1.The expression of FAS-associated factor 1 and heat shock protein 70 in ovarian cancer.

OBJECTIVE: In this study, we evaluated the expression of FAS-associated factor 1 (FAF1) and heat shock protein 70 (HSP70) in normal ovary and ovarian cancer, and also analyzed the correlation between FAF1 and HSP70 in ovarian cancer. METHODS: The patient group consisted of 29 unrelated Korean women diagnosed as ovarian cancers and control samples were obtained from 7 patients who underwent oophorectomy for benign disease of uterus, and normal ovary was confirmed histologically from biopsy. We examined FAF1 and HSP70 expression by western blot analysis and immunohistochemical staining in normal ovary and ovarian cancer. Furthermore, we examined a correlation between FAF1 and HSP70 in ovarian cancer. RESULTS: The expression of FAF1 was lower in ovarian cancer than that in normal ovary (P=0.02), and the expression of HSP70 was increased in ovarian cancer in comparison to that in normal ovary (P=0.03). The expression of FAF1 was decreased in advanced stages (stage III or stage IV) as compared with early stages (stage I or stage II) (P=0.01). The expression of HSP70 was not significantly related with ovarian cancer histology (P=0.10), but the expression of HSP70 was most increased with papillary serous carcinomas and undifferentiated ovarian cancer. The expression of FAF1 was inversely correlated with the expression of HSP70 in ovarian cancer (Spearman correlation coefficience=-0.47). CONCLUSION: We concluded that the expression of FAF1 or HSP70 each seems to have a meaning as a biomarker for early detection of ovarian cancer. The expressions of FAF1 and HSP70 seem to be more valuable in predicting ovarian cancer when used together because of their inverse correlation. This is the first study about the expression of FAF1 in ovarian cancer and the correlation between FAF1 and HSP70 expression in ovarian cancer.

2.Endocervical Glandular Lesions in Invasive and Intraepithelial Squamous Neoplasms of the Uterine Cervix.

The incidence of adenocarcinoma of the uterine cervix had shown an increasing tendency, and which is related to the increased use of oral contraceptives or prevalent in human papillomavirus infection. Endocervical glandular dysplasia or adenocarcinoma in situ are occasionally associated with squamous neoplasms of the uterine cervix. This study was aimed to evaluate the histologic features and the incidence of endocervical glandular lesions associated with squamous neoplasms, the presence of human papillomavirus infection-suggesting histologic findings in adjacent squamous neoplasms and the immmunohistochemical findings of endocervical glandular lesions for carcinoembryonic antigen. The materials used were 105 cases of microinvasive and invasive squamous cell carcinoma, and 83 cases of squamous intraepithelial lesions which are consisted of 142 radical or total hystrectomy products and 46 conization or loop excision products. The results are as follows; 1. Among 188 cases, six cases(3.2%) had shown foci of high grade glandular dyaplasia, and 19 cases(10.1%) revealed the areas of low grade glandular dysplasia. There was no adenocarcinoma in situ case. 2. In four of six high grade glandular dysplasia cases, microinvasive or invasive squamous cell carcinomas were associated. In low grade glandular dysplasias, sqaumous intraepithelial lesions were occcupying 68.4%. 3. HPV infection-suggesting histologic findings had accompanied all high grade glandular dysplasia cases and in 17 cases(89.5%) of 19 low grade glandular dysplasias. 4. In 2 of 5 high grade glandular dysplasias and in 1 of 19 low grade glandular dysplasias, the immunohistochemical reaction for carcinoembryonic antigen was similar to that of adenocarcinoma. In conclusion, the clinicopathologic importance of endocervical glandular lesions associated with squamous neoplasms of the uterine cervix should be kept in mind, and further study for the relationship between endocervical glandular lesions and human papillomavirus infection or hormonal influence will be continued.

3.Association of the E-cadherin gene polymorphism with cervical intraepithelial neoplasm and cervical cancer in Korean population.

OBJECTIVE: The aim of this study was to explore the association between E-cadherin promoter -160C>A polymorphism and the risk of cervical cancer and cervical intraepithelial neoplasm (CIN) in a Korean population. METHODS: We investigate 107 patients with histopathologically confirmed cervical cancer, 119 patients with histopathologically confirmed CIN and 112 control group women who were surgically proven to have normal cervices. The genetic distribution of E-cadherin promoter -160C>A polymorphism were analyzed by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) of PCR products. RESULTS: We found no overall association between each individual E-cadherin promoter -160C>A genotype and the risk of cervical cancer and CIN. In the cervical cancer group, the allele frequency of C was 83.6%, in the control group 83.5%, showing no significant difference (p=0.941). Similarly, in the CIN group, the allele frequency of C was 81.9%, in the control group 83.5%, showing no significant difference (p=0.645). A subgroup analysis of the clinical parameters in CC, CA, AA genotype also showed no significant difference suggesting the lack of an association between E-cadherin promoter -160C>A polymorphism and cervix cancer stages (p=0.413), then its polymorphism and HPV infection (p=0.634). CONCLUSION: our results show that Korean women with specific polymorphism in E-cadherin promoter -160C>A are neither more susceptible to develop the cervical cancer or CIN nor more valuable for the cancer progression.

5.Convulsions with Noroviral Gastroenteritis in Children at a Single Center in Korea.

PURPOSE: Viral gastroenteritis is a common disease in infants and children. Seizures can be associated with viral gastroenteritis as benign convulsions with mild gastroenteritis (CwG). After the development of the rotavirus vaccination, norovirus has become inreasingly significant in children. We retrospectively analyzed the clinical features in a pediatric population presenting with seizures and confirmed enteral viral infections, especially norovirus infections. METHODS: We retrospectively reviewed the medical records of pediatric patients aged < 15 years admitted due to convulsions and gastroenteritis to the Department of Pediatrics of Myongji Hospital between July 2014 and June 2016. RESULTS: A total of 46 patients (24 male and 22 female) were included. Norovirus was detected in 21 (45.7%) patients, adenovirus in three (6.5%), rotavirus in two (4.3%), astrovirus in one (2.2%), and none of agents were detected in 19 (41.3%) patients. Patients in the norovirus gastroenteritis (NGE) group had a higher incidence of diarrhea than that among in the non-norovirus gastroenteritis (NNGE) group (61.9% vs 28.0%; P < 0.05). Twelve patients experienced status epilepticus, including five (23.8%) in the NGE group and seven (33.0%) in the NNGE group (P=0.837). Seizures were effectively terminated by intravenous benzodiazepines in 8 (66.7%) of 12 patients. Additional long-acting antiepileptic drugs such as fosphenytoin or levetiracetam were required in three (25%) of 12 patients. CONCLUSION: Patients with CwG with and without noroviurs infection did not differ in terms of clinical features. However, status epilepticus was not uncommon among patients with CwG by definition.

6.Effectiveness of autogenous tooth bone used as a graft material for regeneration of bone in miniature pig.

INTRODUCTION: This study examined the effect of autogenous tooth bone used as a graft material for bone regeneration in an artificial bony defect of minipigs. MATERIALS AND METHODS: Four healthy minipigs, weighing approximately 35-40 kg, were used. Four standardized artificial two-walled bony defects, 5 mm in length and depth, were made on the bilateral partial edentulous alveolar ridge on the mandible of minipigs, and autogenous tooth bone was augmented in the right side as the experimental group. On the other hand, only alloplastic bone graft material HA was grafted with the same size and manner in the left side as the control group. All minipigs were sacrificed at 4 weeks after a bone graft and evaluated histologically by Haematoxylin-eosin staining. The specimens were also evaluated semi-quantitatively via a histomorphometric study. The percentage of new bone over the total area was evaluated using digital software for an area calculation. RESULTS: All specimens were available but one in the left side (control group) and two in the right side (experimental group) were missing during specimen preparation. The amount of bone formation and remodeling were higher in all experimental groups than the control. The mean percentage area for new bone in the experimental and control groups was 43.74+/-11.96% and 30.79+/-2.93%, respectively. CONCLUSION: Autogenous tooth bone is a good alternative to autogenous bone with the possible clinical feasibility of an autogenous tooth bone graft in the reconstruction of bony defects.

7.Major health related problems among adolescents in Seoul area.

BACKGROUND: Adolescence offers unique opportunities for investment in health and well-being. The purpose of this study was to examine the common physical symptoms and diseases and medical utilization rate and the prevalence of health behaviors among school-aged adolescents. METHODS: This study was carried in 1998. Subjects consisted of 3,771 adolescents (ages 10 to 18) who attended an elementary, middle, and high school in Seoul and 3,246 parents. Self-report questionnaire was administered to the students and their parents to measure self-reported physical and psycho\social problems, practice of health behavior, medical care utilization rate, and the most common diagnosis. RESULTS: 1) Academic performance was the most common psycho\social problem for adolescents and their parents. 2) The perceived health status was highest in middle school students, lowest in female high school students. 3) As adolescents went to higher grades, the medical institute and dental clinic utilization rate tended to be decreased, and oriental clinic utilization rate did not change. The most common diagnosis during the past year was dental caries. The next most common diagnosis was respiratory infection. Accidents (males), headache (females) were more common in high school than middle school. 4) The most frequently reported symptom was fatigue in middle and high school students. More than half of female high school students reported fatigue, dizziness, headache, dysmenorrhea, hypersomnia during the last month. 5) The higher their grade was, the less adolescents practiced health behaviors. 21.9% of males and 1.9% of female high school students were smokers. 34.5% of males and 13.3% of female high school students drank alcohol on a regular basis. The prevalence of obesity was 12.0% in males, 10.0% in female high school students. Male adolescents tended to deal with stress with leisure activities, female adolescents did by talking about their problems with related people. CONCLUSION: This overview of health associated issues among adolescents provided a unique view of differences in health care needs between younger and older teens and between male and female teens. These data have meaningful implications for health promotion program for adolescents.

8.The Effects of Self-directed Practice using Peer-tutoring on Confidence, Performance and Learning Satisfaction of Nursing Students in Practicing Core Nursing Skills.

PURPOSE: The purpose of the non-equivalent control group pretest-posttest study was to test the effects of self-directed nursing practice using peer-tutoring, on the level of confidence, performance and learning satisfaction of nursing students in practicing core nursing skills. METHODS: The data were collected from 80 junior nursing students at a nursing college in Daejeon before and after 3 weeks of intervention on practicing six core nursing skills, according to the highest order of priority. The subjects were divided into the peer-tutoring practice group (PTPG, n=40) and the lecturer-guided practice group (LGPG, n=40). The data were analyzed using mean, frequency, and t-test. RESULTS: The PTPG scored statistically higher than the LGPG on the performance of ‘indwelling urinary catheterization’ and ‘wearing protection equipment while entering quarantine room & disposing waste’ The PTPG scored statistically higher than the LGPG on the confidence of ‘indwelling urinary catheterization’ and ‘inserting intravenous catheterization’. The PTPG scored statistically higher on learning satisfaction than LGPG. CONCLUSION: The results showed that self-directed nursing practice using peer-tutoring could be effective for nursing students in improving proficiency in core nursing skills and might be applied to core nursing skills training.

9.Clear Cell Sarcoma of the Kidney: A case in 39 year old man.

Clear cell sarcoma of kidney(CCSK) is a rare pediatric neoplasm characterized by a predominating component of clear cells, a predilection for metastases to bone, and a poor prognosis. The incidence of CCSK peaks during the 2nd year of life and adult cases are very rare. We report a case of CCSK encountered in the right kidney of a 39-year-old man. Grossly, it was a lobulated mass showing infiltrative margin, measured 7x5.5x5cm and had a homogeneous gray-tan color with a soft, fish-flesh consistency. Microscopically, about half of the tumor revealed the classic pattern of CCSK, having tumor cell cords or nests separated by the characteristic alveolar capillary networks. The tumor cells had clear pale cytoplasm, bland looking round nuclei and inconspicuous nucleoli. The other half showed the epithelioid-trabecular pattern forming pseudorosette or cord-like structures. Immunohistochemically, there was only a focal positive reaction to vimentin. Ultrastructurally, the tumor cells showed the primitive nephrogenic mesenchymal differentiation such as electron lucent cytoplasm, a small amount of organelles, scanty heterochromatin, inconspicuous nucleoli, and a lack of flocculant basal lamina material around the cytoplasmic membrane. We consider that this is a case of CCSK occuring in the oldest patient ever reported, confirmed by both immunohistochemistry and electron microscopy.

10.Bilateral Massive Ovarian Leiomyoma in an Adolescent Female.

Ovarian leiomyoma is a very uncommon tumor that usually presents as an unilateral neoplasm typically associated with uterine leiomyoma in middle aged to postmenopuasal women. These probably originate from smooth muscle cells of the ovarian blood vessel or from the smooth muscle fibers near the attachment of the ovarian ligament. We report a case of ovarian leiomyoma, bilateral and massive in 17 year-old woman. We document the smooth muscle origin of the tumors with immunohistochemical studies that show appropriate staining with antibodies to actin, desmin, vimentin, S-100 protein.
