Calcification is exceedingly rare in schwannomas. In the literatures, we found only three reports of schwannomas with calcification. We report two cases of intracranial calcified schwannomas, one in the anterior cranial fossa and the other in the middle cranial fossa.

Nontyphoidal Salmonella are gram negative bacilli organism, which may induce systemic infection such as febrile enteritis, bacteremia, and osteomyelitis. Main route of infection is known as food but also possible through reptile, amphibian, and fish raised as pets in the house. There is no known cases report of Salmonella infection through pets in Korea and also rare in the overseas. We report 2 patients who visited Severance children's hospital with chief complaint of fever and diagnosed as nontyphoidal salmonellosis. Each case had a history of raising turtle or tropical fish with possibility of Salmonella infection through these pets. Increasing incidence of raising pet reptile and fish lately, contact precaution and proper prevention and control of Salmonella infection of these pets especially in children under 5 years old are necessary due to higher risk of serious complications of salmonellosis.

Thirteen patients were operated for cubital tunnel syndrome and followed for an average of 26 months postoperatively. Ten patients had a history of relevant trauma and three patients had degenerative osteoarthritis of the elbow. The average duration of symptoms was 18 months (range, 2 to 96 months). Diagnosis was made by physical examination, electromyography and nerve conduction study. Among these, nerve conduction study was found to be the most valuable diagnostic method for the patients with atypical clinical findings. Most of the operations were performed by anterior transposition of the ulnar nerve. At the most recent follow-up, the result was excellent in two patients, good in eight, and fair in three; thus ten patients(77%) showed satisfactory results. The rating system for ulnar neuropathy based on sensory, motor dysfunction and pain was useful for evaluating the operative results. The postoperative gain of score for pain and sensory function were larger than that of motor function. Factors known to influence the result of the operation (age, duration of symptom, history of trauma, method of operation) did not effect the outcome in this study. For successful operation, the ulnar nerve must be thoroughly examined, all possible levels of compression must be released and new foci of compression must be created.

Authors reviewed clinical records of 92 patients with tetanus from January 1974 to December 1983 at Severance Hospital and the following results wre obtained. 1) There were about 2.2 times more cases of neonatal tetanus than that of nonneonatal. The admitted cases of tetanus decreased in 1980's compared to those of 1970's. 2) The male versus female ratio was 3 : 1 not only in nonneonatal but also in neonatal teanus. 3) There was a tendency of higher mortality when the site of injury was on the central part of the body than on the distal. 4) Most cases of tetanus developed after the injury with bactrial contamination as expected, but a few cases are due to only trivial injury such as acupuncture or injection without overt contamination. 5) There was a close relationship between the incubation period and mortality in neonatal tetaus but no such definite relationship in nonneonatal tetaus. 6) The duraton of admission was about 3 weeks and the majority of deaths occureed in less than one week of admission. 7) There was no statistically significant difference in mortality between the group who used tetanus antitoxin from animal and that who used human tetanus immuneglobulin.

Blood flow generally decreases to areas of the lung that become hypoxie, thereby reducing pulmonary shunt and preventing arterial hypoxemia. The mechanism of blood flow reduction in the hypoxic area is believed to be hypoxic pulmonary vasoconstriction(HPV). Over the years, serious attention has been given to the question of whether hypoxia of the airway(alveolar hypoxia) is the only way to elicit pulmonary vasoconstriction. the current consensus favors this traditional view. Even though the usual experiments have succeeded in proving that a large drop in alveolar PO2(PAO2) does elicit pulmonary vasoconstriction, they have not excluded the possibility that a sufficient drop in mixed venous PO(PvO2) may have the same eonsequenee. The purpose of this study is to test the hypothesis that a decrease in PvO2 may elicit hypoxic pulmonary vasoconstriction. For the selective change of PvOwe used venovenous extracorporeal circulation(VVECMO), a technique which had been tested for adequacy in hemodynamics and oxygenation in preliminary studies on 6 mongrel dogs. In eight pentobarbital-anesthetized, mechanically ventilated mongrel dogs, stepwise reductions in oxygen concentration(F1O2,) from 0.21 to 0.15 and 0.10 caused remarkable HPV. The mean pulmonary arterial pressure(MPAP) and the pulmonary vascular resistance(PVR) were increased from 17.3+/-3 mmHg and 185+/-64 dyne sec cm(-5) in normoxia(F1O2=0.21) to 30+/-6 mmHg and 401+/-257 dyne sec cm in severe hypoxia(F1O2=0.10, P(A)O2=30+/-9 mmHg), respectively ; while, in moderate hypoxia(F1OP2=0.15, P(A)O2=53+/-12 mmHg), only the MPAP increased and the increase was from 17.3+/-3 mmHg to 23+/-3 mmHg. The selective increase in PvO2without any change in P(A)O2 by extracorporeal veno-venous bypass from 37+/-6 mmHg and 22+/-4 mmHg to 54+/-8 mmHg and 45+/-5 mnHg in moderate and severe hypoxia, respectively, depressed HPV: decrease in MPAP in moderate hypoxia(MPAP=21+/-4 mmHg) and decrease in MPAP and PVR in severe hypoxia(MPAP=23+/-5 mmHg, PVR=319+/-228 dyne sec * cm(-5)). This study indicates that the mixed venous oxygen tension is an important determinant of the magnitude of hypoxic pulmonary vasoconstriction in diffuse alveolar hypoxia. Moreover, the method used in this study and the result might be useful other pulmonary pathophysiological studies and clinical situations with acute diffuse alveolar hypoxia, such as acute respiratory failure and chronic obstructive pulmonary diseases.

MAGE (melanoma antigen gene) is a tumor specific shared antigen, presented by HLA class I molecules, which is recognized by cytotoxic T lymphocytes. MAGE proteins are expressed in malignant tumor cells, in contrast to no expression in normal or benign tissues except for testis and placenta. MAGE might be a potential target for immunotherapy of malignant tumors. However, its biological aspects associated with cell cycle are not yet described. The flow cytometry is a useful tool for objective and quantitative analyses of heterogenous tumor cell population. To understand the status of MAGE related to cell cycle and its relationship with p53 as the G1 checkpoint regulator, p21, and PCNA as a proliferative index, we investigated expression of MAGE-3 protein, mutant p53, p21, and PCNA by flow cytometry and immunohistochemical stain. In addition, double stains for MAGE-3/p53, p53/PCNA, and p53/p21 were analysed with bivariate flow cytometry. DNA histograms using MAGE-3/PI (DNA) and p53/PI (DNA) were also analysed. The cell line (PNUH- 12) used for this study originated from a hypopharyngeal squamous cell carcinoma, which has point mutation (exon 7, C-->G) of p53. The expression rate of MAGE-3 was 83%, PCNA 85%, and p53 81%. No expression for p21 was identified. MAGE-3 was expressed in cytoplasm, while both PCNA and p53 were expressed in nuclei of tumor cells. With bivariate analyses, coexpression rates of MAGE-3/p53 and p53/PCNA were 0.96 and 0.97, respectively. Both MAGE-3 and p53 showed constantly high level throughout the cell cycle. These results suggest that expression of MAGE-3 and mutant p53 is not dependent on the cell cycle. p21 seems to be inactivated.

The expression of MAGE proteins are confined to malignant tumor tissues, except for the normal testis and placental tissues. Therefore, MAGE may be a potential target for immunotherapy of malignant tumors. However, biological aspects associated with the cell cycle are not yet described. MATERIALS AND METHODS: The material used for this study was a novel human squamous cell carcinoma cell line (PNUH-12) from the hypopharynx, which had one point mutation of 78th base, C to G, in exon 7 of p53 gene. To understand the role of MAGE in relation to cell cycle and its relationship with p53 as the Gl checkpoint regulator, the expressions of MAGE-3 protein and mvtant p53 (Mtp53) were accessed by flow cytometry and immunohistochemistry. Double stains for MAGE-3/Mtp53 was analyzed with bivariate flow cytometry. DNA histograms using MAGE-3/PI (DNA) and Mtp53/PI (DNA) were also analyzed. RESULTS: The expression rate of MAGE-3 and Mtp53 were 83% and 85%, respectively. MAGE-3 was expressed in cytoplasm, while M:p53 were expressed in the nuclei of the tumor cells on the immunohistochemical sections. With bivariate analyses, coexpression rate of MAGE-3/Mtp53 was 0.96, and MAGE-3 and Mtp53 constantly showed high levels throughout the cell cycle except Go. CONCLUSIONS: These results mean that (I) MAGE-3 might have yet unknown relationship with mutant p53, (2) expressions of MAGE-3 and Mtp53 are not dependent on the cell cycle in PNUH-12 hypopharyngeal squamous carcinoma cell line, and suggest that MAGE-3 might have a role as important as p53 during the development of malignant tumors.
