2.Clinical Observation on the Bladder and Urethral Injuries.

A clinical observation of the bladder and urethral injuries was made in the army hospital from Jan. to Dec 1968. The results were obtained as follow; 1. The bladder and urethral injuries were 2.3% of all patients treated in the hospital in the last year. 2 The bladder and urethral injuries were 23 cases of the automobile accidents, excluding 3 cases of expulsive injuries and 2 cases of falling accidents 3. The bladder and urethral injuries associated with fracture of pelvic bone were 32.1%, especially 80% in the pubic ramus fracture. 4. The incidence of urethral injury was higher than bladder injury. 5. The complications were urethral stricture (80%). impotence and etc.

4.A Case of Horseshoe Kidney Associated with Left Side Ureteral Stone.

A 24-year-old Korean soldier was found to have horseshoe kidney with left ureter stone. The diagnosis was made radiographically. Considering the rarity of this disorder, we decided to report this case. According to the literature. horseshoe kidney was described by Berengeri da Carpi for the first time in 1552. After Allen, the ratio of incidence of the horseshoe kidney is approximately 0.25% of the general population. Its etiology, incidence, complication, diagnosis and treatment was discussed. Recent related literatures were reviewed.

5.Management of Tibia Fracture by Closed Intramedullary Nailing

161 patients with tibia fracture were treated by closed intramedullary nailing at the department of Orthopedic Surgery, Eul-Ji General hospital, Dae Jeon, during 5 years from January, 1988 to April, 1993. 134 of these patients were performed with Kiintscher IM nailing and 27 patients with Ender nailing. Rigid IM nailing is the method of choice in tibia shaft fracture with rigid fixation, low complication, wide indication and early weight bearing, but the treatment of segmental fracture of the tibia will have many difficulties because of severe displacement, severe comminution, massive soft tissue damage and lack of blood supply at fracture site. Either antegrade or retrograde Ender nailing was of value for the management of segmental fracture which was too proximally or too dixtally located to insert interlocking screws, open fracture and soft tissue injury around Kiintscher insertion site. The result as fllows; 1. 43 fractures were open and 118 were closed. 69 fractures involved the distal portion, 55 fractures the middle portion, 13 fractures the proximal portion of the tibia and 24 fractures were segmental. 2. Among the 161 eases, 114 cases were male and 47 cases were female, the most common ages were ranged from 21 year to 30 year, involving 44 cases. 3. The most common cause was traffic accident. 4. Average intervals from injury to operation were 6.34 days(closed fracture) and 9.84 days(open fracture). 5. The mean durations of the bone union were 18.90 weeks(closed fracture) and 16.46 weeks(open fracture). 6. Complication included 7 cases delayed union, 3 cases nonunion, 8 cases superficial infection, 4 cases joint stiffness, 3 cases nail migration, 2 cases angular deformity, 2 cases rotational deformity, 1 cases osteomyelitis.

6.Cardiac Arrest. A Report of Application of External Cardiac Massage on Three Patients.

Three patients who were resuscitated with external cardiac massage are reported, who recovered completely without any neurological damage. In all cases, cardiac and respiratory resuscitation was started not later than 2 minutes after arrest-respiration and heart beat returned within 7 minutes. Hypothermia was done in two cases after resuscitation to promote easier recovery. All cases recovered completely without any neurological damage or complications, thus showing a 100% survival rate. Closed cardiac massage proved to be effective in these cases of cardiac arrest.

7.Topographic Central Island after Excimer Photorefractive Keratectomy.

We analyzed the topographic findings of central islands following excimer photorefractive keratectomies(VISX Twenty/Twenty, Inc, Santa Clara, CA) for myopia to evaluate whether the size of the central island was significantly different betwaen 1 week and 6months postoperatively and whether central island at postoperative 6months affected the decrease of corrected visual acuity from preoperative level. There were statistically significant differences in the indices related to the size of central island(peak, height, area) between 1 week(40 patients, 41 eyes) and 6 months(55 patients, 64 eyes) follow-up groups. In 64 eyes of 6months follow-up group, we could not find any statistically significant effect of the indices related to the size of central island on the decrease of corrected visual acuity, except SAI.

8.Anesthesia for Pheochromocytoma Removal A Case Report.

A 54 year-old woman underwent resection of pheochromocytoma under general anesthesia. Following induction of anesthesia with intravenous thiopental sodium, succinylcholine iodide was administered intravenously and endotracheal intubation was performed. Anesthesia was maintained with nitrous oxide, oxygen and diethyl ether using a semi-closed carbon dioxide absorption circle system. The course of anesthesia was rather stormy, reflected by hypertension, arrhythmia and hypotension, but the patient tolerated anesthesia well.

9.Eight Cases of Microbial Growth in Soft Contact Lenses.

Microbial growth(including fungal growth in 6 cases) was detected in 8 soft contact lenses Among the contact lens wearers who visited the Department of Ophthalmology, Asan Medical Center, between May, 1990 and Nov, 1990, contact lens deposits were easily recognized with naked eye and it was not removed by conventional cleaning methods, such as chemical cleaners and enzymatic agents. We examined these deposits under the microscope and found them microbial colonies. Isolation of microorganisms from the contact lenses and storage solutions was attempted in order to identify them. Direct microscopic observation of colonies grown in the contact lenses made it possible to identify 5 out of 10 cases. Growths of four cases were identified as Actinomyces sp., Trichothecium sp., Dermatophilus congolensis, Alternaria sp., and one case was found mixed growth of Humicolor sp. and Fusarium sp. Other 3 cases were highly suspected as fungus. Culure of the eight storage solutions were all revealed Serratia marcescens Our patients showed foreign body sense and redness in all 8 cases, superficial punctate keratitis in 1 case, and corneal erosion in 1 case.

10.Application of HFJV in Tracheal Stenosis .

Due to the increased use of tracheostomy and intermittent positive pressure ventilation, patients with trscheal stenosis have become more frequent. Recently we experienced a patient with tracheal stenosis who was tracheostomized upon admission, but unfortunately the stenotic lesion was located below the end of the tracheostomy tube. The stenotic lesion was l.6cm above the carina, its diameter was 0.5 cm, and the length of the stenotic segment was about 2cm, A3,5 mm(I.D.) endotracheal tube was passed through the stenotic lesion via the tracheostomy site, and high frequency jet ventilation was applied with a swivel connector. Immediately after the start of surgery, CO2retention occurred and the driving gas pressure increased from 4p to 5p psi, the I:E ratio from 1:2 to 1: 3, but the respiration rate (100 bpm) was maintained as before. CO2retention was relieved soon. Following end to end anastomosis a 6. 0 mm(I.D.) cuffed endotracheal tube was intubated orally and inhalation anesthesia using N2O-O2-Halothane was maintained until the surgery was completed.
