1.Effects of Nurses’ Communication Styles and Surgical Patients’ Anxiety and Uncertainty on Patient Satisfaction

Purpose:
: The study was conducted to examine effects of nurses’ communication styles perceived by surgical patients and their anxiety and uncertainty on patient satisfaction.
Methods:
: Data were collected by using a structuralized questionannaire which consists of nurses’ communication style tool, State trail Anxiety Inventory(STAI), Mishel Uncertainty in Illiness scale, and patient satisfaction tool targeting on 276 surgical patients who were hospitalized in general wards from July 24, 2020 to December 18, 2020. The data analysis was done with the correlation analysis and the multiple regression analysis.
Results:
: Nurses’ communication style perceived by surgical patients has found to have the most informative communication styles and the least authoritative communication style. And the variable which has the most effect on patient satisfaction was the affective communication style(β=.35, p<.001), followed by nurse communication importance perceived by surgical patients(β=.15, p=.001). And surgical patients' anxiety (β=-.15, p=.011) and uncertainty(β=-.13, p=.024) has found to have a negative effect on patient satisfaction.
Conclusion
: The patient satisfaction got higher when the surgical patients perceived the Affective & informative communication styles among nurses’ communication styles, and when they perceived the nurses’ communication as important.
2.A Case of Keratitis, Ichthyosis and Deafness(KID) Syndrome.

KID syndrome was named after the initials of the major three symptoms of the disease; keratitis, ichthyosis, and deafness. The syndrome was first introduced by Dr. Burns in 1915 who described one of his patients with those symptoms. In 1981, Dr. Skinner and his colleagues reported 17 patients who had keratitis, ichthyosis, and deafness. They also called the disease KID syndrome for the first time. After that, there have been only 60 cases of KID syndrome reported. KID syndrome may be presented with neurosensory deafness, vascularizing keratitis and such skin disorders as ichthyosis, ichthyosiform skin eruptions, and alopecia. Also, those with KID syndrome may suffer from repetitive infections, dental affections, hypohidrosis, growth delay and hepatic and renal abnormalities. The authors report the first case of KID syndrome in Korea with some references. The patient presented with keratitis, neurosensory deafness, and such characteristic skin disorders as ichthyosis and hyperkeratosis.

3.Osteopontin Expression and Its Prognostic Significances in Human Renal Cell Carcinoma.

BACKGROUND: Osteopontin (OPN) is a glycoprotein and it participates in cell-cell and cell-matrix interactions. In vitro studies suggest that the OPN expression is associated with tumor metastasis, and especially with the metastasis of osteotropic tumors originating in breast, prostate and lungs. Since no human tissue study has suggested the means by which OPN participates in the tumorigenesis, angiogenesis, progression and metastasis of renal cell carcinoma (RCC), we evaluated the expression and prognostic significance of OPN in RCC. METHODS: Immunohistochemistry was performed with using the primary antibody for OPN on the archival paraffin-embedded tissue microarray specimens from 51 RCC patients who underwent radical or simple nephrectomy. RESULTS: In the normal kidney specimens, OPN was expressed in a few compressed distal tubules adjacent to the RCCs. In RCCs, the OPN expression was elevated in larger tumors (p<0.05) and in the tumor with low microvessel density (p<0.01). In the present study, univariate analysis indicated that stage, tumor size, lymph node and distant organ metastasis are significant prognostic factors for disease free survival (DFS) in RCC patients (p<0.01), but OPN is not (p=0.0661). Multivariate analysis indicated lymph node metastasis is the independent prognostic indicator of DFS (p<0.05). CONCLUSION: Though this study has statistical limitations, these results suggest OPN plays a role in tumor progression and metastasis and it may act as a potential prognostic indicator to predict the prognosis of RCC patients.

4.Malignant melanoma on a thermal burn scar

Chronic burn scars often cause various skin malignancies at rates of up to 2%. These lesions are usually squamous cell carcinomas, but rarely, malignant melanoma is reported. We report a 67-year-old male with a malignant melanoma on a burn scar with regional metastasis. This patient presented an ulcerative lesion only in 2 weeks. After histopathological diagnosis, we performed only palliative surgery on patient's demand, and followed up the subsequent deterioration course. Our case reemphasizes the need for rapid diagnosis and treatment when suspect lesions are present on chronic burn scar. Also, physician should be in mind and inform the patient about malignant melanoma and its aggressive course.
