2.Microsurgical Treatment of Cerebral Arteriovenous Malformation.

Five cases of cerebral arteriovenous malformation(AVM) are presented, which underwent total extirpation using the operating microscope. The following are considerations which favor the total extirpation of AVM and the advantages of using the operating microscope, including the rationale for non-use of hypotensive anesthesia or mannitolization which is commonly used in intracranial aneurysm surgery: 1) Relatively small sized AVMs are found to show a high rate of large intracerebral hematoma, including that the degree of bleeding from the malformed vessels is not related to the size of the AVM. 2) The operating microscope facilitates the following procedures: a) determination of the extent of the malformed vessels. b) clipping feeders. c) identification of red vein and laminar flow. d) preservation of normal brain tissue.

5.Dandy-Walker Cyst Case Report.

In 1921, Dandy discussed the clinical diagnosis and treatment of hydrocephalus secondary to occlusion of the foramina of Magendie and Luschka. In 194, Walker reported a case of congenital atresia of the outlets of the fourth ventricle treated surgically. Since a detailed study of this disease entity by Benda in 1954, the condition has commonly been known by the term, the Dandy-Walker syndrome. In 1968, Raimondi and et al refered this condition as the Dandy-Walker cyst. There is considerable discussion as to the nature of the fundamental embryologic error in this condition, but Denda suggested that atresia of developing fourth ventricular foramina was only a part of the syndrome and not its cause, because in some cases reported, the foramina were found to be patent. These patients. Show characteristic clinical symptoms, signs and radiological findings and should suggest the correct diagnosis. This syndrome was one of the first cause of hydrocephalus to be treated successfully by surgery. We are reporting one case of this cyst encountered recently at Yonsei University Medical Center.

8.Surgical treatment of Malignant Tumors on the Lip and Vermilion border.

Treatment of tumors on the lip often present special difficulties in selecting the appropriate modality of treatment that will provide the patient with the best cosmetic result. The complex functions and contours of the lip, the many small cosmetic subunits and the limited availability of adjacent skin make the preservation of function and appearance difficult. Five cases comprised of 4 cases of squamous cell carcinoma, 1 case of basal cell carcinoma on the lip, particulary involved in the vermilion border. Five cases of malignant tumors were removed by total excision, followed by reconstructive surgery, such as A-T advancement flap, and advancernent flap after fusiform excision with angles greater than 30o (i.e., 60o), W-plasty, bilateral opposing advancement flap. We can treat malignant tumors on the lip involved upto one half of the total length by simple excision followed by reconstructive surgery under local anesthesia without great hazards. In our experience, surgical treatment is one of the best treatments for malignant tumors.

9.The Value of Colposcopy for Early Diagnosis of Cervical Cancer.

This study was performed to evaluate the diagnostic value of colposcopy by comparing colposcopic findings with cytology, histology of colposcopically directed biopsy and final biopsy result on hysterectomy. From January 1, 1988 to August 31, 1996, the colposcopic examinations was undertaken in 1200 patients, excluding grossly visible carcinoma of the cervix. The result were as follows : 1. Colposcopic examinations were satisfactory in 95.4% and unsatisfactory in 4.58%. 2. The accuracy rate of cytologic diagnosis was 73.1% and colposcopic impression was 93.1%. 3. The false negative rate of colposcopic impression was 2.6%, much less than 24,7% of cytologic examination. 4. The cytologic accuracy rate in cervicitis was higher than that of colposcopic impression but the accuracy rate of colposcopic impression in diagnosis of cervical intraepithelial neoplasia was higher than that of cytology. 5. Among 97 cases in which colposcopically directed biopsy were performed, more advanced lesion were disclosed in 12 cases(12.4%).
