Forty-six patients (fifty hips) underwent revisions of a total hip arthroplasty that had failed but was not associated with infection, Three of these patients had a third revision. The mean length of follow-up was approximately two years. Thirty patients reported that their condition was improved. On final roentgenographic examination showed, two loosening of the acetabular components and five of the femoral components in which there was one sympto matic loosening (moderate severe pain and probable roentgengraphic loosening) noted. The extralong stems are not necessary in all revision cases, cortical defects at the tip of standard stem obviously should require bypass the stress riser with a longer stem. If the stability can be achieved with host bone, Revision may be carried out with a relatively short stem. Significant postoperative complications as subsidence and progressive loosening were noted in seven out of the forty-six patients. We consider that extensive porocoated cementless stem or standard flute stem seem to be a better outcome in revision arthroplasty.

Korean Journal of Dermatology 1993;31(5):657-663
BACKGROUND: Vitiligo is riot quite a rare diseasep; it has about 1% prevalence. The cause of vitiligo is not clear, however, in recent studies an autoimmune origin is freqluei itily mentioned. OBJECT: We tried to an lyze some clinical features of vitiligo and relate them with the presence of autoantibodies. MEHTODS: A total of 381 vitiligo patients was included for the analysis of clinical features. A laboratory study included rhumatoid factor, antinuclear antibocoly, antithyroglobulin antibody and antimicrosome antibody. Some 62 patients were examined for opl thmologic changes. RESULTS: One hundred and one(26.5%) of 381 patients exam ned showed at least one of the autoantibodies tested. Twenty nine pateints showed 2 different aitintibodies. The age at aonset of vitiligo in the autoantibody positive group was 6.6 years later than that of the autoantibody negative group. Autoimmune and/or endocrine diseases were more frequinty found among aut.oantibody positive patients. These diseas s included hyperthyroidism, diabetes me litus and alopecia areata. One patient revealed retinal hypoigmentation and showed no autoantibcidics. CONCLUSION: About 9% of vitiligo patients who were autoantiocyte positive had clinical evidence of diseases associated with the autoantibody. However, it is prudent. to xpect that more patients with t.he autoantibody may develop later systemic autoimmune diseases or endocrinopathies. A long term follow-up of these patients seem:, to be very important.

8.Cerebral Oxygen Saturation Monitoring during Aortic Dissection Surgery: A case report.

Korean Journal of Anesthesiology 1997;33(5):962-966
Transcranial cerebral oximetry has been successfully used in a variety of neurosurgical conditions, primarily those associated with disturbed cerebral circulation. It has been also used in intraoperative monitoring of aortic dissection and surgical procedures performed under deep hypothermia and circulatory arrest. During disending aortic arch exposure, sudden cerebral oxygen saturation change from 63% to 48% was detected. After therapeutic bypass, cerebral oxygen saturation was increased to 65%. During aortic arch repair, deep hypothermic circulatory arrest with retrograde cerebral perfusion was applied for 130 min and cerebral oxygen saturation slowly decreased from 65% to 52%. Patient was discharged from hospital without neurologic complication and cognitive funtion disturbance.

Korean Journal of Dermatology 1993;31(3):408-411
We report a case of pretibial myxedema with hypothyroidism in a 52-year-old woman. She suffered from primary hypothyroidism for a period of 17 years. She was treated with a daily dose of sodium levothyroxine 0.2 mg intermittently. She had erythematous ftesh colored, hard and thickened plaque. on the left pretibial area for onr year. The histopathologic findings showed considerable amount of muci in the upper and mid dermis. Toluidine blue and alcian blue stain demonstrated an extensive deposition of dermal mucin. We treated her with a topical steroid and oral sodium levothyrcxire.

The Journal of the Korean Orthopaedic Association 1990;25(1):93-102
The metallic failure is one of the annoying problems after fracture surgery. The seventeen cases of metallic failures after compression plate fixation were treated at the Department of Orthopaedic Surgery. Yonsei University Wonju College of Medicine between January, 1980 and December, 1988. The results were summerized as follows: l. Among 17 cases, 12(70.5%) were under 40 years of age and 14(80%) were male patients. 2. The sites of fracture of the seventeen cases; 12 femurs, 3 tibiae, and 2 radii. Of the cases, there were breakage of plates in 13 cases, bending 1 case and failure of screw in 3 cases. 3. The pattern of fracture: communited fracture
