1.Histological Changes of Innervated Muscles after neourolysis: An Experimental Study on Rabbit Sciatic Nerve

The normal vascularization of peripheral nervesand the vascular factor in peripheral nerve les ons have regained increasing interest among surgeons. So, several attempts have been made to assess the relative importance of the vasa nervorum and intrinsic longitudinal vascular plexuses of nerve in maintaining the blood supply.of a segment of nerve trunk. The purpose of our experiment was to determine in laboratory animals the maximum extent to which a nerve can be mobilized without impairing its vascular supply so much that nerve function is jeopardized. All our studies were carried out on both sides of the sciatic-tibial nerve of thirty-two rabbit which were anesthetized intraperitoneally with urethane. The experimental procedure differed in three groups, Group I in which the sciatic nerve was mobilized 3 cm in length, Group II was mob lized 7 cm length and Group III was mobilized 10cm long. The tibialis post. muscles of each group were analyzed at intervals ot 1,2,4,6 and 8 weeks after neurolysis. Each muscles were examined grossly and histologically after hematoxylin and eosin staining. Experimental studies showed that a peripheral nerve is a well vascularized structure with a considerable reserve capacity in its microirculation. The intrinsic collateral system is well developeed and experimental deta supported the view that peripheral nerves may be mobilized over a cons derable length with or the only minium interference with their microvascular flow. The results were as follows: 1. The first evidence of histologic change in the muscle fibers was in the sarcolemmal neclei. 2. Localized atrophy of muscle fibers were observed at the six weeks after neurolysis. 3. With increasing length of neurolysis, abnormal finding were developed in early stage. 4. Massive atrophy of muscle fibers were noted in the muscle fibers which neurolysed more 7cm.

2.A Case of Pericardial Tuberculoma.

We have experienced a case of pericardial tuberculoma, a very rare disease, with massive pericardial effusion in a 63-year-old veteran. He wdimensional echocardiography. Computed tomographic scans confirmed the presence of a pericardial mass and clinically unsuspected "lung mass". The presence of the lung mass led us a diagnostic and therapeutic dilemma. Surgery confirmed the pericardial mass which revealed tuberculosis by patholohy and loculated pleural effusion at the major fissure, so-called "phantom tumor", not a lung mass.

A rare case of traumatic bilateral anterior and posterior dislocstions of the hip wss occured by autamobile accident as a passenger injury. The right hip was dislocated anteroinferior to the righ scetabulum(neer the right obturator froamen), and the left hip was dislocated posterosuperior to the left acetsbulum with a ipsilateral femur head fracture caudsd to the fovea centralis(Fig. 1).

4.Multifocal Pyogenic Arthritis on four Joints: A Case Roport

Multifocal pyogenic arthritis more than four joints is very rare and considered to be related with some conditions such as immune deficiency, alcoholism, diabetes mellitus, steroid abuse, septicemia and gonococcal infection. In early stage, it is important to differentiate from acute rheumatic fever or acute rheumatic polyarthritis. Authors report a case of multifocal pyogenic arthritis on four joints simultaneously with good result.

5.A Clincal Study of Type IIIc Open fracture of the Forearm

In 1984, Gustilo subgrouped type III open fracture into III a, III b and III c according to the severity, soft tissue damage and vascular injury. Type III c open fracture presents serious problems especially possible amputation due to vascular insufficiency, so emergency vascular surgery with bone fixation by using plate, IM nail fixator is mandatory. We reviewed 10 cases of type III c open fracture of the forearm and evaluated the bone healing process according to fixation method, intramedullary nailing and plating treated at Orthopaedic Department of Sung-Ae General Hospital from Feb. 1986 to Aug. 1988. The folliwing results were obtained: l. All cases were industrial accidents, 9 cases in right, and the mean age was 24.1 ranging from 17 to 34. 2. There were 4 cases of the radial & ulnar artery rupture and 6 cases of the radial artery rupture, 7 cases were treated end to end anastomosis and 3 cases with vein graft. 3. The fracture level of ulna & radius was same in all cases, 7 cases in distal one-third, 2 cases in middle one-third and 1 case in proximal one-third. 4. The average time of union was 14.1 weeks in the radius and 14.5 weeks in the ulna. 5. The fixation methods were plating in 7 cases and IM nailing in 3 cases. 6. The complications were non-union in 1 case, osteomyelitis in 2 cases and refracture in 1 case. 7. The functional result was excellent in 1 case, satisfactory in 3 cases, unsatisfactory in 5 cases, and failure in 1 case according to the rating system of Anderson.

6.Natural History of MSA-Clinical Evidence for Single Disease entity.

BACKGROUND & OBJECT10NS: Multiple system atrophy(MSA) is a heterogenous system disorder affecting extrapyramidal, cerebellar and autonomic nervous system. Clinical spectrum is broad, and depending on the system affected, patients are classified into striato-nigral degeneration (SND), olivo-ponto-cerebellar atrophy (OPCA) and Shy-Draper syndrome (SDS). However, evolution of symptoms during follow-up usually occurs, stirring up a debate between "lumpers" and "splitters". Recent pathological documentation of intracytoplasmic inclusions support "lumpers" that MSA is a specific disease entity with specific pathology. The study was done to analyze the natural course of MSA, and examine whether they are separate or part of the same disease. METHOD: We obtained the clinical data of patients with clinically probable MSA by the criteria of Quinn (1994). In addition to review of medical records, all patients were phone-interviewed or examined personally. RESULTS: Forty four patients were included in the study (male 23, female 21). Mean onset age 52.9 years, and mean follow-up period 19.7 months. Nine patients died during follow-up (mean disease duration 5.2 years). The initial predominant features were parkinsonism in 40% (14/35), cerebellar dysfunction in 25.7% (9/35), autonomic dysfunction in 17.1% (6/35) and others in 17.1%. At the latest follow-up, parkinsonism were noted in 77.1%, cerebellar dysfunction in 88.6% and autonomic dysfunction in 80%. With progression, all the patients showed mixed clinical manifestations, the most common being combination of all 3(60%). CONCLUS10N: The data supports that SND, OPCA and SDS are part of the same disease process.

7.Epidermoid Cyst of the Fourth Ventricle.

The authors describe a case of intracranial epidermoid cyst with fourth ventricle involvement. A 43-year-old woman presented with progressive aggravating headache and cerebellar syndrome due to a large tumor in the fourth ventricle, that extended into the left side of foramen of Luschka. The histopathological workup following gross total removal of tumor had revealed the typical features of an epidermoid cyst. Four months after surgery, the cerebellar sings recovered to normal with a remarkable improvement of headache. Epidermoid cysts involving the fourth ventricle have been known to be one of the most rare diseases in the central nervous system. This rare case is discussed and a review of the relevant literature is presented.

8.A Computerized Axial Tomographic Study on the Measurements of Cerebral Ventricles and Sulci in Chronic Alcoholics.

The authors measured the lateral & 3rd ventricles and cortical sulci in computerized axial tomographic films of 15 male patients with chronic alcoholism and 16 controls, who had been admitted to Kyng Hee University Hospital from Jan. 1, 1979 to Mar. 31, 1983. The results were as following: 1. The average age of patients was 47.5 yrs., and that of conntrols was 34.5 yrs. The average duration of habitual drinking was 11.6 yrs., and complicated with delirium tremens, head trauma, liver diseases (46.7% each other) and seizure (33.3%). 2. The sulcal widening was found in 9 cases (60.0%) of patients (mean +/- S.D. = 6.7 +/- 1.76mm) and no cases in control group (mean +/- S.D. = 2.3 +/- 0.88mm). The difference between patients and control group was statistically significant, especially in twenties (P<0.05). 3. The lateral ventricular enlargement was seen in 12 cases (80.0%) of patients (mean +/- S.D. = 17.1 +/- 0.93mm), and 8 cases (50.0%) of control group (mean +/- S.D. = 14.7+/- 1.46mm). There was statistical significance, especially in forties (P<0.05). There were no statistical significance except cella media index (P<0.05), according to various measuring indices. The width of 3rd ventricle was 1.88 +/- 0.152mm (mean+/-S.D.) in patients and 1.12+/-0.311mm (mean +/- S.D.) in control group. And the difference was statistically significant. 4. Cerebral atrophy was noticed in 10 cases (67.0%) among 15 taken C.A.T. of brain.

9.Effects of Applying a Vibration Stimulation to Paretic Side and Non-Paretic Side on Balance and Gait in Stroke Patients: a Pilot Study

Purpose:
The purpose of this pilot study was to assess the feasibility of a proposed study “To investigate the effects of applying a vibration stimulation to the paretic and non-paretic sides on balance and gait in stroke patients” before proceeding with the main study.
Methods:
A total of 30 patients who had suffered a stroke and who met the selection criteria participated in this study. However, only 14 patients completed the study. The study subjects were randomly assigned to vibration stimulation on either both sides (B group) or the paralyzed side (P group). The interventions were applied for six weeks. The patients were assessed using the Berg balance scale (BBS), Timed up and go (TUG), 10-meter walk (10 MW), and 6-minute walk (6 MW) tests.
Results:
There was a significant difference before and after the application of vibration stimulation in the BBS, TUG, 10MW, and 6 MW tests in the B group (p < 0.05). Similarly, there was also a significant difference observed in the 10 MW and 6 MW tests in the P group (p < 0.05). The difference between the two groups was not significant in the BBS, TUG, 10 MW, and 6 MW tests (p > 0.05).
Conclusion
This pilot study suggests that it would be feasible to conduct a large-scale randomized controlled trial (RCT) to examine the effects of applying a vibration stimulation to the paretic and non-paretic sides, on the balance and gait in stroke patients.
10.The Effect of Phenylephrine on Cardiac Performance and Myocardial Oxygen Balance in Resuscitation from Hemorrhagic Shock.

BACKGROUND: The issue of using phenylephrine in hemorrhagic shock treatment has been controversial because it is known that phenylephrine improves the tissue perfusion by increasing arterial blood pressure but deteriorates the myocardium by increasing afterload and decreasing myocardial oxygen delivery via coronary vasoconstriction. This study was aimed to assess the effects of phenylephrine on hemodynamic variables, cardiac performance, and myocardial oxygen balance in resuscitation from hemorrhagic shock. METHODS: Twenty anesthetized dogs were randomly divided into phenylephrine group and control group. After inducing hemorrhagic shock, resuscitation was done with phenylephrine and 0.9% normal saline respectively. We measured hemodynamic indices, blood gas parameter and cardiac enzymes which indicate myocardial demage. RESULTS: In both groups, cardiac output and hemodynamic indices improved. In phenylephrine group, the systemic oxygen delivery and consumption was much higher and the myocardial oxygen extraction ratio was maintained at the lower level than the control group. In addition, the CK-MB was higher at the early phase of resuscitation and the troponin T was also higher than the control group during the whole period of resuscitation. Creatine kinase-MB increased during early resuscitation in phenylephrine group but kept decreasing after that and there's no difference between two groups. Troponin T was higher in the phenylephrine group after resuscitation. CONCLUSIONS: We concluded that phenylephrine improves myocardial oxygen balance and contractility without serious myocardial demage during resuscitation from hemorrhagic shock.
