1.Sacral Perineural Cyst with Lumbar Disc Herniation: A Case Report.

STUDY DESIGN: A case report is presented of a symptomatic sacral perineural cyst with lumbar disc herniation that has reported a few case in the literature. We report the case of symptomatic perineural cyst with lumbar disc herniation. SUMMARY OF LITERATURE REVIEW: Symptomatic sacral perineural cyst is rare and some cases of sacral perineural cyst associated with disc herniation were reported. But it is not well known that the correlation between clinical symptoms caused by mass itself and concomittant lumbar disc herniation. METHODS: A 34-year-old male presented with low back pain, both leg dysesthesia and bilateral radiating pain. Clincal sign and symptom, physical examination, diagnostic imaging studies including MRI, myelography and pathologic findings of surgical specimens were evaluated. RESULTS: After the decompressive partial laminectomy and discectomy, the patient who had diapnosed with herniated nucleus pulposus of L4-5 and L5-S1 complained residual both leg paesthesia and hypoesthesia. Patient's clinical symptom was correlated with sacral perineural cyst. So we removed the cyst. Postoperatively neurologic symptoms and signs were completely relieved. Pathologically, Sacral perineural cyst composed of meningoepithelial cell and neural tissue within cystic wall were confirmed. CONCLUSION: We could find that sacral perineural cyst sufficiently evoked a clinical symptom. So, careful observation was needed in that case of sacral perineural cyst associated with disc herniation.

2.An Epidemiological Study for Relationship Between Risk Factors and Types of Cataract.

The epidemiological study to evaluate the risk factors and types of cataract was performed in 183 cataract patients who received cataract operation in Chung-Ang University Hospital and in 80 control patients from March 1992 to Febraury 1994. Our study revealed that the sun light exposure and smoking were related to nuclear opacity, and drinking and smoking were related to cortical opacity. Also anterior subcortical opacity was more common in patients with heart disease. And the long term use of steroid and patient with diabetes mellitus were risk factors of posterior subcapsular opacity cataractogenesis. In this study we could confirm some risk factors were closely related to the type of lens opacity.

3.Comparative Study on the Healing Process of the Calvarial Defect filled with Hydroxyapatite and Bioglass in Rats.

Bone or bone substitute has been widely used for the reconstruction of bony defect developed by trauma, infecton, cyst, tumor, congenital abnormality, etc. Owing to its variety in quality and quantity, the use of alloplasts is increased rapidly. Among the various graft materials, HA has been studied and applied in clinic most exensively. When HA applied to bony defect, it is considered to help the bone healing by osteoconduction but tends to be encapsulated with fibrous tissue and floated in the applied area by the lack of consolidation. Bioglass is considered as a subsitute which can cover such weak point of HA. Bone defect were artificially prepared in the calvaria of the rats followed by grafts of HA and bioglass into defects. Sequential sacrifice was peformed at the 3rd, 7th, 14th and 28th day of experiment. The staining of the obtained specimen was performed with H&E, MT, immunohistochemical stain and then histological examinaions were carried out under the light and scanning electron microscope. 1. In the features of H&E stain, infiltration of chronic inflammatory cell were present in all group at the 3rd and 7th day of experiment with decrease at the 14th day in bioglass group. The infiltration of inflammatory cells was more active in control and HA group whereas the formation of granulation tissue was more active in bioglass group was larger and more irregular than in HA group. 2. The features of MT stain showed new bone formation from the 7th day in all groups and the evidence of advanced calcification was observed at the 28th day. 3. In the features of immunohistochemical stain, bioglass group showed more PCNA positive response in the osteoblasts at the site of new bone formation and the lower activity of undifferentiated mesenchymal cells than HA group. 4. In scanning electron microscopic features, fibrous tissue formation and osteoid attached to HA was observed in the HA group after the 7th day. In bioglass group, osteoid formation was observed between the absorbed bioglass. Generally in HA group, bone formation progressed with characteristic of the fibrous and osteoid tissue mainly attached to the surface and the high activity of undifferented mesenchymal cells was seen at the graft site. In bioglass group, osteoid tissue formation was seen between the irregulary absorbed bioglass and the increased PCNA positive response was observed in the osteoblast at the new bone formation site.

4.A Case of Cysticercosis Treated with Praziquantel.

We have recently experienced a case of cysticercosis treated with praziquantel. The patient, 22-year-old female, with 43 cystic masses had marked clinical response to the administration of praziquantel (75 mg/kg/day) for 7 days. It seems that praziquantel, anticestodal agent, might be a safe and effective therapy for this infection.

7.Efficacy and Safety of Alfuzosin 10 mg Once Daily in Patients with Lower Urinary Tract Symptoms Suggestive of Benign Prostatic Hyperplasia: A 6-Months Study in Real Life Practice.

PURPOSE: The aim of the study was to evaluate the long-term safety profile and efficacy of Alfuzosin 10 mg once daily in patients with lower urinary tract symptoms (LUTS) suggestive of benign prostatic hyperplasia (BPH), under daily practice conditions in Korea. MATERIALS AND METHODS: In this 6-months, open-label, multicenter, noncomparative, observational study, 511 men were enrolled. International Prostate Symptom Score (IPSS), IPSS 8th question (bother score), maximum flow rate (Qmax), Danish Prostate Symptom Score (DAN-PSS) were evaluated at baseline and after 3, 6 months of treatment. Safety was analyzed in all patients exposed to alfuzosin (n=480). Analysis was performed at end-point in the intent to treat population (n=368). RESULTS: Of the 511 enrolled patients in the study, 218 patients (42.7%) dropped out. With alfuzosin, IPSS and bother score significantly improved from baseline by -6.7+/-6.4 (-31.9%, p<0.001) and -1.2+/-1.2 (-29.3%, p<0.001), respectively. Nocturia also significantly improved from 2.3+/-1.3 at baseline to 1.8+/-1.0 at end-point (-0.6+/-1.1, p<0.001). In the subgroup of patients likely to be obstructed (Qmax <10 ml/sec at baseline), Qmax improved from 8.3+/-1.2 at baseline to 13.7+/-6.1 at end-point (+5.4+/-5.8, p=0.001). In patients with pain/discomfort on ejaculation, weighted score significantly improved from 2.4+/-2.2 at baseline to 1.5+/-1.9 at end-point (-31%, p=0.002). There were no clinically relevant changes in sitting systolic BP and diastolic BP at endpoint. CONCLUSIONS: Alfuzosin 10 mg once daily administered for 6-months is effective in improving LUTS and quality of life, and is well tolerated from a sexual and cardiovascular perspective, including in elderly men and those receiving anti-hypertensive co-medication.

9.A Study on the Perinatal Outcomes of Natural Childbirth Led by Midwives: A Retrospective Study

Purpose:
The purpose of this study was to suggest the importance of natural childbirth by investigating the results of midwife-led perinatal periods and whether they were positive experiences for both individual women and their families.
Methods:
This retrospective study aimed to confirm the perinatal outcomes of midwife-attended vaginal births. Among 1,127 women who gave midwife-led birth in Gyeonggi-do, Seoul between January 1, 2019 and December 31, 2021, data were collected for 829 women. Records were excluded that were missing necessary data. Data on childbirth were analyzed using frequency, percentage, mean, standard deviation, and the chi-square test.
Results:
Among the mothers who underwent natural childbirth, 549 (66.2%) were multiparous. In addition, all 829 patients (100%) breast-fed their infants. First-degree perineal lacerations occurred in 29.2% of patients, and second-degree lacerations occurred in 48%. Non–high-risk subjects made up 56.0% of the study sample. The average bleeding amount was 281.44±4.50, and the average 1-minute Apgar score for infants was 8.25±0.02.
Conclusion
Compared to childbirth in a hospital, natural childbirth was associated with a lower risk of birth canal laceration, postpartum bleeding, and Cesarean section. In addition, the health statuses of the newborns were confirmed to be good . Based on the results of this study, there is a need for a strategy in which midwife-led natural childbirth is integrated into the local medical system, as well as a need to raise women's awareness of natural childbirth and positively influence the experience.
10.Glue Embolization of Lymphopseudoaneurysm for Chylous Ascites after Retroperitoneal Surgery

Objective:
To assess the safety and efficacy of lymphopseudoaneurysm (LPA) glue (n-butyl cyanoacrylate [NBCA]) embolization in the management of chylous ascites after retroperitoneal surgery.
Materials and Methods:
A retrospective analysis from January 2014 to October 2018 was performed in six patients (4 females and 2 males; mean age, 45.3 ± 14.2 years; range, 26–61 years) who underwent LPA embolization for chylous ascites developing after retroperitoneal surgery involving the perirenal space (four donor nephrectomies, one partial nephrectomy, and one retroperitoneal lymphadenectomy). After placing a percutaneous drainage catheter into the LPA or adjacent lymphocele, embolization was performed by filling the LPA itself with a mixture of glue and Lipiodol (Guerbet).
Results:
Daily drainage from percutaneously placed drains exceeded 300 mL/day despite medical and surgical treatment (volume: mean, 1173 ± 1098 mL; range, 305–2800 mL). Intranodal lymphangiography was performed in four of the six patients and revealed leakage in 2 patients. Percutaneous embolization of the LPA was successful in all patients using an NBCA and Lipiodol mixture in a ratio of 1:1–1:2 (volume: mean, 4.3 ± 1.1 mL; range, 3–6 mL). Chylous ascites was resolved and the drainage catheter was removed in all patients within 4 days after the procedure (mean, 2.0 ± 1.8 days; range, 0–4 days). No procedure-related complications or recurrence of chylous ascites occurred during a mean follow-up period of 37.3 months (range, 21.1–48.4 months).
Conclusion
Glue embolization of LPA has the potential to be a feasible and effective treatment method for the management of chylous ascites after retroperitoneal surgery.
