1.Clinical Evaluation of Congenital Dislocation of the Hip

Congenital dislocation of the hip has always presented an unique problem in Orthopedic Surgery. Orthopedic surgeons have been baffled not only by the mystery of its origin, but also by its unpredictable course and the uncertainty of results of treatment. Hippocrates described this condition first with all its distinguishing characteristics, including distortion of the hip, the pathologic gait, the exaggerated lordosis. etc, His prognosis in this luxation of early childhood appeared to be unfavorable. In 1895, Lorenz established a new, decidedly conservative method of treatment. His process involved a complete separation of the two objectives to be fulfilled, namely reduction and retention. To retain this reduction he conceived the motion of fixing the hip for a specific and prolonged period in carefully chosen “primary position” of 90 flexion and 90 abduction by forcibly maintaining the head in its normal location. Ortolani, in 1935, was the first to show that a diagnosis could be establisted immediately after birth. The jerk, click or snap elicited by manipulation of the new born infants hip. Early diagnosis is still the most important aspect of congenital disiocation of the hip. Despite the emphasis on early diagnosis, a disturbingly large number of congenital dislocation of the hip remain undiagnosed until after the child has started to walk. Numerous surgical procedures and modifications have been used. The general types are: 1) Open reduction, 2) Shelf procedure, 3) Innominate osteotomy, 4) Arthroplasty, and 5) femoral osteotomy. These procedures have been useful also with the more difficult problem of residual or recurrent dislocation or subluxation aftertreatment by closed or open method. Congenital dislocation of the hip was not common in Korea like as in western countries. The reasons for this are poorly understood. But racial differences and native customs may offer a partial explanation. The author studied congenital dislocation of the hips in sex incidence, site of predilection, clinical symptoms and signs, X-ray findings, relation to the congenital anomalies and birth history, and the results of treatment. In this study, 57 cases of congenital dislocation of the hip, admitted to the orthopedic department of Severance Hospital during 7 years and 6 months since January 1963 until June 1970, were reviewed. The results of this study are as follows: 1. The annual changes of the numbers of the patients, who were first seen at Severance Hospital, were increasing. 2. The female incidence in Korea is relatively low as compared with the western countries. And the preponderance of females over males is in the ratio of ten to one in bilateral cases and two to one in unilateral cases. 3. The ratio of bilateral cases: right sides: left sides is 1:2: 2.2. 4. In these series, 15 cases of congenital anomalies or diseases in 13 patients are combined and among them, 10 patients are included in teratologic groups. And most common co-existing anomalies are club foot (7 cases). The incidence of the teratologic dislocation in Korea is relatively high as compared with the western countries. 5. The acetabualr index in affected hips showed no relationship to the age factor, but C-E angle, Y-coordinate and height are aggravated according to the increment of age. 6. Closed reduction is satisfactory under the age of 3, but after then the prognosis is poor. 7. Open reduction is recommendable after the age of 3 and under the age of 9. 8. The prognosis of treatment in bilateral cases and teratologic groups is poor.

2.Changes to Balance and Trunk Repositioning Sense According to Frequency of Falls in Stroke Patients.

BACKGROUND: The purpose of this study was to investigate the relationship between the changes to postural sway velocity and trunk repositioning errors and the frequency of falls in stroke patients. METHODS: Fifty-five stroke patients, divided into three groups (no falls group: 20, one fall group: 19, repeated falls group: 16), stood quietly with eyes open and closed on a force platform while postural sway velocity was quantified by the center of pressure measures. Trunk repositioning errors were measured in standing while the subjects performed forward flexion movements. RESULTS: We found a significant difference between the groups for postural sway velocity in medial-lateral and anterior-posterior directions with eyes open and closed (P<0.05). With the eyes closed, there were significant differences between the three groups in the postural sway velocity (P<0.05) and the trunk repositioning errors changed significantly (P<0.05). CONCLUSIONS: Our results demonstrate that the increase in falls may increase visual dependence for postural control velocity and trunk repositioning errors. These results may be useful in balance training to prevent falls in stroke survivors.

3.Repair of Chromate induced DNA-Protein Crosslinks in Rat Lymphocyte.

Genotoxic agents can induce various DNA lesions. DNA-Protein Crosslinks(DPCs) were known as the important DNA lesions which could impair gene expression because DPCs had a high probability of resisting repair and persisting through cell cycle. This repair resistance of DPCs could have biological significance but had not been evaluated clearly yet. Most of the studies that have evaluated the repair of DPCs only compared the extent of DPCs repair with other DNA lesions. We injected K2CrO4, a genotoxic agent, into Sprague-Dawley rats intraperitoneally(5mg/kg) and isolated blood lymphocytes 12 hours later. These lymphocytes were cultured in the mitogen added growth media and mitogen free media separately. The degree of the repair of DPCs was monitored for 4 days by the K-SDS assay. 4 day later, the amount of DPCs decreased by 4.6% in the mitogen added media but in creased by 10.9% in the mitogen free media. These results showed that DPCs induced by K2CrO4 were not repaired easily and the DPCs were biologically significant DNA lesions. We thought the decrease of DPCs in the mitogen added media was not due to the repair of DPCs, but from the increase of normal cell proliferation. Therefore, it is very important to consider the proliferation of normal cells when estimating the repair of DPCs.

6.SHEAR BOND STRENGTH OF PRETREATED DENTIN SURFACE WITH RESIN-REINFORCED GLASS IONOMER CEMENT.

The purpose of this study was to evaluate the effect of dentin pretreatment with Dentin Conditioner,Ultra-Etch,conditioner of Fuji Plus cement on the shear bond strength of resin-reinforced glass ionomer cements to dentin and analyze the fractured surfaces. To evaluate the bond strengh, the extracted human teeth which had uniform area of exposed dentin were cemented with conventional glass ionomer cement. 3M RelyX TM Luting (Vitremer luting cement). Fuji Plus cement after dentin pretreatment. The shear bond strengh was measured using the Universal testing machine(Instron Co., USA) with a crosshead speed of 1mm/m. The effect of dentin pretreatment was evaluated by observing pretreated dentin surfaces under the scanning electron microscope,measuring the shear bond strength and observing the fractured surfaces under the scanning electron microscope. The results were as follows: On the SEM observation of surface morphology, the specimens treated with Dentin Conditioner, Ultra-Etch and conditioner of Fuji Plus cement were removed the smear layer and funneled dentinal tubules in dentin surfaces. In RelyX TM Luting cement group, shear bond strengh of pretreated group was significantly higher than control group. In Fuji Plus cement group and Fuji I group, regardless of the type of pretreatment agents, there was tendency of increase in the shear bond strength. On the SEM observation of fractured surfaces, as the shear bond strength increase, it were shown thicker cement layers and were not shown dentinal tubules. According to these results, it were shown that dentin pretreatment have much effect on bonding states.

7.Expression of Epstein-Barr Virus Gene Products, Bcl-2 and p53 Proteins in Nasopharyngeal Carcinomas.

The authors studied EBV genome expression in 40 conventionally processed samples of nasopharyngeal carcinomas (NPC), using in situ hybridization for EBERs and immunohistochemistry for LMP, Bcl-2 and p53 proteins. The NPCs consisted of 6 keratinizing squamous cell carcinomas (KSCs), 13 nonkeratinizing carcinomas (NKCs) and 21 undifferentiated carcinomas (UCs). The results were summarized as follows: 1) EBERs were expressed in 80.0% of all the NPCs (32/40). As for the subtypes, they were detected in 92.3% of NKCs (12/13), in 90.5% of the UCs (19/21), and in 16.7% of the KSCs (1/6). In positive cases, the nuclei of tumor cells displayed uniformly strong staining. 2) LMP was expressed in 10.0% of all the NPCs (4/40), all of which were UC. The LMP expression in the UCs was not correlated to the expression of EBERs, Bcl-2 and p53 proteins. 3) Bcl-2 protein was detected in 85.0% of all the NPCs (34/40). As for the subtypes, they were detected in 92.3% of the NKCs (12/13), in 90.5% of the UCs (19/21), and in 50.0% of the KSCs (3/6). 4) p53 protein was detected in 75.0% of all the NPCs (30/40). As for the subtypes, they were detected in 81.0% of the UCs (17/21), in 69.2% of the NKCs (9/13), and in 66.7% of the KSCs (4/6). 5) In the NPCs the expression of EBER showed a significantly positive correlation with that of p53 or Bcl-2 protein. The above results indicate that the association of EBV with NPC is chiefly with poorly differentiated and undifferentiated carcinomas. Additionally, carcinomas commonly display widespread, strong immunoreactivity of Bcl-2 and p53 proteins over tumor cells. In conclusion, these observations indicate that the EBV-association in NPC appears to contribute to the overexpression of tumor-related genes during carcinogenesis.

8.A STUDY OF THE STRESS DISTRIBUTION OF THE ABUTMENT AND SUPPORTING TISSUES ACCORDING TO THE SLOPES AND TYPES OF GUIDING PLANES OF THE LAST ABUTMENT IN DISTAL EXTENSION REMOVABLE PARTIAL DENTURE USING THREE DIMENSIONAL FINITE ELEMENTANALYSIS METHOD.

The purpose of this study was to investigate the stress distribution of the abutment and supporting tissues according tot he slopes and types of the guiding plane of distal extension removable partial dentures. The 3-dimensional finite element method was used and the finite element models were prepared as follows. Model I : Kratochvil type guiding plane with 90degree to residual ridge Model II : Kratochvil type guiding plane with 95degree to residual ridge Model III : Kratochvil type guiding plane with 100degree to residual ridge Model IV : Krol type guiding plane with 90degree to residual ridge Distal extension partial denture which right mandibular first and second molar were lost was used and the second premolar was prepared as primary abutment with RPI type retainer. Then 150N of compressive force was applied to central fossae of the first and second molars and von Mises stress and displacement were measured. The results were as follows : 1. Model I and Model IV showed a similar stress distribution pattern and the stress was concentrated on the apex of the root of the abutment. 2. The stress was increased and concentrated on mesial side of the root of the abutment in Model II. The stress was concentrated on buccal and mesiobuccal side of the root of the abutment in Model III. 3. In Model I, the root of the abutment displaced and twisted a little in clockwise. In Model IV, the root of the abutment displaced to distolingually at apical region of the root and mesiobucally at cervical region of the root. 4. In Model II, the root of the abutment displaced to mesiolingually at apical region of the root and more displaced and twisted in counterclockwise at cervical region of the root. In Model III, the root of the abutment displaced to mesiobucally at apical region of the root and more displaced and twisted in clockwise at cervical region of the root.

9.FITNESS TEST USING THE PHASE-SHIFTING PROFILOMETRY ACCORDING TO THE DENTURE CURING METHODS.

According to repeated measurements and correction procedures, the accuracy of the phase-shifting profilometry was developed. At first, after 20 final models for maxillary complete denture were duplicated, the mucosa surfaces of models were measured with the phase-shifting profilometry and each mirror view of these was calibrated. Maxillary cats were divided into 4 groups of 5 casts per each, and wax dentures with 2 sheets of baseplate wax and artificial teeth were made and then cured according to the curing method of each group. Group I ; quick curing with QC-20 acrylic resin Group II ; 9 hour curing with QC-20 acrylic resin Group III ; SR-Ivocap system Group IV ; metal base and quick curing with QC-20 acrylic resin After curing, polishing, and storing at 37degrees C n saline for 30 days, the forms of the impression surface of the dentures were measured with the phase-shifting profilometry. Then, the impression surface form of each denture was placed in the optimal position of comparison with the mirror view of the same final cast. The amount and direction of distortion of each denture was analyzed and the effects of polishing and storage in each denture were compared. The obtained results were as follows : 1. In Group I, the denture was observed as the appearance distorted in the opposite direction of the mucosa and the postero-lateral part of palatal portion of the denture was observed as the appearance separated from the mucosa. Also, the buccal flanges of the denture were observed as the appearance distorted in the direction of the mucosa. 2. In Group II, the postero-lateral part of palatal portion of the denture was observed as the appearance separated slightly from the mucosa. The bilateral buccal flanges of denture were observed as the appearance distorted severely in the direction of the mucosa. 3. In Group III, the bilateral part of the residual ridge crest portions and the buccal flanges of the denture were observed as the appearance distorted in the direction of the mucosa, and specially, the buccal flanges of the maxillary tuberosities were distorted severely. 4. In Group IV, the acrylic resin base of the buccal portion of the denture was observed as the appearance distorted in the opposite direction of the mucosa. 5. The phase-shifting profilometry, done with repeated measurements and correction procedures, was effective in comparing the amount and direction of distortion at every position after the laboratory work and the delivery of maxillary complete denture.

10.The Analysis of Epithelial Ingrowth after LASIK Using a Femtosecond Laser.

PURPOSE: To analyze the incidence and clinical course of epithelial ingrowth after laser in situ keratomileusis (LASIK) using a femtosecond laser. METHODS: We retrospectively evaluated the results of 1158 eyes of 581 patients who received LASIK with the flap created by a femtosecond laser from February 2006 to March 2009 at our institute. We investigated the incidence and clinical course of LASIK in which the flap was created by a femtosecond laser. RESULTS: Epithelial ingrowth was first detected one week after surgery in 12 (57%) of 21 eyes and within one month in 19 eyes (90%). Epithelial ingrowth was localized most commonly near the temporal and nasal flap edge in 12 eyes (57%). In two eyes (9.5%), an isolated ingrowth mass was located in the pupillary area. The development of more than 2.0 mm of epithelial ingrowth was observed in three eyes (0.25%). Among 28 eyes with loose epithelium or epithelial detachment during surgery, epithelial ingrowth was observed in six eyes (21.4%) compared with 15 (1.32%) of 1130 eyes without loose epithelium or epithelial detachment. Therefore, epithelial detachment during surgery was significantly associated with epithelial ingrowth (p = 0.00). CONCLUSIONS: In LASIK in which the flap is created by a femtosecond laser, damage to the epithelial layer increases the chances of epithelial ingrowth. Therefore, avoiding epithelial defects with a gentle maneuver is needed to reduce the chance of epithelial ingrowth during surgery.
