1.A Case of Lentigo Maligna Melanoma.

Herein we report a case of lentigo maligna that evolved into lentigo maligna melanoma. Fifteen years ago, the patient developed a pigmented lesion on the right cheek. The lesion progressed slowly, and 2 months ago, a black nodular lesion was developed in the periphery of the pigmented lesion. Histopathologic examination of the lesions revealed findings consistent with lentigo maligna and lentigo maligna melanorna. She had been performed wide excision and cheek flap for reconstruction.

2.A Case of Desmoplastic Malignant Melanoma.

Desmoplastic malignant melanoma (DMM) is an uncommon variant of malignant melanoma featuring a proliferation of spindle cells with pronounced desmopaasia. DMM commonly occurs on the sun-damaged skin such as head and neck in older persons, and it can be associated with lentigo maligna. We report an unusual case of desmoplastic malignant melanoma that developed de novo on the trunk in a young patient.

3.Immunohistochemical Study of Malignant Melanoma with HMB - 45 Monoclonal Antibody and Anti S - 100 Protein Antibody.

We evaluated the characteristics of HMB-45 monoclonal antibody(MoAb) and compared it with a polyclonal anti-S-100 antibody in immunohistochemical staining of 22 primary melanomas of the skin ; 13 acral lentiginous melanomas(ALM), 3 superficial spreading melanomas(SSM), 4 lentigo maligna melanomas(LMM), 3 nodular melanomas(NMM). All speimens were formalin-fixed and paraffin-embedded, and were studied with the avidin-biotin-peroxidase complex technique. The results can be summarized as follows : l. In all tissue specimens, all or some of the melanoma cells in the dermis reacted with HMB-45 MoAb and anti-S-100 protein. 2. HMB-45 MoAb is more sensitive in detecting malignant melanoma cell heterogeneity t,han anti-S-100 protein. 3. Melanoma cells within the epidermis in 6 of 12 ALMs and 3 of 4 LMMs did not react with anti-S-100 antibody, whereas most melanoma cells within epidermis of 12 ALMs and 4 LMMs reacted with HMB-45 monoclonal antibody. These results indicated that immunohistochemical staining with HMB-45 MoAb and anti-S-100 protein is very useful in the study of biologic behavior of malignant, melanoma.

Lentigo maligna melanoma and lentigo maligna are typically located on photo-exposed sites such as the head and neck, with the cheek being the most common site. Since lentigo maligna melanoma and lentigo maligna are found predominantly on such cosmetically-sensitive areas, it is critical to determine the exact histologic margin for maximal sparing of tissue and complete extirpation of the tumor. Since Mohs micrographic surgery has been recommended for tumors on cosmetically-sensitive locations where the tumor margin is indistinct, it appears to be a reasonable treatment modality for this type of tumor. We describe a case of lentigo maligna melanoma on the cheek, which was resected with Mohs micrographic surgery.

Lentigo maligna melanoma (LMM) is the least common type of melanoma and most commonly affects the sun-exposed skin of the elderly. We herein report a case of LMM located on the left lower eyelid of an 86-year-old woman. This female patient presented with a longstanding irregular brownish patch containing black macules consistent with LMM. There are several treatment modalities for removing the LMM lesion. However, considering the functional and the aesthetic aspect of the lower eyelid, we performed Mohs micrographic surgery to remove the lesion, and lateral based transposition flap was employed to reconstruct the resultant defect on the left lower eyelid.

6.A Case of Lentigo Maligna Developed on the Sole.

Lentigo maligna, a precancerous tumor arising from abnormal melanocytes, is a chronic, slowly progressive, pigmented lesion with a range of colors from pale tan to black and an irregular shape, The commonest location of lentigo maligna is the face during the sixth and seventh decades of life. Less frequently, the lesion occurs on an extrafacial area such as the hand or lower leg. In the late stage, lentigo maligna transforms into invasive malignant melanoma. A 50-year-old male patient had a well-defined, black pigmented patch on the right sole for 30 years. The histopathologic examination revealed numerous vacuolated melanocytes clusters at the dermo-epidermal junction with invasion of the epidermis. There was no evidence of dermal invasion by atypical melanocytes.

We describe a case of lentigo maligna melanoma evolved from lentigo maligna in 82-year-old man. Five years ago, the patient developed a brownish pigmented lesion on the left cheek. The lesion progressed slowly, and several months ago, a black pigmented patch was developed in the periphery of the brownish pigmented patch. Histopathologic examination of the lesions revealed findings consistent with lentigo maligna and lentigo maligna melanoma.

Lentigo maligna melanoma(LMM) is the least common type of melanoma and most commonly affects the sun-exposed skin of the elderly. We herein report a case of LMM evolved from lentigo maligna(LM). A 71-year-old female presented with a longstanding irregular brown patch containing two black papules on the left cheek which developed 8 months ago. Histopathologic findings were consistent with LM and LMM. We performed a wide excision and dufourmental flap for reconstruction with no recurrence for 30 months. Sun exposure has been known as the most important etiologic factor for the development of malignant melanoma including LMM and the incidence has an increasing tendency. Thus, skin biopsy is recommended in the evaluation of a longstanding irregular brown patch on the face to prevent LMM.

BACKGROUND: Malignant melanoma in Korea has been increasing as in other countries, but there is no nationwide survey of malignant melanoma in Korea. OBJECTIVE: The aim of this study was to document the clinical and histopathological features of cutaneous melanoma in Korea. METHODS: The Dermatopathology Research Group conducted a review of nationwide collection of 109 primary cutaneous melanomas, diagnosed at 23 institutes over a recent 3-year period. Clinical records and pathology slides of the patients were reviewed retrospectively. RESULTS AND CONCLUSION: The peak age was the 7th decade with the mean age of 58.1 years. Korea has a higher rate of acral melanoma and much lower rate of lentigo maligna melanoma. .Major component tumor cell was spindle cell type. Clark level of tumor was III or more and Breslow depth was 2mm or more at the time of the first diagnosis in many cases (62%). Pre-existing melanocytic nevus was present in a few cases (3 cases). All these features suggest that the racial difference between the Korean and the Caucasian is evident. We also suggest that an early detection program is very important to cure this malignant tumor.

10.A Case of Lentigo Maligna Melanoma.

We report herein a case of lentigo maligna in a 60-year-old female that progressed to lentigo maligna melanoma. She had an uneven dark brownish pigmented and irregular-bordered macule on the right cheek, which gradually extended peripherally over the last 20 years and became 2.5x2.5cm in size. A dark brownish nodule developed in the inferior portion of the macule 7 months ago. The histopathologic findings were consistent with lentigo maligna and lentigo maligna melanoma. The lesion was excised with a surgical margin of 1cm and a full thickness skin graft was performed.
