We herein describe two cases of sudoriparous angioma consisting of increased number of eccrine sweat glands associated with angiomatous vascular channels. Case 1 is a 16 year-old girl developing two, painful and tender cavernous hemangiomas which sweated whenever compressed. These were located on her left gluteal area since early childhood. The biopsy specimen revealed an increased number of cystically dilated eccrine glands in the mid-dermis in addition to vascular components in the lower dermis. Case 2 is a five year-old girl manifesting non-tender, slightly hyperhidrotic nevus flammeus-like eruption on her left suprapopliteal area since birth, the biapsy apecimen of which revealed an increased number of eccrine glands admixed with capillary hemangioma in the mid-dermis.

PURPOSE: This descriptive correlation study was done to identify how perception of patient safety culture of general hospital nurses affects safety during nursing activities. Data from this study should provide information on management of patient safety as well as improvement in patient safety. METHOD: Participants in this study were 357 clinical nurses working in a general hospital in M city which had two medical evaluations. A survey was conducted to gather the data. RESULTS: The score for perception of patient safety culture of the general hospital nurses was 3.42, out of a possible 5 points, and the score for safety care activities was 3.90. There was a statistically significant positive relationship between the nurses' perception of patient safety culture and their safety care activities, Perception of patient safety culture, Supervisor/manager, communication and procedures, and frequency of accident reporting were factors that impacted significantly on safety nursing activity. CONCLUSION: The results of the study indicate that patient safety cultural perception significantly affects the safety of nursing activities and thus systematic educational strategies to increase perception should be provided to increase the level of patient safety culture. Also, other specific methods that increase the level of patient safety culture should be considered.

In these days, there are two main ways in the treatment of the avascular necrosis of the femoral head in adult by prosthetic replscement; total hip replacement and bipolar endoprosthesis. So far, no study has been performed on the post-operative functional evaluation of them. Therefore it is difficult to decide which one is preferred to the other in the treatment of avascular necrosis of the femoral head in adult. In order to evaluate the functions of the prosthetic joints, the authors reviewed the results of 18 cases of cementless bipolar endoprosthesis(abbreviated as CBE) and 22 csses of cementless total hip replscement arthroplasty(abbreviated as CTHRA), performed at the Department of Orthopedic Surgery, Seoul National University Hospital, during the period from February, 1984 to May, 1986. The following results were obtained. 1. The average ages of CBE and CTHRA groups at the time of operation were 48.5 years and 37.5 years, respectively. 2. The average follow-ups of CBE and CTHRA were 15.7 months and 30.5 months, respectively. 3. By Meyer's modification of Marcus and Enneking classification, the sverage stages of CBE and CTHRA groups were 3.7 and 4.4 respectively. 4. According to Harris hip rating score system, Harris scores were increased as much as 30.5 in CBE group and 30.6 in CTHRA group. 5. In the aspects of functional improvement and relief of pain, the two groups showed no significant difference. However, the operation was essier, and the damage to the acetabular bone was less in CBE group. For these reasons, CBE was believed to be the preferred choice of treatment of avascular necrosis of femoral head in adult.

PURPOSE: For evaluation of the growth hormone and cortisol responses to insulin stimuli in children with short stature, the human growth hormone secretion and cortisol level was monitored 30 and 60 minutes after insulin infusion by intravenousely. METHODS: The inclusion criteria of children for this study were that the height was below 3 percentile compare with the same sex and age children, bone age was younger more than one year of chronologic age, and insulin and L-dopa induced growth hormone secretion was below 10ng/mL. There were 10 children compatible with this criteria(group A) and 11 children was selected for control group(group B). Serum concentration of growth hormone was determined just before intravenous infusion of 0.1U/kg insulin and then 30 and 60 minutes after insulin infusion. Serum level of cortisol was determined simultaneously with the growth hormone but skip the level of the 30 minutes after insulin infusion. Serum glucose level was monitored on every time sampling the blood for determined growth hormone and cortisol level. Serum level of growth hormone and cortisol were determined by radioimmunoassay. RESULTS :The serum glucose concentration were decrease to under 50% of baseline level on 30 minutes after insulin infusion in A and B group. The serum growth hormone level before insulin infusion was 2.5+/-1.78ng/mL and changed to 2.2+/-1.69 ng/mL on 60 minutes after insulin infusion in group A. In group B, the serum growth hormone level alteration before and on 60 minutes after insulin infusion was from 5.5+/-6.67ng/mL to 12.6+/-7.91ng/mL. The serum concentration of cortisol was changed from baseline level 10.7+/-7.10, 7.9+/-3.98microg/dL to 16.7+/-11.47, 26.1+/-3.59microg/ dL on 60 minutes after insulin infusion in group A and B respectively, but some cases in group A show a little increase in cortisol level. CONCLUSION: On the basis of this study, it is suggested that the deficiency of growth hormone secretion is related to cortisol concentration in peripheral blood. But for more detailed assessment of this association, follow up studies will be needed in more cases.

The purpose of this study was to provide basic data useful to the training of hospice care members. For this study carried out to investigate the degree of death orientation of hospice care members. The subjects of study were 22 nurses, 85 clergies, 59 volunteers who registered on Hospice Education Programs. The data were analysed by descriptive analysis, t-test, ANOVA, Duncan test and Person's correlation coefficient. The results of the study can be summarized as follows; 1. The degree of death orientation were 85.95 in nurses group, 73.05 in clergies group, and 88.49 in volunteers group. So, clergies group has more positive death orientation than others. 2. The degree of death orientation showed significant differences between age group(F=4.62, p=.004), education program group(F=27.25, p=.000), and religiosity(t=3.77, p=.000). There were no significant differences between the degree of death orientation and the others general characteristics of hospice care members. In conclusion, the urgent need for educational preparation of death orientation and spiritual care of hospice care team, especially volunteers group and nurses group is emphasized. Furthermore, all of the hospice care members those who complete the hospice education program, should be performed efficient hospice care intervention for dying patients and their families.

We report a patient who had been initially diagnosed as a myelodysplastic syndrome in 1998 presenting purpuric patches on the left arm that started to develop about a year prior. The purpuric lesions were diagnosed as leukemia cutis by skin biopsy. Her subsequent bone marrow biopsy showed progression into an atypical chronic myeloid leukemia with increased numbers of leukocytes in the peripheral blood. Leukemia cutis typically is regarded as a sign of progression of disease or a manifestation of recurrent disease in treated patients with an established diagnosis of leukemia. We suggest that the skin lesion in this patient could have been a sign of con-version into atypical chronic myeloid leukemia.

Undifferentiated sarcoma of the liver is a highly malignant neoplasm that occurs almost exclusively in children. We present a case of adult undifferentiated sarcoma in a 33-year-old man. The neoplasm was typically hypovascular on hepatic angiography and a globular, cystic and mucoid mass separated from the adjacent liver by a psedocapsule was removed from the left lobe of the liver. Necrosis and hemorrhage were found. Microscopically the neoplasm consisted of myxoid and cellular areas and the basic neoplastic cells were stellate cells showing variable degree of anaplasia and pleomorphism. Eosinophilic globules were PAS-positive and immunohistochemically negative for alpha-fetoprotein. Extramedullary hematopoiesis was present and normal-appearing bile ducts and hepatic cell cords were noted.
