2.Causes of necrotic features in fine-needle aspirates from cervical lymph nodes

Background:
Lymph node fine-needle aspiration (LN FNA) cytology indicates necrosis in various diseases. Dominant necrotic features make the diagnosis of underlying conditions very difficult.
Methods:
We retrospectively reviewed 460 patients who underwent cervical LN aspiration cytology that revealed necrotic findings at Keimyung University Dongsan Hospital in Daegu, Korea, from 2003–2017. Each specimen was evaluated and analyzed in association with the clinical findings, biopsy findings, and/or other ancillary tests, including acid-fast bacilli staining and molecular testing for Mycobacterium tuberculosis.
Results:
When necrotic features were noted upon cervical LN FNA cytology, the most common pathologic LN FNA category was necrosis alone (31.5%). The second most common category was granulomatous inflammation (31.3%), followed by Kikuchi disease (20.0%) and malignant neoplasm (8.7%). In cases where the cervical LN FNA revealed necrosis alone, the most common final diagnosis was tuberculosis. In young patients, Kikuchi disease should be considered as one cervical LN FNA category, while metastatic carcinoma should be suspected in older patients.
Conclusions
Even when necrosis alone is observed in LN FNA cytology, it is important to determine the cause through further evaluation.

5.Deposed Queen Yoon Might Have Suffered From Bipolar Disorder

Deposed Queen Yoon was sentenced to death by her husband, King Seongjong, on account of extreme jealousy and inappropriate behaviors. It remains unclear whether her unusual behavior resulted from a psychiatric disorder. In this study, we addressed the issue of whether deposed Queen Yoon suffered from bipolar disorder. We reviewed Seongjong Sillok (Veritable Records of King Seongjong), Joseon Wangjo Sillok (Veritable Records of the Joseon Dynasty), and the available literature on deposed Queen Yoon. A review of the literature showed that deposed Queen Yoon might have experienced depressive and manic episodes. Her symptoms of episodic course, familial tendency of mood symptoms, and postpartum onset suggest that she might have suffered from bipolar disorder. Her symptoms and clinical course suggest that deposed Queen Yoon might have suffered from bipolar disorder. To the best of our knowledge, this is the first report suggesting that deposed Queen Yoon might have suffered from bipolar disorder.

7.Medical Students' Needs Assessment for Counseling Programs

BACKGROUND: This study attempted to develop a student counseling program to help students solve problems effectively by understanding what they want to be counseled by the medical students, what they want to be counseled according to their needs, and investigating their needs.METHODS: A survey was conducted to 278 medical students in a city using a questionnaire developed by three medical education experts.RESULTS: As a result of analyzing the preference for counseling contents, academic achievement counseling was the highest. A result of analyzing the preference of counseling contents by counselors are as follows; professor : selection of major (19.5%) and career and aptitude (16.0%), senior : learning method (22.9%) and major selection (14.1%), professional counseling agencies : selection of major (12.6%), emotion and personality (11.6%), non-anonymous online bulletin board : selection of major (19.5%) and career and aptitude (16.0%), anonymous online bulletin board : interpersonal relationships (15.9%) and selection of major (13.9%)CONCLUSIONS: Based on the analysis of medical student's preference of counseling contents and preference of counseling contents according to the counselor, The results of this study suggest that the counseling system can be used as a basic data for development.

8.Epidemiology and Clinical Characteristics of Enterovirus Infections in Children: A Single Center Analysis from 2006 to 2010.

PURPOSE: This study was performed to investigate the epidemiology of enterovirus (EV) infections in children at a secondary hospital during recent 5 years. METHODS: We collected the cerebrospinal fluid, stool and throat swab samples from the pediatric patients with suspected EV infections in KEPCO Medical Center, Seoul, Korea from July 2006 to September 2010. EV detection and genotype identification were performed by RT-PCR at Korea Centers for Disease Control and Prevention. RESULTS: A total of 386 samples were collected from 277 patients during study period. Ninety-eight patients (35.4%) were diagnosed with EV infections. The RT-PCR positive rate was the highest in throat swab samples (48.3%). The median age of patient was 4.7 years (range, 0.1-12.5 years). Aseptic meningitis (50, 51.0%) was the most common clinical manifestation; herpangina (22, 22.4%) and hand-foot-mouth disease (18, 18.4%). One hundred EVs were isolated from 98 patients and 20 genotypes of EV were identified; Echovirus 30 (28 cases, 28%), Enterovirus 71 (12 cases, 12%), Echovirus 25 (10 cases, 10%), Echovirus 9 (9 cases, 9%) and Coxsackievirus A6 (8 cases, 8%). Aseptic meningitis caused by Echovirus 30 was the most common manifestation in 2008. There was no complicated case caused by Enterovirus 71. CONCLUSION: This study showed the epidemiology of confirmed EV infection in children from 2006 to 2010. There is a need for continuous surveillance of EV infections and its clinical manifestations.

9.Prevalence and Factors related to Irritable Bowel Syndrome in University Students.

PURPOSE: The purpose of this study was to identify the prevalence of irritable bowel syndrome and factors related to irritable bowel syndrome (IBS) in university students. METHODS: A cross-sectional, descriptive study was conducted using a self-report structured questionnaire with 420 university students. Data collection was done during May and June, 2017. The data were analyzed using descriptive statistics, a t-test, χ² test, and binary logistic regression analysis with IBM SPSS/WIN 22.0 program. RESULTS: The prevalence for IBS was 24% among Korean university students. Family history of IBS (OR=2.98, 95% CI=1.32~6.70), history of stomach and intestinal disease (OR=2.24, 95% CI=1.36~3.77), sleep quality (OR=2.01, 95% CI=1.16~3.48), mild anxiety (OR=2.75, 95% CI= 1.19~6.33) and more than moderate anxiety (OR=2.02, 95% CI=1.02~4.00) were found to be factors related to IBS. CONCLUSION: The prevalence of IBS was high among university students, and sleep quality and anxiety significantly influenced prevalence of IBS in these students. Therefore, interventions to decrease symptom of IBS must focus on management of sleep quality and anxiety.

10.Psychological Autopsy of Suicide Victims between Interview Methods through Their Family Members versus Review of Their Police Investigation Records

Objectives:
This study compared the psychological autopsies of suicide victims through interviews of the bereaved family members and investigations of the police death records.
Methods:
A psychological autopsy was performed using both the Korea Psychological Autopsy Checklist (K-PAC) through an interview of the bereaved family members and the Korea Psychological Autopsy Checklist for Police Record (K-PAC-PR) from the police death records at the same suicide victims. The frequency and percentage of each analysis item were checked, and the information collected was compared.
Results:
Of 129 victims, information from two methods showed no significant differences in marital status, employment status, cohabitation status and relationship, location of suicide, method of suicide, and main cause. Among the stress information at the time of death, interpersonal and mental health problems were consistent, but the occupational, economy, family-related, physical health problems were estimated to have greater impact according to the interview methods. The estimates of depression, schizophrenia, bipolar disorder, and dementia were consistent, but the investigation method estimated more sleep disorders and anxiety disorders, and the interview methods estimated more drug use disorders.
Conclusion
Based on the analysis results, the two methods of a psychological autopsy should be properly utilized, and effective suicide prevention using the psychological autopsy information was discussed.
