3.The Influences of Arteriosclerosis on the Development and Progression of Diabetic Retinopathy .

To evaluate the relationship between arteriolosclerosis and diabetic retinopathy, we graded the levels of diabetic retinopathy and arteriolosclerosis and compared each other in 255 patients with non-insulin dependent diabetes. And we also measured serum lipid levels and checked other risk fac-tors for diabetic retinopathy. SPSS was used for statistical analysis. Patients with advanced arteriolosclerosis were at higher risk of severe diabetic retinopathy. And patients with elevated plasma total cholesterol, LDL-C, TG had more severe levels of diabetic retinopathy than whose lipid levels were normal. These results suggested that advanced arteriolosclerosis and elevated serum lipid levels are associated with severe diabetic retinopathy. For preventing visual threatening complications of diabetic retinopathy, the degree of arteriolosclerosis and serum lipid levels should be carefully monitored in the evaluation of diabetic retinopathy.

4.Change of Plasma Glucose Levels in Term Neonates during the First 72 hours using the New Reagent Strip Method.

Blood glucose levels were measured in 89 healthy term neonates during the first 72 hours using the SureStep, a newly developed reagent test strip method by LifeScan. The blood samples were obtained by heel-stick puncture and blood glucose concentrations were monitored at birth(0), 2, 4, 6, 12, 24, 48, and 72 hours after birth. Mean and standard deviation of their measurement were compared according to postnatal hours and type of delivery. Comparison of significance between mean plasma glucose levels were made with the Wilcoxon rank sum test and significance level of 0.05 was used to determine which pair-wise comparisons were significantly different. The mean plasma glucose concentrations of first 6 hours were significantly lower than those of 12, 24, 48, 72 hours. This finding indicates that plasma glucose stabilization in healthy fullterm neonates can be expected with the feedings. The mean plasma glucose concentration at birth in the neonates of cesarean section (64.5+-8.06 mg/dl) was significantly lower than that of vaginal delivery (80.3+-20.7 mg/dl), but there were no significant differences after 2 hours. This may be due to the difference in prenatal care of obstetric department of Horamae hospital (C/S: midnight NPO and Hartmann solution, V/D: NPO with labor pain and 5% dextrose solution intravenously). In summary, the changes in perinatal care, especially prenatal fluid therapy and time of first feeding should be considered in defining neonatal hypoglycemia. Our data suggest that hypoglycemia should he defined as below 40 mg/dl during the first 6 hours and below 55 mg/dl thereafter.

5.The Effect of Intra-articular Injection of Steroid on Lysozyme Activity in Patient with Rheumatoid Arthritis

The lysozyme activities of knee joint fluid in normal Korean people, patients with rheumatoid arthritis, and patients with degenerative arthritis have been reported by Lee in 1974. This time I have studied the chronologic changes of lysozyme activity of knee joint fluid in patients with rheumatoid arthritis following intra-articular injection of steroid preparation (6-methylprednisolone acetate). The results obtained were as follows. 1. The synovial concentration of lysozyme in patients with rheumatoid arthritis was found to be 10.88 mcg/ml which is higher than normal value of 8.69 mcg/ml. 2. The lysozyme activity of joint fluid obtained 5 days after intra-articular injection of steroid prepatation was variable ranging from 7.40 mcg/ml to 10.0mcg/ml with the average of 8.97 mcg/ml, which corresponded to the relief of clinical manifestations. 3. The lysozyme activity of joint fluid obtained 10 days after intra-articular injection of steroid preparation decreased to 8.36 mcg/ml on average with the range from 7.60 to 9.30 mcg/ml which was in accordance with much relief of clinical manifestations. 4. The lysozyme activity of joint fluid obtained 20 days after intra-articular steroid injection was 9.55 mcg/ml on average with the range from 8.25 to 11.25 mcg/ml which was in accordance with the aggravations of clinical manifestations.

6.Osteonecrosis Occuring in the Medial Femoral Condyle due to Prolonged, Excessive Cortisone Therapy: A Case Report

Authors report a rare case of localized osteonecrosis occuring in the medial femoral condyle induced by prolonged steroid administration in a 48 years old male who had kidney transplantation surgery. Initially, he experienced clinical symptoms of discomfort and recurring effusion of both knees after transplantation surgery which did not respond, at all, to conservative treatment even in its early stage.

10.Chronic Midgut Volvulus Due to Movable Pancreas: 1 case report.

Incomplete fixation of pancreas, which may be associated with a wide mobility of root of SMA, result in chronic midgut volvulus. Movable pancreas is an uncommon form of incomplete intestnal fixation and rotation. Herein, we present a caae of movable pancreas. A patient was a 32-year-old male with the complaint of colicky intermittent abdomnal pain. The abdominal ultrasonography and CT showed a movable pancreas. The head of pancreas was located in the left of the abdominal aorta during the inspiratory time and left decubitus position. At surgery, pancreas was mobile and the root of mesentery was very weak and fibrotic. Duodenum and mesentery were fixed by 0.7 mm Dacron patch along the right peritoneal gutter and the retroperitoneum from ligament of Treitz to the ileocecal attachment. Duodenojejunostomy was performed for the prevention of SMA syndrome. The symptoms disappeared and recovery was prompt. Following the procedure, the patient remains asymptomatic until 10 months after operation
