2.Clinical Observation of The Spianl Tuberculosis

In the developed countries in Europe and the United States, The spinal tuberculosis is of no serious problem due to the developed anti-tuberculotic agents and the advanced operative techniques. However, in Korea poor economic and other conditions prevent many patienta from getting proper treatments for spinal tuberculosis. We analysed 45 cases of spinal tuberculosis who were treated at our department during three years from June, 1972 to June, 1975 by operative treatment and being compared with these results that obtained conservative treatment. The results were obtained as follows; 1. There was most common in below 9 years old (28.9%) and common age incidence was below 35 years old (68.8), The sexual incidence was 22 in male and 23 in female. 2. Lower thoracic and upper lumbar were mostly involved, i.e thoracic (52.2%), lumbar (32.7%), thoraco-lumbar in orders. 3. Two affected vertebra was most common in 33 cases, number of affected vertebra averaged 2.5. 4. In laboratory finding, E.S.R. that was 50–60 mm/hr. in average of then admission wss shifted as normal range of 10–20 mm/hr. after cure of 5–6 months. 5. In 28 cases of operative treatment, anterior spinal fusion was done in 19 cases (67.9%), posterior spinal fusion in 8 cases(28.6%), one case(3.5%) was treated by thoracotransversectomy. 6. In consequence of pathologic exaraination and A.F.B. stain, the case of tuberculotic constitution was 18 in tissue pathology, and that of positive reaetion was 9 in direct smear for A.F.B. 7. The case that tuberculosis was limited at spine and was not found in other organs was 27, it got from 60% of the whole. 8. Average kyphotic angle of the thoracic region after operation was 4.3°, in 6 months was reduction of 1.8°, that of lumbar region after operation was 1.2°, in 6 months was reduction after operation was reduction of 0.7°, as change of radiologic kyphosis. 9. As a result of being cured during 18 months by operative or conservative methods, not a case is found in activity.

3.Primary Stent Placement for Chronic Spontaneous Infrarenal Abdominal Aortic Dissection: A Case Report.

Spontaneous infrarenal abdominal aortic dissection (SIAAD) is a rare entity with various clinical presentations. We recently encountered the even rarer condition of a female patient suffering from chronic SIAAD with multiple intimal flaps and prominent lumbar artery collaterals; this all caused stenotic changes of the infrarenal abdominal aorta and produced progressive lower extremity pain and claudication in both her legs. This patient's condition was successfully managed by primary stent placement followed by balloon angioplasty.

4.Effect of Antioxidants on the Incidence of 7, 12-dimethylbenzanthracene-induced Mammary Tumor in Rats.

The inhibitory effect of selenium, vitamin E, and BHA on DMBA-induced mammary tumorigenesis in rats was investigated. Dietary vitamin E (200 IU/Kg diet) alone could not reduce the tumor incidence at 25 weeks after DMBA administration (10mg DMBA/rat) when selenium was deficient. Selenium supplementation (2ppm in drinking water) to rats fed a practical diet (0.17 ppm Se) reduced the tumor incidence to 14.3% from 75% at 27 weeks after DMBA administration. Dietary supplementation of BHA (0.75%) also reduced the incidence of DMBA-induced mammary tumor to 42.9% at 27 weeks after DMBA-treatment. Rats fed a diet deficient in both selenium and vitamin E contained significantly lower glutathione peroxidase activity and higher malondialdehyde in muscle. However, supplementation of selenium or BHA to the rats fed a practical diet did not alter the malondialdehyde content and glutathione peroxidase activities in muscle, skin and mammary gland. Dietary selenium increased the tissue selenium level. DMBA-induced mammary tumorigenesis was reduced by antioxidants tested but the anticarcinogenic effect of selenium or BHA seems to be independent of glutathione peroxi-dase activity.

5.The Usefulness of CT-guided Fine-needle Aspiration of Lymph Nodes in Uterine Cervical Carcinoma.

PURPOSE: The purpose of this study was to evaluate the usefulness of CT-guided percutaneous needle aspiration(PCNA) of paraaortic and pelvic lymph nodes in patients with uterine cervical carcinoma. MATERIALS AND METHODS: CT-guided PCNA was performed in 18 patients with treated cervical carcinoma. initial clinical stages were CIS in one, lb in three, lib in ten, and IIIb in four cases. We used 20 gauge Westcott needles for aspiration. Mean depth from skin to lymph nodes was 10.0cm in paraaortic group (n=13) and was 7.9cm in pelvic group (n=5). The size of lymph nodes ranged 1.0-3.0cm (mean :1.8) and 1.5-5.0cm (mean :2.6cm), respectively. RESULTS: All cases with paraaortic lymph node enlargement were proved to be metastatic lymphadenopathy. In five cases with pelvic lymph node enlargement, three were proved to be malignancy and two were negative. Among 16 cases with metastatic lymphadenopathy, eight patients were treated with chemotherapy, five with radiation therapy, and three with chemotherapy and radiotherapy. In two cases with negative results lymph nodes were disappeared or unchanged on follow up CT scans. No complications were encountered during CT-guided PCNA procedure. CONCLUSION: CT-guided PCNA of paraaortic and pelvic lymph nodes is a useful method in determining metastasis from cervical carcinoma and in planning further treatment.

6.An Observation on the Mortality Rates of Transport Accidents in Korea.

This paper describes the incidence of transport accident for the period, 1955-1965. Transport accidents were classified into three categories, viz. railway(WHO Classification of Diseases, E-802), watercraft(E850-E858) and motor vehicle accidents(E810-E835, E840-E841, E844-E845). Crude data on the subject were collected from the various sources of Government Statistical Books including Statistical Year Books edited by the Central Office of Economic Planning Board, Annual Police Reports by the Ministry of Home Affairs, and the national and local associations for road traffic safety. From the data incidence and mortality rates by year, month and local province were computed and other variables relevant to the epidemiology of accidents were observed. The following summary could be drawn: 1. Death rates due to transport accidents per 100,000 population were 12.3 for 1955 and 9.7 and 1965. The incidence of injury due to the same cause were 34.0 for 1955 and 35.9 for 1965. 2. Death rates by transportation vehicle showed 9.0 due to motor vehicle accidents, 1.7 due to water-crafts, and 1.6 due to railway trains for 1955. In 1965 death rates were 6.0 due to motor vehicles, 1.2 to water-crafts and 2.4 to railway. 3. Seasonal distribution of transport accidents revealed that car accidents occur more frequently in spring and fall seasons while ship accidents do in winter and train accidents more in summer. 4. Both car and ship accidents slightly decreased during the past decade, 1955-1965, whereas the accidents of railway trains showed a tendency of increase. 5. Although the survey on railway accidents excluded the injuries of passengers or railway employees corresponding to WHO classification of diseases, E801, due to inaccuracy of data, it is roughly estimated that the same number of causalities as the incidence among pedestrians or any other than passengers or employees assumed to be at work(E 802).

8.A study on dermatologic diseases of workers exposed to cutting oil.

We investigated the 1,004 workers who worked in a automobile factory to study the epidemiologic characterist of dermatoses due to cutting oils. Among the workers, 667(66.4%) answered the questionnaire. They are belong to 5 departments of the factory-the Engine-Work(86), Power train Assembly(17). We measured the oil mist concentration in air of the departments and examined the workers who had dermatologic symptoms. The results were follows; 1) Oil mist concentration; Of all measured points(52), 9 points(17.2%) exceeded 5mg/m3-the time-weighed PEL- and one department had a upper confidence limit(95%) higher than 5mg/m3. 2) Dermatologists examined 213 workers. 172 of them complained any skin symptoms at that time-itching(32.5%), papule(21.6%), scale(15.7%), vesicle(12.5%) in order. The abnormal skin site found by dermatologist were palm(29.3%), finger & nail(24.6%), forearm(16.2%), back of hand(8.4%) in order. 3) As the result of physical examination, we found that 160 workers had skin diseases. Contact dermatitis was the most common; 69 workers had contact dermatitis alone(43.1%), 11 had contact dermatitis with acne(6.9%), 10 had contact dermatitis with folliculitis(6.3%), 1 had contact dermatitis with acne & folliculitis, and 1 had contact dermatitis with abnormal pigmentation. Others were folliculitis(9 workers, 5.6%), acne(8, 5.0%), folliculitis & acne(2, 1.2%), keratosis(1, 0.6%), abnormal pigmentation(1, 0.6%), and non-specific hand eczema(47, 29.3%). 4) The prevalence of any skin diseases was 34.0 per 100 in cutting oil users, and 13.3 per 100 in non-users. Especially, the prevalence of contact dermatitis was 23.0 per 100 in cutting oil users and 4.3 per 100 in non-users. 5) We tried patch test(standard series, oil series, organic solvents) on 49 patients to differentiate allergic contact dermatitis from irritant contact dermatitis and found 20 were positive. 6) In a multivariate analysis(independent=age, tenure, kinds of cutting oil), the risk of skin diseases was higher in the water-based cutting oil user and both oil user than non-user or neat oil user(odds ratio were 2.16 and 2.78, respectively). And the risk of contact dermatitis was much higher at the same groups(odds ratio were 5.16 and 6.82, respectively).

9.Properties of Hepatitis B Virus Associated DNA Polymerase.

The nature of hepatitis B virus (HBV) particle associated DNA polymerase was studied in relation to various enzyme inhibitors including antiviral agents. HBV DNA polymerase required high concentration of MgCl2(> 30 mM) and neutral pH for its full activity. p-chloromercuribenzoate was a strong inhibitor (85% inhibition at 1 mM) but N-ethylmaleimide had much less inhibitory effect (20% inhibition at 10 mM). Phosphonoformic acid showed the greatest inhibitory effect on HBV-DNA polymerase (almost complete inhibition at 100 microM) among phosphocompounds tested. Adenine arabinoside triphosphate (ara-ATP) and cytosine arabinoside triphosphate (ara-CTP) were competitive inhibitors with respect to their respective deoxyribonucleoside triphosphate (dATP and dCTP). Ara-CPT was a stronger inhibitor of HBV-DNA polymerase compared to ara-ATP. Ki values for ara-ATP and ara-CTP were 15.0 microM and 11.7 microM , respectively. HBV-DNA polymerase is characteristic in its ionic requirements and susceptibilities to certain inhibitors.

10.Predicting Factors of Discontinuation of medication after Cognitive Behavioral Therapy for Panic Disorder.

OBJECTIVE: The authors experienced that cognitive begavioral therapy(CBT) could replace medication for controlling panic attacks and anticipatory anxiety symptoms. The objective of this study was finding out predicting factors of discontinuation of medication after CBT for patients with panic disorder. METHOD: A hundred forty-eight patients who met DSM-IV criteria for panic disorder with or without agoraphobia for at least 3 months had completed 12 weekly sessions of Panic Control Therapy(CT ; Barlow et al). Eighty-one patients who could discontinue medication and sixty-seven patients who could not discontinue medication were measured with several scales as the pre- and post-treatment assessment. The scales were Beck Depression Inventory(BDI), Clinical Global Impression(CGI), Spiellberger State Anxiety Inventory(STAI-state), Anxiety Sensitivity Index(ASI), Body Sensation Questionnaire(BSQ), Panic Belief Questionnaire(PBQ), Agoraphobic Cognition Questionnaire(ACQ), Fear Questionnaire(FG), Toronto Alexithymia Scale(TAS). RESULTS: At the pre-treatment assessment, the scores of BDI, CGI, STAI-state, ACQ, BSQ were higher in the patients who could discontinue medication than in the patients who could not discontinue medication(t=-2.68, t=-4.88, t=-3.07, t=-3.68, t=-3.35, p<0.01). CONCLUSION: Patients with panic disorder who were less depressed, less anxious, less agoraphobic and who had less negative cognitions for the bodily sensation and who had higher scores in the therapist's assessment could discontinue their medications.
