1.Measurements of Mid-arm Circumference(MAC) and Mid-arm Circumference/Head Circumference(MAC/HC) Ratio as Indices of Nutritional Status in Newborn Infants.

Mid-arm circumference and mid-arm circumference/head circumference ratio(MAC/HC) were measured in 207 AGA(appropriate for gestational age) infants delivered at 26 to 42 weeks of gestation from January 1990 to December 1993 in Yeungnam University Hospital, Taegu, Korea. There were linear relationships between MACs and MAC/HC ratios and gestational age(MAC : y=03181x -2.2069, r=0.81, p<0.001 ; MAC/HC ratio : y=0.049x+0.1128, r=0.62, ; < 0.001). Using standard curves of MAC and MAC/HC ratio according to the gestational age, measurement of MAC or MAC/HC ratio can be a noninvasive, simple method to evaluate the intrauterine growth of newborn infants and the nutritional status of growing premature infants.

2.Angiokeratoma Circumscriptum: Successful Treatment with the Flashlamp Pulsed Tunable Dye Laser.

Angiokeratoma circumscriptum is present at birth or early childhood and is an uncommon dermatosis characterized by papules and small nodules that may coalesce to form plaques. Histopathologically, there are varying degrees of hyperkeratosis, papillomatosis, and irregular acanthosis. In the papillary dermis, greatly dilated capillaries are observed, The acanthotic epidermis encircles the vascular spaces(blood cysts) where, occasionally, organized thrombi may be found. The use of pulsed-dye lasers to treat cutaneous vascular lesions is based on the theory of selective photothermolysis. We report a case of an angiokeratoma circumscriptum in an 48-year-old woman for whom the flashlamp pulsed tunable dye laser proved to be a highly successful means of treatment.

3.Extramammary Paget's Disease of the Penis and Scrotum with a Renal Cell Carcinoma.

Extramammary Paget's disease is a rare cutaneous malignancy and frequently associated with an underlying adnexal carcinoma and perhaps with underlying internal malignancy. In our case, genital Paget's disease was followed by the discovery of renal cell carcinoma. We report a rare case of extramammary paget´s disease of the penis and scrotum which associated with a renal cell carcinoma.

4.Therapeutic Effects and Limitations of Lacrimal Endoscopy without Silicone Tube Intubation

Purpose:
To evaluate the effects and limitations of lacrimal endoscopy without silicone tube intubation in patients with epiphora.
Methods:
We conducted a retrospective chart review of 64 eyes in 49 patients who underwent lacrimal endoscopy between May 2021 and May 2022. The clinical characteristics, irrigation test results, lacrimal endoscopic findings, and type of surgery were analyzed.
Results:
The mean duration of symptoms was 31.2 months, and was significantly longer in the failure group than in the success group (p = 0.043). Irrigation tests showed passage, partial obstruction, and complete obstruction in 20 (31.3%), 16 (25.0%), and 28 (43.7%) eyes, respectively. Lacrimal endoscopy showed narrowing, mucus, fibrosis, granulation, and stones in 41 (64.0%), 12 (18.8%), 6 (9.3%), 3 (4.7%), and 2 (3.1%) eyes, respectively. Following lacrimal endoscopy, 32 (50.0%) eyes each were included in the success and failure groups. Preoperative irrigation test results did not affect the success rate (p = 0.203). Silicone tube intubation and dacryocystorhinostomy were performed in 5 (7.8%) and 8 (12.5%) eyes, respectively, because the symptoms did not improve after lacrimal endoscopy.
Conclusions
Lacrimal endoscopy, performed without silicone tube intubation, was effective in improving symptoms and may guide the choice of surgical technique, if required.
5.Effects of Extranasal Molding after Primary Cleft Lip Nasal Repair : Photogrammetric Analysis.

PURPOSE: In the correction of cleft lip, there have been various methods to minimize recurrence of the nasal deformity after primary nasal surgery. After cheiloplasty and primary nasal surgery, we tried to elongate the columella of the cleft side, to stretch the vestibular lining of cleft side, and to elevate the alar cartilage of the cleft side with a molding prong. METHODS: We had fifteen cleft lip patients; 12 unilateral cases(6.3-8.2 months), and 3 bilateral cases(3 -7.5 months). Immediately after primary repair of the cleft lip, the toboggan shaped molding prong was located to deep inside of vestibular web of the cleft side. It was persistently suspended by a silicone tube which was connected to the prong and the frontal scalp. The results were analyzed with Photoshop(R) photogrammetrically for 6-48 months with on average of 20.6 months. We measured the proportion index of columellar length-interalar distance for three times(preoperation, immediate postoperation, and postoperation) on the nasal base views. RESULTS: In unilateral, the index had a significant increase statistically between preoperation(10.73) and immediate postoperation(23.96). It is supposed that columellar length was reconstructed to 105.80% of normal side. But, it was decreased to maintain 87.7% of normal side in postoperation(20.54). The results were similar in bilateral. The linear scars by suture penetrating nose skin were not discernable. CONCLUSION: In summary, placement of the molding prong could elongate the reconstructed columella with some relapse postoperatively.

6.The Study of EGFR Expression on Malignant Skin Tumor and Molecular Labelling Using Gold Nanoparticles.

BACKGROUND: The potential benefits of a molecular characterization of cancer are clear. Because of this, there is pressing need to perform in vivo imaging of the molecular features of cancer. However, before designing an appropriate molecular targeting technique utiilizing a cancer-related target molecule such as epidermal growth factor receptor (EGFR), it is necessary to characterize the exact expression of the target molecule. OBJECTIVE: The objective of this study was twofold. Our first goal was to characterize the expression of EGFR in skin cancer, and second, to develop nanoparticles conjugated with antibodies, demonstrating their use as optical probes for detecting cancer cells in vitro. METHODS: We performed immunhistochemical analysis of EGFR expression in skin cancer using monoclonal antibodies. Gold nanoparticles were synthesized and allowed to conjugate to anti-EGFR in epithelial cancer tissue. Following an incubabtion period, we recorded surface plasmon resonance images using gold nanoparticles conjugated to anti-EGFR antibodies. RESULTS: Specific membranous binding of EGFR was detected in all of the 10 non-melanoma skin cancers. Surface plasmon resonance images using gold nanoparticles also showed light scattering around tumor cells. CONCLUSION: These ex vivo results demonstrate that optical imaging using gold nanoparticles can allow selective detection of human epithelial cancer cells. Our study demonstrates the potential of gold nanoparticles to target, probe, and illuminate cancer cells making them an effective biomolecular cancer detection tool.

7.A Case of a Patent Omphalomesenteric Duct Presenting with Meconium Discharge from the Umbilicus

The omphalomesenteric duct is a link between the primitive midgut and the yolk sac. Normally, the duct obliterates around 6 weeks of gestation, yet varying degrees of incomplete obliteration can take place in 1%–4% of infants. This study described the case of a newborn with a patent omphalomesenteric duct remnant fistula identified at birth with meconium in the umbilical cord. At birth, the infant presented meconium staining and meconium discharged within the umbilical cord. Physical examination and other examinations showed no other specific findings. The omphalomesenteric duct fistula was confirmed through the imaging study (abdominal ultrasonography, gastrografin enema). A surgery was carried out where the remnant was resected. The patient did well and was discharged soon after without complication.

8.A Case of Tick bite Caused by Ixodes monospinosus.

The occurrence of tick bites and related dermatoses have been rarely described in Korea. Ticks may cause various infectious diseases associated with dermatoses by transmission of the mediating pathogens such as virus, bacteria and rickettsia. We describe herein, to our knowledge, the first tick bite with Ixodes monospinosus in Korea, The patient, a 64-year old male, was bitten by a tick while climbing a mountain which is located in the Kanghwado. He had an asymptomatic, yellowish pea-sized tick attached on the right antecubital area for 7 days' duration, which was identified as an adult female of I. monospinosus by the scanning electron microscopic examination.

9.Analysis of Anatomical Relationship between Stensen's Duct and Buccal Branch of Facial Nerve.

BACKGROUND: When using the anterior approach for performing superficial parotidectomy, the first thing to do is to find the buccal branch of the facial nerve and the parotid duct. The buccal branch usually runs transversely with the parotid duct from the anterior border of the parotid gland. We wanted to check the relationship between the two structures during the operation and to get clinically helpful information. METHODS: Twelve patients with parotid mass were treated with superficial parotidectomy between May 2012 and August 2012. The outline of superficial and deep lobes of the parotid gland, parotid duct, and the buccal branch of the facial nerve were drawn on the transparent film by tracing the structures intraoperatively. RESULTS: In 7 (58.3%) of 12 cases, the buccal branch of the facial nerve was located more caudally than the parotid duct at the anterior border of the superficial lobe of the parotid gland. In 3 cases (25%), the buccal branch was located more cephalically than the parotid duct. The mean distance between two structures were 2.54+/-1.48 mm. In 11 cases, the parotid duct was located deeper than the buccal branch. CONCLUSION: The buccal branch of the facial nerve tends to be located more caudally than parotid duct and runs more superficially than parotid duct in all cases. We identified the relationship between the parotid duct and the buccal branch of the facial nerve during the operations on living subjects, not from the cadavers, so it would be a clinically helpful study which supplied more accurate anatomical information.

10.Factors Related to Sleep Time Underestimation in Obstructive Sleep Apnea: Retrospective Study Based on Polysomnographic Data

BACKGROUND: The perception of sleep time in obstructive sleep apnea (OSA) is not well understood, some studies have reported that subjects with OSA have abnormal sleep perception. We hypothesized that the severity of OSA would affect the sleep perception of patients with OSA and investigated the associated factors that affect the sleep perception in OSA. METHODS: Four hundred and sixty eight subjects with OSA were included in present study. Subjects with OSA were divided, depending upon their sleep perception. The first group included individuals who underestimated their time spent sleeping, the second group included those who did not underestimate their sleep time. The underestimation of sleep time is defined as the perceived total sleep time being less than 80% of that measured in polysomnography (PSG). All participants were analyzed their demographics, PSG parameter and questionnaires such as Beck Depression Inventory, Epworth Sleepiness Scale. RESULTS: Of 468 participants, 179 (38.2%) subjects were included in the group that underestimating sleep. Gender (female, odds ratio [OR]=2.01, 95% confidence interval [CI]=1.25–3.22), depression (OR=1.75, 95% CI=1.03–2.97) and proportion of slow wave sleep (OR=0.98, 95% CI=0.96–0.99) were related to the underestimation of sleep. CONCLUSIONS: The underestimation of sleep in OSA is not directly related to OSA severity. Gender, psychiatric disorder, and sleep architecture are associated with the underestimating sleep in OSA.
