1.Pathological Analysis of the Basal Cell Carcinoma.

Though basal cell carcinoma is the most frequent and increasing malignant tumor of the skin in Korea, its pathological analysis has been done only on the small numbers. So, we did a comprehensive pathologic study on the 283 patients with basal cell carcinoma diagnosed in the Department of Pathology, Seoul National(233cases) and Chung-Ang(50cases) University Hospital during 1975-1992. The age distribution was ranged from 15 to 84 years with highest incidence rate in the age group of 5th-8th decades and 83.7% of all patients were over 40 years of age. Sex difference was not noted. The most common site was face occuring in 235 out of 283cases(83.0%) especially in the eyelid(25.5%), nose(17.9%) and cheek(16.6%). The most frequent histopathologic type was solid type(54.0%) followed by mixed(23.9%), adenoid(7.5%), and metatypical(4.7%). Among 51 mixed type, all showed solid components with adenoid(51.0%) followed by morphea(25.5%) and metatypical type(13.7%). And among 14 recurrent cases, solid type is found in 50% of cases. The characteristic clinicopathological findings are solid arrangement of tumor cells with various histological pattern and predominant occurence on the face.

2.A Case of Extrapelvic Endometriosis with Abdominal Pain in a 13-Year-Old Girl.

Endometriosis is defined as the presence of endometrial tissue, e.g. gland and stroma, outside the uterus. Extrapelvic endometriosis is a rare condition but it occurrs in the gastrointestinal tract, kidney, pancreas, umbilicus, perianal area, abdominal scar tissue, lung or central nervous system except spleen. We have experienced a case of extrapelvic endometriosis in an adolescent girl, who complained of severe left upper quadrant abdominal pain and vomiting. The lesion was diagnosed by abdominal ultrasonography and computerized tomography, and confirmed histologically by exploratory laparotomy and mass excision biopsy. During menstration after surgery, she had no complaint of abdominal pain nor dysmenorrhea. We report this case with a review of associated literature.

3.Association between Tuberculin Responses and Serum IgE.

PURPOSE: Atopy is a state of allergic response, mediated by IgE, to common environmental allergens. In recent decades, there has been an increase in prevalence of atopic disorders in children in developed countries. The factor associated with the rise of atopy incidence may be declining exposure to some infection opportunities. The aim of this study is to determine if the immune response to Mycobacterium tuberculosis can suppress atopy by investigating the association between tuberculin responses and serum IgE. METHODS: We divided the patients into two groups according to tuberculin skin test. Group A showed positive response (induration diameter > or =10mm), while Group B showed negative response (induration diameter< l0mm). A comparison was made between the two groups in terms of age, sex, serum total IgE and allergen-specific IgE. RESULTS: Twenty-one patients comprised Group A and 72 comprised Group B. The mean serum total IgE concentration and the number of patients showing positive MAST in Group B was significantly higher than that of Group A (P<0.05). CONCLUSION: In our study, there was an inverse association between delayed hypersensitivity to Mycobacterium tuberculosis and atopy.

4.A Case of Primary Gastric Lymphoma in Puberty.

Primary gastric lymphoma is relatively uncommon, accounting for 3% to 8% of all malignancies arising in the stomach. The most common symptom is abdominal pain, closely followed by weight loss, anorexia, weakness due to anemia, nausea, and vomiting. The diagnosis of gastric lymphoma usually requires a biopsy at the time of gastroscopy or laparotomy. Microscopically, the vast majority of gastric lymphoid tumors are non-Hodgkin's lymphomas of B cell origin. Survival rates for all types of gastric lymphoma generally exceed those for adenocarcinoma and other malignancies of the stomach. We experienced one case of primary gastric lymphoma in puberty with a brief review of the literature.

5.Lysophosphatidylcholine induces azurophil granule translocation via Rho/Rho kinase/F-actin polymerization in human neutrophils

Translocation of azurophil granules is pivotal for bactericidal activity of neutrophils, the first-line defense cells against pathogens. Previously, we reported that lysophosphatidylcholine (LPC), an endogenous lipid, enhances bactericidal activity of human neutrophils via increasing translocation of azurophil granules. However, the precise mechanism of LPC-induced azurophil granule translocation was not fully understood. Treatment of neutrophil with LPC significantly increased CD63 (an azurophil granule marker) surface expression. Interestingly, cytochalasin B, an inhibitor of action polymerization, blocked LPC-induced CD63 surface expression. LPC increased F-actin polymerization. LPC-induced CD63 surface expression was inhibited by both a Rho specific inhibitor, Tat-C3 exoenzyme, and a Rho kinase (ROCK) inhibitor, Y27632 which also inhibited LPC-induced F-actin polymerization. LPC induced Rho-GTP activation. NSC23766, a Rac inhibitor, however, did not affect LPC-induced CD63 surface expression. Theses results suggest a novel regulatory mechanism for azurophil granule translocation where LPC induces translocation of azurophil granules via Rho/ROCK/F-actin polymerization pathway.
6.Follow-up Management State of Lead Battery Workers in Periodic Health Examination.

To evaluate the follow-up management state and related factor of lead battery workers in periodic health examination as part of program of group occupational health service, author studied 293 workers with questionnaire on knowledge of results and follow-up management state and related factors, and compared the responses to their periodic health examination result charts. The results were as follows: 1. 252(86%) workers responsed that they had received the health examination result chart, but only 116(39.6%) workers responsed that they had been educated or explained about the result of health examination, and 11(57.9%) workers among 19 workers with non-occupational disease D, 101(44.3%) workers among 228 workers with non-occupational disease C, and 19(28.4%) workers among 67 workers with occupational disease C knew accurately their health examination results. 2. 78(24.8%) of the workers responsed that they had follow-up management, and contents of follow-up management were follow-up(36.6%), out-patient treatment(31%), change worksite(8.5%), temporary retirement(7.0%) and others(16.9%). 3. Most of the workers responsed that the health examination were necessary, but three-fourths of the workers responsed that the health examination had been superficial or that they didn't know. 4. In this study, follow-up management show significant association with only explanation or education about health examination result chart.

7.Relationship between Chest Radiography and Cell-mediated Immunity in Mycoplasma pneumoniae Pneumonia.

PURPOSE: Mycoplasma pneumoniae pneumonias have been one of the most common causes of lower respiratory tract diseases during childhood. It is suggested that pathologic changes seen in the lung tissues were the histologic expression of the host immune response. The aim of this study was to determine the relationship between the chest radiographic pattern of Mycoplasma pneumoniae pneumonia and the level of the cell-mediated immunity of the host. METHODS: Chest radiographic changes and the results of tuberculin skin test (5TU PPD) were studied during the acute stage of infection in 76 patients with Mycoplasma pneumoniae pneumonia. Chest radiographic findings were used to divide the patients into two groups; one group had a predominance of diffuse reticulonodular infiltration (Group A; n=40), while the remainings showed a predominance of segmental or lobar consolidation (Group B; n=36). A comparison was made between the two groups in terms of age, gender, total leukocyte and lymphocyte count, mycoplasma antibody titer, severity of pneumonia, and pleural effusion. RESULTS: Sixty out of 70 patients had negative tuberculin skin test and the positive rate of PPD reaction was higher in Group A (14/40) compared to Group B (2/40) (P<0.05). Pleural effusion was observed significantly more often in Group B (10/36) than in Group A (2/40) (P<0.05). There was no significant difference between Group A and B in age, gender, total leukocyte and lymphocyte count, mycoplasma antibody titer and severity of the disease (P>0.05). CONCLUSION: Our study suggests that the cell-mediated immunity of patients with Mycoplasma pneumoniae pneumonia might influence the radiographic pattern of the pulmonary lesions.

8.Primary Intracranial Teratoma.

Two case of primary intracranial teratomas are presented. 9.Glycine induces enhancement of bactericidal activity of neutrophils

Severe bacterial infections are frequently accompanied by depressed neutrophil functions. Thus, agents that increase the microbicidal activity of neutrophils could add to a direct antimicrobial therapy. Lysophosphatidylcholine augments neutrophil bactericidal activity via the glycine (Gly)/glycine receptor (GlyR) α2/ TRPM2/p38 mitogen-activated protein kinase (MAPK) pathway. However, the direct effect of glycine on neutrophil bactericidal activity was not reported. In this study, the effect of glycine on neutrophil bactericidal activity was examined. Glycine augmented bactericidal activity of human neutrophils (EC50 = 238 μM) in a strychnine (a GlyR antagonist)-sensitive manner. Glycine augmented bacterial clearance in mice, which was also blocked by strychnine (0.4 mg/kg, s.c.). Glycine enhanced NADPH oxidase-mediated reactive oxygen species (ROS) production and TRPM2-mediated [Ca2+ ]i increase in neutrophils that had taken up E. coli. Glycine augmented Lucifer yellow uptake (fluid-phase pinocytosis) and azurophil granule-phagosome fusion in neutrophils that had taken up E. coli in an SB203580 (a p38 MAPK inhibitor)-sensitive manner. These findings indicate that glycine augments neutrophil microbicidal activity by enhancing azurophil granule-phagosome fusion via the GlyRα2/ROS/calcium/ p38 MAPK pathway. We suggest that glycine could be a useful agent for increasing neutrophil bacterial clearance.
10.Usefulness of Additional Prone pad compression Study in Upper Gastrointestinal Series for Detecting EarlyGastric Cancer.

PURPOSE: To evaluate the usefulness of prone pad study in upper gastrointestinal series(UGIS) for thedetection of early gastric cancer(EGC). MATERIALS AND METHODS: During an eight-month period, 88 of 170 patientswho underwent gastrectomy due to EGC were also the subjects of prone pad study as well as double contrast(n=92),mucosal relief(n=76), or compression(n=91) studies. The EGCs were single in 84 patients and double in four. Wecompared prone pad study with the three other techniques for detecting a tumor and depicting the surroundingmucosal changes. Lesional conspicuity was rated 'complete', 'incomplete','suspicious'. or 'undetected'. Thedepiction of surrounding mucosal change was rated 'excellent', 'good', 'fair', or 'poor'. RESULTS: Mean tumorsize was 3.2cm, with a range of 0.3-9cm. Tumors were located in the antrum(n=55), angle(n=13), lower or midbody(n=16), or the sign body and cardia(n=5). Among the 92 EGCs evaluated, UGIS missed the lesion in threecases(sensitivity, 97%). The rates of 'complete' lesional conspicuity were 49% inn prone pad, 29% in compression,20% in double contrast, and 9% in mucosal relief. The rates of excellent' in depicting surrounding mucosal changewere 45% in prone pad, 11% in double contrast, 9% in mucosal relief, and 9% in compression. The tumor wasdemonstrated only in prone pad study in five(5%) of the 92 EGCs. CONCLUSION: prone pad study during UGIS improvesboth the detection rate of EGC and the depiction of mucosal change around the tumor.
