1.A photoelastic study on the initial stress distribution of the upper anterior teeth when intrusive force applied (by utility archwire. burstone intrusion archwire. and "J" hook headgear).

The purpose of this study was to analize the initial stress distribution around apex and the alveolar bone of the upper anterior teeth when applying intrusive force by the use of utility archwire, Burstone 3-piece intrusion archwire, and "J" hook headgear which is usually used in clinical practice. By the use of the polarization plate, initial stresses were analized when 80g and 150g force applied. The results were as follows. 1. With the utility archwire, moderate levels of stress were evenly distributed on the apical areas of the anterior teeth and concentrated on the apical areas of the first molars. 2. With the Burstone's 3-piece intrusion archwire, moderate levels of stress were evenly distributed on the apical areas of the anterior and posterior teeth. 3. With the "J" hook headgear, severe levels of stress were widely distributed on, the alveolar- bone and apical areas of the upper anterior teeth, and concentrated on the apical area between the central and the lateral incisors. Especially. weak levels of stress appeared along the periodontal ligament space of all teeth.

3.Clinical features of olivary hypertrophic degeneration in brain magnetic resonance image.

Olivary hypertrophic degeneration (OHD) is caused by lesions in dentato-rubro-olivary pathway(myoclonic triangle) and responsible for palatal myoclonus and other involuntary movements. We report the clinical features and magnetic resonance imaging(MRI) findings of 5 patients with OHD. All patients had previous brainstem hemorrhage mainly involving pontine tegmentum in four patients and right superior cerebellar peduncle in one patient T2-weighted MR] revealed round or oval shaped high signal area in the ventral or ventrolateral portion of the medulla. Their clinical presentations were as followings: palatal myoclonus (4 case), ocular myoclonus (3 case), pharyngeal and laryngeal myoclonus (2 case), limb myoclonus (2 case), head shaking (I case) and perioral tremulous movement (1 case). The frequency of myoclonus was 70-170/minute and the median time interval between the insult of the primary lesion and the onset of myoclonic symptoms was 2 months. OHD shown as hyperintense lesions on T2 weighted MRI should be differentiated from ischemic, neoplastic or other pathologic lesions. The characteristic clinical presentations and the location of primary lesions involving myoclonic triangle may be helpful in differential diagnosis from primary medullary lesions.

Treatment of pelvic malignancy persistent or recurrent after surgery, radiation therapy or chemot-herapy presents a difficult therayeutie challenge. Those patients with central recurrence can be considered as candidate for radical resection. However we must consider many factors for the choice of treatment modality for advanced or recurrent pelvic rnalignancy as follows; the size and location of the recurrent cancer, dose of radiation therapy given, general conditions of the patient and the experience of the surgeon. Pelvie exenteration, when appropriate, is suecessful treatment for recurrent or persistent pelvic malignancy; 5-year survivai has been reported to be as high as 50%. We experienced seven cases of pelvic exepteration for pelvic malignancy between Mar. 1990 and Feb. 1993, Accordingly we present the cases with a brief review of the literatures. 8.The Effect of Rocuronium during Concomitant Propofol and Rocuronium Intravenous Injection.

BACKGROUND: The induction and maintenance of intravenous anesthesia by propofol-TCI has been popular. When infusing propofol with other drugs at the same intravenous access line, incompatibility may result. We attempted to evaluate the incompatibilities between propofol and rocuronium by subtherapeutic dosing of rocuronium. METHODS: Eighty patients were randomized into two groups. Induction of anesthesia was done by propofol-TCI. The patients in group 1 received rocuronium 0.6 mg/kg at the same intravenous access line with propofol infusion and in group 2 received rocuronium at the different intravenous access line with propofol infusion. One Hz single twitch responses were measured by acceleromyography after rocuronium administration. Intubation was done when the twitch height depressed more than 95% of the baseline. The intubating conditions were assessed and graded as excellent, good, poor and bad. Onset time, recovery index and duration were compared between the two groups. RESULTS: The onset was significantly delayed in group 1 (124.9 +/- 35.5 vs. 101.8 +/- 22.1), but there were no significant differences in the intubating condition, recovery index, duration and the incidence of withdrawal response to rocuronium injection. CONCLUSIONS: Onset time was delayed when administrating rocuronium at the same intravenous access line with propofol-TCI. So giving rocuronium at the different intravenous access line with propofol infusion would be helpful to achieve adequate intubating condition rapidly.

10.c-Fos Immunoreactivity in the Neurons of the Lateral Geniculate Nucleus in Albino Rats by Light Exposure after Dark Rearing.

PURPOSE: To investigate the effect of dark rearing immediately after birth on the maturation of the visual relay neurons in the lateral geniculate nucleus. METHODS: Fifty neonatal rats were used. Neonates of the control groups were raised under a normal light/dark cycle. Neonates of the experiment groups were dark reared and isolated from light during the entire experimental period, then exposed to the sun light for 1 hour before sacrifice. RESULTS: In the control groups, the neurons in the dorsal lateral geniculate nucleus developed normally at each age tested. In the experiment groups, the cytoplasm of the large neurons in the dorsal lateral geniculate nucleus of 2-week-old rats contained small vesicles, and the cytoplasm of the large neurons of 4-week-old rats was converted into a vacuole-like space. Moreover, c-Fos immunoreactivity of the large neurons in the dorsal lateral geniculate nucleus in the experiment groups was significantly increased compared to that of the control groups. CONCLUSIONS: We suppose that the maturation of the neurons in the lateral geniculate nucleus might be influenced by light stimulation during the critical period. Furthermore, c-Fos could be a marker of the functional activity of the visual relay neurons of the lateral geniculate nucleus in albino rats.
