3.Predictors of Turnover Intention among Nurses in Small and Medium-sized Hospitals.

PURPOSE: The purpose of this study was to explore the predictors of turnover intention among nurses working in small and medium-sized hospitals in South Korea. METHODS: Data were obtained from a survey conducted in 2016 on welfare policy and system improvement. The participants were nurses of the Korean Nurses Association who worked in small and medium-sized hospitals. The responses of 2,011 nurses were analyzed using hierarchical multiple regression analysis, which was performed to explore the predictors of nurses' turnover intentions. RESULTS: The findings of this study showed that the predictors significantly reducing nurses' desire to leave their hospitals were a 40-hour workweek, satisfaction with the salary, and six out of the 40 fringe benefits provided by their hospitals: the provision of uniforms and work shoes, paid menstrual leave, operational costs for the department, job-related educational costs, and in-house nurse training program. CONCLUSION: These findings suggest that hospitals should provide proper work hours and reasonable rewards for nurses' work in order to reduce turnover intention among their nurses.

4.The Significance of Brainstem Auditory Evoked Potential in High-Risk Neonates.

OBJECTIVE: To observe the prognostic value of brainstem auditory evoked potentials (BAEP) for prediction of cerebral palsy (CP) in the high-risk neonates. METHOD: Eighty-one high-risk neonates were subjected to take the history of illness, neurological examination, developmental assessment, BAEP study within one month after birth. They had been checked for detection and management of the CP in period of 12 to 56 months after birth. Associating factors were observed about their gestational age at birth, 1-minute Apgar score, history of asphyxia and/or intubation, and hyperbilirubinemia and/or exchange transfusion. RESULTS: Abnormal BAEP findings were seen in 38 of 81 (46.9%) high-risk neonates and 6 of 81 (7.4%) were diagnosed as a CP. Five of 6 CPs and 42 of 75 non-CPs had been abnormal in BAEP study. Neonatal BAEP study showed 83.3% sensitivity, 44% specificity, 98.4% false positive and 2.9% false negative in predicting CP. Asphyxia showed high correlation with abnormality of BAEP and CP (p<0.01). Gestational age, low birth weight, toxemia, germinal matrix hemorrhage (GMH) grade II and intubation were correlated with CP (p<0.05) but not with the abnormality of BAEP. CONCLUSION: It is suggested that BAEP study of high-risk neonate is useful in prediction of CP because of low false negative and high sensitivity.

5.Comparison of Thiopental Sodium and Propofol as an Induction Agent in Propofol-N2O Anesthesia for Cesarean Sections.

BACKGROUND: We compared thiopental sodium with propofol as induction agents under propofol-N2O anesthesia for cesarean sections. METHODS: We selected 68 pregnant women with a single fetus undergoing an elective cesarean section under general anesthesia and randomly allocated them to the thiopental sodium group (group N) or the propofol group (group P). Without premedication, thiopental sodium 5 mg/kg and succinylcholine 1 mg/kg were injected for induction in group N, and propofol 2 mg/kg and succinylcholine 1 mg/kg in group P. Propofol 10 mg/kg/hr was infused continuously with 50% N2O in both groups. We checked the blood pressure and the heart rate before and after injection. We analysed blood gas of maternal artery, umbilical artery, and umbilical vein at delivery and checked Apgar scores at 1 minute and 5 minutes after delivery. RESULTS: There was no significant difference in blood pressure, heart rate and Apgar scores between groups. Oxygen partial pressure (35.6 +/- 5.8 mmHg) and oxygen saturation (66.2 +/- 12.0%) of the umbilical vein in group P was higher than in group N (32.7 +/- 4.9 mmHg, 58.7 +/- 11.5%). Carbon dioxide partial pressure and pH did not differ between groups. CONCLUSIONS: There was no beneficial effect of thiopental sodium compared with propofol as an induction agent under propofol anesthesia. Propofol is a useful drug for cesarean sections.

6.Patient Outcomes according to Blood Glucose Level in Neonates with Cardiovascular Surgery.

PURPOSE: The purpose of this study was to provide basic data for suitable neonate blood glucose maintenance by investigating and analyzing the blood glucose level of post cardiovascular surgery neonates in the ICU for the effect of blood glucose levels on the following outcomes; ICU stay, hospital stay, mechanical ventilation time, morbidity, and mortality. METHODS: The participants were 143 neonates in the ICU after having had cardiovascular surgery. The design for this study was an investigation of the blood glucose levels of the neonates and retrospective analysis of patient outcomes according to blood glucose level. RESULTS: The results for the neonate groups showed that the factors of hospital stay, ICU stay, mechanical ventilation time and mortality, for the group with a blood glucose level over 140 mg/dL were longer and higher than for the group with blood glucose of less than 100 mg/dL or the group between 100-139 mg/dL. CONCLUSION: The results of this study indicate that when caring for neonates after cardiovascular surgery, it is important to recognize the influence of blood glucose levels on patient outcomes like hospital days, ICU stay, length of time on mechanical ventilation and mortality. Further, care guidelines for neonates' glucose level management need to be developed.

8.A Mitochondrial Mutation in Leber's Hereditary Optic Neuropathy.

Leber's hereditary optic neuropathy is caused by a single nucleotide change in the mitochondrial deoxynucleic acid(mtDNA). We identified a single guanine to adenine transition mutation in the mitochondrial DNA at nucleotide position 11778(Wallace mutation)in a 13 year old boy. To our knowldge, this is the first report confirming mtDNA mutation in Korea. This would be very helpful for the correct diagnosis of optic neuritis, optic neuropathy and optic atrophy of unknown etiology as well as for genetic counselling in the future.

9.A Case of Bilateral Coronary Ostial Stenosis in Takayasu's Arteritis.

Takayasu,s arteritis is a systemic disease characterized by occlusion of aorta and its major branches because of a panaortitis with thickening of the adventitia. Coronary artery narrowing is due to extension of inflammations that occur in aorta. Angina pectoris may be the first symptom of the disease if the coronary arteries are the initial site of severe arterial narrowing. We present a case of bilateral coronary ostial stenosis where Takayasu,s arteritis was pathologically proved as an etiology. The patient was taken coronary ostial angioplasty with good result.

10.Carbon monoxide poisoning as an epigenetic factor for Leber's hereditary optic neuropathy.

A 45-year-old Korean woman visited our hospital complaining of poor vision after carbon monoxide (CO) poisoning. We have confirmed the presence of a point mutation at position 11778 in the ND4 gene of mitochondrial DNA. This case suggests that CO poisoning may precipitate the clinical expression of Leber's hereditary optic neuropathy (LHON). To our knowledge, this would be the first case report of clinical expression of LHON precipitated by CO poisoning.
