1.Diagnostic performance of 64-channel multislice computed tomography in assessment of significant coronary artery disease in subjects suspected as acute coronary syndrome and stable angina

Objective:To evaluate the diagnostic accuracy of 64-channel multislice spiral computed tomography(MSCT) in subjects with presentations suggestive of stable angina or acute coronary syndrome. Methods: Subjects received both 64-channel MSCT and coronary angiography, from Feb. 2006 to Feb. 2007, were enrolled for retrospective analyses. Results of the quantitative coronary angiography were used as the "Golden criteria", accuracy of 64-channel MSCT were evaluated in the overall sample, as well as in subjects suspected of stable angina and acute coronary syndrome. Results: A total of 120 subjects were enrolled in the analyses. On the patient level, the sensitivity, specificity, and accuracy of 64-channel MSCT in detecting significant stenoses were 92.5%, 50%, and 87.5%, respectively. The overall ROC area under curve was 0.71. On the artery level, sensitivity, specificity, positive predictive value, negative predictive value, and ROC area under curve of 64-channel MSCT were 69.9%, 83.8%, 81.1%, 73.7%, and 0.77, respectively. Further analyses showed the sensitivity, specificity, positive predictive value, negative predictive value, and ROC area under curve of 64-channel MSCT in subjects presenting as stable angina and in subjects presenting as acute coronary syndrome were as follows: 70.2% vs 69.2%, 76.2% vs 86.0%, 74.7% vs 85.6%, 71.9% vs 69.9%, and 0.73 vs 0.78. Conclusion: The accuracy rates of 64-chanel MSCT in subjects presenting as stable angina and in subjects presenting as acute coronary syndrome were similar.
2.Spleen autotransplantation plus portaazygous devascularization in the treatment of portal hypertension

Objective To evaluate spleen autotransplantation with lower esophagus transection for the treatment of portal hypertension by three-dimensional dynamic contrast-enhanced magnetic resonance angiography (3D-DCE MRA). Methods Twenty-eight patients were randomly divided into study group undergoing splenic autotransplantation and cardia-esophageal devascularization plus lower esophagus transection and control group without splenic transplantation. The cross section area, blood velocity and blood flow of main portal vein (MPV) were measured by 3D-DCE MRA, and the blood flow, collateral circulation of transplanted spleen were assessed. Results The cross section areas (cm ) of MPV of the two groups decreased postoperatively ( study group 1. 80 ? 0. 69 vs. 1. 20 ? 0. 73 , t = 13.96, P = 0. 00; control group 1. 78 ? 0. 43 vs. 1. 29 ? 0. 57, t = 11. 38, P = 0. 00). The mean blood velocity ( cm/s) of MPV decreased postoperatively (study group 9. 85 ? 0. 09 vs. 7. 06 ? 1. 98, t = 18. 98 , P = 0. 00; control group 10. 01 ?0. 43 vs. 8. 19 ?2. 44, t =22. 32, P =0. 00) in the two groups, and the mean blood velocity ' of MPV in study group was lower postoperatively than that in control group ( t = - 8. 02, P = 0. 00 ) . The mean blood flow (ml/s) of MPV decreased postoperatively (study group 15. 05 ? 2. 43 vs. 10. 52 ? 2. 55, 1 = 16.93, P=0. 00; control group 14. 81 ?2. 29 vs. 11.58 ?2. 96, t = 15. 90, P=0. 00) , and the mean blood flow of MPV in study group was lower postoperatively than that in control group (t = - 2. 73, P = 0. 02). Extensive collateral circulation developed around the transplanted spleen. Conclusions 3D-DCE MRA clearly shows the autotransplantated spleen and portal hemodynamics. It is an important non-invasive way to evaluate the result of the procedures.
3.Possible regulation channel of Xiaoyao San in treating rats of syndrome of liver invading the spleen by immunohistochemistry

Objective: Pervious RT-PCR results revealed the possibility of effective regulation channel of Xiaoyao San in treating rats of syndrome of liver invading the spleen. This experiment was designed to explore CIS rats’ immunohistochemistry findings in hippocampal subregion CA1 and amygdaloid subregion BLA after suppressing both amygdaloid AMPA receptors by microinjection of CNQX. To compare Xiaoyao San group with CNQX group, and to analysis of these findings may prove the possibility of effective regulation channel of Xiaoyao san treating this syndrome. Methods: 75 male SD rats were randomly divided into 5 equal groups: normal group, CIS group, sham-operation group, CNQX group and Xiaoyao San group. The number of GluR2 positive cells in hippocampal subregion CA1 and amygdaloid subregion BLA in these 5 groups were examined by immunohistochemistry test. Results: Compared with control group, the number of GluR2 positive cells decreased in hippocampal subregion CA1. Statistical analysis showed that no difference existed in control, CNQX and Xiaoyao San groups in CA1 region. Except CNQX group, tendency of changes were contradictory in BLA and CA1 region. In addition, the least in BLA in CNQX group. This finding revealed that Xiaoyao San may act the same as CNQX, thus Xiaoyao San afforded effective protection against this depression syndrome through suppression of both amygdaloid excitability. Conclusion: The balance coordination between hippocampus and amygdala may be the key in transformation from adaption to damage in a state of stress, was one of the central mechanisms of liver governing smoothing qi flow (LGSQF) on regulating the stress. Xiaoyao San may effectively regulate the balance of the excitability of hippocampus and amygdala. This hypothesis may reveal the regulation channel on Xiaoyao San affording effective protection against liver invading the spleen syndrome.
4.Expression and significance of calcyclin binding protein in rat brain tissues after traumatic brain injury

Objective To investigate the expression and significance of calcyclin binding protein (CacyBP)in the brain of rat model of traumatic brain injury(TBI).Methods Sixty 60 male SD rats were divided randomly into normal control group (n=10) and TBI group (n=50).The TBI model was created by using lateral head rotation device and subdivided into 6 h,24 h,72 h,7 d and 14 d group (10 rats per group).The expression and distribution of CacyBP in the rat brain was investigated immunohistochemically.The presence of the brown stained particles was considered aspositiveand lack of the stained particles agnegative. Results CacyBP was mainly distributed in the hippocampus,dentate gyrus and cortical neuron cytoplasm.Compared with the high level expression of CacyBP in the normal control group,the expression of CacyBP was decreased to the lowest in the rat brain at 6 h post TBI (P<0.01),became stronger gradually at 24 hours and recovered to normal at day 14,with no statistical difference compared with normal control group (P>0.05). Conclusion The lowest level expression of CacyBP after TBI indicates that CacyBP may play an important role in development of brain injury under effect of difierent mechanisms.
5.Comparative analysis on the effective competence of clinical head nurses'in Macau and Guangzhou

Objective To explore the clinical head nurses'effective competence through interviewing nursing supervisors by in depth interview method and to provide reference for constructing training content for clinical head nurses in Macau and Guangzhou.Methods In depth interview was carried out among 8 nursing supervisors from 2 hospitals in Macau and 9 nursing supervisors from 2 tertiary hospitals in Guangzhou.Content analysis was utilized through organizing records and then forming written documentations.Results The themes were drawn after the interview,which were good moral character accomplishment,good communication skill,prospective and creative thinking,favorable executive ability.Forwardlooking and innovative were significantly different in Macau and Guangzhou.Conclusions The effective clinical head nurses' competence is multi-faceted,the results can provide reference for selection,evaluation and training for head nurses.
7.Treating cervical spinal canal stenosis by open-door cervical laminoplasty combined modified buyang huanwu decoction.

OBJECTIVETo explore the clinical efficacy of open-door cervical laminoplasty combined Modified Buyang Huanwu Decoction (MBHD) to treat cervical spinal canal stenosis (CSCS).
METHODSTotally 32 CSCS patients were randomly assigned to two groups, Group A (17 cases, treated by laminoplasty) and Group B (15 cases, treated by laminoplasty combined MBHD). All patients received open-door cervical laminoplasty. Those in Group B took MBHD additionally for 2 weeks after surgery. The visual analogue scale (VAS), the Japanese Orthopedic Association (JOA) score, and the neck disability index (NDI) were measured preoperative, postoperative 3 months and 12 months, respectively.
RESULTSThere was no statistical difference in preoperative VAS, JOA, or NDI (P > 0.05). The VAS, JOA, and NDI were obviously improved 3 months and 12 months after surgery in the two groups, showing statistical difference when compared with before surgery in the same group (P < 0.01). At 3 months after surgery the aforesaid indices in Group B were superior to those in Group A (P < 0.05). There was no statistical difference in the aforesaid indices between the two groups at 12 months after surgery (P > 0.05).
CONCLUSIONMBHD favorably improved early recovery of neural functions of CSCS patients (3 months after surgery).

8.Minimally invasive surgical treatment for lumbar degenerative disease with IsoC-3D navigation under Mast Quadrant system.

OBJECTIVETo evaluate the effects of surgical treatment of IsoC-3D navigation assisted percutaneous pedicle screw fixation,vertebral canal decompression and interbody fusion using Mast Quadrant system in patients with lumbar degenerative disease.
METHODSFrom January 2009 to February 2010,21 patients with lumbar degenerative disease were treated with IsoC-3D navigation under Mast Quadrant system. There were 12 males and 9 females, with an average age of 50.2 years (ranged,36 to 72 years). All patients underwent discectomy,vertebral canal decompression, cage implantation using Mast Quadrant system and IsoC-3D navigation assisted sextant lumbar fixation. Clinical outcomes were evaluated with Oswestry disability index (ODI), Visual analog scale (VAS) and degree of satisfaction of patients.
RESULTSEighteen patients (85.7%) were followed up from 6 to 18 months with an average of 10 months. No surgery-related complications were found. The preoperative, postoperative ODI scores was 49.6 +/- 12.2 and 17.2 +/- 9.2, respectively (P < 0.01); VAS score of leg pain decreased from preoperative 75.2 +/- 10.0 to 12.2 +/- 11.8 at final follow-up (P < 0.01); VAS score of lumbago decreased from preoperative 59.9 +/- 17.3 to 16.6 +/- 11.5 at final follow-up (P < 0.01). Sixteen patients obtained satisfactory results.
CONCLUSIONIsoC-3D navigation assisted percutaneous pedicle screw fixation,vertebral canal decompression and interbody fusion using Mast Quadrant system could achieve satisfactory clinical results in treating lumbar degenerative disease and may be a better alternative to conventional surgical procedures. It has advantages such as limited tissue damage, less blood loss, short time in hospital.

9.Clinical application of alpha1 adrenoceptor antagonist Naftopidil to the treatment of chronic non-bacterial prostatitis.

OBJECTIVETo study the efficacy and safety of alpha1 adrenoceptor antagonist Naftopidil in the treatment of chronic non-bacterial prostatitis.
METHODSAn opened, self-controlled, multicentral clinical trial was conducted. One hundred and six cases of patients who had been diagnosed as chronic non-bacterial prostatitis (NBP) were treated with Naftopidil (25 mg once a day) for 4 weeks. The efficacy was evaluated by the NIH Chronic Prostatitis Symptom Index (NIH-CPSI) and the WBC in the examination of prostatic secretion (EPS) after the treatment.
RESULTSAfter 4 weeks therapy, 105 cases were evaluable. After treatment, NIH-CPSI total score were averagely decreased 12.0 points (P <0.001), symptom score 7.9 points (P <0.001) and QOL score 4.1 points (P <0.001). There was a statistically significant difference in WBC count ([(15.2 +/- 15.1)/HP vs (9.5 +/- 12.0)/HP, P < 0.01] and max flow rate(MFR) [(19.2 +/- 4.8) ml/s vs (22.7 +/- 4.9) ml/s, P < 0.01]. The total effective rate were 84.8% in the whole group. The clinical adverse rate was 3.81%, including 3 cases of mild dizziness and 1 case of mild inappetence.
CONCLUSIONalpha1 adrenoceptor antagonist Naftopidil is effective and safe for the treatment of chronic non-bacterial prostatitis.

10.Repairing of lower eyelid ectropion with expanded flap.

OBJECTIVETo investigate the effect of reconstruction of lower eyelid ectropion with expanded flap.
METHODSFourty patients with lower eyelid ectropion were reconstructed using tissue expander. The volume of the smallest expander was 30 ml, and that of the biggest one was 150 ml. The expand time was from 2-months to 3-months, then advancement or transposition flaps were created and employed in the defected lesion where the scar was removed just in one operation.
RESULTSAll patients have been followed up for 2-year with satisfactory results and no recurrences was appearance.
CONCLUSIONSApplication of expander reasonable may get satisfactory result in reconstruction of lower eyelid ectropion. The incision in donor site is hidden and the symptom seldom recurs.
