1.Molecular Characterization of papA Gene from Uropathogenic Escherichia coli.

The papA gene in uropathogenic Escherichia coli strains was characterized by PCR-restriction fragment length polymorphism (RFLP) and enterobacterial repetitive intergenic consensus (ERIC)- PCR. One hundred four E. coli strains from patients with urinary tract infections and 32 strains from healthy persons were examined. Thirty seven (27.2%) strains (33 from patients, 4 from healthy persons) in the 136 E. coli strains were positive in mannose resistant hemagglutination (MRHA) test. The adherence of MRHA positive strains to HEp-2 cells was greater than those of MRHA negative isolates (p<0.001). PapA-PCR were positive in 25% (26/104) of the strains from patients, and 3.1% (1/32) of the strains from healthy persons. Among 27 papA-positive isolates, subtypes were identified by RFLP as 8 (29.6%) F7z, 3 (11.1%) F9, 4 (14.8%) F12, and 4 (14.8%) F13. Six groups with novel RFLP patterns were detected, also. The subtypes of P-fimbriae was highly similar to each other by ERIC-PCR.

2.Evaluation of Dermal Measurement for Workers in Benzidine Dihydrochloride and Benzidine Based Dye Manufacturing Factory.

This study is performed to evaluate usefulness of dermal measurement of benzidine and benzidine based dye as one of the occupational exposure assessment method for these compounds. We selected one benzidine manufacturing factory and one dye manufacturing factory in Incheon area. Eleven workers were for benzidine manufacturing factory and twenty four for dye. We analyzed relationships among air level, amount on skin and concentration of urinary metabolites for these compounds. Airborne levels of benzidine and dye were measured by NIOSH 5509, 5013 methods. Amount of these compounds on skin was measured with skin wipe method. Concentration of benzidine metabolites in urine was measured by High Performance Liquid Chromatography after alkaline hydrolysis. The amount of benzidine on hand skin was 25.05( - 233.2) ng/ur, and the amount of the neck was 2.01 ( - 11.9) ng/cm2 in the benzidine dihydrochloride manufacturing factory. The amount of benzidine on hand and neck skin has positive correlation with concentration of urinary monoacetyl benzidine (r=0.644, p < 0.05) . The amount of benzidine based dye on hand skin was 55.75( - 457.7) ng/cm2, and the amount of the neck skin was 18( - 284.7) ng/cm in benzidine based dye manufacturing factory The amount of dye on hand and neck skin has positive correlation with concentration of urinary benzidine for dye workers (r=0.467, p < 0.05). When assessing the exposure of workers who deal with benzidine, the amount of benzidine on skin should be measured for an accurate exposure assessment.

3.Trends of Study and Classification of Reference on Occupational Health Management in Korea after Liberation.

The purposes of this study are to define the scope of occupational health management and to classify occupational management by review of related journals from 1945 to 1994 in Korea. The steps of this study were as follows: (l)Search of secondary reference; (2) Collection and review of primary reference; (3) Survey; and (4) Analysis and discussion. The results were as follows; 1. Most of the respondents majored in occupational health(71.6%), and were working in university (68.3%), males and over the age 40. Seventy percent of the respondents agreed with the idea that classification of occupational health management is necessary, and 10% disagreed. 2. After integration of the idea of respondents, we reclassified the scope of occupational health management. It was defined 3parts, that is, occupational health system, occupational health service and others (such as assessment, epidemiology, cost-effectiveness analysis and so on). 3. The number of journals on occupational health management was 510. It was slightly increased from 1986 and abruptly increased after 1991. The kinds of journals related to occupational health management were The Korean Journal of occupational Medicine(18.2%), several kinds of Medical College Journal(17.0%), The Korean Journal occupational Health(15.l%), The Korean Journal of preventive Medicine(15.l%) and others(34.6%). As for the contents, the number of journals on occupational health management systems was 33(6.5%) and occupational health services 477(93.5%). Of the journals on occupational health management systems, the number of journals on the occupational health resource system was 15(45.5%), occupational finance system 8(24.2%), occupational health management system 6(l8.2%), occupational organization 3(9.1%) and occupational health delivery system l(3.0%). of the journals on occupational health services, the number of journals on disease management was 269(57.2%), health management 116(24.7%), working environmental management 85(18.1%). As for the subjects, the number of journals on general workers was l85(71.l%), followed by women worker, white color workers and so on. 4. Respondents made occupational health service(such as health management, working environmental management and health education) the first priority of occupational health management. Tied for the second are quality analysis(such as education, training and job contents of occupational health manager) and occupational health systems(such as the recommendation of systems of occupational and general disease and occupational health organization). 5. Thirty seven respondents suggested 48 ideas about the future research of occupational health management. The results were as follows: (l) study of occupational health service 40.5%; (2) Study of organization system 27.1%; (3) Study of occupational health system (e.g. information network) 8.3%; (4) Study of working condition 6.2%; and (5) Study of occupational health service analysis 4.2%.

4.Newly Formed Hepatic Masses in Children with Biliary Atresia after Kasai Hepatic Portoenterostomy.

BACKGROUND: This report describes the clinicopathologic findings of six hepatic masses that developed after Kasai hepatic portoenterostomy (HPE) in six patients with longstanding biliary atresia (BA). METHODS: Hepatic masses were found in six of 55 pediatric patients who underwent liver transplantation for BA after Kasai HPE from 1997 to 2009. Clinicopathologic analysis was performed and immunohistochemical staining was carried out for CD34, smooth muscle actin (SMA) and cytokeratin 7. RESULTS: Of the six hepatic masses, two were diagnosed as focal nodular hyperplasia (FNH)-like lesions, two were large regenerative nodules (LRN), one was a mesenchymal hamartoma (MH) and one was a cholangiocarcinoma. The immunohistochemical staining findings for SMA and CD34 were more prominent for the FNH-like nodules than for the cirrhotic background liver. Dysplastic biliary epithelium arising from intestinal metaplasia was found in the cholangiocarcinoma. CONCLUSIONS: Our findings suggest that FNH-like lesions, LRNs and MH are the results of vascular hemodynamic changes after Kasai HPE and that cholangiocarcinoma is due to recurrent cholangitis after BA. All the lesions in this series must be included in the differential diagnosis of a newly formed hepatic mass in patients after portoenterostomy.

5.Pathology of Epstein-Barr Virus-Associated Gastric Carcinoma and Its Relationship to Prognosis.

Among Epstein-Barr virus (EBV)-associated neoplasms, EBV-associated gastric carcinoma (EBVaGC) is the most common tumor worldwide. In contrast to the predominant site of occurrence of EBV-negative gastric carcinoma in the antrum, EBVaGC occurs most frequently in the proximal stomach, including the cardia, fundus and body. Microscopically, EBVaGC can be subclassified into three histological subtypes according to the host cellular immune responses: lymphoepithelioma-like carcinoma, carcinoma with Crohn's disease-like lymphoid reaction, and conventional-type adenocarcinoma. Recent studies have shown that patients with the lymphoepithelioma-like carcinoma subtype of EBVaGC have the best overall and disease-free survival, followed by Crohn's disease-like reactions, which in turn have better survival than patients with conventional-type adenocarcinoma. Histologic subclassifications of EBVaGCs are based on the differing degree and pattern of infl ammatory response and the extent of desmoplasia. Because these subclassifications appear to be a powerful prognostic parameter, further research into the underlying mechanisms of the cellular immune reaction in these pathologic subtypes of EBVaGCs may play a key role in understanding the innate immune response of patients with this highly aggressive carcinoma.

8.Micronodular Thymoma with Lymphoid Stroma: A Case Report.

Micronodular thymoma with lymphoid stroma (MNT) is an extremely rare tumor and has not been reported in Korea. Herein, we report a case of MNT diagnosed in a 75 year-old male. The mass was incidentally identified on thoracic computer tomography (CT) during work-up for a cardiac mass. The resected thymic mass was ovoid and solid measuring 3.5x3 cm in size and showed homogenously white tan, solid and firm cut surface. Microscopically, the tumor had a thin fibrous capsule except small foci of invasion to thymic adipose tissue, and consisted of characteristic multiple discrete epithelial nodules in abundant lymphoid stroma. The epithelial nests anastomosed each other, forming a vague cord-like structure. The cells in the epithelial nests were bland, spindle to oval shape with relatively abundant cytoplasm, slightly vesicular chromatin and one definite nucleolus. The lymphoid stroma contained prominent germinal centers. No evidence of recurrence or metastasis has been found within 4 years of surgery.

9.A Case of Bilateral Endogenous Enterococcus Faecalis Endophthalmitis in Liver Abscess.

PURPOSE: To report a case of bilateral endogenous Enterococcus faecalis endophthalmitis in liver abscess. METHODS: The patient was transferred for bilateral endophthalmitis because she had decreased bilateral visual acuity for three days. We performed bilateral pars plana vitrectomy, lensectomy, and intravitreal antibiotic injection. We determined the presence of Enterococcus faecalis in vitreous culture and liver abscess using abdomen computerized tomography. RESULTS: Thirty-four days after the operation, she was discharged with both eyes with flat retina and filled with silicone oil. Her vision was poor. CONCLUSIONS: Bilateral endogenous Enterococcus faecalis endophthalmitis has rapid progression and a poor visual prognosis.
