1.Relationship of Atopic Dermatitis to Molluscum Contagiosum.

This study is statistical investigation on the relationship of atopic dermatitis to molluscum contagiosum. The study was included 1,215 cases of atopic dermatitis and. 354 cases with mollescum contagiosum during the period of 1974 to 1977. According to our clinical experiences it was not uncommon to develop the combined cases with atopic dermatitis and mollucum contagiosum. On the immunological point of view the association of defective cell-mediated immunity and increased susceptibility to viral and fungaI infections in some cases with atopic dermatitis has been reported by many authors. The results are as follows: 1) The order of combined diseases in atopic dermatitis was rnolluscum contagiosum (8. 9 %), impetigo (6. 8 %), contact dermatitis (5. 1%), insect bite (3. 5 %) and urticaria (2. 0 %). The highest combined rate was seen in molluscum contagiosum. (p<0. 05) 2) The order of combined diseases in molluscum contagiosum was atopic dermatitis. (30.5%), impetigo (6.0%), contact dermatitis (5.7%) and insect bite (2.4%). The highest combined rate was seen in atopic dermatitis(p<0. 01). 3) Atopic dermatitis was more frequent in male than in female(p<0. 01) but in the age group above 11, female was more prevalent(p<0. 01). There was no sex difference for developing molluscum contagiosum in all our studied cases, but the frequency was higher in male below 5 year of age(p<0. 01%) and in female above 11 year old (p<0. 01). 4) Atopic dermatitis was frequently seen in the age group below 5 year old(72. 0%), and also moIluscum contagiosum was common in the same age group(60. 5 %). There were two peaks in incidence, below 10 year old(78. O%) and the age group of 21 to 3O(12. 4%). in molluscum contagiosum. The incidence was significantly higher in woman(10. 2%) than that of man(2. 3%) in the later group. The combined cases with atopic dermatitis and molluscurn contagiosum were more frequently seen in the age group below 5(84. 3%)but no case was found in more than 15 year old. 5) Atopic dermatitis was more frequent in winter and spring (62. 7%) than in summer and autumn(37. 3%), (p<0. 01). The combined cases with atopic dermatitis and moIluscum contagiosum were most frequent in spring(85. 2 %),(p<0. 05).

2.Syringoma Localized on the Vulva.

Syringoma of the vulva is a rare disorder and limitation of the lesions to this site appears unique. Lesions can be clinically diagnosed without difficulty, since it is characterized by multiple, bilateral, asymptomatic skin-colored papules. Vuluar syringoma should be considered in the differential diagnosis of multicentric papular lesions of the vulva. The patient presented here is a 30-year-old healthy woman; lesions are multiple, biiateral, asymptomatic, angular or round, skin-colored to yellowish brown, firm papules localized on the vulvar region and showing a characteristic histologic findings.

3.A Case of Exfoliative Dermatitis with Renal and Bone Marrow Failires due to Topical Metalic Mercury.

Mercury had been an important constituent of the remedial drugs for last few centuries, but substantially replaced by more specific and effective mordern medicines. However, metalic mercury, tbe vestige of the last decades, has still been employed for the patients of scabies by the herbal doctors in Korea. Therefore, we have often experienced patients with such complications as exfoliative dermatitis following topical applications of metalic mercury or inhalation of the vapor. There are three major toxic chernical forms of mercury, such as metalic (ele- mental) mercury vapor, salts of the mercury, and organic mercur.'als. Metailc mercury is the most volatile of the inorganic forms of the metal, Inhalation of the vapor of the metalic mercury is very toxic, but ingestion of the globular forms is rarely harmful. Acute mercury poison.ng due to topical mercurials is very rare. This case was a such patient who showed the mercury poisoning can result from the percutaneous absorption of topically applied mercury. The patient was an 11 year-old schoolboy with scabies. Three days after topical applications on the whole body with the mixture of rneta,lic rnercury, phosphorus and perilla oil, as prescribed by a herbal doctor, he was suffered from headache, fever, facial flushing, sore throat, a,nd stomatitis for a few days, and then followed by exfoliative dermatitis, renal and bone marrow fa'.lures. However, bone marrow failure due to mercury was very rarely described in the literatures. Virtually, the whole body was covered by thick scales on the dirty greyish pigmented or erytheinatoas Cohfluent payules. It whs oozing on the external genital area, and all nails were spontaneously extracted. Tbe findings of renal failures included puffy face, ascites, BUN 92. 8 mg/dl, serum creatinine 7. 3 mg/dl. creat.nine clearance 12rnl/min., proteinuria, rnicro and gross hematuria, and non or poor visualizations of IVP. The peak in severity was at about 50 days after onset and bacame normal except mild proteinuria at about 70 days after onset. The findings of aplastic anemia were shown by pancytopenia(Hb 7. 6g/dl, WBC 1, 700, seg. neutro. 18%, lympho. 40% mono. 26%, eosino. 16%, Hct25%, thrombocytes 40, 000, RBC 3, 000, 000, reticulocytes 0. 1%) and about 20 per cent of the cellularity in bone marrow aspiration from about 70 to 90 days after onset. In fact, the aplastic anemia was coincided with nasal bleeding, staphylococcal septisema, and staphylococcal pustulation at the site of IM injection. However, the levels of onset 80 day-urine and blood rnercury were high: 208 ug/1. (norm.alless than 100 ug/l)in urine, and 126. 3 ug/100g (normal less than 5 ug/100g) in blood.

4.Chromosome Breakage Test for the Diagnosis of Fanconi's Anemia.

BACKGROUND: Fanconi's anemia (FA) is an autosomal recessive disease characterized by aplastic anemia, pre-malignancy, congenital malformations and chromosome breakage syndromes. As up to 30% of patients have no detectable congenital anomalies, the modern diagnosis of FA rests on chromosomal breakage of patient's cells induced by chemical clastogens such as diepoxybutane (DEB) or mitomycin-C (MMC). METHODS: We have done chromosome breakage test to differentiate FA from 11 aplastic anemia, three Diamond-Blackfan syndrome, three myelodysplastic syndrome, one acute leukemia with congenital anomaly and three siblings of FA. The peripheral blood lymphocytes from each individual were co-cultured in phytohemagglutinin-containing medium by the three methods, i.e., DEB treated, MMC treated and un-treated. RESULTS: Five cases were found to have increased chromosomal breakages to DEB and MMC, confirming diagnosis of FA. Other 21 cases showed no increased chromosomal breakages. No overlap was found between FA group and others (P<0.01). In one FA, there was no increased spontaneous breakage, but increased breakage to DEB and MMC. Of five FA, one case showed no congenital anomalies. CONCLUSIONS: Chromosme breakage test was shown to be simple, reliable and useful in ascertaining the diagnosis of FA.

5.Acute Compartment Syndrome Induced by Rhabdomyolysis Due to Antipsychotic Drug Overuse

A 49-year-old male was found unconscious at his accommodation and visited the emergency room. He was on antipsychotic and antidepressant drugs (vortioxetine hydrobromide, mirtazapine, sertraline hydrochloride, quetiapine, and alprazolam) for schizophrenia and major depression. At the time of discovery there were signs of overdose of the drugs around the patient. A physical examination revealed, pain, pallor, and edema in the left buttocks and lateral thigh. Active ankle movements below the left ankle were not possible and sensations in the tibia and peroneal nerves were lost. The pressure in the buttock compartment was measured at 42 mmHg. Magnetic resonance imaging revealed edema and high intensity signals in the left hip muscles and surrounding soft tissue. An emergency fasciotomy was performed and partial restoration of the lower extremity sensation and muscle strength were achieved after 24 hours.
6.Lumbar Juxtafacet Cyst Treated with Direct Needle Aspiration Under the Guidance of Image Intensifier

A lumbar juxtafacet cyst is a rare disease that causes low back pain, radiculopathy and neurological claudication by compressing the nerve roots. A 34-year-old male complained of severe low back pain and radicular pain in the right lower extremity. Magnetic resonance images revealed a cyst at the lateral recess of the spinal canal between the L3-4 disc and posterior facet joint that extended to the L4 body level. Under the guidance of an image intensifier, needle aspiration of the cyst was performed, which extracted 1.5 ml of serous, yellowish colored fluid. After the aspiration, the symptoms subsided dramatically. The follow-up magnetic resonance images showed no recurrence of the cyst. To the best of the author’s knowledge, there are no reports of lumbar juxtafacet cyst treated with needle aspiration in Korea. This case is reported with a review of the relevant literature.
7.Nodular scalp mass as the first presentation of pulmonary large cell neuroendocrine carcinoma: a case report

Metastasis of lung cancer to the skin is uncommon, presenting in 0.22% to 12% of lung cancer patients, and it is extremely rare for skin metastasis to be the first clinical manifestation of lung cancer. In the few cases where skin metastasis has been reported as the first sign of lung cancer, the patients were typically heavy smokers or had preexisting respiratory diseases and symptoms. This prompted clinicians to consider skin metastasis of a pulmonary malignancy. Large cell neuroendocrine carcinoma (LCNEC) is a rare type of lung cancer that accounts for approximately 3% of lung cancers. LCNEC mainly metastasizes to visceral organs, such as the liver, bone, and brain, and it only shows metastasis to the skin in very rare cases. Herein, we report an unusual case of a metastatic skin lesion as the first sign of primary pulmonary LCNEC, in a 63-year-old woman with no pulmonary symptoms or personal history of smoking or pulmonary disease.
8.The Effect of Exercise on Prevention of the Common Cold: A Meta-Analysis of Randomized Controlled Trial Studies.

BACKGROUND: Because there is no specific treatment for the common cold, many previous studies have focused on prevention of the common cold. There were some studies reporting that regular, moderate-intensity exercise increases immunity and prevents the common cold. We conducted a meta-analysis to determine the effects of exercise on prevention of the common cold. METHODS: We searched MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trials (CENTRAL), CINAHL for studies released through June 2013. We manually searched the references. Two authors independently extracted the data. To assess the risk of bias of included literature, Cochrane Collaboration's tool for assessing risk of bias was used. Review Manager ver. 5.2 (RevMan, Cochrane Collaboration) was used for statistical analysis. RESULTS: Four randomized controlled trials were identified. A total of 281 participants, 134 in the exercise group and 147 in the control group, were included. The effect of exercise on the prevention of the common cold had a relative risk (RR) of 0.73 (95% confidence interval [CI], 0.56 to 0.95; I2 = 7%). The mean difference of mean illness days between exercise group and control group was -3.50 (95% CI, -6.06 to -0.94; I2 = 93%). In the subgroup analysis, the RR of under 16 weeks exercise was 0.79 (95% CI, 0.58 to 1.08). CONCLUSION: In this meta-analysis, regular, moderate-intensity exercise may have an effect on the prevention of the common cold. But numbers of included studies and participants were too small and quality of included studies was relatively poor. Subsequent well-designed studies with larger sample size are needed to clarify the association.

9.Effect of Sertraline on the Regulation of Serotonin Transporter mRNA Levels in Rat Brain.

The knowledge of gene regulation of serotonin transporter mRNA may provide clues to understanding how antidepressants affect their therapeutic actions. Recently, the effects of antidepressants on the serotonin transporter have been investigated but yielded controversial results. To study this further, we performed in situ hybridization for serotonin transporter mRNA in rats(treatment group, n=5)receiving long term(14 days)treatment with a selective serotonin reuptake inhibiting antidepressant, sertraline(10mg/kg, i.p) Following sertraline treatment, a significant(p<0.05)ncrease in hybridization of serotonin transporter mRNA was observed compared to that observed in vehicle-treated rats(control group, n=5) This result may be interpreted as a compensatory mechanism to reduce synaptic levels of serotonin which were increased by long term sertraline treatment.

10.The Influence of IMF-related Economic Stress upon Mental Health.

This study was designed to investigate the influence of the so-called IMF-related economic stress upon the mental health and to analyze the process of the influence. As the possible variables moderating the stress process, social resource, problem solving style, and hopelessness were included. 221 male and female subjects were recruited according to the intensity of economic stress and classified into four groups(employees of solid companies, employees of the companies at risk, short-term unemployed and long-term unemployed). Relevant measures including economic stress scale, selected SCL-90R subscales(somatization, depression, hostility, anxiety), social support scale, problem-solving scale, and job hopelessness scale were administered. As assumed, it was proved that economic stress had adverse effects on the mental health. In addition to this basic causal relationship, moderating effects of several variables were found as follows: First, social support had the buffering effect upon the economic stress. That is, strong social support played the role suppressing the development of psychiatric symptoms under high stress. Second, men were more vulnerable to the adverse effects of economic stresses than women. Third, economic stresses had influence on the helplessness concerning the problem solving. However other problem solving strategies such as creative style, approach style, avoidance style, etc were proven to have no relationship with the economic stress. Fourth, in the analysis of unemployed group, it was found that hopelessness mediates the relationship between economic stress and mental health. In conclusion, the clinical implications of such findings in economic stress process were discussed and several limitations and the suggestions for future researches were also addressed.
