2.Iatrogenic Subcutaneous Aspergillosis in a Patient with Lumbar Spondylitis Treated with Ketoconazole.

We report herein s case of subcutaneous aspergillosis in a 41-year-old woman with lumbar spondylitis(L4~L5). The patient had received high doses of corticosteroids and broad-spectrum antibiotics and two times of operation due to lumbar problems. The diagnosis of Aspergillus fumigatus infection was established by the mycologic study of cultures from wound exudate and histopathologic examination of the second surgical specimens of subcutaneous soft tissue, which was assumed to be iatrogenically susceptible infection. Intravenous amphotericin B therapy was discontinued because of the immediste adverse effects. After twelve-week therapy of oral ketoconazole(400mg/day), open wound healed and spondylitis with neurologic signs eventually resolved. No evidence of recurrent infection was noted during six months of follow-up.

3.Clinical analysis of dissecting aortic aneurysm in emergency department.

Dissecting aortic aneurysm is an emergency condition which requires prompt diagnosis and management. From January 1992 to December 1996, 54 patients were admitted to our department. Patients were categorized according to the Stanford classification of aortic dissection and investigation of the clinical records utilized the retrospective method. Male constituted the majority, according for 38(70.4%) of the total number of patients, whereas females accounted for 16(29.6%). Mean age was 52.1+/-2.2 years old (range:25~82). Clinical findings included chest pain in 46 cases(85.2%), renal failure in 11(20.4%), aortic insufficiency in 10(18.5%) and stroke in 9(16.7%). Predisposing factors were hypertension in 48 cases(88.9%), Marfan's syndrome in 5(9.3%) and diabetes mellitus in 1(1.8%). 21 patients(type A:12, type B:9) underwent surgical treatment. There were three(2 in type A and 1 in type B) deaths in the surgical treatment group and nine(5 in type A and 4 in type B) deaths in the medical treatment group. These results reflect our current policy in the management of dissecting aortic aneurysm.

4.Syphilis Prevalence in Teagu Hope Village.

Serologic tests for syphilis, including VDRL and TPHA tests, and Physical examination were carried out on 1, 605 subjects who lived in Taegu Hope Village, a social welfare center, from April, 198R to May, 1985. We prepared this study comparing with previous reports for healthy individuals and prostitutes. The results were as follows, The reactive rate of VDRL. qualitative test was 11. 6% in total, 10. 3%, in men and l3. 5%. in women. The reactive rates of VDRL. qualitative tests were highest in 7 th decade as 28, 7% and followed by 6 th decade as 16. 7% the decacile as 16. 0% 8 th decade as 14,3% 4th decade as 8. 8% and 3rd decade as 5. 9%, 3 In ]Hg sero-positive subjects, VDRL quantitative tests were done, and 35. 5% were reacted with 1: 8 or high and 29. 0% had one or more syphilitic lesions. 4 In 54 subjects who had one or more syphilitic lesions, 19 had chancres, 17 had alopecia syphilitica, 7 had rnaculopapular syphilid, 7 had condylomata lata. 3 had papular syphilid, and 1 had maculopapular syphilid and alopecia syphilitica. The biological false positive rate was 10. 1,4 in 138 VDRL reactive subjects, using 1;he TPHA test standard.

5.A Case of Excess Granulation Tissue Response during Etretinate Therapy.

We experienced a case of granulation tissue occuring in lateral sulci of the right thumb, index, middle and left thumb, middle finger nails in 71-year-old psoriatic male patient taking 5pmg daily(0.6mg/kg/day) during 4th week of etretinate therapy. The excision of the granulation lesion and partial nail ablation was performed and his doses was lowered to 25mg daily (0. 3mg/kg/day). The condition were resolved with no recurrence 4 weeks later after the operation.

6.Two Cases of Keratosis Palmaris et Plantaris.

We described here-in a 36-year-old male patient (case 1) with a severe form of keratosis palmaris et plantaris(Thost-Unna type) which was diffuse hyperkeratosis with hyperhidrosis on the entire surface of the palms and soles. His son, 8-year- old boy, showed symmetrical distribution of hyperkeratotic patches on the inner sides of both great toes(case 2). Therefore, it was suggested this genodermatosis occured in two generations of a family. The biopsy taken from the right palm of case 1 showed features of epidermal hyperplasia with hyperkeratosis, hypergranulosis and acanthosis. We tried the oral Vit. A therapy(50,000 unit, daily) in case 2. The hyperkeratotic lesions became slightly improved in 4 months after therapy.

7.Four Cases of Porphyria Cutanea Tarda.

Porphyria cutanea tarda(PCT) is a photocutaneous disorder due to excessive porphyrins in the skin. We experienced 4 male patients who showed typical clinical manifestations. Histologic findings revealed subepidermal bullae with festooned derrnal papi11ae, and typical porphyrin excretion pattern of PCT was detected. One case had on 500mg daily of chloroquine with r.linical and biochemical improvement 4 months later without any adverse effects. Other 2 cases had on 125mg of chloroquine twice a week and the other one had perforrned phlebotomy.

9.Pulmonary Consolidation Pattern on the Chest CT: Malignant vs Benign.

PURPOSE: It is not easy to determine the cause of pulmonary consolidative lesion. Even without any definite mass, malignancy cannot be ruled out. And sometimes, it is difficult to differentiate tuberculosis from pneumonia. To differentiate malignant consolidative lesion from benign one, we studied patterns of air bronchogram, mucoid impaction, and computed tomographic anglogram etc. MATERIALS AND METHODS: Fifty seven cases of pulmonary consolidative lesions(23 cases of malignancy,34 cases of benign lesion) were retrospectively reviewed by three radiologists. RESULTS: Among the 28 cases which showed a little air bronchogram(less than 1/3 of the whole lesion in volumetric measure with the eye) 19 cases were malignancy and nine cases were benign lesions. All of the 12 cases which showed profound air bronchogram over 2/3 of the whole lesion were benign lesions. Bronchiectasis was detected in 31 cases(four of malignancy and 27 of benign lesions). Among the 20 cases which didn't show the mucoid impaction five cases were malignancy and 15 cases were benign lesions. Out of eight cases with mucoid impaction filling the long segments(branching tree shape), seven cases were malignancy and one case was benign lesion. So called CT anglogram was detected in nine cases of malignancy and two cases of benign lesions. All of nine cases of malignancy showed CT anglogram which was like arborizing tree. CONCLUSION: Scanty air bronchogram, profound arborizing mucoid impaction and/or CT angoigram within consolidative lesion could suggest malignancy.
