3.Clustered Microcalcifications on Mammography: Histopathologic Correlation in Benign and Malignant Lesions.

PURPOSE: To evaluate differential points of clustered microcalcification in malignant and benign lesions on histopathology and correlate with mammographic findings. MATERIALS AND METHODS: Authors evaluated differential points of malignant and benign calcifications in histopathologically confirmed 24 cases. In two patients, mapping studies and paraffin block mammographic examinations were done to correlate calcification on mammography with histopathologic morphology. RESULT: On histopathology, malignant calcifications were located in ductal lumen, while benign ones were usually located in ductal epithelium(p<0.05). However, associated three benign lesions of the 10 malignant lesions showed still benign calcifications adjacent to proven cancer. The typical malignant calcifications were the shape of lamination, while benign calcifications showed granular and salty on light microscope. In 2 mapping studies, malignant-looking calcifications within the area of predominant benign calcifications yielded atypical ductal hyperplasia. Mammographic pepper, granular, punctate, salty and S-shaped calcifications were correlated with granular and salty calcification on light microscope. Mammographic linear, branching, comma, tadpole and wormiform calcifications were correlated with histopathologic laminated calcification. CONCLUSION: Authors have recognized about characteristics of malignant Vs. benign clustered microcalcifications on histopathology. Authors concluded that if malignant-looking calcification such as linear, branching, comma, tadpole and wormiform are present in an are a of predominant benign calcifications, specimen radiographic correlation and mapping study will be necessary for better demonstration of the atypical ductal hyperplasia, hidden or associated breast cancer.

4.The Effect of Muscle Relaxation Therapy on Mood State and Job Stress of Clinical Nurses.

PURPOSE: This study is to identify the effects of muscle relaxation therapy on mood state and job stress of clinical nurses, finally to purpose the bases of nursing intervention to increased of work efficiency and job satisfaction. METHOD: The research design was a nonequivalent control group pretest-posttest design. The subjects were 50 clinical nurses (23 for experimental group), who were working for 1-5 years in medical and surgical ward from the 2 C-University medical center in Seoul. This study was conducted from May 12 to July 25, 2000. The muscle relaxation therapy was performed 5 weeks to experimental group. The experi-mental group participated in the muscle relaxation training sessions for 20minutes twice a week during five weeks. The level of job stress was measured by nurses occupational stress scale. The level of Psychological response was measured by Profile of mood state. The data was analyzed using descriptive statistics, x2-test, t-test, paired t-test. RESULT: The results of this study were as follows: The experimental group who received the muscle relaxation therapy showed more reduction of job stress level (t=1.35, p=0.038) and profile of mood state level (t=2.27, p=0.027) after therapy than those in control group. CONCLUSION: This study reveals that the muscle relaxation therapy is effective in reduction of nurse's job stress and promotion of mood state. So the researcher thinks that it is useful in reduction of job stress and increased of job satisfaction in hospital.

5.Correlations between electrophysiological parameters of repetitive nerve stimulation and single fiber electromyography tests in myasthenia gravis.

BACKGROUND AND PURPOSE: For diagnosis of acquired autoimmune myasthenia gravis (MG), single fiber electromyography (SFEMG) is known to be much more sensitive than repetitive nerve stimulation test (RNST) in demonstrating the electrophysiological defects of neuromuscular transmission. Neuromuscular blocking in SFEMG is presumed to have the same physiologic basis with decremental response in RNST. The authors performed the study to know the correlations between the electrophysiological parameters of the RNST (decremental response) and the SFEMG (jitter and blocking) and to understand the basic physiological mechanisms in MG. METHODS: In 28 patients with definite MG (12 : ocular MG, 16 : generalized MG), RNST and SFEMG tests were done in orbicularis oculi and abductor digiti quinti and in extensor digitorum communis, respectively. The correlations between several factors, such as maximum decremental response (%) in RNST, mean value of mean consecutive differences (MCD's) (?sec) and blocking pairs (%) were analyzed. RESULTS: In 12 ocular MG patients, RNST and SFEMG abnormalities were found in 4 (33.3%) and in 10 (83.3%), respectively. In 16 generalized MG patients, abnormal decremental responses were found in 13 (81.3%) and increased jitters in 15 (93.8%). SFEMG showed significant correlations between blockings and mean MCD's (R=0.54, p<0.01). When the correlation between RNST and SFEMG was analyzed, increased percentage of blockings in extensor digitorum communis were correlated with maximum degree of decremental responses in abductor digiti quinti or orbicularis oculi (R=0.60, p<0.001). The degree of mean MCD's was poorly correlated with maximum decremental responses (R=0.23, p=0.229). But the correlation became significant (R=0.43, p<0.05) by analyzing mean MCD's and maximum decremental responses only from abductor digiti quinti, reflecting that blockings have similar electrophysiological meanings with increased mean MCD's. CONCLUSION: The good correlation between decremental response in RNST and blocking or mean MCD's in SFEMG was found. Therefore the authors concluded that the parameters of the RNST and the SFEMG show similar electrophysiologic phenomena of the abnormal neuromuscular transmission in MG.

6.Correlation between Fatigue and Quality of Life Caused by Stress of Researchers' Work.

PURPOSE: The purpose of this research was to provide baseline data by utilizing solutions to health problems caused by work-related stress and to examine a way of treatment by comparing and analyzing how the stresses link to fatigue and quality of researchers' life. METHODS: A survey was conducted with 50 researchers working for in H research institute in Daejeon Metropolitan City from April 10th to May 10th, 2012 to examine the correlation of work stress, fatigue and quality of life. The data were analyzed using the Kruskal-Wallis test to look into examine the difference in work stress, fatigue, and quality of life. Also, the correlation of work stress, fatigue and quality of life was measured through by the Pearson correlation coefficient. The data collected through the questionnaires was were analyzed by SPSS 21.0 based on the significant level with a p-value, 0.05. RESULTS: The results showed that fatigue was caused by work stresses in researchers and quality of life was ranked in a relatively low level. Correlation of work stresses, quality of life and fatigue and work stresses against mental health index was negative and fatigue against physical index was also negative. CONCLUSION: It is necessary to maintain more detailed provisions in order to improve mental health and stress level of researchers. Moreover, systematic, in-depth evaluation should be done to reduce researcher's work-related stress and fatigue. In addition, stress management and prevention program should be implemented for these researchers.

7.A case of Guillain Barre Syndrome showing pupillary paralysis.

It has been reported that pupillary paralysis is rare findings in typical Guillain-Barre syndrome(GBS). We experienced a 56-year-old male with fulminant GBS who pupillary paralysis. He showed quadriplegia, total paresis of motor cranial nerves including bilateral ptosis and complete opthalmoplegia. Pupils were dilated up to 8mm and fixed. After a one year follow up, his pupil slowly constricted to light. The instillation of 0.1% pilocarpin caused both pupillary constriction. These results pointed to postganglionic involvement of pupillary parasympathetic nerves.

8.Structural Relationships among Job Embeddedness, Emotional Intelligence, Social Support and Turnover Intention of Nurses.

PURPOSE: This study was done to elicit basic data for effective human resource management by identifying the relationships among job embeddedness, emotional intelligence, social support, and the turnover intention of Nurses. METHODS: Research design was to build a hypothetical causal model between variables and to verify its fitness. The sample for this study was 283 nurses with careers of more than 6 months in one hospital of more than 800 beds located in Seoul. They agreed in writing and this study was approved by the Institutional Review Board. Data were analyzed using SPSS 18.0 and AMOS 18.0 program. RESULTS: Differences in general characteristics for the variables were significant for age, marital status, education, work experience, job title, income, and department. Job embeddedness, emotional intelligence and social support were significantly correlated to turnover intention. Job embeddedness to emotional intelligence and social support showed positive effects and a negative effect to turnover intention. Emotional intelligence to turnover intention showed a positive effect, but social support was not significant. CONCLUSION: Organizations should provide ways to minimize voluntary turnover of a competent workforce and demonstrate their competency. Also it should develop training and management programs to effectively utilize emotional intelligence.

9.A Case of Primary Anetoderma.

Anetoderma is a rare cutaneous disorder characterized by circumscribed loss of skin elasticity. This has been historically divided into inflammatory(Jadassohn-Pellizzari) and non-inflammatory(Schw eninger-Buzzi) type. We herein, report a case of Schweninger-Buzzi type anetoderma occurring on a 6 year-old girl. The lesions did not have an inflammatory onset. Histologically, the focal loss of elastic fibers was seen on the lesion.

10.Pro-inflammatory Cytokine Expression of Spleen Dendritic Cells in Mouse Toxoplasmosis.

Dendritic cells have been known as a member of strong innate immune cells against infectious organelles. In this study, we evaluated the cytokine expression of splenic dendritic cells in chronic mouse toxoplasmosis by tissue cyst-forming Me49 strain and demonstrated the distribution of lymphoid dendritic cells by fluorescence-activated cell sorter (FACS). Pro-inflammatory cytokines, such as IL-1alpha, IL-1beta, IL-6, and IL-10 increased rapidly at week 1 post-infection (PI) and peaked at week 3 PI. Serum IL-10 level followed the similar patterns. FACS analysis showed that the number of CD8alpha+/CD11c+ splenic dendritic cells increased at week 1 and peaked at week 3 PI. In conclusion, mouse splenic dendritic cells showed early and rapid cytokine changes and may have important protective roles in early phases of murine toxoplasmosis.
