1.A Case of Epidural Emphysema Caused by Vigorous Coughing.

A 13-year-old boy was admitted via emergency room with dyspnea after vigorous coughing. Subcutaneous emphysema was palpated on both eyelids and around the neck. Breathing sound was slightly wheezy. On initial chest roentgenogram no parenchymal lesion or pneumothorax except subcutaneous emphysema on the thoracic wall was found. On the second day of hospitalization he showed agitation much more severe than the roentgenographic findings on the chest. Diffuse severe subcutaneous emphysema, pneumomediastinum, pneumopericardium, retroperitoneal pneumoperitoneum and epidural emphysema were found on computed tomography. He did not showed neurologic deterioration or respiratory difficulty demanding mechanical ventilation. He was treated with oxygen, bronchodilator and corticosteroids. After four days of hospitalization, tension pneumothorax finally developed which was successfully treated by closed thoracic tube drainage with water sealed bottle. The patient recovered without any sequelae after conservative care. On the follow up computed tomography no underlying pathologic lesion or residual emphysema was found.
2.The Correlation between the Scores of Written Examination, the Clinical Clerkship Examination, the Clinical Skill Assessment, and the Graduation Examination of the Medical Students.

PURPOSE: The aim of this study was to investigate correlations between medical student scores on 4 examinations: the written examination, clinical clerkship examination, clinical skill assessment, and graduation examination. METHODS: Scores for 51 students who entered Daegu Catholic Medical School in 2005 on the written examination, clinical clerkship examination, clinical skill assessment, and graduation examination were included. Correlations between the scores were analyzed statistically. RESULTS: The scores on the written examination showed a strong correlation with those of the clinical clerkship assessment (0.833) and graduation examination (0.821). The clinical clerkship assessment scores correlated significantly with graduation examination scores (0.907). In addition, clinical skill assessment scores correlated with the written examination (0.579), clinical clerkship examination (0.570), and graduation examination (0.465) scores. CONCLUSION: Overall, the correlation between the scores on the clinical clerkship examination and the written examination was more significant than the correlation between scores on the clinical clerkship examination and clinical skill assessment. Therefore, we need to improve the evaluation method for the clinical clerkship examination and clinical skill assessment.

3.THE TENSLE BOND STRENGTH AND ELASTIC MODULUS OF THE SOFT DENTURE LINING MATERIALS.

This study was to compare the tensile bond strength and flexibility of four different soft liners(Coe Soft, Soft Relining, Soft Liner, Dura Base Soft) before & after thermocycling. Each soft liner was bonded to denture base resin block, and measured the tensile bond strength and modulus of elasticity using Universal testing machine. The mean value of tensile bond strength and modulus of elasticity for each experimental groups were statistically processed by SPSS(Statistical Package of Social Sience). The obtained results were as follows : 1. Dura Base Soft had the highest tensile bond strength and Coe Soft had the lowest tensile bond strength. 2. Coe Soft had the lowest modulus of elasticity, and Dura Base Soft had the highest modulus of elasticity of all the soft liners. 3. Thermocycling \ no effects on the tensile bond strength and modulus of elasticity of all the soft liners. 4. The failure modes of Coe Soft, Soft Relining, Soft Liner were mainly cohesive failure and that of Dura Base Soft were mainly adhesive failure.

5.Re-evaluation of the Orbital Dimensions in Modern Korean Adult Skulls.

This article describes the normal range of orbital linear dimensions (eq. orbital breadths, orbital height, orbital depth, interorbital breadths and biorbital breadth) and orbital indicies. One hundred and eighty-seven Korean skulls from grown-up individuals are studied with analysis for the purpose of evaluating anthropometric characteristics of Korean orbits. The results are compared to previously published data for modern Korean skulls. The width of the orbit is larger on the right side than the left and sexual difference was also shown. On the other hand, the heights of both orbits are much the same. Consequently, there is a slight tendency that chamaeconch is met with more frequently on the right side, whilst hypsiconch on the left side. The orbital depth is larger on the right side, however, depth index did not show any side difference. There are sexual differences on the biorbital breadth and upper facial breadth but not on the interorbital breadth on the basis of dacryon. These results represent the asymmetry of Korean orbit and that measurement on both sides must be necessary for anthropological comparison.

6.Inhibitory Effect of a Sesquiterpene from Artemisia iwayomogi on Expression of Inducible Nitric Oxide Synthase by Suppression of I-κBα Degradation in LPS-stimulated RAW 264.7 Cells.

A sesquiterpene was purified from Artemisia iwayomogi methanolic extract during the course of searching anti-inflammatory principle from medicinal plants. A sesquiterpene identified as armefolin inhibited the production of nitric oxide (NO) and attenuated inducible nitric oxide synthase (iNOS) protein level in lipopolysaccharide (LPS)-activated RAW 264.7 cells. Armefolin also down-regulated mRNA expressions of iNOS and pro-inflammatory cytokines, interleukin-1β and interleukin-6 in LPS-activated macrophages. Moreover, armefolin suppressed the degradation of inhibitory-κBα (I-κBα) in LPS-activated macrophages. These data suggest that armefolin from A. iwayomogi can suppress the LPS-induced production of NO and the expression of iNOS gene through inhibiting the degradation of I-κBα. Taken together, armefolin from A. iwayomogi might be a candidate as promising anti-inflammatory agent.

7.Smart Device Usage-Related Factors are Correlated with Self-Regulation Ability in Early Childhood.

PURPOSE: To investigate the relationship between the smart devices usage-related factors and self-regulation ability development in early childhood. METHODS: Parental questionnaires of 187 children aged 3–6 years were analyzed. The metrics included smart device usage frequency (times/week, scored as uFreq), smart device usage time (hours/day, scored as uTime), parental scale for appropriate smart device usage level (scored as uLevel), the Korean-developmental screening test (K-DST), and the scale for self-regulation ability in young children (scored as SRS, and including four sub-categories: self-appraisal, self-determination, behavior inhibition, and emotionality). The correlations were analyzed by total age group and by each age. RESULTS: In the total age group analysis, uFreq and uTime were negatively correlated with mean SRS (rs =−0.366, −0.330; P < 0.001) and sub-category SRS (rs =−0.186 to −0.370; P < 0.05). Mean uLevel score was positively correlated with mean SRS (rs =0.406; P < 0.001) and most of the mean sub-category SRS (rs =0.174 to 0.362; P < 0.05). In 3-year-old children, the mean SRS was strongly negatively correlated with uFreq (rs =−0.751; P < 0.001), negatively correlated with uTime (rs =−0.518; P < 0.001), and positively correlated with mean uLevel score (rs =0.533; P=0.013). Such correlations seemed to decrease at the age of 4–6 years. CONCLUSION: Self-regulation ability was significantly correlated with smart device-related factors and was the highest in the 3-year-old children. Encouraging appropriate smart device usage will be helpful for self-regulation development of young children.

8.Perfusion Hyperintensities in Patients With Seizures: Evaluation With Arterial Spin Labeling Magnetic Resonance Imaging Within 24 Hours After Onset

Purpose:
We used arterial spin labeling (ASL) perfusion magnetic resonance (MR) imaging to evaluate cerebral perfusion abnormalities in patients with seizures within 24 hours of symptom onset.
Materials and Methods:
A retrospective search of our institutional database identified 27 patients who had undergone ASL perfusion studies for seizures or seizure-like symptoms.The inclusion criteria were as follows: 1) history of seizure, 2) MR examination performed within 24 hour of seizure onset, and 3) localized perfusion abnormality on ASL. We evaluated the presence, location, and extent of perfusion abnormalities on ASL and signal abnormalities on fluid-attenuated inversion recovery (FLAIR), diffusion-weighted image (DWI), and susceptibility-weighted image (SWI), respectively. All pathological MR findings, accompanying focal neurological symptoms, and electroencephalogram (EEG) findings were compared.
Results:
The mean time from symptom onset to MR examination was 5 h 54 min. All patients (n = 27) showed localized increased perfusion on ASL perfusion imaging. On FLAIR imaging, 20 patients (74.1%) showed hyperintensity in the area of perfusion abnormality.In 19 patients (70.4%), DWI showed hyperintensity of the lesion with decreased apparent diffusion coefficient value (ADC). Seven patients (25.9%) showed a focal parenchymal area of pseudo-narrowed cortical veins on SWI, associated with focal hyperperfusion. In 20 patients (74.1%), the extent of perfusion abnormalities on ASL was greater than that of signal abnormalities on FLAIR or DWI. In 14/16 patients (87.5%) with abnormal EEG findings, the area with EEG findings and the location of the hyperperfusion abnormality on ASL corresponded.
Conclusion
In patients with seizures within 24 hours of symptom onset, ASL perfusion imaging revealed localized hyperperfusion, which was more frequent than signal intensity abnormalities on FLAIR or DWI. The locations of hyperperfusion areas correlated with EEG abnormalities. Thus, the ASL sequence may be a useful clinical assessment protocol for evaluating patients with seizures.
9.PAUF promotes adhesiveness of pancreatic cancer cells by modulating focal adhesion kinase.

Pancreatic cancer is a notorious disease with a poor prognosis and low survival rates, which is due to limited advances in understanding of the molecular mechanism and inadequate development of effective treatment options over the decades. In previous studies, we demonstrated that a novel soluble protein named pancreatic adenocarcinoma up-regulated factor (PAUF) acts on tumor and immune cells and plays an important role in metastasis and progression of pancreatic cancer. Here we show that PAUF promotes adhesiveness of pancreatic cancer cells to various extracellular matrix (ECM). Our results further support a positive correlation of activation and expression of focal adhesion kinase (FAK), a key player in tumor cell metastasis and survival, with PAUF expression. PAUF-mediated adhesiveness was significantly attenuated upon blockade of the FAK pathway. Moreover, PAUF appeared to enhance resistance of pancreatic cancer cells to anoikis via modulation of FAK. Our results suggest that PAUF-mediated FAK activation plays an important role in pancreatic cancer progression.

10.A Case of Osteo-nevus of Nanta.

Nevus cell nevus in bone has been called osteo-nevus of Nanta since Nanta reported it in detail in 1911. We report a 46-year-old female patient with osteo-nevus of Nanta on her forehead. Histologic examination revealed nests of nevus cells, with numerous osteocytes and osteoblasts in the bony tissue. Inflammatory cell infiltrate and foreign body granuloma were not found.
