2.Comparison between Immunohistochemical Stains and Serum Hormone Level on Pituitary Adenomas.

The current classification of pituitary adenomas is based on cell type, largely ascertained by immunohistochemistry and electron microscopy. In an application of immunohistochemistry, pathologists have some problems in judging the results. An immunostaining result does not always correspond with a serum hormone level. It is also difficult to determine the nature of a tumor when a few cells are positive. We performed the immunohistochemical stains on 34 pituitary adenomas using polyclonal antibodies to six pituitary hormones [PRL (prolactin), GH (growth hormone), ACTH (adrenocorticotropic hormone), FSH (follicle-stimulating hormone), LH (luteinizing hormone), TSH (thyroid-stimulating hormone)] and compared with serum hormone level. The serum hormone level was increased in 14 cases (41.2%) of PRL, 7 cases (20.6%) of PRL & GH, 6 pleurihormonal cases (17.6%), 4 nonfunctioning cases (11.8%), 2 cases (5.9%) of FSH, and 1 case (2.9%) of GH. The most common immunohistochemical type of pituitary adenoma was 10 prolactinoma cases (38.5%), followed by 7 pleurihormonal cases (26.9%), 4 null cell cases (15.4%), 3 cases of mixed PRL & GH (11.5%), 1 case of ACTH (3.8%) and 1 FSH & LH case (3.8%). The corresponding rates of the serum hormone level and immunostaining results were 94.1% in GH, 88.9% in TSH, 85.7% in LH, 82.4% in ACTH, 66.7% FSH, and 61.8% in PRL. In the immunostaining for FSH, 12 cases showed less than 5% positivity and most of them exhibited the normal serum hormone level. In conclusion, the most common elevated serum hormone and immunohistochemical type of pituitary adenoma was prolactinoma. The corresponding rate of the serum hormone level and immunostaining result was the highest in GH cell adenoma and was the lowest in prolactinoma. The cells showing less than 5% positivity seem to be entrapped normal cells.

3.The Affinity of Calmodulin-Affigel for Inositol Triphosphate Kinase From Bovine Brain.

The one event on signaling mechanism is the cleavage by adenyl cyclase of ATP into second messenger, cyclic AMP. The other transfer system of inositol metabolism, it is widely recognized that hydrolysis of the minor membrane lipid phosphoinositide bisphosphate (PIP₂) initiated by occupation of certain receptors and catalyzed by phospholipase C, lead to toe generation of the two intracellular messengers, inositol triphosphate (IP₃) and diacylglycerol (DG). IP₃ is converted to inositol tetrakisphosphate (IP₄) by IP₃ kinase. In the present study, it is that purification of calmodulin is used by phenyl-Sepharose CL-4B chromatography, it's molecular weigh, 17,000 in SDS-polyacrylamide gel electrophoresis. In order to observe the affinity between calmodulin (CaM)-Affigel 15 and IP₃ kinase, and isolated IP₃ kinase, was applied in CaM-Affigel with Ca²⁺ equilibrium buffer and EGTA equilibrium buffer. We compared with binding and elution effect of IP₃ kinase in several condition of buffer. In affinity of binding, Ca²⁺ equilibrium buffer was in the most proper condition, and elution, CaM/Ca²⁺buffer (CE 1 10.36, CE2 12.76pM/min/mg of protein) was effected much more than EGTA buffer (E2 1.48, E 2.43pM/min/mg of protein), but CaM/Ca²⁺stimulate the activity of IP₃ kinase. And then, several detergents such as sodium deoxycholate, tween 20, cholic acid, polyethylene glycol, chaps were applied. The 0.2% chaps buffer (E2 23.19, E3 8.05pnM/min/mg of protein) was the most effective in elution of IP3 kinase.

4.Life Style Changes of Adults who become Disabled in Adulthood.

PURPOSE: This study aimed at uncovering the disabled adult's way of thinking and behavior due to their changed life style. METHOD: In depth interviews with 11 participants were held and analysed based on the Grounded Theory Methodology by Strauss and Corbin (1990). RESULT: This study showed us that the disabled are getting independent by selfconfidence through developing coping strategies for their physical, emotional, social, sexual problems. And in the process of creating these strategies, support structures, satisfaction for performance effect, financial burdens, and mobility had an effect on the intervening factors. Adults who become disabled go through many changes. First, they have vague hopes of improving. Second, they become discouraged. Third, they accept their disability. Next, they learn coping strategies and finally, they integrate the changes into their new life style. CONCLUSION: This study can be utilized in rehabilitation nursing so that the disabled can adapt to their changed life style quickly and lead independent and self-confident lives.

5.On the Test of the Reliability and Validity of the Disabled's Motivation Scale for Rehabilitation.

PURPOSE: to test of the reliability and validity of the Disabled's Motivation for Rehabilitation Scale. which was developed in 2002. METHOD: An experimental version of the scale was distributed to a sample of 441 disabled with ages above 18 and below 80 years. The subjects of the test-retest were 60 disabled. RESULTS: revealed a satisfactory level of test-retest and internal consistency. The overall fit of the factor model to the data was good. Correlation among the subscales revealed a simple pattern that, in general. provides support for the self- determination continuum and the construct validity. In testing concurrent, criterion-validity, there was a positive correlation between the motivation scores for rehabilitation and the Health-Related Hardiness scores and a negative correlation between the motivation scores for rehabilitation and the Learned Helplessness scores. CONCLUSION: The Disabled's Motivation for Rehabilitation Scale revealed a useful instrument with a high degree of reliability and validity. In this sense, this instrument can be effectively utilized in rehabilitative nursing for the disabled.

6.Angiosarcoma of Jejunum Following Therapeutic Irradiation: A case report.

We report a case of angiosarcoma of the jejunum in a 65-year-old female. This angiosarcoma developed 20 years after postoperative irradiation for cervical carcinoma of the uterus. Grossly, the resected jejunum showed a 8 cm long segmental hemorrhagic lesion with multiple, small hemorrhagic nodules or cysts on its serosa and mesentery. Microscopically, the wall of jejunum was infiltrated by epithelioid or polygonal tumor cells arranged in solid nests or lining irregular vascular spaces. The case is of interest that the tumor occurred in the very unusal site for angiosarcoma and the patient had a past history of irradiation for uterine cervical carcinoma 20 years ago. In addition, the resected intestine showed histologic changes of chronic irradiation effect. Therefore, this case supports the view that there is cause and effect relationship between irradiation adn angiosarcoma.

7.Clinical Application of Polymerase Chain Reaction for Diagnosis of Pulmonary Tuberculosis in Pneumoconiotic Patient.

Recent development in thepolymerase chain reaction (PCR) has brought an extraordinary opportunity for the rapid detection of M. tuberculosis in clinical specimens for the diagnosis of tuberculosis. Pneumoconiosis is a sort of pulmonary fibrosis consequent to inhalation of the respirable dust. The association between pulmonary tuberculosis and pneumoconiosis is well recognized. There is a 10-fold increase in the tuberculosis risk among the workers who have pneumoconiosis demonstrated by chest roentgenogram. The physicians managing the patients with pneumoconiosis have to maintain a high index of suspicion for the development of mycobacterial infection, since the diagnosis of tuberculosis is often difficult. Mycobacterium tuberculosis is a very slow growing organism and acid-fast bacillus (AFB) staining frequently shows false negative results, and therefore PCR would be a very rapid, easy and sensitive diagnostic method for the diagnosis of Mycobacterium tuberculosis in pneumoconiotic patients. To compare the PCR method with the conventional methods in diagnosing Mycobacterium tuberculosis in sputum, we used the sputa of 115 pneumoconiosis patients in Munkyeong Cheil Hospital. Of 32 pulmonary tuberculosis in the pneumoconiosis patients, 29 were PCR positive and were higher than 28, 20 positive by culture and AFB stain. Overall sensitivity, specificity, and which were 90.6, 91.5 % respectively for the PCR assay, 87.5, 100 % for the culture method ; 62.5, 98.7 % for the AFB stain. The PCR assay is a rapid, efficient, sensitive method which can detect M. tuberculosis directly in pneumoconiosis patients, and further study should be followed for the development of the easier method.

9.Sirenomelia: An autopsy case report.

Sirenomelia is a severe form of caudal regression syndrome that results in a fusion of the lower extremities, which is not compatible with life. A various spectrum of anomalies affecting primarily the musculoskeletal, genitourinary and gastrointestinal systems also can occur. This rare malformation has a reported incidence rate of approximately 1 in 60,000 births, with a range of 1 to 1 percent of all malformed infants. We experienced a sirenomelic case with combined anomalies of genitourinary, cardiovascular and gastrointestinal systems. Maternal obstetric history revealed oligohydramnios and intrauterine fetal growth retardation, and the baby was spontaneously delivered at 37 weeks of gestational age, but died I hour after birth.
