3.Correlation between CD44 Variants Expression, Microvessel Density and VEGF Expression and HPV 16/18 Subtypes in Squamous Neoplasia of the Uterine Cervix.

OBJECTIVE: In the development of squamous neoplasia of the uterine cervix, high risk HPV infection has been followed by CD44 variant expression and angiogenesis. The aim of this study is to evaluate the sequential changes and relatianship of CD44 variant expression, microvessel density (MVD), and VEGF expression in CIN III, microinvasive (MI) and invasive (CA) squamous cell carcinoma. METHODS: The materials were 17 cases of CIN III, 14 cases of MI and 15 cases of CA. In situ PCR for HPV 16/18 and immunohistochemical studies for CD44std, v6, v7/8, CD 31 for MVD, and VEGF were performed. RESULTS: CD44std expression was decreased in squarnous neoplasia compared to normal, and CD44v6 and v7/8 expressions were increased, however, there was no statistical significance. Accentuated staining of CD44v6 and v7/8 along the infiltrating borders was noted in 76.9% and 69.2% of MI and in 71.4% and 42.9% of CA, respectively. High MVD and VEGF 2+ expression were higher in the squamous neoplasia compared to narmal, however, there was no significant difference between the squamous neoplasia and no significant relationship between MVD and VEGF expression. The expression rates of CD44std and CD44v6 were higher in HPV 16/18 negative squamous neoplasia and that of CD44v7/8 was higher in HPV 16/18 pasitive squamous neoplasia, however, their differences were not significant. The incidences of high MVD and VEGF 2+ expression were higher in HPV 16/18 positive suqmaous neoplasia with no statistical significance. CONCLUSIONS: These results suggest that CD44 variants expression and angiogenesis are involved in the development of squamous neoplasia of the uterine cervix, however, HPV 16/18 might not be related to CD44 variant expression and angiogenesis.

4.Endocervical Glandular Lesions in Invasive and Intraepithelial Squamous Neoplasms of the Uterine Cervix.

The incidence of adenocarcinoma of the uterine cervix had shown an increasing tendency, and which is related to the increased use of oral contraceptives or prevalent in human papillomavirus infection. Endocervical glandular dysplasia or adenocarcinoma in situ are occasionally associated with squamous neoplasms of the uterine cervix. This study was aimed to evaluate the histologic features and the incidence of endocervical glandular lesions associated with squamous neoplasms, the presence of human papillomavirus infection-suggesting histologic findings in adjacent squamous neoplasms and the immmunohistochemical findings of endocervical glandular lesions for carcinoembryonic antigen. The materials used were 105 cases of microinvasive and invasive squamous cell carcinoma, and 83 cases of squamous intraepithelial lesions which are consisted of 142 radical or total hystrectomy products and 46 conization or loop excision products. The results are as follows; 1. Among 188 cases, six cases(3.2%) had shown foci of high grade glandular dyaplasia, and 19 cases(10.1%) revealed the areas of low grade glandular dysplasia. There was no adenocarcinoma in situ case. 2. In four of six high grade glandular dysplasia cases, microinvasive or invasive squamous cell carcinomas were associated. In low grade glandular dysplasias, sqaumous intraepithelial lesions were occcupying 68.4%. 3. HPV infection-suggesting histologic findings had accompanied all high grade glandular dysplasia cases and in 17 cases(89.5%) of 19 low grade glandular dysplasias. 4. In 2 of 5 high grade glandular dysplasias and in 1 of 19 low grade glandular dysplasias, the immunohistochemical reaction for carcinoembryonic antigen was similar to that of adenocarcinoma. In conclusion, the clinicopathologic importance of endocervical glandular lesions associated with squamous neoplasms of the uterine cervix should be kept in mind, and further study for the relationship between endocervical glandular lesions and human papillomavirus infection or hormonal influence will be continued.

5.Pulmonary Endometriosis: A case report.

Pulmonary endometriosis is a rare disease which is characterized by hemoptysis during menstruation (catamenial hemoptysis). We report a case of pulmonary endometriosis in a 33-year-old housewife. She has had regular menses with moderate flow and minimal dysmenorrhea. She had undergone curettage in May 1995 for artificial abortion. In July 1995, she experienced the first episode of hemoptysis. A chest CT scan revealed a 2.0 1.0 cm sized ill-defined soft tissue density in the periphery of anterior segment of the left upper lobe with a surrounding irregular ground-glass opacity. A left upper lobectomy was done under the diagnosis of pulmonary endometriosis. Cut section of the resected lung showed a round red-brownish solid lesion, measuring 2.0x1.0cm in cross. Microscopically a focus of the endometrial tissue, which was composed of endometrial glands and stroma, was found in the lung parenchyme and many hemosiderin laden macrophages were seen in the surrounding alveoli. The postoperative course was favorable with no further episodes of hemoptysis.

6.Translocation of p53 Protein in Melanocytes and Malignant Melanoma Cells After UVB Irradiation.

BACKGROUND: UVB is responsible for most of the carcinogenic effects of sun exposure. OBJECTIVE: Aim of this study was to investigate the regulation and intracellular redistribution of p53 protein after UVB irradiation. METHODS: Cultured normal human melanocytes and G361 melanoma cells were used for west-ern blotting and confocal microscopic examination for determining expression and distribution of p53. RESULTS: UVB irradiation increased p53 expression in cultured normal human melanocytes and G361 malignant melanoma cells according to increasing doses of UVB. Furthermore, p53 moved from cytosol to nucleus after UVB irradiation. CONCLUSIONS: UVB irradiation induced overexpression and redistribution of p53 in cultured normal human melanocytes and G361 malignant melanoma cells.

7.A large cardiogenic thrombus lodged at the carotid bifurcation mimicking severe carotid stenosis

A 62-year-old woman without vascular risk factors presented with left-sided weakness and numbness.
Magnetic resonance imaging (MRI) brain depicted acute right hemispheric infarcts in the cortical and
subcortical white matter. Initial MR angiography (MRA) showed large thrombus at the right carotid
bifurcation. Valvular atrial fibrillation (vAF) with severe tricuspid regurgitation (TR) was found on
two-dimensional echocardiography. Serial follow-up computed tomography angiography (CTA) or
MRA at 3, 9, and 15 days after anticoagulation alone showed complete resolution of the thrombus
with no neurological deterioration. Our case suggests that prompt institution of anticoagulantion alone
may result in radiologic resolution of the thrombus with improvement in patient’s clinical status.

8.A clinical and histologic study of 71 cases of basal cell carcinoma.

A clinical and histologic survey was done on 71 cases of basal cel carcinoma(BCC) filed at Department of Clinical Fathology, Kang Nam, Han Kang, and Kang Dong Sacred Heart Hospital during past 13 year s from 1979 to 1991. The results were summerized as follows: 1. Of all malignant skin tumors BCC accounted for 31.6%. The frequency of BCC in the total number of outpatients visiting the Department of Dermatology was 0.06%. 2. The ratio of men to women with HCC was 1:1.03. 83.8% of the pat.ients with BCC were older than the age of l0 years with the mean age of 56.9 years. 3. 83.8% of the BCC appared in the face, especially on the nose(25.4%), eyelid(20.3%). 4. On the basis of classification of Lever et al, the solid type(66.7% ) was the most com mon histologic pattern, followed by the adenoid(8.8%), pigmented(8.8%), and etc. By the classification of Farmer et al, the nodulocystic type(35.1% ) was the most common histologic pattern followed by infiltrative(31.6%), adenoid(8.8%), pigmented(8.8%), and etc. By the classification on Sexton et al, in the order of decreasing frequency, liistologic subtypes were the nodular(43.9%), mixed (22.8%), infiltrative(19.3%), supeficial(3.5%), morpheic(3.5%), and micronodular(3.5%).

9.Rapid and Simple Screening of Mitochondrial DNA in Koreans by the Analysis of Highly Variable Control Region SNPs.

Human mitochondrial DNA (mtDNA) is generally used to identify highly degraded forensic samples, particularly when the extracted DNA is not sufficient for nuclear DNA analysis. However, direct sequencing, the most widely used mtDNA analysis method, is laborious and time-consuming, and precludes the simultaneous analysis of many samples. Here, we describe a rapid and simple screening method for mtDNA analysis in Koreans using single base extension (SBE) methods. Sixteen highly polymorphic mtDNA SNPs from the control region were selected, and a multiplex SBE system was constructed to analyze them. Because the developed system consists of two duplex PCRs, which produce small amplicons with fewer than 270 bp, it works well with highly degraded samples such as old skeletal remains. Using this multiplex SBE system, 145 different haplotypes were expected to be observed from 593 unrelated Koreans. Seventy-three haplotypes were expected to be observed only once, and the most frequent haplotype was expected to occur 80 times. Since the mean number of pairwise differences was estimated to be 4.55, the developed system could be useful to exclude samples that do not match evidence and reference samples. Therefore, the multiplex SBE system used in this study will be a useful tool to analyze many samples simultaneously and to efficiently screen out non-matching mtDNA sequences in forensic casework.
