1.A Case of Unusual Lung Abscess Induced by Psoas Abscess in Tuberculosis of the Spine

A case of unusual lung abscess induced by psoas abscess in tuberculosis of the thoracolumbar spine complicated with complete spastic paraplegia is reported. Usually the tuberculous psoas abscess drains out along the fascia and ligament by it's own gravity. The common site of drainage of the tuberculous psoas abscess are the Petit triangle, the femoral triangle, the sciatic notch, the adductor region of the thigh and the lumbar region. In the reported case the left psoas abscess penetrated into the diaphragm and induced a lung abscess. This was confirmed by operation as no communication was inspected between the paravertebral abscess. The abscess was completely evacuated through thoracotomy and the tuberculous lesion in T-11,12 was operated by complete removal of focus and simultaneous anterior spinal fusion, another lesion in the lumbar spine was operated by radical operation three weeks after first operation. The paraplegia was recovered three months after first operation with satisfactory bony union and the patient was allowed to be up and about. A satisfactory result was obtained by radical ressection and anterior interbody fusiao of T-11,12 of the diseased vertebrae through the left extrapleural approach. This was followed by focal deberidement of right psoas abscess.

3.Sparganosis in Subcutaneous Tissue of Thigh: A Case Report.

A case of rare subcutaneous sparganosis in thigh treated by surgical excision is reported. In this 49year-old male with a palpable mass on the anteromedial aspect of mid-thigh (5x7x5cm sized) which was misdiagnosed with a soft tissue tumor initially, a sparganosis was suspected by a plain x-rays, bone scan and his past history which he frequently had raw snakes, frogs and raw fishes before but confirmed by MRI and surgical excision. This represents tor warning to some Koreans who have frequently comsumed raw fishes, snakes or frogs etc., and to some doctors because it is easily confused with a soft tissue tumor.

4.Clinical Study of Osteoid Osteoma

Osteoid osteoma is a small, oval or roundish, nidus like neoplasm of bone. The lesion usually does not exceed a centimenter in its greatest dimension, and it consists of osteoid, trabeculae of newly formed osseous tissues and a substratum of highly vascularized osteogenic connective tissue. In a differential diagnosis, consideration should be given to Brodies abscess, sclerosing osteomyelitis of Garre, Fibrous dysplasia and healing fractures. From Janua 1960 to December 1980, we have seen and studied 18 cases of osteoid osteoma and the results obtained from this study were follows: l. Eleven cases were found in second decade and 17 cases were developed before thirty years old. 2. The sex ratio was nearly equal and duration of disease from onset was average 16 months. 3. Location of lesions were predominantly in lower extremities, 9 in tibia and 3 in femur. 4. Pain was noted in 16 cases and aspirin affected in 8 cases. 5. Nidus was found in 11 cases. 6. All cases were treated by radical excision of lesions with disappearance of pain and satisfactory results.

5.Computed Tomography in the Evaluation of Childs with White Pupil.

White pupil is one of the most disturbing problems in the pediatric age group and many conditions can cause white pupil, but the differential diagnosis between the retinoblastoma and other conditions simulating it, is the prime importance in the evaluation of the white pupil. The ophthalmoscopy is the most accurate diagnostic method for evaluating intraocular diseases because it is able to see directly. However, its usefulness seems to be limited in the case of extreme lens opacity or vitreous opacity and in the evaluation of orbital diseases extending outside of the globe. Recent advances of high resolution computed tomography(CT), the axial tomographic images of the orbit were markedly improved and represented good anatomic details of the orbit and the topographic relationship with surrounding structures. We reviewed CT findings of 15 children who presented white pupil and had results demonstrating specific findings in the cases of retinoblastoma, persistent hyperplastic primary vitreous and vitreous opacity due to retinal detachment and retrolental fibroplasia. The CT findings of each disease are discussed with detail.

6.Surgical Treatment of Intracranial Arachnoid Cyst by Excision.

Arachnoid cysts are benign developmental cysts that occur between aachnoid layers, which represent approximately 1 percent of all intracranial space-occupying lesions, their most common location being the middle cranial fossa. Current surgical treatment options for intracranial arachnoid cyst are either craniotomy and excision of cyst or shunting of cyst contents extracranially. We treated 10 cases of intracranial arachnoid cyst by excision during last two years. For the best surgical curability, we have, not only removed the cyst wall and adhesive gliotic tissue as possible, but also made the opening to basal cistern for communication. During average 8.4 months follow up, we observed, not specific morbidity and mortality, but good clinical improvement and brain expansion. As above result, if haven't specific problem, the direct surgical removal may be the primary choice for intracranial arachnoid cyst, especially in children with noncommunicating type in sylvian.

7.An Ipsilateral Crossed Pinning Technique to Fix Supracondylar Fractures of the Humerus in Children: A Report on the Technique to Escape form Ulnar Nerve Injuries

Of several possible configurations of pin fixation of a displaced supracondylar fracture of the humerus in children, the medial and lateral crossed pinning technique has been known to provide the greatest resistance to gross rotational displacement. A new technique with ipsilateral two lateral crossed pins was devised so as to avoid the ulnar nerve injury with good stability for fracture site. During the period from 1992 to 1994, 18 children with displaced supracondylar fracture of the humerus were treated by closed reduction and ipsilateral two lateral crossed pinning. Eighty nine percents of the final results were satisfactory. There were no ulnar nerve injuries and fixation loss in all cases from the treatment. This is a safe, simple and reliable technique for providing the good stability of fracture site as well as avoiding the ulnar nerve injury.

The treatment of recurrent supratentorial glioma is one of the most challenging and at the same time one of the oldest problem in neurosurgery, especially in case of multiple recurrence. We have experienced 15-year-old male, multiple recurrent supratentorial glioma patient who was subjected to 6 times of operation, radiation therapy and chemotherapy druing 5 year follow-up period. We reveiewed the literature and discussed about recurrent rate, malignant change, method of management and indication of reoperation in low grade glioma.

9.A Clinical Analysis of Treatment of Cervical Fracture and Dislocation.

Recently, the authors experienced the thirty-one cases of cervical fracture and dislocation. Among them, nineteen cases underwent surgical treatment and remained conservative treatment. There are three ways in operation ; 1) anterior approach(Cloward, Smith-Robinson method). 2) posterior approach. 3) combined approach(modified Smith-Robinson and posterior fusion). We did a combined approach and the results were excellent.
