3.Cellular Schwannoma Arising in a Facial Nerve.

Cellular schwaninoma is a variant of schwannoma, which is characterized by predominance of cellular Antoni A area, presence of mitotic activity, nuclear hyperchromasia, pleomorphism, and absence of Verocay body. These pathologic features often prompted a misdiagnosis of malignancy. However, the clinical outcome has indicated the benignity of the tumor. We have experienced a case of cellular schwannoma arising from right facial nerve with right hemifacial weakness and erosion of mastoid process. Grossly, it was a 3.5 x 3 cm sized and relatively well encapsulated mass with yellowish, friable cut surface. Microscopically, cellular growth with moderate cellular pleomorphism and some mitotic activity (5/40 HPFS, up to 2/HPF) were noted. Immunostaining for S-100 protein showed diffuse strong positive reaction.

5.A Case of Pyogenic Liver Abscess Caused by Staphylococcus epidermidis in a Healthy Child.

Staphylococcus epidermidis is a normal inhabitant of skin, throat, mouth, vagina, and urethra. It is not usually pathogenic, particularly in immunocompetent hosts. This report describes a case of a pyogenic liver abscess caused by Staphylococcus epidermidis in a healthy 12-year-old boy. He was admitted to Seoul St. Mary's Hospital with intermittent fever for 6 days. Findings on abdominal computed tomography (CT) showed a mass measuring 7.0x6.5 cm in the right hepatic lobe. Culture of the abscess resulted in growth of Staphylococcus epidermidis as a causative organism. The patient was successfully treated with intravenous administration of antibiotics and percutaneous drainage of the abscess.

7.Music Program Development Using the Ukulele in Community-dwelling Old Adults and Its Effect

Purpose:
This study is a pseudo-experimental study of design before and after the non-equivalent control group, which was attempted to verify that the application of the ukulele to the elderly has the effects of reducing depression, improving self-efficacy, strengthening social bonding, and improving cognitive function.
Methods:
46 (23 in the experimental group and 23 in the control group) participants were selected. The experimental group was provided with three sessions of a music program using eight Ukuleles for the elderly, while the control group was provided with three sessions for the elderly. IBM SPSS 25.0 was used for data analysis, and the independent t-test, x2 -test, and Fisher's exact probability test were performed to verify the homogeneity of the subject's general characteristics.The effect verification after the experimental treatment was analyzed by Fisher's exact probability test, Friedman test, and Mann-Whitney U test.
Results:
Depression showed a statistically significant difference between the two groups (F=39.88, p<.001), self-efficacy showed a statistically significant difference between the two groups (z=-4.96, p<.001), social bonding showed a statistically significant difference between the two groups (z=-5.19, p<.001), and cognitive function showed a statistically significant difference between the two groups (z=-3.98, p<.001).
Conclusion
It was found that the ‘Music Program using the Ukulele’ was effective in reducing depression of the elderly, improving self-efficacy, reinforcing social bonding, and improving cognitive function. We hope that the Music Program using the Ukulele can be used in the elderly nursing curriculum in the future, and we suggest it should be applied as a nursing intervention to those who are experiencing cognitive decline.

9.Regional disparities in major cancer incidence in Korea, 1999-2018

OBJECTIVES:
This study investigated regional disparities in the incidence of 8 major cancers at the municipal level in Korea during 1999-2018 and evaluated the presence or absence of hot spots of cancer clusters during 2014-2018.
METHODS:
The Korea National Cancer Incidence Database was used. Age-standardized incidence rates were calculated by gender and region at the municipal level for 4 periods of 5 years and 8 cancer types. Regional disparities were calculated as both absolute and relative measures. The possibility of clusters was examined using global Moran’s I with a spatial weight matrix based on adjacency or distance.
RESULTS:
Regional disparities varied depending on cancer type and gender during the 20-year study period. For men, the regional disparities of stomach, colon and rectum, lung, and liver cancer declined, and those of thyroid and prostate cancer recently decreased, despite an overall increasing incidence. For women, regional disparities in stomach, colon and rectum, lung, liver, and cervical cancer declined, that of thyroid cancer recently decreased, despite an overall increasing incidence, and that of breast cancer steadily increased. In 2014-2018, breast cancer (I, 0.61; 95% confidence interval [CI], 0.53 to 0.70) showed a high probability of cancer clusters in women, and liver cancer (I, 0.48; 95% CI, 0.40 to 0.56) showed a high probability of cancer clusters in men.
CONCLUSIONS
Disparities in cancer incidence that were not seen at the national level were discovered at the municipal level. These results could provide important directions for planning and implementing local cancer policies.
10.A Clinical Observation on Lupus Nephritis in Children.

We reviewed the clinical features, histrologic patterns and clinical courses of 30 children with lupus nephritis retrospectively, and the results were summerized as follows; 1) The male to female ratio was 1:2.8, and the mean age at the onset was 10 8/12 years. 2) The clinical symptoms were diverse, and malaise, weight loss, anorexia, fever and malar rash were the most frequent findings. 3) Among the immunologic tests, FANA and anti-ds-DNA test revealed the highest sensitivity with positive rates of 97% and 87%, respectively. 4) Clinically, 57% of patients had active nephrotic syndrome at the onset, and 33% showed (?) was the most common findings (70%). 5) During the follow-up period, one children with Class IV lupus nephritis expired. And 8 out of 9 cases with renal insufficiency at the onset showed improvement of renal function after treatment with corticosteroid and cytotoxic agents. In conclusion, the clinical features and histologic findings of lupus nephritis in children were diverse. Early diagnosis and proper treatment can prevent rapid deterioration of renal function and improve long-term survival rate.
