5.Antihypertensive Effect of Captopril on Essential Hypertension.

The antihypertensive effect of captopril was evaluated in 15 patients with mild to moderate essestial hypertension(Mean age : 55, Mean blood pressure : 171/102mmHg). Captopril was administered 25 to 50mg twice daily with or without hydrochlorothiazide by mouth according to our protocol which was presented in the text. All the patients were followed up to 12 weeks. Captopril with or without hydrochlorothiazide significantly lowered the systolic and diastolic pressure in almost all patients. The mean systolic and diastolic pressure were reduced to 134/86mmHg(-37/-16mmHg ; -22/-16%)(p<0.001). Heart rate did not change significantly. No unwanted effects were observed. We conclude captopril has exellent antihypertensive effect in most patients with mild to moderate essential hypertension.

6.A Case of Multiple Dermatofibromas in a Healthy Adult.

Multiple dermatofibromas are rare and some case reports show this disease has been related to generalized defective immune function: autoimmune disease and altered immunity, such as AIDS and systemic lupus erythematosus(SLE), can be treated with immunosuppressive agents. But there are no specific reports of this disease developing in healthy people. We describe a case of multiple dermatofibromas in a 38-year-old healthy man who showed no abnormality of the immune function.

7.Two Cases of Solitary Leiomyoma Cutis.

Leiomyoma Cutis is an unusual benign tumor of smooth muscle derivecl either from the arrector pili muscle, from the media of blood vesseles, or from smooth muscles of scrotum, labia or nipple. Two casea of leiomyoma cutis confirmed by H&E stain and Massons stain is presented, one with lesion on the left thigh and the ather with lesion of the left ankle. The first case was a 49 year-old female with solitary pinhead sized subcutaneous firm mass with intermittent pain and tenderess for 10 months, The second case waa a 36 year-old female with painful solitary peanut sized firm nodule which had pinkish center of 1 year duration.

8.Histologic study of Coral Template Wrapped with Perichondrial Flap.

Autogenous costal cartilage graft has been commonly used for reconstruction of auricular deformity. However, the risk of complication and discomfort at the donor site, as well as distortion of the graft due to morphological change in the cartilage have been serious drawbacks to this procedure. Previous studies examining the chondrogenic potential of perichondrium have suggested that perichondrium may be used as graft for cartilage reconstruction. When a perichondrial flap or a free perichondrium was used as graft, new cartilage formed appositional to the grafted perichondrium. However, the neocartilage was often irregular in shape and varied considerably in quantity. In this study, the feasibility of controlling the shape and the mass of neocartilage was investigated using coral, a porous biomaterial, as a template. A coral a template was wrapped with perichondrial flap from the ears of New Zealand white rabbits and placed into a subcutaneous pocket in the rabbits and placed into a subcutaneous pocket in the rabbit's back by incision. A total of 12 animals were used. Formation of new cartilage was later evaluated by gross and histological examination of the perichondrial flap and the coral template. New cartilage was formed in 11 animals. Immature chondrocytes were visible by 3 weeks after the surgery, and by 8 weeks the immature chondrocytes had formed a cartilage. New cartilage was formed only on the surface of the coral template. These results indicated that the shape and the mass of new cartilage may be controlled by using coral template. Therefore, the desired shape of cartilage may be achieved using a coral template of corresponding shape, and this may help in correcting subtle auricular contour defect and in correcting other structural defects that also require new cartilage formation.

9.CLINICAL EVALUATION ON THE MOUTH REHABILITATION USING DENTAL IMPLANTS.

The use of osseointegrated implants is an accepted procedure for the treatment of Total, or partial edentulism and offers good predictability of long-term success. Osseointegration implies a firm and direct interlocking between vital bone and screw-shaped titanium implants. There should be not to interposed tissue between fixture and bone. This study was undertaken to assess the clinical condition, complication, and prosthodontic aftercare of different implant systems. One hundred fifty-nine patients treated with a total of 503 endosseous implants (364 Steri-oss threaded type, 69 Integral cylinder with HA coated type, 35 Steri-oss threaded with HA coated type, 21 Steri-oss cylinder with HA coated type and 14 3i implant type), Most of the implant were placed in type B and C bone quantity and type 2 and 3 bone quality according to Lekhorm and Zarb. The success rate of Steri-oss threaded type during healing and function was 92%, Steri-oss threaded type with hydroxyapatite coated was 91%, Steri-oss cylinder type with hydroxyapatite coated was 90%, Integral cylinder type with hydroxyapatite coated was 90% and 3i implant type was 93%. One hundred twenty-nine patients had been treated with implant prosthesis. 79 of these patients had received a fixed type prosthesis and 50 patients had received a removable type prosthesis. There were no differences between the implant systems with regard to age, gender. Failures were associated with poor bone quality, smaller implant sizes, a surgical installation technique and stress distribution when in function. Visual analgoue scales recorded as satisfied results functionally and esthetically, but 15% dissatified with chewing ability.
