2.Cataracts Induced by Dialysis Because of Chronic Renal Failure and It's Operation Results.

There has been increased dialysis because of chronic renal failure, and also has been increased cataractous changes. We found 8 eyes(5 patients) who were under dialysis and also had cataractous changes. They received cataract operations. The results were obtained as follows: 1. Male to female ratio is 4:1 and the average age of cataract patients is 45 years old. 2. The cataractous changes after dialysis occurred within 1 year(except example 1). 3. We found that the cataracts developed from posterior subcapsule of lens(except example 1) and the corrected visual acuity at 2 months after surgery were over 0.5. 4. There were no serious problems after surgery(routine I.C.C.E.). 5. On blood chemistry, the average calcium level was 7.88mg% (hypocalcemic state), and we suggest that this condition may be related with cataractous changes.

3.The effect of cyanate methacrylate on the shear bond strengths to dentin.

The purpose of this study was to evaluate the effects of cyanate methacylate on the shear bond strengths to bovine dentin surfaces as a dentin primers. Seven experimental adhesives were made with different mass fraction of Isocyanatoetylmethacrylate (IEM), 40wt% HEMA (Wako Pure Chemical Industries Osaka, Japan), 0.6% camphoroquinone, 0.4% amine and ethanol as balance. dentin bonding agents (0, 2, 4, 6, 8, 10, 12%) were made and applied on the surface of bovine dentin specimens of 7 experimental groups. Shear bond strengths were measured using a universal testing machine (Instro 4466). To identify the ratio and modes of cohesive failures, microscopic examinationn was performed. The ultra-structure of resin tags were observed under scanning electron microscope. The results were as follows ; 1) A higher shear bond strengths (33.62 MPa) in group 8% of Cyanate methacrylate to dentin were found, but there were no statistically significancy between Groups (p > 0.05). 2) The higher ratio of cohesive failures mode in group 2, 6, an 10% could be seen than that in any other groups. 3) A shorter resin tags were observed in all experimental groups. This could be resulted that the preventing from the cyanate methacrylate penetrate into dentin owing to reacting it with dentin collagen. Therefore the resin tags were shorter in lengths. Whether the higher bonding strengths of dentin bonding agents can be affected was not been assured with statistic results. The results indicated that the relation between tensile strengths of the dentin adhesives to bovine dentin and resin tags formed into the dentin could not affected. The main reason of increasing the shear bond strength to bovine dentin in experimental groups could not be assured.

5.Incidence of Cesarean Section Following a Continuous Lumbar Epidural Analgesia for Labor Pain.

BACKGROUND: This retrospective study was subjected to evaluate a causal relation in the incidences of Cesarean section with or without epidural analgesia during labor. METHODS: All of the subjects was divided into two groups which consisted of 394 cases wanted epidural analgesia as Epidural group and 2938 cases unwanted it as Non-epidural group. Continuous lumbar epidural analgesia was performed at L3-4 interspace with catheter advancing 3 cm cephalad when cervix was dilated to at least 3~5 cm and then patient was placed lateral decubitus or sitting posture. 0.125% bupivacaine 10 ml was injected initially via the epidural catheter and then followed by a mixture (10 ml) of bupivacaine 16.7 mg and fentanyl 16.7 microgram hourly to be infused continuously. The data were analysed using Pearson's x2 test with p<0.05 taken as a significant difference. RESULTS: The incidence of normal spontaneous vaginal delivery and Cesarean section, the cause of alteration to Cesarean section, and gestational frequency followed by Cesarean section rate showed no significant difference between two groups. The occurrence of neonate below 7 points of Apgar score at one and five minute after Cesarean section was rather more in Non-epidural group (p<0.05) than that in Epidural group. CONCLUSION: It is concluded that the continuous lumbar epidural analgesia with the dose of bupivacaine and fentanyl as mentioned above does not affect to Cesarean section rate during labor, which provides safe and effective for labor pain control.

6.Effects of different add-back regimens on hypoestrogenic problems by postoperative gonadotropin-releasing hormone agonist treatment in endometriosis.

OBJECTIVE: To compare the efficacy of different add-back regimens on hypoestrogenic symptoms during postoperative gonadotropin-releasing hormone (GnRH) agonist treatment in endometriosis patients. METHODS: This prospective cohort study included reproductive-aged women who underwent conservative laparoscopic surgery for ovarian endometriosis and received add-back therapy during a 6-month course of GnRH agonist therapy after surgery. Participants received one of four different add-back regimens: 1 mg of estradiol valerate, 2.5 mg of tibolone, or a combination of 1 mg of estradiol and 2 mg of drospirenone or 0.5 mg of norethisterone acetate. Changes in quality of life, hypoestrogenic symptoms, and bone mineral density were compared according to add-back regimens. RESULTS: A total of 57 participants completed a 6-month course of GnRH agonist and add-back therapy. All components of quality of life did not differ across groups. However, within the same treatment group, social relationship factors decreased significantly with estradiol valerate and tibolone alone, and environmental factors decreased significantly with estradiol valerate alone. Menopausal Rating Scale score did not change significantly, but the incidence of hot flushes significantly decreased with a combination of estradiol and norethisterone acetate. Bone mineral densities at the lumbar spine declined significantly after treatment in all groups except with a combination of estradiol and norethisterone acetate. CONCLUSION: This preliminary study suggests that an add-back regimen containing estradiol valerate and norethisterone acetate may have better efficacy in terms of quality of life, hypoestrogenism-associated symptoms, and bone mineral density.

7.Effects of Preservation of the Lamina Propria of the Maxillary Sinus Mucosa on Mucosal Regeneration in Experimentally Induced Maxillary Sinusitis with Polyposis in Rabbits.

BACKGROUND AND OBJECTIVES: It remains unclear whether the lamina propria (LP) of the sinus mucosa should be preserved in the management of the paranasal sinuses with severely diseased mucosa. The aim of this study is to evaluate the effects of preservation of the LP of the maxillary sinus (MS) mucosa on mucosal regeneration in maxillary sinusitis with polyposis in rabbits. MATERIALS AND METHODS: Maxillary sinusitis with polyposis was induced in 180 of the 190 MSs in 95 rabbits and, from these, 10 MSs were used as control. After 4 weeks, the ostia were reopened and 180 MSs were divided into 3 groups according to the following different procedures on the MSs; preservation of the whole mucoperiosteum without removal of polyps and polypoid mucosa (group A), complete removal of the whole mucoperiosteum (group B), and preservation of part of the LP and the periosteum with removal of polyps and polypoid mucosal surface (group C). Each of the experimental groups was explored after 0, 2, 4, 6, 8 and 12 weeks, respectively. Changes of mucociliary transport (MCT) speed, ciliary beat frequency (CBF), and inflammation score in histopathology were evaluated. RESULTS: Eight and/or 12 weeks after the procedures, there were significant improvements in MCT speed, CBF and inflammation score of the group C, compared with those of the group A and/or the group B. CONCLUSION: Preservation of the LP and the periosteum with removal of only polyps and polypoid mucosal surface may be more beneficial to mucosal regeneration than preservation or complete removal of the whole mucoperiosteum including polyps and polypoid mucosa in the surgical management of maxillary sinusitis even though the MS mucosa was severely diseased.

Primitive neuroectodermal tumor (PNET), also referred to as extraskeletal Ewing's sarcoma, is a highly aggressive tumor, and it is seen typically in the soft tissues of the chest wall and paraspinal region but rarely arises in the kidney. A 27-year-old woman was presented with a left renal mass and underwent a radical nephrectomy. The pathological diagnosis was a primitive neuroectodermal tumor. Microscopically, there were sheets and lobules of cells with round to oval nuclei and coarse chromatin, as well as a Homer Wright rosette formation. Immunohistochemical analysis showed strongly positive staining of CD99 and negative staining of other neuroendocrine markers, such as synaptophysin and chromogranin.

9.Retroperitoneal Castleman's Disease with Myxoid Degeneration: A Case Report.

Castleman's disease is characterized by the presence of a rare benign tumor of lymphoid origin which usually arises in the mediastinum but rarely occurs in the retroperitoneum. In the radiology literature, the unenhanced CT appearance of the disease has been decribed as hypodense or isodense as compared to that of the liver, and as showing homogeneous contrast enhancement. In this case, CT revealed a well-circumscribed, homogeneous enhancing mass with an internal cystic portion, a finding different from those described in the literature. We report a case of retroperitoneal Castleman's disease in which there was unusual mucoid degeneration resulting in a large cystic component, and which has not been previously reported.

An "overlap syndrome" is used to describe patients who have two or more well-defined connective tissue diseases such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), systemic sclerosis (SSc), and polymyositis. Their coexistence is defined clinically and often by specific serologic tests. We report a case of dermatomyositis associated with IgA nephropathy that had overlapping features of Sjogren's syndrome. This dermatomyositis and Sjogren's overlap is a rarely reported overlap syndrome worldwide.
