4.Epidemiologic Findings of Behcet's Syndrome.

For the purpose of epidemiologic study, the patients who visited Severance Hospital Behcet's Syndrome Specialty Clinic were grouped by Shimizu classification into the complete, incomplete, suspected types and evaluated. The results of the evaluation with respect to personal characteristics, geographic distribution, socioeconomic characteristics, family history, and clinical characteristics are as follows. 1. The incidence among the subgroups was incomplete type(46.3%), suspected type(36.7%), complete type(17.0%) and the male to female ratio was 0.60: 1, and the average age of onset was 29.1. years for males and 27.6 years for females. 2. The occupational distribution among males included office workers, 31.5%; drivers, 10.6% and among females was office workers, 6.7%; teachers, 3. 7 %; unemployed, 81.7%. 3. Among the presumed aggrevating factors affecting oral cavity, recurrent tonsillitis(20.8%) was most common, and premenstrual exacervation(12.8%) was most common. 4. The most common initial major manifestation was oral ulcer(80.6%), and the longest duration of major manifestations was oral ulcer(average 6.4 years), and the most frequent relapse of major manifestations was oral ulcer(average 14.6 times/year). 5. The average duration between the appearance of the first and second major manifestations was 5.2 years for complete type, 6.3 years for incomplete type, and 6.6 years for suspected type. 6. Combinations of major manifestations most commonly included oral ulcer, genital ulcers and skin lesions(65.3%) for incomplete type, and oral ulcer, skin lesion(54.8%) for suspected type.

6.Tuberculous Spondylitis Aggravated by Spinal Manipulative Therapy: A case report.

A 43 years old woman had suffered from a lower back pain for 2 months. She experienced pain aggravation after spinal manipulative therapy that was practiced by non-licentiate. Physical examination showed tenderness on L1 and L2 spinous processes. Radionuclide bone scan with 99mTc-MDP showed increased radioactivity of L1, L2 vertebral bodies. The MRI finding showed low signal intensity of L1 and L2 vertebral bodies in T1-weighted image and high signal intensity in T2-weighted image. Needle biopsy finding showed fibrosis and inflammatory cell invasion of bone marrow. We concluded that she had tuberculous spondylitis and non-detection or negligent treatment of a preexisting disease contributed to aggravation of her symptoms. We report one case of tuberculous spondylitis aggravated by spinal manipulative therapy with review of literatures.

7.Establishment of a chronic obstructive pulmonary disease mouse model based on the elapsed time after LPS intranasal instillation.

Chronic obstructive pulmonary disease (COPD) was the 3rd leading cause of death in 2012 worldwide. It is particularly severe in the elderly, who are at risk of death by coughing, mucous hypersecretion, and finally breathlessness. Recently, anti-COPD drug development has increased, and many animal screening systems have been studied. Tobacco smoke animal models are the best known animal screening system, but have several preparation requirements, such as a tobacco smoke generator and a separate facility to prevent smoke release. Accordingly, we evaluated the properties of a lipopolysaccharide (LPS) murine model for COPD screening and the effect of the time elapsed from 0 to 72 hr after LPS intranasal instillation on various biomarkers of COPD severity, such as WBC and neutrophils in bronchoalveolar fluid (BALF), IgE in serum, histopathology in the lung, and cytokines (IL-8, TNF-α, IFN-γ, and TGF-β) and chemokines (CCL-2, CXCL1, CXCL9, CXCL10, and CXCL11) in the respiratory system. Although from 48 hr after LPS treatment several factors which could be evaluated as biomarkers for COPD establishment such as WBC and neutrophil in BALF, IgE in serum, cytokines (IL-8, TNF-α, and IFN-γ), and chemokines (CCL-2, CXCL1, CXCL9, CXCL10, and CXCL11) increased at 72 hr the increment of important factors for COPD establishment such as IgE, fibrosis in the lung, and cytokines (IL-8, TNF-α, and IFN-γ) was more clear. Based on our results, we concluded that the optimal time after LPS intranasal instillation is 72 hr.

As volar plate fixation of distal radius fracture becomes more common, reports of ruptured extensor pollicis longus tendon by a protruding distal screw tip are also increasing steadily. Authors have experienced a rare case of ruptured extensor pollicis longus tendon at the prominent proximal screw of fixed volar plate for distal radius fracture, and we report it herein with a review of the literature.

10.Effect of Hyperbaric Oxygenation on Pulmonary Superoxide Dismutase Activity in Neonatal and Adult Rats.

To investigate the effect of hyperbaric oxygenation on superoxide dismutase activity, neonatal rats (7-10 days old) and adult rats (approximately 100 days old) were continuously exposed to hyperbaric oxygen environment of 2.4ATA for 8 hours and their superoxide dismutase activity were measured. Neonatal rats, all survived through exposure, showed significant increases in the pulmonary superoxide dismutase activity at immediately and 24 hours after exposure. Adult rats, whose 8 hour survival rates were 14 %, did not show any significant increase in the activity of pulmonary superoxide dismutase as compared to the control adult rats. These findings are indicating that increased tolerance to oxygen toxicity in neonatal animals during exposure may be attributed to the increase in activity of superoxide dismutase in neonatal rats.
