collected through face-to-face interview with 55 CHD patients who were hospitalized at one of the general hospitals in Taegu city from May 11 to July 27, 1977. The instruments used for this studyy were the Jenkiness Activity Scale(Jenkins, 1979) and Health Locus of Control Scale(Wallstone & Wallston, 1978). Analysis of data was done by using of Person Correlation Coefficient, t-test, ANOVA, stepwise multiple regression and Cronbach with the SAS program. The results were as follows; 1. Total type A behavior pattern score was 67.2(total 88), job involvement /hard-driving subscale 35.9(total 47), sped subscale 12.8(total 16) and impatient subscale 19.09(total 25). The highest score of the type A pattern was the speed subscale 3.75 when it was transferred as a full mark 4. 2. When the total score of Health Locus of Control Scale of CHD was made 5, internal health locus control was the highest with 3.9, powerful others health locus control 3.6 and chance health locus control 3.0. 3. Women were significantly higher than men on total type A behavior pattern(p=.0453), job involvement/hard-driving subscale(p=.0492) and impatient subscale(p=.0460) according to general characteristics. Angina pectoris patients showed significantly higher than myocardial Infarction patients in total type A behavior pattern score(p=.0266), involvement/hard-driving subscale(p=.0365) and impatient subscale(p=.0185). 4. It was not significantly different for the Health Locus of Control Scale according to general characteristics. 5. Correlations between subscales of type A behavior pattern and subscales of Health Locus of Control showed that there were significantly positive correlation between internal health locus control and job-involvement/ hard-driving subscale(p=.0500), and between powerful others health locus control and important subscale(p=.0204). 6. The variances that can explain the causes of type A behavior pattern were systolic blood pressure 9%, quantity of smoking per day 9.6%, dyastolic blood pressure 8.6%, internal health locus control 6.2%, which accounted for 34.3% of the total variances. The following suggestions are made on the basis above results ; 1) The instrument that measures type A behavior pattern of CHD patient need to be modified and developed as an instrument that is applicable with Korea environment and easy to understand for subjects. 2) Replication of the research is need with nation wide samples. 3) Follow-up study for correlation between subscale of the type A behavior pattern and subscale of Health Locus of Control is needed. 4) Intervention program for CHD patients to reduce type A behavior pattern is to be developed.

3.The Effect of Phase 1 Cardiac Rehabilitation Nursing Care on Knowledge, Anxiety and Self-Care Behavior in Patients with Acute Myocardial Infarction.

The purpose of this study was to identify the effects of phase 1 cardiac rehabilitation nursing care on cardiac rehabilitation knowledge, anxiety, and self-care behavior in patients with acture myocardial infarction. The study design was composed of a nonequivalent control group non-synchronized design and a non-equivalent control group post-test design. The subjects of the study consisted of thirty-four acute myocardial infarction patients hospitalized at a university hospital in Taegu between February 16, 1998 and May 12, 1998. the 34 research subjects were assigned to experimental( 17 patients ) and control( 17 patients ) groups. The phase 1 cardiac rehabilitation nursing care was composed of cardiac rehabilitation education taken from a rehabilitation manual and booklet, and participating in a progressive exercise program. After discharge, a phone interview was conducted in order to encourage the self-care behavior. The modified Knowledge Scale developed by Rahe et al.(1975) and translated into Korean by Hwang(1986), and the modified Self-Care Scale of exercise and diet based on Hickey et al.(1992), were used for data collection. Analysis of data was done by use of Chi-square test, t-test, Repeated measure ANOVA, Simple Main Effect, and Time contrast. The results of this study are as follows : 1. The first hypothesis, "The experimental group which received the phase 1 cardiac rehabilitation nursing care will have a higher level of knowledge than the control group", was supported(F=24.07, p=.000). 3. The third hypothesis, "The experimental group which received the phase 1 cardiac rehabilitation nursing care will have higher self-care behavior scores than the control group", was supported( t=-15.49, p=.000 ). From the above results, it can be concluded that phase 1 cardiac rehabilitation nursing care is an effective nursing intervention knowledge, reducing anxiety, and improving self-care behavior in patients with acute myocardial infarction.

4.Quantitative Analysis of Distribution of the Gastrointestinal Tract Eosinophils in Childhood Functional Abdominal Pain Disorders

BACKGROUND/AIMS: Although functional abdominal pain disorders (FAPDs) are common in children, the accurate pathogenesis of FAPDs is not known yet. Micro-inflammation, particularly tissue eosinophilia of gastrointestinal (GI) tract, has been suggested as the pathophysiology observed in several GI disorders. We aimed to evaluate eosinophilic infiltration throughout the entire GI tract in children with FAPDs, compared to those with inflammatory bowel diseases (IBD) and to normal reference values. METHODS: We included 56 children with FAPDs, 52 children with Crohn’s disease, and 23 children with ulcerative colitis. All subjects underwent esophagogastroduodenoscopic and colonoscopic examination with biopsies. Tissue eosinophil counts were assessed in 10 regions throughout the GI tract. RESULTS: Eosinophil counts of the gastric antrum, duodenum, terminal ileum, cecum, and ascending colon were significantly higher in children with FAPDs compared to normal reference values. Eosinophil counts of the stomach and the entire colon were observed to be significantly higher in children with IBD than in those with FAPDs. Even after selecting macroscopically uninvolved GI segments on endoscopy in children with IBD, eosinophil counts of the gastric body, cecum, descending colon, sigmoid colon, and the rectum were also significantly higher in children with IBD than those with FAPDs. CONCLUSIONS: Significantly high eosinophil counts of the stomach and colon were observed in the order of IBD, followed by FAPDs, and normal controls, regardless of endoscopically detected macroscopic IBD lesions in children. This suggests some contribution of GI tract eosinophils in the intrinsic pathogenesis of FAPDs in children.

6.Nutrient Intakes and Hair Mineral Contents of Young Children.

PURPOSE: This study was performed to evaluate the dietary nutrient intake status and hair mineral content of Korean young children. METHODS: Fifty-five children who visited Seoul National University Bundang Hospital were divided into three groups by age: infants, toddlers, and preschoolers. The 24-hour recall method was used to collect the food intake data of the subjects. Hair mineral analysis was conducted using a Mass Spectrometer. Serum iron, ferritin, and calcium were also measured. RESULTS: The mean energy intakes of the subjects were 730.3 kcal, 994.3 kcal, and 1,482.9 kcal for each age group. The mean percentage of energy intake compared to recommendation was 101.4% and was not different by age group. Toddlers of 37.8% and preschoolers of 54.5% consumed less than the Estimated Average Requirement (EAR) of calcium. Infants of 28.6%, toddlers of 10.8% and preschoolers of 9.1% consumed less than the EAR of iron. In the case of zinc, copper, and selenium, only 0% to 5% of toddlers and none of the preschoolers consumed less than the EAR of those minerals. The hair calcium, iron and copper concentrations were lower in toddlers and preschoolers than those in infants. Serum calcium levels of preschoolers were significantly lower than those of infants, whereas serum iron and ferritin levels were not. CONCLUSION: Hair calcium, iron, and copper concentrations were significantly lower in toddlers and preschoolers than in infants. Insufficient dietary intake of calcium and iron seems to be related with decreased hair mineral contents in young children.

7.Nationwide Population-Based Epidemiologic Study on Childhood Intussusception in South Korea: Emphasis on Treatment and Outcomes

Purpose:
This was a nationwide population-based study conducted to investigate the epidemiology, treatment, disease outcomes, and associated factors of childhood intussusception in South Korea.
Methods:
Data from the Korean National Health Insurance Service database on all patients <18 years old diagnosed with intussusception from 2007 to 2017 were analyzed.
Results:
A total of 34,688 cases were identified among 30,444 patients. The overall incidence was 28.3/100,000 person-years with a male predominance. Most cases (83.1%) occurred in children <3 years old, with an annual incidence of 195.2, 200.1, and 118.6 cases per 100,000 children in their first, second, and third year of life, respectively. The median age at the first occurrence was 18.7 months, and it was higher in boys than in girls. The post-discharge recurrence rate was 10.6% (3,226/30,444) and the in-hospital recurrence rate was 6.1% (1,842/30,444). The total recurrence rate (post-discharge recurrence and/or in-hospital recurrence) was 15.0% (4,580/30,444). Enema reduction was successful in 90.0% of cases. Enema reduction was more successful in girls than in boys. A total of 3,296 (10.8%) patients underwent 3,481 surgeries, including 735 (21.1%) laparoscopic surgeries. Post-discharge recurrence and surgery were significantly affected by age, sex, and hospital type. Mortality was noted in nine cases (0.03%).
Conclusion
Our study provides accurate epidemiologic data on the treatment and outcomes of intussusception through complete enumeration during an 11-year-period.
8.A Case of Encephalopathy Presented with Motor Aphasia and Quadriplegia Following Intrathecal Methotrexate .

Intrathecal administration of methotrexate(IT-MTX) has constituted the standard approach to prophylaxis and treatment of central nevous system(CNS) leukemia. We experienced a quadriplegia and motor aphasia in a 14-year-old boy following repeated IT-MTX for the prophylaxis of meningeal leukemia. He was diagnosed as ALL without CNS involvement and treated by CCG- 1882 protocol. IT-MTX was administered for CNS prophylaxis. The patient began complaining of urinary incontinence, motor aphasia and weakness in his right leg from 12 days after the 5th dose of the IT-MTX therapy. Even though the IT-MTX was discontinued, loss of muscle power progressed upward resulting in quadriplegia. The patient showed slow and partial recovery on right extremities over 3 months. We report this case with brief review of literature.

9.Two Cases of Neonatal Osteomyelitis due to Extended Spectrum beta-lactamase Producing Klebsiella pneumoniae.

The most common etiologic agents of neonatal osteomyelitis in the last decades were Staphylococcus aureus, Group B streptococcus, and E. coli, but Klebsiella pneumoniae as a cause of neonatal osteomyelitis appears to be rare. Klebsiella pneumoniae is one of the most important organisms associated with hospital acquired infections in the neonate and outbreaks with multiresistant strains have been reported from neonatal intensive care units around the world. These multiresistant Klebsiella pneumoniae have been shown to produce tranferable plasmid mediated beta-lactamases that are able to hydrolyze oxyimmino- beta-lactamas and these confer resistance to the third generation cephalosporin and named extended spectrum beta-lactamase producing Klebsiella pneumoniae. We experienced two cases of neonatal osteomyelitis due to extended spectrum lactamase producing Klebsiella pneumoniae in our neonatal intensive care unit during the same period. To our knowledge this represents the first documented neonatal osteomyelitise to extended spectrum beta-lactamase producing Klebsiellae pneumoniae in Korea.

10.Effects of Preoperative Pain Management Education on the Control of Postoperative Pain: Focused on the PCA used Surgical Patients with Uterine Tumor.

PURPOSE: The purpose of this study was to examine the effects of preoperative pain management education on postoperative pain control in patients with uterine tumor using patient controlled analgesia. METHODS: This study used non-equivalent control group non-synchronized design. Data were collected from September, 2008 to March, 2009 at one university hospital in Daegu, Korea. There were 60 participants, 30 in both the experimental and control group. The experimental group was given preoperative pain education using videos, leaflets, and a PCA model. Postoperative pain intensity, frequency of the PCA button being pressed, and doses of additional analgesics were observed through 24 hours postoperative and knowledge of pain and attitude about the use of the pain medicine were measured at 3 days postoperative. Collected data were analyzed using t-test, chi2 test, repeat measured ANOVA, and Bonferroni methods. RESULTS: There were no significant differences in the postoperative pain level between the experimental and control group. Postoperative frequency of the PCA button pressed, doses of additional analgesics, pain knowledge and attitude about the use of the pain medicine of the experimental group were significantly higher than those of the control group. CONCLUSION: Pain management education is an effective nursing intervention for pain control after surgery.
