2.Effect of Dexamethasone on Experimental Candida Endophthalmitis.

The use of intravitreal corticosteroids in the management of endophthalmitis remains controversial. This study was performed to evaluate the effectiveness of dexamethasone(DXM) in Candida endophthalmitis. In rabbits, Candida albicans(3,000 blastospore: Group A, 30,000 blastopore: Group B) was inoculated intrabitreally. At 24, 48, and 72 hours postinoculation, single intravitreal dose of amphotericin B(AMP, 5microgram/0.1ml) with DXM(400microgram/0.1ml) or AMP alone was given. Clinical examination, protein analysis, electrophysiological and histopathologic measures were utilized to rate DXM effectiveness. Dosage of 5microgram AMP resulted in no culture in both Group A and B, On examinations including the electrophysiologic and histopathologic studies, eyes treated with AMP and DXM did not differ from eyes treated with AMP alone in the Group A(p>0.05). On protein analysis for vitreous in the Group B, however, eyes treated with AMP alone at 24 and 48 hours postinoculation(p<0.05). On electrophysiologic study the decrease rate of the amplitude showed the difference between at 24 hours postinoculation and at 48 or 72 hours postinoculation(p<0.05). Histologic preparations showed a mild or moderate cellular infilteration in the vitreous and swelling in the sensory retina, but small abscess in the vitreous observed when treatment was delayed more than 72 hours following inoculation in both Group A and B. DXM in the experimental Candida endophthalmitis had the beneficial effect in eyes with 30,000 blastspore inoculation and in eyes treated with AMP and DXM at 24 hours postinoculation.

4.Vitamin D Deficiency Rickets.

Vitamin D deficiency rickets is a metabolic disorder of growing bone, which is still important nutritional problem in developing countries. Recently we had expirienced 2 cases of typical vitamin D deficiency rickets, who were fed with only thin rice gruels and not fed any nutrient diets, breast milk, vitamin D supplements and almostly not to be exposured to sunlight for a long period of time due to poor socio-economic condition. Treatments were consisted of 600,000 units of vitamin D injection, high protein, high calorie diets and frequent exposures to sunlight. All of two were improved in clinical and laboratory findings after above therapy.

5.Characterization of a New Integron Containing VIM-2 Metallo-beta-Lactamase Gene Cassette in an Isolate of Morganella morganii from a Urine Specimen.

PURPOSE: The main objectives of this work were to characterize the mechanism of resistance to imipenem of Morganella morganii KU158 isolated in Busan, Korea and to analyze the structure of the integron, which carries the resistance gene that confers resistance to imipenem. MATERIALS AND METHODS: Antimicrobial susceptibilities of M. morganii KU158 were tested by using the disk diffusion method. The modified Hodge and EDTA-disk synergy tests were performed for the screening of metallo-beta-lactamase-producing isolates. blaIMP and blaVIM genes were detected using the polymerase chain reaction (PCR) amplification. To detect the presence of the integron, the PCR method was used. The PCR product was cloned through the use of primers, 5'CS-F and 3'CS-R, and it was used to determine the sequence of the integron through the dideoxy-mediated chain termination method. RESULTS: M. morganii KU158 was intermediately resistant to imipenem and showed a positive result for the modified Hodge and EDTA-disk synergy tests, which suggest the production of metallo-beta-lactamase, and also was positive in the PCR result for the detection of blaVIM gene. The genotype of the PCR product from the blaVIM gene was blaVIM-2. Sequencing of the 5,031 bp-cloned fragment revealed the structure of the class I integron, such as the 5'-CS element containing an Intl1 integrase gene with its own promoter region, the attI1 recombination site, and the 3'-CS element containing qacE1. The integron contained insert gene cassettes blaVIM-2, aac(6')-Ib, aadA1, "orfII", and "orfIII". The blaVIM-2 gene was located immediately downstream of the aac(6')-Ib gene. CONCLUSIONS: M. morganii KU158 acquired the resistance to imipenem through the production of metallo-beta-lactamase VIM-2. The gradual increase in the number of VIM-2-producing bacterial species may indicate the highly mobile nature of the blaVIM-2 cassette. The spread of blaVIM-2 could compromise the future usefulness of carbapenem in treating gram-negative bacilli infections.

6.A Case of Primary Central Nervous System Lymphoma with Ocular Involvement.

PURPOSE: To report one case of primary central nervous system lymphoma with ocular involvement confirmed by pars plana vitrectomy. METHODS: A 52-year-old healthy man visited our hospital complaining of progressive dysarthria, memory impairment, and chronic headache. Brain CT scans and MRI scans revealed a brain tumor in the left basal ganglia and the frontal lobe. Stereotactic biopsy of the tumor showed a large diffuse B-cell type malignant lymphoma. Three months later, he presented with decreased vision and sensation of floaters in the right eye. Funduscopic examination of the right eye revealed a vitreous opacity peripherally. He underwent pars plana vitrectomy and vitreous biopsy to rule out ocular involvement of lymphoma. Smears from the cytospin of the specimen prepared from vitrectomy in the right eye showed a greater number of large, pleomorphic lymphoid cells. Flow cytometric immunophenotyping showed a clonal population of CD 20 which was conclusive for infiltration with B-cell lymphoma. RESULTS: The patient underwent treatment with radiotherapy to the brain and the right eye in combination with systemic and intrathecal chemotherapy. After five cycles of chemotherapy and radiotherapy, the brain mass disappeared, the neurological symptoms were improved, and the visual acuity of the right eye increased.

9.Results of Nuvita Phakic Anterior Chamber Intraocular Lens for High Myopia.

PURPOSE: The study was designed to evaluate the clinical results of an anterior chamber intraocular lens implantation (Nuvita MA 20 implantation) on high myopic patients. METHODS: We reviewed the clinical results of Nuvita MA 20 implantation surgery in 6 high myopic patients (11 eyes) with more than -7.0 diopters (D) for a year after the surgery. One patient was male,five patients were female and the mean age was 31.5 years old (25~41 years old). RESULTS: Mean preoperative spherical equivalent was -16.61 4.9 D and mean postoperative spherical equivalent was -0.9+/-0.5 D at 1 week, -1.275+/-1.14 D at 1 month, -1.14+/-1.22 D at 3 months, -1.18+/-1.18 D at 6 months and -1.05+/-1.2 D at 12 months. Mean uncorrected visual acuity preoperatively was 0.03+/-0.06 and postoperatively was 0.6+/-0.25 at 1 week, 0.57+/-0.25 at 1 month, 0.67+/-0.4 at 6 months, 0.8+/-0.32 at 12 months. Postoperative complications included 6 eyes of IOL rotation (55%), 4 eyes of pupil distortion (36%), 1 eye of intraocular pressure elevation (9%), 1 eye of pupillary block (9%), 1 eye of IOL decentration (9%) and 1 eye of nuclear cataract (9%), but serious postoperative complications such as retinal detachment and bullous keratopathy were not found. Mean endothelial cell density was 2677+/-148 (cells/mm2) at preoperative period, 2576+/-167 (cells/mm2) at 1 month, 2546+/-127 (cells/mm2) at 3 months, 2490+/-171 (cells/mm2) at 6 months and 2408+/-133 (cells/mm2) at 12 months. CONCLUSIONS: The implantation of Nuvita MA 20 into anterior chamber is considered to be a safe method of treatment for patients with high myopia.

10.Endothelial Cell Changes after Penetrating Keratoplasty.

The purpose of this study was to investigate the correlations between endothelial cell loss rate at one week, one month, three months, and six months after penetrating keratoplasty and clinical factors including recipient age, donor age, preoperative endothelial cell densities of the donors and preoperative diagnoses of the recipients. Among the 70 patients who underwent penetrating keratoplasty at Chonbuk University Hospital from December 1996 to January 1999, 30 eyes of 30 patients that showed reasonable endothelial resolution by non-contact specular microscopy during the follow up period of 6 months were chosen and evaluated.The cell density decreased continuously during the observation period. Endothelial cell loss rate averaged 11.5%at one week, 19.0% at one month, 25.0%at three months and 30.0%at six months after penetrating keratoplasty. Endothelial cell loss rate showed no significant correlation with donor age, recipient age, preoperative cell density at any examination period[r+/-0.4, p0.05]. Preoperative endothelial cell density showed significant correlation with postoperative endothelial cell density at each examination period[r0.7, p<0.05]. The rate of endothelial loss in the keratoconus group was significantly lower than those of bullous keratopathy or corneal leukoma groups at three months after penetrating keratoplasty[Wilcoxon test, p<0.05].
