1.Plasma Renin Activity on Postural Change and Blood Sodium in Essential Hypertension.

The response of plasma renin activity and Na, K content to physiological stimuli; supine, standing after liberal salt intake and salt loading, was observed in the normal human and patients with essential hypertension. The results are as follow: 1) The substance obtained from sample that exert contractile activity to rat colon, had blood pressure raising activity. Method in this experiment was able to detect angiotensin-II for concentration of 1 ng. 2) In normal, plasma Na content of supine state with liberal salt intake showed 142.2+/-1.8 mEq/L, but it was increased to 151.0+/-5.9 mEq/L after salt loading. In standing, plasma Na content showed 141.5+/-2.5 mEq/L with liberal salt intake. 3) In normal, plasma Na content of liberal salt intake showed 142.2+/-1.8 mEq/L in supine and 141.5+/-2.5 mEq/L in standing. The pasma Na content in supine after salt loading was significantly reduced by standing (143.7+/-1.5 mEq/L). 4) In normal, plasam renin activity of supine showed 7.3+/-1.6 mg/ml with liberal salt intake and 4.8+/-1.1 ng/ml with salt loading. The plasma renin activity in standing showed 12.8+/-3.1 ng/ml witn liberal salt intake and 7.3+/-1.1 ng/ml with salt loading. In both cases the salt loading decreased the plasma renin activity significantly. 5) In normal, the plasma renin activity of liberal salt intake or salt loading was significantly increased by standing compared with that of supine state. 6) In hypertensive patients with subnormal plasma renin activity, the plasma Na content in supine state showed 144.5+/-0.7 mEq/L with diuretics and 145.5+/-3.3 mEq/L with salt loading. In hypertensive patients with normal or high plasma renin activity, the plasma Na content in supine state showed 129.5+/-7.3 mEq/L with diuretics and 136.5+/-3.0 mEq/L with salt loading. In standing, plasma Na content was 132.5+/-3.1 mEq/L with diuretics and 135.7+/-2.5 mEq/L with salt loading. In hypertensive patients, the lower renin activity cases showed higher plasam Na content. 7) In hypertensive patients with subnormal renin activity, the plasma Na content tend to decrease by standing compared with that of supine state. 8) In hypertensive cases of low renin activity, the plasma renin activity in supine was 3.6+/-1.5 ng/ml with diuretics and 2.4+/-1.1 ng/ml with salt loading, and in standing, it was 6.0+/-2.1 ng/ml. with diuretics and 3.7+/-1.9 ng/ml with salt loading. In cases of high renin activity, the plasma renin activity in supine was 9.3+/-2.3 ng/ml with diruetics and 6.0+/-1.2 ng/ml with salt loading and in standing, it was 18.0+/-3.5 ng/ml with diuretics and 9.7+/-0.5 ng/ml with salt loading. 9) In patients with essential hypertension, we found that the plasma renin activity was incrased or not. It is suggest that the increased renin activity is not the cause of essential hypertension but is caused by essential hypertension.

4.A Clinicopathological Study of Fixed Drug Eruptions.

BACKGROUND: Fixed drug eruptions(FDE) are a cutaneous reaction characterized by one or more circumscribed lesions that recur at the same site in response to a given medication. OBJECTIVE: The purpose of this study was to find the clinical and histopathological characteristics of FDE and to compare early FDE with late FDE histopathologically. METHOD: We clinically investigated 54 cases of FDE that visited the department of dermatology at the Kyunghee medical center from January 1993 to December 1996. Among them, 31 patients had skin biopsies and were evaluated histopathologically on the basis of duration. RESULTS: The results were summarized as follows: 1. Development of FDE did not show any difference according to sex and was evenly distributed over all the ages. 2. The latent periods of FDE were diverse in appearance from 30 minutes to 10 days, but mostly, the skin lesions erupted within 4S hours. 3. The most common skin lesions were erythematous macules. 4. Distribution of the lesions came out as solitary: 20.4%, multiple: 79.6%. S3.7% of the multiple lesions were localized to a part of body, and 16.3% were distributed over the whole body. 5. The areas in which the eruptions developed were (in descending order): upper extremity(37.0%), hand(31.5%), trunk(24.1%), face(24.1%). 6. In most cases(68.6%), the size and the number of lesions were greater in recurrente, rather than in first attacks. 7. The histopathological findings commonly showed perivascular mononuclear cell(MNC) infiltration (100%), pigmentary incontinence(77.4%), basal hydrophic degeneration(71.0%), eosinophil infiltration in dermis(61.3%), etc. 8. The epidermal histopathological findings such as spongiosis, exocytosis of MNC, basal hydrophic degeneration, keratinocyte necrosis and subepidermal vesicles could be seen more frequently in early lesions than in late ones. CONCLUSION: In our study, we were able to obtain meaningful results based on data from the combination of clinical and histopathological investigations. This study may give help to understand the characteristics of fixed drug eruptions and to plan future studies.

6.A STUDY ON ACCUARACY AND DIMENSIONAL STABILITY ACCORDING TO IMPRESSION METHODS.

Accurate reproduction of intraoral tissue is essential in the fabrication of fixed prosthesis. This selection of impression material and method is very important. In this study, vinyl polysiloxane type impression material Panasil was used. Three impression methods ; one-step technique, individual tray technique, and individual tooth tray technique using the vinyl polysiloxane type bite registration material Futar Occlusion, were used to take horizontal and vertical impression. Improved stone models were fabricated. The amount of occlusal surface discrepaneies, interabutment distance discrepancies, amount of marginal defect were evaluated with the X-PLAN360d and photoscanning. The results were as follows. 1. There was no significant difference in occlusal surface according to the directions and techniques of impression taking. 2. There was no significant difference in interabutment distance discrepancies according to the directions and techniques of impression taking 3. There was no significant difference in marginal discrepancies according to the direction of impression taking but there was significant difference between one-step technique and individual tray technique, individual tooth tray technique (P<0.05). Taken together, these results suggest that individual tray technique and individual tooth tray technique are more accurate for impression taking than one-step technique.

9.Transpedicular Screw instrumentation and Spinal Fusion after Laminectomy in Lumbar Spinal Stenosis

Lumbar spinal stenosis is a common category of spinal disease in which a decrease in the volume of the spinal canal results in compression of the neural elements. Its pressure symptoms are characterized by vague back-pain and chronic sciatica. The goals of surgical treatment in lumbar spinal stenosis are the relief of pain and the preservation or restoration of neurologic function. Extensive laminectomy in lumbar spinal stenosis results in postoperative instability and postoperative vertebral subluxation with pain, restricted movement, and further neural compression occurs after decompressive laminectomy. Therefore internal fixation devices have been used in these circumstances to prevent postoperative instability while the fusion being consolidated. From February 1988 to October 1988, 36 patients of lumbar spinal stenosis had undergone operations with decompressive laminectomy and segmental spinal instrumentation with transpedicular screws at the department of Orthopaedic Surgery, Yonsei University College of Medicine. In 18 patients degenerative changes in lumbar spine were the principal aetiologic factor:7 patients had spondylolisthetic spinal stenosis:ll patients had combined spinal stenosis. All patients were followed up for periods varying between twelve and ninteen months. According to Kim's criteria, the postoperative results were as follows;excellent in 17 cases(47.2% ), good in 15 cases(41.7% ) and fair in 4 cases(11.1% ). The study revealed that segmental spinal instrumentation with transpedicular screws provided a rigid fixation, early ambulation, minimal fusion and reducing of recovery period were possible. In conclusion, segmental spinal instrumentation with transpedicular screws is a good and reliable method of stabilization after extensive decompressive laminectomy.

10.Tne effets of anti-inflammatory ageets on bloed-aqueous-barrier in experimental uveitis.

The purpose of this paper is to study the effects of anti-inflammatory agents on the blood-aqueous-berrier in experimentally induced uveitis. 10% solution of sodium fluorescein was injection intravenously in the ear vein of rabbits (0.25 ml/kg) and fluorescein curve was drawn after the method of Amsler and Huber. Experimental uveitis was induced by the injection of 0.2 ml of human serwn into the vitreous body. After uveitis had been induced. three kinds of anti-inflammatory agents-Prednisolone, Benzyron and Methotrexate-were Biven oralIy. In control animals in which no uveitis was induced, no changes in fluorescein curve were demonstrated before and after the administrations of drugs. In animals with uveitis, maked increases of the blood-aqueous barrier were noted. When Benoyron and Prednisolone were given to these animals, normaliration of the fluorescein curves were demonstrated. Methotrexate had no effects on the fluorescein curves in uveitis animals, showing no improvements in the permeability of uveal vessela. This ineffectiveness seemed to be due to the resistance of rabbits to methotrexate.
