1.Electromechanical Gait Trainer for Gait Rehabilitation for Patients with Stroke.

We developed an electromechanical gait trainer for rehabilitation of the persons with disabilities, which was designed to simulate normal gait with partial body weight support. The purpose of this study was to apply it to stroke patients and to evaluate its usability. Four nonambulatory stroke patients had 20 sessions of gait training with newly developed gait trainer for 5 to 6 weeks. Knee and ankle joint angles were measured by electrogoniometer during training. Main outcome was assessed by functional ambu lation category (level 0~5). Knee and ankle joint angles during exercise in the gait trainer showed repetitive pattern of gait-like movement similar to normal gait, though it was variable among the subjects. Functional ambulation category levels after training were 2 in 3 patients and 1 in 1 patient. The electromechanical gait trainer was a useful device for gait retraining in stroke patients with severe neurologic impairments.

2.CT Number Measurement of Residual Foreign Bodies in Face.

PURPOSE: Computed tomography theoretically should improve detection of foreign bodies and provide more information of adjacent soft tissues. And the CT scanner and PACS program proved to be an excellent instrument for detection and localization of most facial foreign bodies above certain minimum levels of detectability. The severity of injury in penetrating trauma to the face, it is often underestimated by physical examination. Diagnosis of a retained foreign object is always critical. METHODS: From March, 2005 to February 2008 a study was done with 200 patients who had facial trauma. Axial and coronal CT images were obtained with a General Electric(Milwaukee, Wis) 9800 CT scanner at 130kV, 90mA, with a 2-mm section thickness and a 512x512 matrix. RESULTS: Axial and coronal CT images at various window widths should be used as the first imaging modality to detect facial foreign bodies. The attenuation coefficients for the metallic and nonmetallic foreign bodies ranged from -437 to +3071 HU. As a general rule, metallic foreign bodies produced more Hounsfield artifacts than nonmetallic foreign bodies, thus providing a clue to their composition. All of the metallic foreign bodies were represented by a single peak and had a maximum attenuation coefficient of +3071 HU. Of the nonmetallic foreign bodies, glass had an attenuation coefficient that ranged from +105 to +2039, while plastic had a much lower coefficient that ranged from -62 to -35. wood had the lowest range of attenuation coefficients: -491 to -437. CONCLUSION: The PACS program allows one to distinguish metallic from nonmetallic foreign bodies and to individually identify the specific composition of many nonmetallic foreign bodies. This program does not, however, allow identification of the specific composition of a metallic foreign body. We recommend this type of software program for CT scanning of any patient with an injury to the face in which a foreign body is suspected.

3.Trends in antimicrobial sales for livestock and fisheries in Korea during 2003-2012.

In this study, trends in the sales of antimicrobials for use in livestock facilities and fisheries from 2003 to 2012 were investigated with regard to antimicrobial group, antimicrobial usage, and animal species. The overall amount of antimicrobials sold each year from 2003 to 2007 was 1,500 tons, after which they decreased, with the lowest sales being 936 tons in 2012. The total volume of antimicrobials used for feed additives decreased markedly by 94% from 2003 to 2012, which was mainly attributed to banning of feed additives. However, antimicrobial consumption through self prescription by farmers for disease prevention and treatment increased by 25% from 2003 to 2012. The largest volume of antimicrobials sold was for use in pigs (48~57%), followed by poultry (18~24%), fisheries (11~25%), and cattle (5~8%). Tetracycline was the highest selling antimicrobial, followed by penicillins and sulfonamides, although the overall sale of all three antimicrobials gradually decreased over the study period. This study demonstrated that the total consumption of antimicrobials has gradually decreased since 2008. Nevertheless, usage by nonprofessionals increased, which can ultimately cause emergence and spread of antimicrobial resistance. Thus, early establishment of veterinary prescription guidelines for prudent use of antimicrobials is urgently needed in Korea.

4.Epidemiological characteristics of bovine brucellosis in Korea, 2000~2004.

This paper describes the epidemiological characteristics of bovine brucellosis in Korea during January 2000~September 2004, which encompasses the period when the incidence of bovine brucellosis increased abruptly. Data from the National Animal Infectious Disease Data Management System were used for this study. A range of epidemiological measures was calculated including annual herd and animal incidence. During the study period, there were 1,183 outbreaks on 638 farms. In beef cattle, annual herd incidence increased from 0.2 (2000) to 11.5 (2004, to September) outbreaks per 10,000 and annual animal incidence varied between 3.4 (2000) and 105.8 (2004, to September) per 100,000, respectively. On 401 (62.9%) infected farms during this period, infection was eradicated without recurrence. Recurrence of infection was significantly higher on farms where abortion was reported (53.3%), compared to farms where it was not (30.0%). On beef cattle farms, infection was introduced most frequently through purchased cattle (46.2%). Based on the results of this study, the establishment and spread of brucellosis in the Korean beef cattle population were mainly due to incomplete or inappropriate treatment of aborted materials and the movement of infected cattle.

BACKGROUND: This study was to evaluate a nationwide nosocomial infection rate and antimicrobial resistance in intensive care units(ICUs) in Korea. METHODS: The study was carried out at 16 university-affiliated teaching hospitals from July through October 2004. We performed a prospective multicenter study to investigate nosocomial infection rates, device-associated infection rated, and causative pathogens and their antimicrobial resistance. RESULTS: The urinary tract was the most commonly involved site. Nosocomial infection rate was 12.48 in medical. ICU (MICU), 9.59 in medical surgical ICU (MSICU), 14.76 in surgical ICU (MSICU), and 11.60 in other lCU. Device-associated infection rates were as follow: 1) rates of urinary catheter-associated urinary tract infection were 4.26 in MICU, 3.17 in SICU, 4.88 in MSICU, and 5.87 in other ICU; 2) rates of central line-associated bloodstream infection were 3.24 in MICU, 1.56 in SlCU, 2.36 in MSICU, and 1.78 in other ICU; 3) rates of ventilator-associated pneumonia were 3.61 in MlCU, 13.05 in SICU, 1.68 in MSICU, and 4.84 in other lCU. Staphylococcus aureus was the most frequently identified microorganism in this study; 93% of S. aurues were resistant to methicillin; 17% of Pseudomonas aeruginosa isolated were resistant to imipenem; 11% of Enterococcus faecium and 18% of Enterococcus faecalis showed resistance to vancomycin. Over a half of Acinetobacter spp, Stenotrophomonas maltophilia, Klebsiella pneumoniae, and Escherichia coli showed resistant to fluoroquinolone. Conclusion: This study shows the seriousness of antimicrobial resistance and the importance of infection control in the lCU in Korea. This study should provide a theoretical strategy to enforce the infection control.
