1.A New Embedding Method for Optimal Tissue Preparation for Mohs Mierographic Surgery.

BACKGROUND: The principle of Mohs micrographic surgery is to examine microscopically the entire peripheral margin of an excised tissue specimen to precisely localize the site of any residual tumor and enable its subsequent removal. In processing in the tissue for frozen horizontal sections, a technical difficulty arises in manipulating the deep and lateral margins into the same flat plane. OBJECTIVE: Our aim was to develop a new method allowing the optimal tissue preperation for Mohs micrographic smgery and to evaluate the usefulness of this methad. METHODS: The technique involves conversion of a non-planar surgical margin to a planar surface by applying and freezing the specimen against a flat glass surface. We compared this modified method used in our laboratory with standard methods for tissue preparation of Mohs micrographic surgery.

2.Clinical Study of Total Hip Prosthesis Replacement: Report of 69 cases

The goals of hip arthroplasty have been to eliminate pain, decrease deformity, increase morbility and obtain stabiIity. Currently total hip replacement is the best avaliable procedure to accomplish these goals. A prospective study of total hip arthroplasty was begun at the orthopedic dept of Kyung Hee Medical College in 1974. Between May, 1974 and June, 1977. 69 total hip replacements of the Charnley type, the Trapesoidal-28 type and the Muller type were peformed by the authors. Of the 60 Patients involved in this study, 9 had bilateral operations. The Charnley type Prosthetic device was used in 56 cases, the Trapesoidal-28 type in 7 cases and the Muller type in 6 cases. Complications included wire breakage, dislocation, loosening, femur shaft perforation, nonunion of greater-trochanter, acetabular protrusion, transient femoral nerve palsy, femur shaft fracture, infection, socket lateralization, ectopic bone formation and death. In 59 patients of the 60 patients, 53 patients felt that they were definitely improved, 5 patients felt their condition was unchanged and 1 patient felt her condition was worse.

3.Purification of the major allergens from Korean Dermatophagoides pteronyssinus and production of the recombinant antigens.

BACKGROUND: Purified major allergens of house dust mite are essential for evaluation of the allergic mechanism in molecular basis and development of new modalities of immunemodulation. OBJECTIVE: In this study, we aimed to purif group 1 and group 2 allergens from Dermatophagoides pteronyssinus (Dp). In addition, cDNAs corresponding to Der pI and II in Korean Dp were isolated and recombinant Der p1 and Der pII were synthesized. MATERIALS AND and METHOD: Der pI allergen was purified by ammonium sulfate precipitation, anion -exchange column chromatography, and gel filtrat,ion chromatography. Der pII allergen was purified by anion exchange chromatography, gel filtration chromatography, and a preparative isoelectric focusing method. RESULTS: Eight hundred ug of Der pI and 50 ug of Der pII were obtained from 100 g of culture medium and 1 g of mite bodies, respectively. The purities of these allergens were confirmed by SDS PAGE and the strong reactivity to the patient sera was identified. In order to produce a recombinant allergens, poly(A) RNA from house dust mites were isolated and used for cDNA synthesis by RT PCR. The cDNA was inserted into prokaryotic expression vector and the vectors were transformed into E. coli. A little amount of recombinant Der pI protein was produced due to the low solubility, and 1.2 mg of recombinant Der pII was produced from 1 L of E. coli culture medium. The antigenicity of Der pI was relatively weak, however, Der pII showed a strong antigenicity. Amino acid sequence of the amplified cDNA deduced from DNA sequences of Der pII showed 6 different variants. The variation of amino acid sequences suggests the possibility of high incidence of mutation of Der pII protein. CONCLUSION: A simplified method for the purification of Der pI and Der pII was developed. Recombinant allergens will be useful for the diagnosis and treatment of allergy with lower costs.

4.Therapeutic Uses of T-lens (Soflens) in Bullous Keratopathy.

21 patients suffering from bullous keratopathy were fitted with T-Iens (Soflens) continuously for average 12 months without adverse effect. The T-Iens (Soflens) produced dramatic relief of ocular pain occurred in 9 patients among the 12 paitents. The T-Iens (Soflens) produced mild clinical improvement of vesicle. visual auity and corneal edema.

5.Extramammary Paget's Disease of the Penis and Scrotum with a Renal Cell Carcinoma.

Extramammary Paget's disease is a rare cutaneous malignancy and frequently associated with an underlying adnexal carcinoma and perhaps with underlying internal malignancy. In our case, genital Paget's disease was followed by the discovery of renal cell carcinoma. We report a rare case of extramammary paget´s disease of the penis and scrotum which associated with a renal cell carcinoma.

6.Influence of a Soft Contact Lens (Soflens) on the Penetration of 24NaCI and 3H Cortisol Solution into the Anterior Chamber.

A soft contact lens is used not only for the correction of refractive errors, but also for therapeutic purpose. Soft contact lens is hydrophilic, it takes up and releases medicaments so that it promotes and prolongs the penetration of drugs through the cornea. It has been also recently report!,!d that a soft lens increases pressure lowering effect in pilocarpine therapy. The purpose of this study is to evaluate the penetration rate of drugs immersed into a soft contact lens (Soflens) through the cornea. In this study the authors used 24NaCl and [3H] cortisol solution on the rabbit cornea with or without Soflens. The results were as follows: 1. During four hours following the administration of 24NaCl the pent ration rate into the anterior chamber when Soflens, immersed in 24NaCl solution for 2 minutes was applied, showed 2.4 times higher than that of topical instillation. In topical instillation, the Soflens group revealed a higher penetration rate (1.6 times) than non-Soflens group. It was revealed that Soflens promoted the penetration of 24NaCl solution into the anterior chamber of rabbits. 2. Intracorneal penetration of 24NaCl solution was 1.5 times higher in topical instillation in the Soflens group than the non-Soflens group. Meanwhile, the highest penetration (2.2 times) was noted in the group of Soflens which were soaked in 24NaCl solution. The highest concentration in the iris (1.5 times) was noted in the soaked Soflens group. 3. In topical instillation of cortisol solution in the group of Soflens without treatment and the group of Soflens immersed in cortisol solution showed much lower penetration rates into the anterior chamber with 0.9 and 0.5 times than those of the topical instillation group. Based on the above results, it is clear that Soflens does not always promote the penetration of drugs into the anterior chamber. The drug-sponging effect is supposed to very according to various factors such as solubility, molecular weight of drug and physico-chemical properties of a soft con tact lens.

7.Angiokeratoma Circumscriptum: Successful Treatment with the Flashlamp Pulsed Tunable Dye Laser.

Angiokeratoma circumscriptum is present at birth or early childhood and is an uncommon dermatosis characterized by papules and small nodules that may coalesce to form plaques. Histopathologically, there are varying degrees of hyperkeratosis, papillomatosis, and irregular acanthosis. In the papillary dermis, greatly dilated capillaries are observed, The acanthotic epidermis encircles the vascular spaces(blood cysts) where, occasionally, organized thrombi may be found. The use of pulsed-dye lasers to treat cutaneous vascular lesions is based on the theory of selective photothermolysis. We report a case of an angiokeratoma circumscriptum in an 48-year-old woman for whom the flashlamp pulsed tunable dye laser proved to be a highly successful means of treatment.

10.A Case of Inflammatory Metastatic Carcinoma of the Breast.

A 51-year-old Korean woman presented with a non-tender, well-demarcated, reddish, edematous patch on the right anterior chest where a previous mastectomy and radiation therapy had been performed. She had been diagnosed as having infiltrating ductal carcinoma of the right breast about 1 year ago. Histopathological findings of the skin lesions were consistent with inflammatory metastatic carcinoma of the breast. Inflammatory carcinoma or carcinoma erysipeloides is a well-established entity most frequently associated with carcinoma of the breast. It is characterized by dermal lymphatic invasion by malignancy and clinically should be distinguished from erysipelas or cellulitis. We describe a case of inflammatory metastatic carcinoma derived from an infiltrating ductal carcinoma of the breast which can be clinically confused with radiation dermatitis.
