1.The Effect of Hormonal Replacement Therapy on Breast.

PURPOSE: To evaluate mammographic and sonographic breast parenchymal changes and the risk of breast cancer in women on hormonal replacement therapy (HRT). MATERIAL AND METHODS: The study group consisted of 50 patients examined with serial mammograms and/or ultrasonograms during HRT. The control group consisted of 50 patients examined with serial mammogram for a routine health check. Mammographic parenchymal changes in both the study and control groups and so- nographic findings of 27/50 patients in study group were evaluated. RESULTS: Follow-up mammogram of the control group showed no interval change or slight evolution of parenchyma with increasing age, but the study group showed increasing parenchymal densities. Most frequently encountered finding on SOhogram in 11 women treated by estrogen alone, was ductal dilatation (7cases ;64%), whereas in 16 women treated by estrogen and progesteron it was ductal epithelial hyperplasia (8 cases; 50%). Overall, four breast cancers developed;one infiltrating ductal carcinoma and three ductal carcinoma in situ. CONCLUSION: HRT causes the changes of breast parenchyma on mammogram and sonogram of postmenopausal women, and increases the risk of developing breast cancer. Therefore, careful and regular examination should be followed in those on postmenopausal HRT.

2.Comparison of Thiopental Sodium and Propofol as an Induction Agent in Propofol-N2O Anesthesia for Cesarean Sections.

BACKGROUND: We compared thiopental sodium with propofol as induction agents under propofol-N2O anesthesia for cesarean sections. METHODS: We selected 68 pregnant women with a single fetus undergoing an elective cesarean section under general anesthesia and randomly allocated them to the thiopental sodium group (group N) or the propofol group (group P). Without premedication, thiopental sodium 5 mg/kg and succinylcholine 1 mg/kg were injected for induction in group N, and propofol 2 mg/kg and succinylcholine 1 mg/kg in group P. Propofol 10 mg/kg/hr was infused continuously with 50% N2O in both groups. We checked the blood pressure and the heart rate before and after injection. We analysed blood gas of maternal artery, umbilical artery, and umbilical vein at delivery and checked Apgar scores at 1 minute and 5 minutes after delivery. RESULTS: There was no significant difference in blood pressure, heart rate and Apgar scores between groups. Oxygen partial pressure (35.6 +/- 5.8 mmHg) and oxygen saturation (66.2 +/- 12.0%) of the umbilical vein in group P was higher than in group N (32.7 +/- 4.9 mmHg, 58.7 +/- 11.5%). Carbon dioxide partial pressure and pH did not differ between groups. CONCLUSIONS: There was no beneficial effect of thiopental sodium compared with propofol as an induction agent under propofol anesthesia. Propofol is a useful drug for cesarean sections.

3.A Case of Bilateral Coronary Ostial Stenosis in Takayasu's Arteritis.

Takayasu,s arteritis is a systemic disease characterized by occlusion of aorta and its major branches because of a panaortitis with thickening of the adventitia. Coronary artery narrowing is due to extension of inflammations that occur in aorta. Angina pectoris may be the first symptom of the disease if the coronary arteries are the initial site of severe arterial narrowing. We present a case of bilateral coronary ostial stenosis where Takayasu,s arteritis was pathologically proved as an etiology. The patient was taken coronary ostial angioplasty with good result.

4.Chemotherapy-related Cognitive Impairment and Quality of Life in People with Colon Cancer: The Mediating Effect of Psychological Distress.

PURPOSE: The purpose of this study was to test the mediating effect of psychological distress in the relationship between chemotherapy related cognitive impairment (CRCI) and quality of life (QOL) in people with cancer. METHODS: A purposive sample of 130 patients undergoing chemotherapy was recruited for the cross-sectional survey design. Data were collected from November 2014 to June 2015. The instruments were K-MMSE (Korean Mini-Mental State Examination), Everyday Cognition (ECog), Hospital Anxiety Depression Scale (HADS), and Functional Assessment of Cancer Therapy-General (FACT-G). Data were analyzed using descriptive statistics, correlation, and multiple regression using Baron and Kenny steps for mediation. RESULTS: The mean score for objective cognitive function was 27.95 and 69.32 for perceived cognitive decline. Overall quality of life was 91.74. The mean score was 17.52 for psychological distress. The prevalence was 56.2% for anxiety and 63.1% for depression, and 20.0% for CRCI. There were significant correlations among the variables, objective cognitive function and self-reported cognitive decline, psychological distress, and quality of life. Psychological distress was directly affected by CRCI. (R2=29%). QOL was directly affected by CRCI. Psychological distress and CRCI effected QOL (R2=43%). Psychological distress had a partial mediating effect (beta= -.56, p <.001) in the relationship between self-reported cognitive decline and quality of life (Sobel test: Z= -5.08, p <.001). CONCLUSION: Based on the findings of this study, nursing intervention programs focusing on managing cognitive decline, and decreasing psychological distress are highly recommended to improve quality of life in cancer patients.

5.Effect of Diet and Apoliprotein E (Apo E) Polymorphism on the Variation of Serum Lipid Profile in Korean Males.

BACKGROUNG AND OBJECTIVES: Diet is the basic and principal therapeutic modality for hyperlipidemia. However, diet therapy alone showed variable responses in lowering lipid levels in different studies. This research is to prove the effect of diet and Apo E polymorphism on the variation of serum lipid profile in Korean males. MATERIALS AND METHODS: To evaluate the gene-diet interation, serum total cholesterol (Chol), triglycerides (TG), HDL-cholesterol and LDL-cholesterol (HDLc, LDLc), lipoprotein (a) (Lp (a)), and fasting blood glucose (FBS) were measured with Apo E genotyping in vegeterians (group A, n=154) and in healthy Korean male adults (group B, n=150) of similar mean age (50.1 vs. 49.3). RESULTS: Lipid profiles showed significantly lower levels in group A compared to group B (Chol 168.3+/-30.5 mg/dL vs. 181.3+/-33.4 mg/dL, p<0.001;TG 131.0+/-62.9 mg/dL vs. 149.4+/-76.7 mg/dL, p=0.023;HDLc 56.0+/-11.0 mg/dL vs. 56.9+/-11.5 mg/dL, p=0.509;LDLc 92.5+/-28.1 mg/dL vs 100.6+/-29.9 mg/dL, p=0.016;Lp (a) 22.1+/-14.6 mg/dL vs. 26.9+/-13.8 mg/dL, p=0.004;FBS 85.1+/-14.1 mg/dL vs. 102.7+/-16.6 mg/dL, p<0.001). The Apo E genotyping showed Epsilon3/3, 64.1%; Epsilon3/4, 20.7%;Epsilon2/3, 11.8%;Epsilon2/2, 1.3%;Epsilon4/4, 0.6% in the combined groups. The distribution was similar in both groups. Chol and LDLc were significantly (p<0.05) higher in Epsilon3/4 allele group compared to other allele groups among non-vegetarians. On the other hand, Chol and LDLc were significantly (p<0.01) lower in vegetarians compared to non-vegetarians only in Epsilon3/4 allele group. CONCLUSION: Vegetarian diet significantly lowered Chol, TG, LDLc, Lp (a) and FBS levels. Significant lipid lowering effect of vegetarian diet was noted in Apo E allele Epsilon3/4 group which had significantly higher Chol and LDLc levels without diet intervention. These data suggest that the influence of diet on serum lipid profiles differ according to apo E genotypes.

6.Factors Influencing Unmarried Mothers' Parenting Stress: Based on Depression, Social Support, and Health Perception.

PURPOSE: To identify factors affecting parenting stress of unmarried mothers. METHODS: The subjects were 108 unmarried women who were parenting their children under the age of six. The data was collected from December 15, 2016 to March 5, 2017. The instruments consisted of self-reported questionnaires that included93 items: 21 on depression, 16 on social support, 20 on health perception, and 36 on parenting stress. The collected data were analyzed by t-test, ANOVA, Pearson correlations, and hierarchical multiple regression. RESULTS: The factors influencing parenting stress were health perception (β=−.55, p < .001), evaluative support (β=−.42, p=.005), informative support (β=−.35, p=.040), emotional support (β=−.24, p=.045), partner support (β=−.20, p=.048), and depression (β=.14, p=.033), and the explanatory power was 57%. CONCLUSION: In order to alleviate the parenting stress of unmarried mothers, it is necessary to develop programs considering the physical, psychological, and social factors of unmarried mothers. Their subjective assessment of their health can increase or decrease their parenting stress, so it is necessary to develop parenting-stress intervention focusing on health perception.

7.Changes in Chemotherapy-induced Peripheral Neuropathy, Sleep Quality, and Quality of Life following Chemotherapy in Stomach Cancer Patients: a Prospective Study

Purpose:
The purpose of this study was to identify the changes of chemotherapy induced peripheral neuropathy (CIPN), sleep quality, and quality of life and their interrelationships following chemotherapy.
Methods:
A sample of 52 patients who had been diagnosed with stomach cancer receiving oxaliplatin containing chemotherapy were included in a prospective longitudinal study. The assessment tools were Chemotherapy-induced Peripheral Neuropathy 20, Pittsburgh Sleep Quality Index, and EORTC Quality of Life- Cancer. The data were collected at three time points: pre-chemotherapy, post-chemotherapy, and three months after the completion of chemotherapy, using questionnaires given between June 2017 and March 2019. Data were analyzed using descriptive statistics, repeated measures analysis of variance, and multiple regression analysis.
Results:
Post-chemotherapy, 19.2% of patients complained of CIPN and 21.2% exhibited CIPN at three-month follow-up. Repeated measures ANOVA showed a significant increase in CIPN after chemotherapy and it remained high at three-month follow-up (F = 39.90, p< .001). Functional quality of life (F = 8.23, p< .001) and symptom quality of life (F = 7.88, p= .001) also showed significant decreases after chemotherapy, and symptom quality of life remained low at three-month follow-up. However, for sleep quality, no significant main effect of time point was shown. CIPN and sleep quality were a factor influencing quality of life with an explanatory power of 48.3% at post-chemotherapy and 65.3% at three-month follow up.
Conclusion
These results suggest that chemotherapy is highly associated with CIPN and symptoms affecting quality of life in cancer patients. Nursing intervention is needed to monitor and relieve these symptoms of patients during the cancer care trajectory.
8.Pathologic Analysis of Endomyocardial Biopsies in Heart Transplantation.

Endomyocardial biopsy (EMB) is a valuable diagnostic procedure for the surveillance of cardiac allograft rejection. Interpretation of individual cases is still problematic due to variations of findings for grading of rejection and other associated lesions. We reevaluated an experience on endomyocardial biopsies to develop better diagnostic criteria for rejection and other complications. Immunohistochemical studies against cytokines were performed to assess the usefulness of the method for the diagnosis or researches. A total of 249 EMBs taken from 33 cardiac allograft recipients were reviewed. There were 25 males and 8 females. Dilated cardiomyopathy was present (24 cases) and valvular heart disease (4 cases), restrictive cardiomyopathy (3 cases) were also common conditions. We applied the grading system of the International Society for Heart Transplantation (ISHT) for the assessment of acute cellular rejection. Grades of 0, 1A, 1B, 2, 3A and 3B were 39.0%, 28.1%, 11.2%, 11.5%, 12.4% and 1.6% respectively, but 3.2% were inadequate. Thirty five episodes of grade 3A or 3B were present in 17 patients. The response to therapy was assessed using a next follow up biopsy, which revealed resolving or resolved rejection in 85% of patients. The intensity of immunohistochemical stains for IL-6 and TNF-alpha was increased in proportion to the histologic grade but Quilty lesion and cardiomyopathy also showed a positive reaction. The other pathologic findings were ischemic change, previous biopsy site, interstitial edema and fibrosis, and Quilty lesion. These findings showed usefulness of endomyocardial biopsy not only for the evaluation of cardiac allograft rejection but also for the diagnosis of associated cardiac lesions. Immunohistochemical study of the cytokines was related to the degree of inflammation rather than degree of rejection.

9.A Case of Henoch-Schonlein Purpura with Hemorrhagic Bullae in a Child.

Henoch-Schonlein purpura(HSP) is a systemic small vessel vasculitis characterized by cutaneous purpura, arthritis, abdominal pain and nephritis. The characteristic rash of HSP consists of palpable purpura on the buttocks and lower extremities. Bullous lesions often appear in adults with HSP, whereas they are very rare in children with HSP. In this case report, the patient presented with arthralgia and abdominal pain and had hemorrhagic bullae as a prominent manifestation of the disease. The skin biopsy of the patient revealed typical leukocytoclastic vasculitis of dermal vessels and prominent IgA and fibrinogen deposits on capillary walls by direct immunofluorescence. We confirmed the diagnosis of HSP and observed improvement of clinical symptoms and signs within a few days after corticosteroid treatment. We therefore report a case with a review of the literature.
