2.Improvement of HACCP Verification Checklist in School Foodservices: A Case Study on Cooked Squid with Seasoned Fresh Vegetable.

The purpose of this study was the improvement and modification of the HACCP verification checklist in school foodservices. For this, the HACCP verification checklist was modified on the basis of an existing school foodservice format. The modified checklist was composed of 28 items, including CCPs (critical control points), microbial test, and other components of the HACCP system than CCPs. To confirm the suitability of the modified checklist, comparisons were made based on the microbiological quality of cooked foods, utensils, and number of aerial microbes in the working area. In this study, the applicability of the modified checklist was determined by focusing on cooked squid with seasoned fresh vegetables (Ojingeochaesomoochim). The following results were obtained from 14 schools in Changwon. The checklist scores for maintaining hot foods over 60degrees C or serving within 2 hours, microbial tests of drinking water, food contact surfaces and cooking utensils, monitoring tools, and usage of suitable sanitizers were 2 points each (The possible highest score is 2 points). On the contrary, the checklist score for microbial test of cooked foods was the lowest of all the items. The correlation coefficient (r) between the improved checklist and microbiological quality of cooked foods was 0.699 (P<0.01), whereas that between the improved checklist and microbiological quality of cooking utensils was 0.612 (P<0.05). The correlation coefficient between the improved checklist and aerial plate count in the working area was -0.556 (P<0.05). Our results indicate the potential possibility of using the HACCP verification checklist in school foodservices.

3.The clinical significance of serum CA125 and CA19-9 levels in endometriosis.

OBJECTIVE: Because endometriosis is difficult to diagnose and has a high recurrence rate after treatment, a reliable serum marker of endometriosis is necessary. Therefore, the aim of this study is to measure the serum levels of CA125 and CA19-9 in patients with endometriosis before and after treatment and during recurrence, and to assess the usefulness of these levels in the diagnosis, clinical follow up and prediction of recurrence in endometriosis. METHODS: Eighty-eight patients who visited the department of Obstetrics and Gynecology of Ewha Mokdong Hospital from January 1994 to December 1998 and were diagnosed as endometriosis by laparoscopy or explo-laparotomy were enrolled as subjects. A retrospective analysis of serum CA125 and CA19-9 levels at 1 month before and 3 to 6 months after initiation of treatment was done. RESULTS: The serum CA125 and CA19-9 levels of endometriosis group(81.0+/-252.5, 36.6+/-53.4 ; mean+/-2SD, U/ml) before treatment was significantly higher than control group(11.6+/-12.8, 9.4+/-8.6)(p<0.05). Overall sensitivity rate for CA125, CA19-9 levels and both was 53.4%, 42.9% and 64.3% respectively. The sensitivity rate for endometriosis, stage 3 and 4(85.4%, 55.0%) was significantly higher than that, stage 1 and 2(p<0.05). The serum CA125 level in endometriosis group showed a significant increment according to stages(p<0.05) while the serum CA19-9 level showed an increasing trend(p=0.055) and both levels decreased significantly after treatment(p<0.05). The serum CA125 level was also higher at recurrence after treatment(p<0.05). CONCLUSIONS: The serum CA125 and CA19-9 levels are a useful marker for diagnosing severity of disease, monitoring efficacy of treatment and predicting recurrence in endometriosis.

7.Clinical review of abnormal cervical cytology in pregnancy.

PURPOSE: This study was performed to evaluate the clinical significance of abnormal cervical cytology in pregnancy and the effectiveness of cytologic and colposcopic surveillance of these women. METHODS: A retrospective analysis was conducted in pregnant women referred to the colposcopic clinic with abnormal cervical cytologic results above ASCUS between Jan. 2002 and Dec. 2007. The Initial colposcopy was performed at the end of 1st trimester. Cervical cytology and colposcopy were repeated once or twice during remaining period of pregnancy and at postpartum 6~8 weeks. Colposcopy directed cervical biopsies were taken in case of colposcopic abnormalities. Without evidence of invasion, no invasive procedure was attempted during pregnancy. Above CIN2 patients were treated postnatally. The postpartum diagnoses were compared to the worst antepartum impressions and initial cytologies. RESULTS: One hundred three pregnant women were referred to colposcopic clinic for 5 years. There was no invasive cervical cancer antenatally. After delivery, 53 patients were followed up and 17 patients above CIN2 lesions treated. Two microinvasive squamous cell carcinoma and 1 invasive adenocarcinoma were found postnatally. In 33 initial ASCUS cytologies, there were 7 CIN2,3 lesions and 3 invasive cancers. One CIN3 was found in 13 LSIL cytologies. All 7 HSIL patients had CIN3 lesions. There were 2 microinvasive cancers postnatally in 17 antenatal CIN2,3 patients. CONCLUSION: ASCUS cervical cytologic results in pregnancy have 30% possibility of CIN2,3 and cancer. LSIL results are normal in 85% and HSIL group 100% CIN3. Even though there is 12.5% risk of microinvasion in antenatal CIN2,3 lesions, noninvasive conservative management is reasonable.

8.Comparing Chewable and Manual Toothbrushes for Reducing Dental Plaque: A Pilot Study.

This study aimed to compare the effectiveness of chewable toothbrush and manual toothbrush and provide basic data for recommendation of the chewable toothbrush in specific groups and situations. A total of 20 subjects participated in this study (rolling method, 10; non-rolling method, 10). After professional prophylaxis, participants used the manual toothbrush to brush their teeth for 3 minutes. After a 7-day wash-out period, participants used the chewable toothbrush according to the manufacturer's instructions. Pre- and post-plaque indexing of the teeth was performed. The dental plaque index was assessed using the Turesky Modification of the Quigley-Hein Plaque Index (TMQHPI) for amount of plaque and Silness-Löe Plaque Index (SLPI) for plaque thickness. The difference between pre- and post-dental plaque index was analyzed using a paired t-test and the Wilcoxon signed-rank test. The Mann-Whitney U test was also used to compare the dental plaque index reduction rates. The dental plaque index differed significantly between the chewable toothbrush and the manual toothbrush. The TMQHPI reduction rate was significantly different between the rolling and non-rolling method groups for the manual toothbrush but not the chewable toothbrush. The difference in SLPI reduction rate between the rolling and non-rolling method groups was significant for the manual toothbrush but not for the chewable toothbrush. Differences in the dental plaque index reduction rates between the chewable and manual toothbrushes were not significant in the non-rolling method group. The results of this study showed higher reduction rates in dental plaque with manual toothbrush use than with chewable toothbrush use. However, the non-rolling method group did not show statistically significant differences according to toothbrush type. The present study showed that a chewable toothbrush can be an alternative to a manual toothbrush for individuals who have difficulty using the generally recommended rolling method.

9.An Individualized Teaching Program for Atherosclerotic Risk Factor Reduction in Patients with Myocardial Infarction.

This study was conducted to evaluate the effect of a teaching program on patients with myocardial infarction. Forty-five patients were randomly selected 22 were assigned to a teaching group and 23 to a control group. An individualized teaching program was delivered to the teaching group during the hospitalization period. It covered aspects such as: the characteristics of heart disease, the anatomy and physiology of the heart, risk factors of atherosclerosis, medication and diet and exercise therapy. When these subjects were discharged to their homes, they received regular supportive care via telephone or mail for 12 weeks. Atherosclerotic risk factors, including, smoking, exercise, blood lipid profile and BMI were measured before and after the teaching program. Post-testing revealed that the numbers of those who exercised and the number of non-smokers were significantly higher in the teaching group than in the control group. Increased HDL cholesterol (High-Density Lipoprotein cholesterol) was significantly greater in the teaching group than in the control group. The above findings suggest that this individualized teaching program might be helpful at reducing the risk factors of atherosclerosis in myocardial infarction patients.

10.Effects of Cardiac Rehabilitation Teaching Program on Knowledge Level and Compliance of Health Behavior for Patients with Myocardial Infarction.

PURPOSE: The purpose of this study was to investigate the effects of cardiac rehabilitation teaching program on knowledge level and compliance of health behavior for the patients with myocardial infarction. METHOD: The subjects were 47 patients 23 were assigned to the experimental group and 24 were for the control. The cardiac rehabilitation teaching program is a individualized teaching program which was delivered to the experimental group during hospitalization period by present researcher. Data were collected through questionnaire surveys for knowledge level and compliance of health behavior from September 15, 1999 to December 31, 2000. The collected data was analyzed by using the SAS program. RESULTS: 1. With regard to the knowledge scores 1) The total knowledge level in the experimental group was significantly higher than in the control group. 2) As to the knowledge domains, nature of disease, risk factors, diet, medication, exercise, and daily activities were significantly higher in score in the experimental group than in the control group. 2. With regard to the compliance of health behavior 1) The average compliance with good health behavior was significantly higher in the experimental group than in the control group. 2) As to the health behavior domains smoking cessation, diet, stress management, regular exercise, and other measures for lifestyle modification were significantly higher in score in the experimental group than in the control group. 3. The pre-treatment knowledge score was positively correlated to the post-treatment knowledge score and post-treatment knowledge score was positively correlated to the post-treatment compliance of health behaviors. CONCLUSION: The above findings indicate that the cardiac rehabilitation teaching program for the experimental group was effective in increasing level of knowledge and improvement f compliance with good health behavior of patients with myocardial infarction.
