1.Surgical Treatment of Spontaneous Pneumothorax by Thoracoscopic Wedge Resection with Fibrin Glue.

BACKGROUND: To evaluate the efficacy of Fibrin glue to decrease recurrence in video-assisted thoracoscopic surgery(VATS) for a treatment of spontaneous penumothorax. MATERIAL AND METHOD: All medical records of 17 patients who underwent a thoracoscopic wedge resections of bullae with stapling device with Fibrin glue in our institute between May 1998 and December 1999 were reviewed. variables analyzed include affected sites primary indication of VATS. duration from admission to discharge duration of postoperative stay duration of chest tube drainage recurrence and complication. There were 16 men and 1 woman. RESULT: There was no evidence of hemodynamic instability or arterial blood gas abnormalities encountered during the procedure. Mean age at the time of the VATS was 26.9 years (range 15 to 61 years) The mean duration from admission to discharge was 7.8 days and mean postoperative stay was 5.1days mean chest tube indwelling period was 4..0 days. There was no recurrence of pneumothorx. CONCLUSION: Thoracoscopic wedge resections with introduction of fibrin glue are safe and effective and requires only a short hospital stay. We believe that this thoracoscopic technique will further simplify the surgical treatment of pneumothorax.

4.A Study on Dietary Intakes and Nutritional Status in College Women Smokers - I. Anthropometric Measurements and Nutrient Intakes.

In order to investigate the dietary intakes and physical characteristics in college women smokers, interviews using questionnaires were done on 33 smokers and 42 nonsmokers residing in seoul area. General living habits, dietary habits, food consumption frequency and nutrient intake by quick estimation were investigated through direct interviews with subjects. Subjects height, weight and blood pressure were measured, and body fat percentage were statistically analyzed using Bio-electrical Impedence Fatness Analyzer(GIF-891). All data were statistically analyzed by SAS PC package program ; percentage or mean and standard error were examined for each item, and the significant difference was evaluated by chi-square test or Student's t-test at alpha=0.05. In the analysis of taste and food preference, smokers consumed larger amount of alcohol and coffee than nonsmokers ; they also disliked sweet taste. The results of food consumption frequency data also showed that smokers consumed less fish, milk and fruits but consumed more instant foods than nonsmokers. As a result of anthropometric measurements, height, age, and body fat percentage showed no difference, but there was a significant difference in weight, BMI, systolic blood pressure, and diastolic blood pressure. Energy intake in nonsmokers was 1640 kcal/day(CHO : Pro : Fat=66.0 : 14.7 : 19.3), in smokers. Intakes of calcium, iron, vitamin C, vitamin A, vitamin B1, vitamin B2, and niacin in smokers were not significantly different from those of nonsmokers.

5.Levels of Serum Lipids, Copper, Zinc, Ceruloplasmin and Ferroxidase Activity in Smoking College Women.

This study was done to compare serum levels of lipids, trace minerals, ceruloplasmin and ferroxidase activity between female college smokers and nonsmokers. Serum levels of lipids, Cu, Zn, ceruloplasmin and ferroxidase activity were determined in 33 smokers and 42 nonsmokers residing in the Seoul area. Serum levels of TG, total cholesterol and HDl-cholesterol were measured using test kits. Serum Cu Zn were determined by the AAS. Serum ceruloplasimin level and ferroxidase activity were determined by spectrophotometric methods. All data were statistically analyzed by the SAS PC package program. Mean and standard errors were calculated for each item, and the significance of the difference between two groups was evaluated by Student's t-test at p<0.05. There was no significant difference in levels of total cholesterol, TG, VLDL-cholesterol, LDL-cholesterol, LDL/HDL and atherogenic index, but HDL-cholesterol level was significantly lower(p<0.05) in female college smokers than in nonsmokers. Serum zinc level showed no significant difference, whereas copper level was significantly higher in smokers, consequently yielding higher Cu/Zn ratio in smoking college women. Ceruloplasmin level was 32.6mg/dl in smoking college women, which was significantly higher(p<0.05) than that of 28.6mg/dl in nonsmokers. However, cerulopasmin ferroxidase activity, 0.34U/mg in nonsmokers was higher(p<0.05) than that of 0.29U/mg in smokers. From these results, it is confirmed that cigarette smoking itself has a very detrimental effect on health and mineral nutritional status although the smoking history is relatively short among college women(pack-years=1.7).

6.Pulmonary Thromboembolism Developed in Recovery Period of upper Abdominal Surgery: Case report.

Pulmonary thromboembolism (PTE) is a serious postoperative complication. Prompt diagnosis of PTE is important but it is difficult because clinical manifestations of PTE are not obvious in most cases. A 59 year-old woman received cholecystectomy and choledocholithotomy under general anesthesia. At the 9th postoperative day, syncope, hypoxemia and hypotension were developed suddenly and the patient was transfered to intensive care unit. PTE was suspected with hemodynamic monitoring from pulmonary arterial catheter and echocardiography and diagnosed with lung perfusion scan and venogram of lower extremities. Proper cardiopulmonary support was done and the patient was recovered and discharged in improved condition. In conclusion, if a patient have syncopal attack with shock in postoperative period, PTE must be thought to be an one of possible causes of syncope and thoroughly investigated.

7.In vitro Stimulation of Tumor - Draining Lymph Node Lymphocytes with the 30 kDa Antigen of Mycobacterium tuberculosis Leads to the Differentiation of Th1 Cells and Cytotoxic Effector Cells.

Tumor-draining lymph node (TDLN) lymphocytes contain immunologically sensitized to tumor but functionally deficient T cells. The 30 kDa protein antigen, a major secreted protein antigen of Mycobacterium tuberculosis, exhibits strong T cell stimulatory effect. In this study, it examined that the feasibility of using M tuberculosis 30 kDa antigen to stimulate tumor-draining lymph node cells for the generation of specific immune effector cells. Freshly isolated TDLN lymphocytes could directly respond to the 30 kDa antigen alone and their proliferative responses were markedly augmented by stimulation with rIL-2. TDLN cells were stimulated with the 30 kDa antigen for various time intervals and examined for the induction of IFN-r and IL-4 mRNA using RT-PCR. The expression of IFN-r mRNA was greatly augmented after 1 wk, whereas IL-4 mRNA is markedly decreased after 1 wk. Cytotoxic T cell activities induced by the 30 kDa antigen was also evaluated. TDLN cells stimulated with the 30 kDa antigen alone were able to generate remarkable cytotoxic response to K562 or Daudi cell lines after 6 days of culture. And their cytotoxic effects were highly augmented by stirnulation with rIL-2. These results suggest that the 30 kDa antigen of M. tuberculosis may selectively activate Thl cells of TDLN lymhocytes and induce the cytotoxic T cell activities. In conclusion, the 30 kDa antigen can be used as a biologic response modifier in tumor immunology.

8.Leiomyosarcoma of the Pancreas: A case report.

Most of the malignant tumors of the pancreas are adenocarcinomas arising from the ductal epithelium. Primary leiomyosarcoma of the pancreas, even though it is the most common sarcoma of the pancreas, is very rare. We present a case of leiomyosarcoma of the pancreas, probably primary, with metastases to the stomach, lymph nodes, and abdominal wall. A 52-year-old woman visited the hospital with vague right upper abdominal pain and weigh loss of 6 kg for 2 months. The radiological and endoscopic examination revealed that she had a large heterogeneous mass, 11 cm in size, in the pancreatic body and tail, a 4 cm-sized mass in the paraaortic area, and a 3 cm-sized polypoid mass in the stomach. Histologically, they were all similar to one another and composed of markedly pleomorphic cells. Immunohistochemical and electron microscopic studies showed definite smooth muscle differentiation of the tumor cells. Two months later, the patient underwent an excision of a new 3 cm mass in the right lower abdominal wall, showing features of well differentiated leiomyosarcoma.

9.The Effect of Quinidine on Digoxin Clearance.

BACKGROUND: Quinidine appeared to increase serum digoxin levels when given with quinidine. Therefore elevated serum digoxin concentrations and clinical toxicity have been reported in patient receiving quinidine. Currently, Bayesian method which estimates the most probable parameters of the drug for each patient from population parameters data is useful approach for adjusting digoxin dosage. To increase the accuracy of Bayesian method, it is desirable to use population parameters of Korean. Therefore we evaluated the effect of quinidine on digoxin clearance in Korea. METHOD: Patient's records from 19 adult cardiac disease without CHF having normal renal and liver function from Seoul National University of Hospital respectively wre evaluated. Digoxin pharmacokinetic parameters, CL and Vd, were obtained from serum concentration of digoxin of single and combined therapy at each steady-state by using bayesian method. RESULTS: This study show that quinidine reduced the total body clearance of digoxin from 2.39+/-0.17 to 1.51+/-0.08ml/min/kg(p<0.05) and reduced the digoxin volume of distribution from 8.57+/-0.29 to 4.98+/-0.19L/kg(p<0.05). This results show that digoxin dosage reduced to 40-50% in Korean, if quinidine therapy is initiated.

10.Mediating Effects of Hope and Therapeutic Relationship in the Relation between General Social Functions and Mental Health Recovery of Community People with Mental Illness

PURPOSE: This study tries to test mediating effects of hope and therapeutic relationship in the relation between general social functions and mental health recovery of community people with mental illness. METHODS: This study was carried out in a cross-sectional research design. The participants included 217 people with mental illnesses who were enrolled at eight Mental Health Welfare Centers in the Gyeonggi Province. Data were collected from February to May, 2018. The collected data were analyzed using a regression analysis, and SPSS PROCESS macro was used to test the mediating effects. RESULTS: This study analyzed the direct effects of general social functions on the hope, therapeutic relationship and mental health recovery. And general social functions had indirect effects on their mental health recovery via hope and therapeutic relationship. Both hope and therapeutic relationship had dual mediating effects in the influence of general social functions on mental health recovery. CONCLUSION: The role of hope and therapeutic relationship in the mental health recovery of community people with mental illness is important, and it is confirmed that hope is a powerful factor influencing mental health recovery.
