PURPOSE: This study was conducted to identify relationships between depression, family support, and rehabilitation-motivation in patients with cerebrovascular accident. METHOD: The data were collected from 'S' hospital in Seoul from Feb. 1, 2004 to Apr. 30, 2004. The data were analyzed using a SPSS 10.0 program for descriptive statistics, Pearson correlation coefficients and stepwise multiple regression. RESULT: The results were as follows: The mean rehabilitation-motivation was 119.89, which was slightly high. The rehabilitation-motivation was influenced by religion, education level, occupation, economic status, treatment fee, duration of hospitalization, level of language ability and ADL. Rehabilitation-motivation has showed correlation with family support (r=.538, p=.000) and depression (r=-.417, p=.000). The variables related to rehabilitation-motivation were identified as family support, level of ADL, occupation, depression, and education level, using stepwise multiple regression. They were accounted for 57% of predictability of the rehabilitation-motivation, with the family support the highest at 29.3%. CONCLUSION: With increasing post stroke patients in Korea, we need to pay more attention to the rehabilitation of them. This study indicates that health professionals need to focus on the intervention especially in increasing family support and in decreasing depression of the patients with cerebrovascular accident.

A clinical study was made on 71cases of Mycoplasma Pneumoniae Pneumonia from March, 86 to February, 89. The results were as follows; 1. Among the 315 cases of pneumonia, the incidence of mycoplasma infection was 22.5% 2. The peak incidence of age was between 5 to 9years of age (53.5%) 3. The sex ratio of male to female was 1.3:1 4. Monthly distribution showed relatively high frequency from October to January (59.2%) 5. Most common clinical symptoms were cough (98.6%) and then followed by fever (49.3%), coryza (19.7%). Rales were the most common finding (95.7%) and followed by pharyngeal injection (49.3%) and wheezing (18.3%) 6. The leukocyte counts in peripheral blood were most common in the range of 5000-10000/mm³ (47.9%) and the ESR was increased in 57.7%, and positive CRP cases were 87.3% 7. The most common radiologic finding of pulmonary infiltration was interstitial infiltration (45.1%) and then followed by disseminated lobular (39.4%) and lobar pneumonia (15.5%) 8. There are a few cases associated disease or complication: otitis media (5.6%), hepatitis (4.2%) acute glomerulonephritis, bronchial asthma and sinusitis (2.8%), thrombocytopenia (1.4%)

BACKGROUND: There is increasing concern in many countries about the problem of drug-resistant tuberculosis. Prevalence of primary drug-resistant tuberculosis is the optimal epidemiological indicator for long term monitoring of national tuberculosis control program. Our purpose was The purpose of our study is to assess clinical characteristics and socioeconomic status of patients with drug-resistant tuberculosis. METHODS: We studied 68 cases with durg-resistant Mycobacterium tuberculosis infection diagnosed at the Ewha Womans University Mokdong Hospital from March, 1995 to February, 2000. RESULTS: Patients with primary drug-resistant tuberculosis(PDR) were younger(39.6±16.3 years vs. 48.2±16.5 years; p<0.05), had more population of less than more were under the age of 40 years aged-group(62.9% vs. 36.4%; p<0.05) and were more highly educated than those with acquired drug-resistant tuberculosis(ADR)(38.9% vs. 11.1%; p<0.05). In patients with ADR, the rates of familial history of tuberculosis and living in a rented house residence in a rented house were increased higher than compared with to those of patients with PDR. Patients with ADR had more involved lobes(2.0±0.8 vs. 1.4±0.7; p<0.01) and longer treatment duration than those with PDR(18.3±7.2 months vs. 10.6±6.3 months; p<0.05). Patients with ADR showed larger numbers of resistant were resistant to more number of drugs, lower hospitalization rate and higher rate of self-interruption of medication than those with PDR. In patients with PDR, mono-drug resistance was increased, whereas poly-and multi-drug resistances were decreased compared with those with ADR. Resistance to isoniazid was the highest among antituberculosis drugs, and resistance to isoniazid in patients with ADR was higher than that in patients with PDR(90.9% vs. 71.4%; p<0.05). CONCLUSIONS: Patients with ADR were more likely to include more population be of lower socioeconomic class, and patients with PDR seem seemed to be young and socially active population. For control of drug-resistant Mycobacterium tuberculosis infection, proper isolation and prevention of patient with drug-resistant tuberculosis are needed.

The aim of this study was to examine the factors influencing perceived oral health in elderly individuals residing in the community.

PURPOSE:To evaluate the role of unenhanced CT in diagnosing the acute appendicitis. METHOD AND MATERIALS: We retrospectively analyzed conventional contrast-enhanced abdominal CT scans of 197 normal patients and barium enemas of 26 out of the 197 patients. Additional unenhanced CT scans of right lower abdomen were performed on 30 patients who were suspected of having acute appendicitis;barium enema was performed in 26 patients. RESULTS: in the analysis of the conventional CT scans, we could detect 132(67%) normal appendicies. On barium enema of 26 out of 197 patients, abruptly narrowed appendix was visualized in 5 cases and luminal irregularity in 4 cases but all 26 cases showed normal appendix on CT. In the 30 cases of unenhanced CT, all appendicies were visualized. Among 12 Cases which were proved as acute appendicitis, barium enenma was done in 10 case;abruptly narrowed appendix was shown in 3 and luminal irregularity in 1. Among 18 cases which were diagnosed as normal appendix, nonvisualization of appendix' was in 1 case, abruptly narrowed lumen in 3, and luminal irregularity in 9. CONCLUSION:Unenhanced CT in the area of the cecum may be an effective and safe diagnostic tool for acute appendicitis.

BACKGROUND/AIMS: Although the only therapy of proven benefit for chronic hepatitis C is interferon alpha, the rate of sustained response after treatment with interferon is less than 25%. A 6-month course of combination therapy with interferon and ribavirin was associated with higher rate of long-term response than either interferon or ribavirin alone. Pilot studies suggested that combination of interferon and ursodeoxy-cholic acid (UDCA) resulted in higher biochemical response than interferon alone. We investigated the rates of end of treatment response(ETR) and sustained response(SR) of combination therapy of interferon e2a, ribavirin and UDCA and compared it with interferon a 2a alone. METHODS: Ninty-five naive patients with chronic hepatitis C who have been positive for anti-HCV by 3rd generation EIA and HCV RNA by RT-PCR and had elevated level of ALT over 6 months were included. They were assigned to three groups. Thirty seven patients in group 1 were treated with interferon a 2a (3MU thrice weekly) in combination with ribavirin (600mg/day) and UDCA (600mg/day) for 6 months. Twenty nine patients in group 2 were treated with the same dose of interferon a 2a alone for 6 months. Changes of ALT and HCV RNA were observed over 12 months (average 3029 mos) after the end of treatment in both groups. Twenty nine patients in group 3 were observed over 12 months without antiviral treatment. HCV genotypes were tested by Innop-Lipa in 24 patients in group 1. RESULTS: In group 1, not only ETR (68%) but also 12 month SR rate (54%) was significantly higher than group 2(31%, 21% respectively). There was no difference in relapse rate between two groups. The level of ALT became normalized and HCV RNA negative within 1 month after treatment in most responders in group l. Genotype 1b was associated with lower ETR and SR than non-lb, although not significant stastistically. CONCLUSION: Both the ETR and 12 month SR rate were significantly higher after combination treatment of interferon a 2a, ribavirin and UDCA than interferon e 2a alone in chronic hepatitis C. It is suggested that this combination is preferable to interferon alone in the treatment of naive patients with chronic hepatitis C.

PURPOSE: It is not easy to determine the cause of pulmonary consolidative lesion. Even without any definite mass, malignancy cannot be ruled out. And sometimes, it is difficult to differentiate tuberculosis from pneumonia. To differentiate malignant consolidative lesion from benign one, we studied patterns of air bronchogram, mucoid impaction, and computed tomographic anglogram etc. MATERIALS AND METHODS: Fifty seven cases of pulmonary consolidative lesions(23 cases of malignancy,34 cases of benign lesion) were retrospectively reviewed by three radiologists. RESULTS: Among the 28 cases which showed a little air bronchogram(less than 1/3 of the whole lesion in volumetric measure with the eye) 19 cases were malignancy and nine cases were benign lesions. All of the 12 cases which showed profound air bronchogram over 2/3 of the whole lesion were benign lesions. Bronchiectasis was detected in 31 cases(four of malignancy and 27 of benign lesions). Among the 20 cases which didn't show the mucoid impaction five cases were malignancy and 15 cases were benign lesions. Out of eight cases with mucoid impaction filling the long segments(branching tree shape), seven cases were malignancy and one case was benign lesion. So called CT anglogram was detected in nine cases of malignancy and two cases of benign lesions. All of nine cases of malignancy showed CT anglogram which was like arborizing tree. CONCLUSION: Scanty air bronchogram, profound arborizing mucoid impaction and/or CT angoigram within consolidative lesion could suggest malignancy.

Purpose: The purpose of this study was to investigate the moderating mediation effect of health-promoting lifestyle in the relationships between clinical nurses’ job stress, appreciation and mental health problems. Methods: The participations were 230 clinical nurses working in general hospitals in Seoul City and Gyeonggi Province. Data were collected in November 2019 via an online survey that covered job stress, appreciation, health-promoting lifestyles and mental health problems. The collected data were analyzed using SPSS 25.0, SPSS PROCESS Macro(Model 4, Model 7) and bootstrapping method. Results: Appreciation had a significant mediation effect in the relationship between job stress and mental health problems. Health-promoting lifestyle had a significant moderation effect in the relationship between job stress and appreciation. Further, health-promoting lifestyle significantly moderated mediation effect of job stress on mental health problems through appreciation. Conclusion The results of this study suggest that it is necessary to effectively manage health-promoting lifestyle in the context of clinical nurses’ mental health problems due to job stress. In order to improve clinical nurses’ mental health, it is necessary to provide them with increased appreciation and lead health-promoting lifestyle.

Purpose: The purpose of this study was to develop an instrument to evaluate the needs satisfaction of nurses and examine its validity and reliability. Methods: The initial items for the instrument were developed through a literature review and interviews, using the conceptual framework of Maslow’s hierarchy of needs theory. The initial items were evaluated for content validity by 14 experts. Four hundred and eighty-six clinical nurses participated in this study through offline and online surveys to test the reliability and validity of the instrument.The first evaluation (n = 256) was used for item analysis and exploratory factor analysis, and the second evaluation (n = 230) was used to conduct a confirmatory factor analysis and to assess the criterion-related validity and internal consistency of the instrument. Test-retest reliability was analyzed using data from 30 nurses. Results: The final instrument consisted of 30 items with two sub-factors for five needs that were identified through the confirmatory factor analysis. The criterion-related validity was established using the five need satisfaction measures (r = .56). Cronbach’s a for total items was .90, and test-retest reliability was .89. Conclusion The findings from this study indicate that this instrument has sufficient validity and reliability. This instrument can be used for the development of nursing interventions to improve the needs satisfaction of clinical nurses.
