1.The Clinical Observation at the Urinary Tract in Patients with Carcinoma of Cervix.

A study was made of 234 patients with carcinoma of the cervix seen during the period from Aug.-1959 to Dec.-1963, who had come to the Departments of Obstetric and Gynecology of Yonsei Medical Hospital. Intravenous pyelogram, cystoscopy, and urine examination were done and N. P, N. was checked in addition to clinical examination before treatment. Intravenous pyelogram were classified into 1. Non-functioning 2. Hydronephrosis 3. Hydroureter as abnormalities Also cystoscopic finding were classified into 1. Elevation of bladder wall 2. Bullous edema 3. Neoplasm invasion. There were 51 patients who showed abnormality of intravenous pyelogram among the 234 patients with carcinoma of cervix, that was a unilateral abnormality in 38 cases and bilateral in 13 cases. Again, among 24(10.26%) non-functioning kidney in this series the left kidney was affected in 11 cases and right kidney in 6 cases. The 125 of 234 patients showed elevation of bladder wall in cystoscopic finding, and the posterior wall of bladder was elevated similarly in all stages. The urological study of patients with carcinoma of cervix are most essential works in addition to clinical examination, and it should be considered as a systemic disease for all physician especially gynecologist, radiologist, and urologist.

2.Primary Lymphosarcoma of the Prostate Gland.

A case of primary lymphosarcoma of the prostate gland proved by Silvermann needle biopsy was presented. Lymphosarcoma of the prostate is a rare condition, and only 23 cases have been reported in medical literature. In Culver's series, 74 per cent were sarcoma of fibrous origin (round cell, spindle cell sarcoma, myxosarcoma), 5 per cent rhabdomyosarcoma, 6 per cent lymphosarcoma, and 10 percent pleuriplastic or un-or de-differentiated sarcoma. A large tumor of the prostate, which is not hard and nodule occurs in a middle aged patient, should be considered of lymphosarcoma of the prostate. A case of lymphosarcoma of the prostate is added to the literature.

This study is to evaluate therapeutic effects between interferon-a combined chemotherapy and chemotherapy(5-fluorouracil, cisplatin) only in invasive uterine cervical cancer. The study included 35 cases of interferon-a combined chemotherapy group and 50 cases of chemotherapy(5-FU, cisplatin) only group. Then we analyzed the therapeutic effects with respect to size of tumor, number of lymphocyte subsets and NK activity, and SCC Ag(squamous cell carcinoma antigen) level in peripheral blood. (continue)

5.Diagnostic Conization of the Cerivx.

To evaluate the effectiveness of conization in the diagnosis of cervical cancer, the histopathologic finding of 464 consecutive cases were analized with respect to the cytology, punch biopsy and hysterectomy. Cold knife conization was performed under direct visualization(naked eye conization) from January, 1987 to Octorber 1994 at department of Obstetrics and Gynecology, Kosin Medical Center. The results were summerized as follows : 1. The rate of agreement and underdiagnosis of between pap smear and naked eye conization were 43.7% and 47%. 2. The rate of agreement and underdiagnosis of between punch biopsy and naked eye conization were 64.5% and 21.8% 3. Subsequent hysterectomy was done on the 382 case and their incidence of residual disease rate was 44.2%(169 cases). Their incidence of residual lesion was increased with severity of the carcinomatous change. 4. The rate of complication undergoing conization was 8.4%(39 cases).

7.Expression of bcl-2 and p53 Protein in Invasive Cervical Cancer.

Recently, the bcl-2 and p53 protein have been recognized as important factors that is contributed to programmed cell death. The objective of this study was to evaluate the prognostic significance of bcl-2 and p53 protein expression in uterine cervical carcinoma. The expression of bcl-2 and p53 in 59 cases of uterine cervical carcinoma (stage IB to IIB) were surgically treated from January 1993 to June 1994. The expression of bcl-2 and p53 was examined by immunohistochemical method using formalin fixed paraffin embedded tissue specimens. The 48 cases were squamous cell carcinoma and 11 cases were adenocarcinoma. The results were as follows: 1. The expression rate of bcl-2 protein was 28.8%(17/59) and there was no significant correlaltion between the expression of bcl-2 protein and the clinicopathologic parameters (histologic type, grade, FIGO stage, cervical invasion depth, lymph node metastasis, parametrial invasion, tumor size, neoadjuvant chemotherapy response, recurrence, survival). 2. The expression rate of p53 protein was 32.2%(19/59) and there was no significant correlation between expression of p53 protein and the clinicopathologic parameters. 3. There was significant correlation between and expression of bcl-2 and p53 protein (P 0.05). In conclusion, bcl-2 and p53 protein are thought to be possible factors in the carcinogenesis of uterine cervical carcinoma and correlate with progression of it. But further study will be required to clarify the role of bcl-2 and p53 in carcinogenesis of the uterine cervix.

9.Factors Associated with Unmet Dental Needs among Single-Person Households in Korea

BACKGROUND: The purpose of this study is to provide the data for discussions related to oral health promotion policies for single-person households by analyzing the status of unmet dental needs and related factors in single-person households in Korea, based on the Anderson model. METHODS: The data, obtained from 544 single-person households of those over 20 years old who were targeted for the 6th Korea National Health and Nutrition Examination Survey, were analyzed through a complex sample frequency analysis, complex sample cross analysis (Rao-Scott chi-square test), and complex sample binary logistic regression analysis on a complex sampling design. RESULTS: The most frequently given reason for an unmet dental need among single-person households was economic (52.4%). Factors related to the unmet dental needs of single-person households are smoking, which is a predisposing factor; personal income levels, which are an enabling factor; chewing discomfort; and limited daily activities, which are need factors. Smokers, the high-income group, the chewing-discomfort group, and the limited activity group showed high unmet dental care experience. Smokers had a 2.75 times higher rate of unmet dental care than non-smokers, and the high-income group had a 5.29 times higher rate of unmet dental needs than the median group. The rate of unmet dental needs for the chewing discomfort group was 3.27 times higher than the non-chewing discomfort group, and the limited activity group had a 7.87 times higher rate of unmet dental needs than the non-limited activity group. CONCLUSION: It is necessary to map out policies designed to help maintain and promote met dental needs considered to be internally heterogeneous to single-person householders, based on the Anderson model.
