1.Pattern of Adenomas after Colorectal Cancer Surgery.

Postoperative colonoscopy is an effective tool for management of colonic adenomas. Authors analyzed the pattern of colonic adenomas detected during follow-up colonoscopy after colorectal cancer surgery and evaluated the characteristics of adenornas, risk groups, and effective fo1low-up schedule. Study group were 222 patients and colonoscopy was performed 389 times. Patterns of adenornas were analyzed by variables as age, sex, preoperative serum CEA level, location of primary colorectal cancer, Borrmann type, Duke's stage, histologic differentiation, DNA ploidy, recurrence and histology. Metachronous adenomas were detected in 79 patients(35.6%) and both metachronous and syachronous adenomas were observed in 29 cases(13.1%). High risk variables for adenomas were male, old age and presence of synchronous adenoma. There were 2 patients with maligant change of adenomas. Yearly follow-up by complete colonoscopy over 3 years or more is recommended and follow-up interval should be shortened in the high risk groups.

5.Antihypertensive Effects and Safety of Lisinopril in Essential Hypertension.

BACKGROUND: Antihypertensive effect and safety of the newer, long acting, nonsulfhydryl angiotensin converting enzyme inhibitor, lisinopril, were studied. METHODS: Twenty eight patients of mild to moderate essential hypertension were administered 10-20mg of lisinopril once daily for ten weeks. Patients were evaluated every two weeks concerning the changes of blood pressure and pulse rate in the sitting position and also any untoward sumptoms and signs attributable to the side effect. Chest X-rey, ECG and laboratory examination were performed in principle two times before and after the completion of medication. RESULTS: The blood pressure declined from 165.4/107.6mmHg to 141.3/92.4mmHg at the end of ten weeks of medication, thus the reduction of 24.1mmHg of systolic pressure and 15.2mmHg of diastolic pressure were observed and the overall effective rate was 85.7%. The pulse rate and laboratory findings were not sigificantly changed before and after the administration of lisinopril. The side effects were observed in 2 cases(7.1%) of mild dry cough and in 2 cases(7.1%) of transitory mild headache and in 1 case(3.6%) of dizziness but no one discontinued medication due to adverse effects. CONCLUSION: Lisinopril proved effective and safe in the treatment of mild to moderate essential hypertension.

7.Echocardiographic Diagnosis of Left Ventricular Hypertrophy.

Echocardiography was done on 51 cases with various cardiovascular disease and on 23 cases of normal control subjects from April, 1981 to March, 1982 in the Department of internal medicine, Eul Ji General Hospital, Seoul, Korea. The results obtained were as follows: 1. The left ventricular mass estimated by the measurements made with standard convention was 261+/-8gm in patient group and 126+/-6gm in control group. With penn convention, the left ventricular mass of the patients group was 297+/-11gm and that of control group was 127+/-6gm. The differences between both conventions were significant statistically. 2. In measurements made with standard convention, those who showed increased left ventricular posterior wall thickness were 37 cases(73%) and those who showed increased left ventricular internal dimension were 15 cases(29%). On the other hand, number of cases who showed increased left ventricular mass were 45 cases(88%) and this results suggested estimation of left ventricular mass seems to be more useful method in the diagnosis of left ventricular hypertrophy than simple measurements of left ventricular posterior wall thickness of left ventricular internal dimension. 3. The correlation coefficient between left ventricular mass and maximum voltage of electrocardiography, left ventricular posterior wall thickness left ventricularinternal dimension was 0.70, 0.74 0.51 respectively in standard method and 0.82, 0.76, 0.44 respectively in penn convention. Left ventricular mass was correlated fairly closely with the maximum voltage of electrocardiography and left ventricular posterior wall thickeness, less closely with the left ventricular internal dimension.

9.Noninvasive ambulatory blood pressure monitoring in 22 healthy normotensive young adolescents.

BACKGROUND: Ambulatory blood pressure monitoring is widely available in clinical practice. To evaluate the pattern of normal 24 hour variation of blood pressure and the problems in analysis of data which was obtained with use of automatic blood pressure recorder, 24 hour ambulatory blood pressure was measured. METHODS: Ambulatory blood pressure was recorded for 24 hours with automatic blood pressure monitor in 22 normotensive young adolescents. RESULTS: 1) Average 24-hour BP was 109mmHg in systolic(115+/-30 for male, 102+/-30 for female), 66mmHg in diastolic(69+/-18 for male, 63+/-19 for female) and pulse rate was 72 beats per minute(70+/-23 for male, 75+/-26 for female). 2) Data obtained from ambulatory blood pressure monitor should be analyzed after deletion of unacceptable data with use of conventional criteria. Blood pressure increase which is not accompanied by increase in pulse rate increase can be used as another criteria to rule out unacceptable blood pressure data. 3) Blood pressure was low from mid-night to 6 A.M. and began to rise slowly till mid-day and then maintained steadily through the remainder of the day. And 24 hour variation of blood pressure was more adequately assessed after application of Fourier analysis. CONCLUSIONS: These results suggest that certain deletion criteria was inevitable during analysis of the data which were obtained from ambulatory blood pressure recorder and Fourier analysis can be used as valuable smoothing technique to assess the 24-hour blood pressure profiles.

10.Persistent Genital Swelling after Hydrocele Ligation in a Patient Receiving Continuous Ambulatory Peritoneal Dialysis.

We report a case of genital swelling in a patient receiving continuous ambulatory peritoneal dialysis. A physical examination did not identify any defect. Ultrasonography revealed a large hydrocele, and surgical repair brought resolution of the genital swelling. Two months later, however, the genital swelling had recurred and was not improved until peritoneal dialysis was replaced by hemodialysis 3 months later.
