2.Factors Influencing Self Care Performance in Patients with Parkinson's Disease.

PURPOSE: This study aims to find the factors that affect the degree of self-care performance in Parkinson's disease (PD) patients. METHOD: This study used a descriptive correlational design. The data were collected using a sample of 80 PD patients, from the university affiliated Parkinson Center in Busan. Pearson's correlations and multiple regression analyses were conducted using the SPSS 18.0. RESULTS: The mean score of the self-care performance was 3.61 (+/-0.40). The highest score observed of self-care performance subscale was medication subscale (4.55+/-0.50) and the lowest score was observed in the exercise subscale (3.03+/-0.64). The self-care performance had significant correlations with depression (r=-.32, p<.01), self-care knowledge (r=.28, p<.05), function of motion (r=.25, p<.05), ability of daily activity (r=.22, p<.05), self-efficacy (r=.24, p<.05), and support from medical staff (r=.24, p<.05). The significant predictors of self-care performance included depression (beta=-.28), status of employment (beta=-.27), self-care knowledge (beta=.21), support from medical staff (beta=.28), and educational level (beta=.28), accounting for 34% of the variance in the self-care performance. CONCLUSION: Depression, self-care knowledge, medical support are significant predictors which affect the self-care performance with PD patients.

3.A study on the menstruation of Korean adolescent girls in Seoul.

PURPOSE: Westernized eating habits have been associated with early-age menstruation, which increases the incidence of dysmenorrhea and premenstrual syndrome among adolescent girls. We therefore surveyed changes in menarche timing and the general menstrual characteristics of adolescent girls in Seoul, Korea. METHODS: We surveyed 538 teenage girls who visited our hospital between July and November 2007. Items explored included age at menarche, general menstrual characteristics, occurrence of premenstrual syndrome and treatment thereof, and an association between present dysmenorrhea and a family history of the condition. RESULTS: Average age at menarche was 12.6 years, with 29% (n=156) subjects beginning menstruation at age 12 years. The prevalence of dysmenorrhea was 82% (n=435). The main symptoms were abdominal (53.2%) and lower back pain (34.2%), and 15.2% of girls who experienced such symptoms required medication. Present dysmenorrhea, and a family history thereof, were statistically correlated (P<0.05). In addition, 58.8% (n=316) of teenage girls had symptoms of premenstrual syndrome. The most frequent psychological symptoms were fatigue (36.4%) and nervousness (38.7%), whereas the most common physical symptom was menstrual cramps (46.5%). Most subjects (87.6%) tolerated the symptoms of premenstrual syndrome without medication; 11.4% took medicines including painkillers; but only 0.1% of subjects visited a doctor. CONCLUSION: The average age at menarche in Korean girls was 12.6 years, thus younger than in the past. Most teenage girls experienced dysmenorrhea and premenstrual syndrome, but few consulted a doctor. Organized treatment plans are required to manage menstrual problems in teenage girls.

4.Three-dimensional finite element stress analysis of single implant restoration using different fixture and abutment screw diameters.

STATEMENT OF PROBLEM: As the effects of the various diameters of fixture and abutment screw on stress distribution was not yet examined, this study focused on the different design of single implant restoration using three dimensional finite element analysis. PURPOSE: This study was to compare five different fixture-abutment combinations for single implant supported restorations with different fixture and abutment screw diameters. MATERIALS AND METHODS: The five kinds of finite element models were designed by 3 diameter fixtures (O/ 3.3, 3.75, 5.0 mm) with 3 different abutment screws (O/ 1.5, 1.7, 2.0 mm). The crown for mandibular first molar was made using UCLA abutment according to Wheeler's anatomy. 244 N was applied at the central fossa with two different loading directions, vertically and obliquely (30degrees) and at the buccal cusp vertically. Maximum von Mises stresses were recorded and compared in the supporting bone, crowns, fixtures, and abutment screws. RESULTS: 1. The stresses in supporting bone and implant-abutment structure under oblique loading were greater than those under vertical or offset loading. The stresses under vertical loading were the least among 3 loading conditions regardless of the implant and abutment screw diameters. 2. The stresses in the narrow implants were greater than the wider implants. The narrow implant with narrow abutment screw showed highest stresses in the lingual crest, but the narrow implant with standard abutment screw showed highest stress in abutment screw. 3. The stresses of abutment screws were influenced by the diameter of fixtures and loading conditions. The wide implants showed least difference between two different abutment screw diameters. CONCLUSIONS: The wide implants showed lesser stresses than the narrow implants and affected least by the different abutment screw diameters. The narrow implants with standard abutment screw showed highest stresses in the lingual bony crest under oblique loading.

5.Clinical Characteristics of the Respiratory Subtype in Panic Disorder Patients.

OBJECTIVE: Panic disorder has been suggested to be divided into the respiratory and non-respiratory subtypes in terms of its clinical presentations. The present study aimed to investigate whether there are any differences in treatment response and clinical characteristics between the respiratory and non-respiratory subtypes of panic disorder patients. METHODS: Among the 48 patients those who completed the study, 25 panic disorder patients were classified as the respiratory subtype, whereas 23 panic disorder patients were classified as the non-respiratory subtype. All patients were treated with escitalopram or paroxetine for 12 weeks. We measured clinical and psychological characteristics before and after pharmacotherapy using the Panic Disorder Severity Scale (PDSS), Albany Panic and Phobic Questionnaire (APPQ), Anxiety Sensitivity Index-Revised (ASI-R), State-Trait Anxiety Inventory (STAI-T, STAI-S), Hamilton Anxiety Rating Scale (HAM-A), and Hamilton Depression Rating Scale (HAM-D). RESULTS: The prevalence of the agoraphobia was significantly higher in the respiratory group than the non-respiratory group although there were no differences in gender and medication between the two groups. The respiratory group showed higher scores on the fear of respiratory symptoms of the ASI-R. In addition, after pharmacotherapy, the respiratory group showed more improvement in panic symptoms than the non-respiratory group. CONCLUSION: Panic disorder patients with the respiratory subtype showed more severe clinical presentations, but a greater treatment response to SSRIs than those with non-respiratory subtype. Thus, classification of panic disorder patients as respiratory and non-respiratory subtypes may be useful to predict clinical course and treatment response to SSRIs.

6.Transverse uterine incision closure: One versus Two layers.

OBJECTIVE: Closure of a low transverse cesarean incision with one layer and two layer, we compared operative time, hemostasis, blood loss and postoperative sonohysteroperitoneographic findings. Study design: At our hospital 103 women were randomized to closure of a low transverse cesarean incision with either one continuous layer of a locking No. 1 chromic suture or two continuous of No. 1 chromic suture with the first layer locked between Mar. 1, 1998 and Dec. 31, 1998. After three months later, sonhysteroperitoneography was taken and then we evaluated uterus indirectly. RESULTS: A one layer closure required less operative time, 16 versus 20.8 minutes (p<0.01), less hemoglobin change, 0.44 versus 1.3 (p<0.01) and less suture materials (p<0.01). But postoperative follow up sonohysteroperitoneography, a one layer closure shows slightly thinning of isthmic layer of myometrium, two layer closure shows more adhesional band and both are similar other findings. CONCLUSION: A one layer does not significantly affect the clinical course than traditional two layer closure. So we recommended a one layer closure when its use is anatomically feasible.

7.A photoelastic stress analysis of fixed partial dentures with endopore implants according to splinting, contact tightness, and crown length.

STATEMENT OF PROBLEM: A difficulty in achieving a passive-fitting prosthesis can be overcome by individual crown restoation of multiple implants. But individualized crown has another difficulty in control of contact tightness and stress distribution. PURPOSE: This in vitro study is to evaluate the stress distribution and the magnitude in the supporting tissues around Endopore implants with different crown lengths, interproximal contact tightness, and the splinting effects. MATERIAL & METHODS: Three Endopore implants(4.1*9 mm) were placed in the mandibular posterior edentulous area distal to the canine and photoelastic model was made with PL-2 resin(Measurements Group, Raleigh, USA). Restorations were fabricated in two crown lengths: 9, 13 mm. For non-splinted restorations, individual crowns were fabricated on three custom-milled titanium abutments. After the units were cemented, 4 levels of interproximal contact tightness were evaluated: open, ideal(8 micrometershim stock drags without tearing), medium(40 micrometer), and heavy(80 micrometer). For splinted restorations, 3-unit fixed partial dentures were fabricated. This study was examined under simulated non-loaded and loaded conditions(6.8 kg). Photoelastic stress analysis was carried out to measure the fringe order around the implant supporting structure. RESULTS: 1. When restorations were not splinted, the more interproximal contact tightness was increased among the three implants, the more stress was shown in the cervical region of each implant. When crown length was increased, stresses tended to increase in the apex of implants but there were little differences in stress fringes. 2. When nonsplinted restorations were loaded on the first or third implant, stresses were increased in the apex and cervical region of loaded implant. Regardless of interproximal contact tightness level, stresses were not distributed among the three implants. But with tighter interproximal contact, stresses were increased in the cervical region of loaded first or third implant. 3. When the nonsplinted restorations were not loaded, there were little stresses on the supporting structure of implants, but low level stresses were shown in the splinted restorations even after sectioning and soldering. 4. With splinted restorations, there were little differences in stresses between different crown lengths. When splinted restorations were loaded, stresses were increased slightly on the loaded implant, but relatively even stress distribution occurred among the three implants. CONCLUSIONS: Splinting the crowns of adjacent implants is recommended for Endopore implants under the overloading situation.

8.Prenatal Sonographic Diagnosis of Cephalopagus Twins Associated with Multiple Anomalies.

Conjoined twins are uncommon complications of monoamniotic twinning. Among them, cephalopagus twins are very rare. The accurate prenatal diagnosis of conjoining is essential for the optimal obstetric counseling and management. We describe a case where prenatal sonographic diagnosis of cephalopagus associated with multiple anomalies including meningomyelocele, diaphragmatic hernia, omphalocele, and club feet was made at 18 weeks of gestation.

9.Medical education program implementation experience in an elective course: a case study in South Korea

Purpose:
This study was conducted to examine the results of designing and implementing a teaching program for medical education as the elective course for 4th-year students of medical course.
Methods:
In order to design the teaching program for medical education as an elective course, we conducted literature review, five medical education experts were interviewed, and the literature required in the design process was reviewed. A developing teaching program was implemented as an elective course in a medical school of Korea, and 4th-year students of medical course participated in the program.
Results:
In the elective course, the medical education program process competencies were derived into three categories: theoretical educational knowledge, teaching competency, and research competency for education. Moreover, instructional materials were developed to help students achieve these competencies. And project-based learning strategy was selected and implemented for 4th-year students in medical course, and positive satisfaction was confirmed.
Conclusion
As a study designed and implemented in a medical education program in a medical school in Korea, it is expected to be helpful when introducing medical education to undergraduate students or developing a medical education program to strengthen the teaching capacity of residents.
10.Quantitative Analysis of Myofascial Pain Syndrome in Trapezius Muscle Using Pressure Algometer and Surface Electromyography

Purpose:
Myofascial syndrome is a chronic muscle pain caused by repetitive motions with stress-related muscle tension. This study aimed to investigate the validity and reliability of the evidence for diagnosing myofascial pain syndrome in trapezius muscle using a pressure algometer and surface electromyography.
Methods:
The experiments were performed using a total of 10 subjects, and the target locations were determined by means of a pressure algometer in the right upper trapezius muscle. The part with the lowest pain value as the trigger point and the part with the highest pain value as the non-pain trigger point were selected for measuring the locations. The median frequency and average frequency were measured in those locations with electromyography. To check the muscle fatigue, the upper trapezius muscle was moved up and down for 2 seconds at 5-second intervals in 30 seconds. The measured values were evaluated using the independent paired t-test and MannWhitney U-test.
Results:
The median frequency at the non-trigger point (13.7) was significantly higher than that at the trigger point (7.3). Furthermore, the mean frequency (14.7) at the non-trigger point was significantly higher than that at the trigger point (6.3).
Conclusion
The results showed the correlations between the trigger points of the muscle pain and frequency analysis of surface electromyography. Thus, this study may be possible to use as a diagnostic tool for myofascial pain syndrome.
