2.A Case of Angio: Behcet's Syndrome, Arterial Occlusion Type.

We report herein a. case of angio-Behqets syndrome, arterial occlusion type in a 31 year-old male. He had a history of scrotal ulceration, frequent oral ulceration and ocular pain with decreased visual acuity. About 8 months ago, moderately dull pain developed on the volar aspect of left index finger. Subsequently, a bean sized bluish patch developed on the previous painful area and on the ipsilateral thumb tip. Subungual splinter hemorrhages developed on the distal portions of left thumb and index finger nails at the same time. The result of saline puncture test showed the positive reaction in both intradermal and subeutaneous injections. The hrachial angiography showed an obliteration of digital artery at the second proxirnal phalanx of ulnar side.

3.Infantile Hemangioendothelioma of the Liver: Brief case report.

Infantile hemangioendothelioma of the liver is a common vascular tumor in infancy. The tumor is usually multinodular or diffuse and classified into two types. We present a case of infantile hemangioendothelioma of the liver, which predominantly consists of type 2. A 4-month-old female was admitted for an evaulation of an abdominal distension. A CT scan of the liver showed a multinodular mass. The right lobectomy was done. Grossly, the mass consisted of round nodules ranging from 2cm to 5cm in diameter. Microscopically, the tumor revealed proliferation of small vascular channels lined by endothelial cells. Bizarre cells and mitotic cells were frequently noted. Vesicular nuclei and multilayering of the endothelial cells were also noted.

4.Changes in Urinary Biochemistry in Machinists Exposed to a Hot Environment.

PURPOSES: To demonstrate changes in urinary biochemistry that lead to a supersaturation of some salt, which makes a higher prevalence of urinary stone in workers chronically exposed to a hot environment and massive sweating. MATERIALS AND METHODS: The study was performed at a metal plant and exposure to heat stress was estimated by the Wet Bulb Globe Temperature climatic index. Stone risk profile from our laboratory included volume, specific gravity, creatinine, sodium, potassium, calcium, magnesium, chloride, uric acid, phosphorus and pH. On a randomly selected sample of 19 workers exposed and 19 workers not exposed to heat stress without any evidence of stone disease, and family history of nephrolithiasis or predisposing factors such as gout, we performed a stone risk profile by collecting the urine during the 8-hour work shift for 1 consecutive days. The independent student t-test was used to compare each parameter between the different groups. RESULTS: Among a randomly selected urine sample of 19 workers exposed and 19 workers not exposed to heat stress without any evidence of stone disease, significant differences were found in fluid intake (2816.0+/-1981.0 ml. versus 1200.0+/-4147.0), and concentration of creatinine (1481.0+/-691.0 ml./l. versus 1032.0+/-574.0), sodium (201.0+/-55.0 mg./l. versus 147.4+/-58.0), calcium (202.0+/-76.0 mg./l. versus 132.0+/-71.0), uric acid (475.0+/-138.0 mg./l. versus 359.0+/-196.0), phosphorus (638.0+/-283.0 mg./l. versus 328.0+/-196.0), pH(6.05+/-0.61 versus 6.72+/-0.92), specific gravity(1.029+/-0.005 versus 1.014+/-0.006) respectively. CONCLUSIONS: This study confirms the concept that heat stress represents a real lithogenic risk factors. Therefore, we recommend adequate fluid intake during hot occupations.

8.Chest Wall and Fissural Invasion of Peripheral Lung Cancer: Evaluation with HRCT.

PURPOSE: We studied the accuracy of high resolution computed tomography in staging chest wall/fissural invasion of peripheral lung cancer. MATERIALS AND METHODS: HRCT findings in 27 patients with suspected chest wall (n=18) or fissural (n=16) invasion of peripheral lung cancer were correlated with surgical and pathologic findings. The HRCT images were restrospectively evaluated for pleural thickenings adjacent to mass, maximal contact length (cm) between mass and chest wall/fissure, angle between the mass and chest wall/fissure, ratio of maximal contact to mass diameter, abnormality of extrapleural fat layer, mass extension across the fissure and fissural irregularity adjacent to mass. Various CT findings and the presence or absence of chest pain were correlated with surgical and pathologic findings. RESULTS: For the evaluation of chest wall invasion, abnormality of extraphleural fat layer was the most useful finding (sensitivity 100 %, specificity 36 %, accuracy 61 %). The remaining HRCT findings proved to have high sensitivity but low specificity, and a high false positive rate. For chest pain, sensitivity was 43 % and specificity, 82 %. In cases without chest pain, the positive predictive value of extraphleural fat abnormality was 44 % ; in the absence of chest pain and extrapleural fat abnormality,positive predictive value was zero. The evaluation of transfissural tumor invasion using variable HRCT findings proved to be accurate, especially when the criteria of mass extension across the fissure and fissural irregularity adjacent to the mass were used (accuracy 81 % and 75 %, respectively). CONCLUSION: Using the finding of 'extrapleural fat abnormeality', HRCT was accurate in the staging of chest wall invasion and its predictability was betten than that of other results obtained with conventional CT. Chest pain had high specificity but low prevalence, and extrapleural fat abnormality was more valuable in cases without chest pain. HRCT proved to be accurate in the evaluation of transfissural invasion of lung cancer using the findings 'fissural cross' and 'fissural irregularity'.

9.Polymerase Chain Reaction Analysis of Human Papillomavirus in Esophageal Squamous Cell Carcinoma with its Correlation to p53 mutation.

HPV infection has been implicated strongly in the pathogenesis of human squamous cell carcinoma(SCC). We analysed a series of 28 surgically removed, invasive squamous cell carcinoma of the esophagus by polymerase chain reaction to detect HPV DNA using consensus primers and 8 type-specific primers of HPV (6, 11, 16, 18, 31, 33, 35, 51). HPV 6, 31, 35 or 51 DNA were detected in 20 out of 28 cases (71.4%) of the esophageal SCCs. HPV 51 was the most frequently detected type, occuring in 13 out of 28 cases (46.4%). p53 immunohistochemical staining was also performed to demonstrate any relationship to HPV DNA positivity. It showed positivity in 16 out of 28(57.1%) esophageal SCCs, and HPV DNA and p53 positivity were concurrently detected in 11 out of 28 cases of SCCs. There was no significant inverse relation between HPV DNA positivity and p53 expression(p>0.05). Our results supported HPV involvement in esophageal squamous cell carcinoma, and suggested there may be another pathway not related to the p53-binding pathway in the carcinogenesis of esophageal SCCs by HPV.
