2.Improvement of Smooth Pursuit Eye Movements after Cigarette Smoking in Chronic Schizophrenic Patients.

This study was done to examine whether cigarette smoking improved smooth pursuit eye movement(SPEM) abnormalities in chronic schizophrenic inpatients. Fifteen schizophrenic and twelve alcoholic subjects abstained from their usual cigarette smoking for a minimum of nine hours and their baseline performances during the constant velocity smooth pursuit tasks were assessed. Then, the subjects smoked as much as they desired in a 10 minutes period and were retested immediately after smoking and 15 minutes after smoking. Electrooculographic recordings during the eye movements were converted and saved as digitized files. Power spectral density curves and natural logarithm value of signal/noise(Ln S/N) ratios were computed from them. In the schizophrenic patients, Ln S/N ratios increased significantly immediately after smoking compared to baseline. But Ln S/N ratios showed no statistically significant changes after 15 minutes compared to baseline. In alcoholic subjects Ln S/N ratios showed no statistically significant changes immediately after smoking and after 15 minutes compare to baseline. In conclusion, SPEM was improved in schizophrenic patients immediately after smoking and we hypothesized that nicotinic receptor dysfunction maybe a candidate mechanism for smooth pursuit eye movement abnormalities in schizophrenia.

4.Urinary Retention after Perianal Operation.

BACKGROUND: Urinary retention is the most common complication after perianal surgery. The authors tried to evaluate the influence of the types and duration of the operation and the types of anesthesia on the incidence of urinary retention. METHODS: The medical and anesthetic records of 106 patients were reviewed retrospectively. They got perianal surgerys after recieving one of the regional blocks; 0.5% hyperbaric bupivacaine 5~8 mg intrathecally, 0.5% hyperbaric tetracaine 5~8 mg intrathecally, or 2% lidocaine 300 mg with epinephrine 5 g/ml caudally. The incidences of urinary retention were compared with each other by Chi-square test and Student t-test, according to the above mentioned points. RESULTS: Fifty percent of these patients underwent urinary catheterization. The incidence of urinary retention after hemorrhoidectomy (56.6%) was higher than that of other anorectal procedures (p<0.05, Chi-square test). The longer operating time was associated with urinary retention (p<0.05, Student t-test). The influence of local anesthetics (bupivacaine, tetracaine, and lidocaine) was absent (p>0.05, Chi-square test), but the difference between spinal and caudal anesthesia was slightly significant (p<0.05, Chi-square test), i.e. the incidence of urinary retention after caudal anesthesia was low. CONCLUSIONS: Short duration of operation, less traumatized perianal surgery, and caudal anesthesia are thought to lead to the lower incidence of urinary retention.

6.Time-sequential Development of Multiple Squamous Cell Carcinomas on Palms and Soles.

Squamous cell carcinoma of the skin arises mostly in the head and neck regions, less frequently in the rest of the body, and rarely in the palms and soles. We report a case of a 6S-year-old man who had had time-sequential development of multiple squamous cell carcinomas on his palms and soles for the past 12 years. These lesions were, in order of time, ulcerative nodules, ulcerative hyperkeratotic papules, hyperkeratotic plaques, maceratied plaques, dark discolored patchs, and hyperkeratotic papules. They were treated by total excision and cryotherpy apart from the last squatnous cell carcinoma in 1994.

7.Therapeutic Effects of Low Energy Laser on Acute Phase of Herpes Zoster.

BACKGROUND: Herpes zoster(HZ), caused by reactivation of latent varicella-zoster virus, is manifested by dermatomal distribution of skin rashes, acute pain and post-herpetic neuralgia by pathologic involvement of the sensory ganglia. Low-energy laser(LEL) deliver a small amount of energy without elevation of the tissue temperature. LEL is reported to have various biologic effects such as anti-inflammatory, analgesic, regenerative, antiallergic, immunocorrective, and antibacterial effects. OBJECTIVE: We attempted to evaluate the therapeutic effect of LEL on an acute phase of HZ. METHODS: A total of 60 patients with acute HZ were enrolled in the study. The control group(30 patients) received medications including analgesics, antihistamine, and topical care without LEL irradiation. The experimental group(30 patients) were irradiated with LEL daily for 7 days in addition to the same kinds of medications for the control group. The efficacy of LEL irradiation for HZ was assessed by analyzing changes in skin rashes, pain scores, and postherpetic neuralgia.

9.The Effect of Incubation Temperature on Susceptibility Test of Dermatophytes to Oral Antifungal Agents.

BACKGROUND: thermotherapy has been shown to be effective in the treatment of some fungal infections. Dermatophyte are well grown at 25degrees C rather than 37degrees C or high temperature. OBJECTIVE: An vitro test was done to assess the complemental effect and optimal conditions of local heating on the susceptability of t. rubrum to systemic antifungal agents. METHODS: Microdilution susceptability test to ketoconazole and itraconazole was done using 96 well microplate. Eight strains of T. rubrum were isolated from patients withtinea pedis and were cultured at 25degrees C, 37degrees C and 42degrees C for 1, 8 or 24 hours per day. MIC were checked at 4th, 7th, 9th day after inoculution. RESULTS: The growth without antifungal agents at 37degrees C and 42degrees C were decreased by 805 and 50% of the growth at 25degrees C respectively. Seven day after inoculation was the proper time to check the MIC. MIC50 of ketoconazole was the lowest at 42degrees C for 24hours per day in value of 0.006microgram/ml, and 0.09microgram/ml at 37degrees C for 24hours per day, 0.37microgram/mlat 42degrees C, for 8hours per day and 37degrees C for 8hours per day. MIC at 42degrees C for 1 hours er day, 37degrees C for 1 hyours and 25degrees C for 24hours per ady MIC were the same in value of 0.05microgram/ml. MIC50 of itraconazole was the lowest at 42degrees C for 24hours per day in value of 0.006microgram/ml, 0.01microgram/ml at 37degrees C for 24hours per day, 0.02microgram/ml at 37degrees C for 8hours per day. MIC at 42degrees C for 8hours per day, 42degrees C for 1hours per day, 37degrees C for 24hours per day MIC were the same in value of 0.05microgram/ml. CONCLUSION: Incubation at 37degrees C for 24 hours per day or 42degrees C for 24 hours per day increased the susceptability of T. rubrum to ketoconazole and itraconazole.

10.Clinical Study of Febrile convulsion and Factors Related to Recurrence.

We have reserched the relationship of their clinical pictures and factors related to the risk of recurrence of 75 patients with simple or complex febrile convulsions, who were admitted to the Departmenrt of Pediatrics, Chungnam National University Hospital from January 1987 to July 199. The 75patient were followed up and consisted of the 55 patients with initial or non-recurrent febrile convulsions and the othere 20 patients with recurrent febrile convulsions. The results were of follows; 1) The age of first episodes was under the 6 years in 94.6% and the first episode under the 12 months was 14.6% in initial cases and 55% in recurrent cases. 2) There were family history of convulsive disorder in 40% of recurrent cases, compared to 10.9% of initial cases. 3) In gestational age, there was no significant difference between initial and recurrent cases. 4) In sex distribution, the boys (66.7%) outnumbered the girls(33.7%) and the ratios was 2:1. Among the recurrent cases of 20 patients, the boys(90%) were much more than the girls(10%). 5) The patients of low birth weight had more febrile convulsions than large birth weight in both initial and recurrent cases. 6) There was no significant difference between initial and recurrent cases in the causes of febrile convulsion. 7) In abnormal EEG findings, recurrent cases were 30% more than 7.3% of initial cases. 8) There was no significant difference of number of seizure during a day in both initial and recurrent cases 9) In episodes of duration over 15 minutes, recurrent cases were 40% more than 23.7% of initial cases. 10) The resident of urban was 78.6% and of rural was 21.4%. 11) The types of convulsion were generalized in 92% of total cases and focalized in 8%. In the focal types, recurrent cases (15%) were much more than initial cases (5.5%). 12) In occurrence of seizures in body temperature above 40 degrees C, it was 5.5% in initial cases, whereas it was 20% in recurrent cases. 13) 13) There was no significant difference between initial and recurrent cases on fever duration before seizure.
