We present two cases of congenital epulis in female newborns. Congenital epulis is a very rare lesion of uncertain histogenesis. The present lesions were located on the gingiva of the anterior alveolar ridge of the maxilla and the mandible, respectively. Both tumors consisted mainly of large eosinophilic granular cells arranged in solid nests. The neoplastic granular cells showed positive reactions for neuron specific enolase and vimentin in their cytoplasms, while they were entirely negative for other antibodies used in this study.

We present a case of ovarian leiomyoma without related clinical symptoms in a 68-year-old woman. Leiomyoma arising primarily in the ovary is rare. However, it is believed that there are actually more cases than those reported because this condition is usually mistaken for a fibrothecoma or parasitic leiomyoma. Most cases previously reported were incidentally presented and coexisted with other ovarian lesions. The present case was characterized by a 9 cm, round lobulated mass that totally replaced the left ovary without uterine leiomyoma or coexisting ovarian lesions.

A 55-year-old woman, with a retroperitoneal cystic mass, incidentally found during evaluation for the cause of right upper quadrant pain, was referred to our hospital from a local clinic. Computed tomography, performed at our hospital, revealed a multilobulated cystic lesion in the right perinephric space, encasing the renal vessels, suggestive of a cystic lymphangioma. A cystic abdominal lymphangioma is a rare tumor in adults, which is difficult to diagnose preoperatively. Surgical excision is known as the treatment of choice; however, only three cases of laparoscopic excision have been reported. We report a case of a successful laparoscopic excision of a retroperitoneal cystic lymphangioma encasing the renal vessels of the renal hilum.

The Committee of Clinical Practice Guidelines of the Korean Diabetes Association revised and updated the '3rd Clinical Practice Guidelines' at the end of 2010. In these guidelines, the committee recommends active screening of high risk individuals for early detection and added HbA1c level as a diagnostic criterion of type 2 diabetes to produce a more practical approach based on clinical studies performed in Korea. Furthermore, committee members emphasize that integrated patient education for self-management is an essential part of patient care. The drug treatment algorithm was also updated based on the degree of hyperglycemia and patient characteristics.

BACKGROUND: This study investigates the perception of the general public regarding the concentration to metropolitan, hospitals of cardiac and cerebrovascular surgeries, and the perceived public need for government policies to resolve this issue. METHODS: A total of 800 participants were recruited for our telephone interview survey. Quota sampling was performed, adjusting for age and sex, to select by various geographic regions. Sampling with random digit dialing was performed; we called the randomly generated telephone numbers and made three attempts for non-responders before moving on to a different telephone number. RESULTS: Our sample population was 818 participants, 401 men (49.0%) and 417 women (51.0%). Our data showed that 85.5% of participants thought that cardiac surgery and neurosurgery patients are concentrated in large hospitals in Seoul. The principle reason for regional patients to want to receive surgery at major hospitals in Seoul was because of poor medical standards associated with regional hospitals (87.7%). We found that a vast majority of participants (97.5%) felt that government policies are needed to even out the clustering of cardiac surgery and neurosurgery patients, and that this clustering may be alleviated if policies that can specifically enhance the quality and the capacity of regional hospitals to carry out surgeries are adopted (98.3%). CONCLUSION: Government policy making must reflect public desiderata, and we suggest that these public health needs may be partially resolved through government-designated cardiac and neurosurgery specialist hospitals in regional areas.

Bronchiolitis obliterans (BO) is a serious noninfectious complication following an allogeneic bone marrow transplant (BMT). A 21-year-old female received an allogeneic BMT as a treatment for myelodyplastic syndrome. Four months after the BMT, progressive dyspnea developed and BO was also diagnosed by a lung biopsy. The patient was administered steroid and immunosuppressive agents for 1 year but there was no improvement in pulmonary function. Azithromycin was prescribed (500 mg q.d. for 3 days followed by 250 mg three time a week) because macrolides might decrease the inflammatory reaction leading to BO. The patient's pulmonary function improved after administration of azithromycin for 1 year. The forced expiratory volume in a one second (FEV1) increase was 220 mL (28.2%) and the forced vital capacity (FVC) increase was 460 mL (25.7%). We report the improvement in the pulmonary function after the administration of azithromycin for 1 year in a patient with BO after a BMT.
