Two Filipino sisters from Capiz presented with facial muscle weakness followed by weakness of both shoulders and arms, and scapular winging. Both presented with difficulty in performing activities of daily living. The younger sister also had bilateral lower extremity weakness with difficulty in ambulation. The results of electrodiagnostic testing in both sisters were compatible with myopathic disease. Comprehensive individualized rehabilitation addressing different functional limitations, focusing on maintaining mobility and functional capacity, and resulting improvements are presented.

2.Return to sports WeChat applet for evaluating the rehabilitation effects after anterior cruciate ligament reconstruction.

OBJECTIVE:
To clarify the intervention guidance of return to sports WeChat applet and evaluate the rehabilitation effectiveness after anterior cruciate ligament (ACL) reconstruction.
METHODS:
Between September 2020 and September 2022, 80 patients who met the selection criteria and underwent ACL anatomical single bundle reconstruction were selected as the research objects. According to the double-blind random method, they were divided into the applet group and the regular group, with 40 cases in each group. Patients in the applet group were rehabilitated under the guidance of the return to sports WeChat applet, and the patients were asked to perform the test once a month after operation, including patients' subjective scores [Tegner score, knee injury and osteoarthritis outcome score (KOOS), International Knee Documentation Committee (IKDC) score, American Hospital for Special Surgery (HSS) score], psychological assessment [ACL recovery sports injury scale (ACL-RSI) score], jumping test, balance test, bending angle test. Patients in the regular group were followed up by doctors and nurses regularly by telephone every month. All the patients were reexamined at 3, 6, 9, and 12 months after operation, and the range of motion of the knee joint with 6 degrees of freedom (flexion and extension angle, varus and valgus angle, internal and external rotation angle, anteroposterior displacement, superior and inferior displacement, and internal and external displacement) recorded by Opti_Knee three-dimensional knee joint motion measurement gait analysis system was observed. The anterior tibial translation difference (ATTD) was measured by Ligs knee measuring instrument when a forward thrust of 120 N was applied to the posterior part of the proximal tibia. Tegner score, IKDC score, KOOS score (including KOOS-Pain score, KOOS-Symptoms score, KOOS-Activities of daily living score, KOOS-Sport score, and KOOS-Quality of life score), HSS score, ACL-RSI score, jumping ability, balance ability, patients' satisfaction with the rehabilitation process, and ACL healing grading according to ACL continuity and signal intensity shown by MRI.
RESULTS:
There were significant differences in various indicators between different time points after operation in the two groups ( P<0.05). At 3 months after operation, except that the ACL-RSI score of the applet group was significantly higher than that of the regular group ( P<0.05), there was no significant difference in the other indicators between the two groups ( P>0.05). At 6 months after operation, the ACL-RSI score, IKDC score, Tegner score, KOOS scores of different items, HSS score, balance and jumping ability of the applet group were significantly higher than those of the regular group ( P<0.05), and there was no significant difference in the other indicators between the two groups ( P>0.05). At 9 months after operation, there was no significant difference in all indicators between the two groups ( P>0.05). At 12 months after operation, 27 cases (67.5%) in the applet group and 21 cases (52.5%) in the regular group returned to sport, with a significant difference of the return to sports incidence between the two groups [ RR(95% CI)=1.50 (1.00, 2.25), P=0.049]. In the applet group, 27 cases were very satisfied with the rehabilitation process, 10 cases were satisfied, 2 cases were basically satisfied, and 1 case was not satisfied, while 19, 13, 5, and 3 cases in the regular group, respectively. The satisfaction degree of the applet group was significantly better than that of the regular group ( P=0.049). MRI examination of the two groups showed that the ACL was continuous without secondary rupture or necrosis. The ACL healing grade of the applet group was 31 cases of grade 1 and 9 cases of grade 2, and that of the regular group was 28 cases of grade 1 and 12 cases of grade 2, there was no significant difference in ACL healing grade between the two groups ( P=0.449).
CONCLUSION
The application of return to sports WeChat applet in the rehabilitation of patients after ACL reconstruction can significantly reduce the fear of return to sports and improve the rate of return to sports. The return to sports WeChat applet is convenient to operate, with high utilization rate and high patient compliance, which significantly improves the satisfaction.

3.Clinical Assessment of Aging.

There are two ways to assess aging: to evaluate the function and to estimate the biological age. The biological age is estimated objectively by measuring the level of biological aging, taking apart with disease. It is measured with some biomarkers, which reflect the change of organ function and body with aging. It is, however, difficult to use in clinical practice. The evaluation of function is more practical and easier to clinicians. The parameters used include Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), and Mini-Mental Status Examination (MMSE) to evaluate the level of cognitive function. Balance test, standing up from chair, and walking pace can be checked to evaluate the physical function. Each method has its own limitations in evaluating the degree of aging precisely.

4.Trunk Impairment Scale for Evaluation of Functional Improvement in Acute Stroke Patients.

OBJECTIVE: To evaluate the change of balance of the trunk using the Trunk Impairment Scale (TIS) in acute stroke patients and to compare it with the improvement in activities in their daily living (ADL) and gait. METHOD: The mean days from the onset of stroke were 11.3 (6~17) days, and functional improvement of 24 hemiplegic patients was evaluated using the trunk impairment scale, Berg balance scale, timed up and go test, 6 minutes walk test, and the modified Barthel index every week for 4 weeks' conventional rehabilitation programs. Correlations between the trunk impairment scale and the other parameters were evaluated. RESULTS: The trunk impairment scale and the modified Barthel index, both which showed significant improvement after 4 weeks, had statistically significant correlation (p<0.05). The Berg balance scale had not improved significantly in 4 weeks. The timed up and go test and the 6 minutes' walk test could not be evaluated in most of these patients within 4 weeks. CONCLUSION: The trunk impairment scale could be a useful parameter for evaluating activities in their daily living (ADL) improvement in acute stroke patients who are unable to ambulate independently. And good trunk balance in acute stroke period is positive correlation with ambulation potential.

5.Wrist Rehabilitation after Scapholunate Interosseous Ligament Reconstruction for a Collegiate Basketball Player

For a basketball player who had scapholunate interosseous ligament injury related to sport, it is crucial to secure the active range of motion (ROM) and regain proprioception of the wrist. The player was involved in a 21-week rehabilitation procedure based on controlling pain with inactive treatments, restoring the wrist ROM with active treatments. We measured the visual analog scale (VAS) for pain, upper extremity functional outcome measurement (Disabilities of Arm, Shoulder, and Hand [DASH]) for the functionality, and active ROMs of the wrist. The VAS was decreased (after surgical treatment, 8.7; 1 week, 2.5; 12 week, 3–5; 21 week, 0). The DASH score was decreased when he returned to play (after surgical treatment, 78; end of the rehabilitation, 23). Wrist flexion and extension ROM were increased to 55° and 67°, respectively. To restore the function of the wrist for basketball performance, improvement of active ROM and proprioception is the primary goal to return to play.

6.Visual and Auditory Reaction Times of Patients with Opioid Use Disorder

OBJECTIVE: Reaction time is defined as the time from the start of a stimulus to the start of the voluntary movement. Time plays an important role in undertaking daily living activities. Reaction time is an important factor in respect of both quality of life and of capabilities demonstrated in the work environment. Alcohol and some addictive substances have effect on RT. The aim of this study was to compare the visual and auditory reaction times of patients with opioid use disorder with healthy control subjects. METHODS: The study was applied to two groups as the opioid use disorder group and the control group. A Sociodemographic and Clinical Data Form was prepared for each patient including age, gender, marital status and education level. Using a computer program the response to visual screen color change (red/blue) and to an auditory ‘beep’ sound of the computer system were recorded. The Student’s t-test was applied as a statistical method. RESULTS: The results showed longer reaction times in the patients with opioid use disorder. CONCLUSION: To add improving reaction time approaches in opioid use disorder treatment may contribute to treatment by increasing quality of life and work performance.

7.Workplace Violence Experienced by Substitute (Daeri) Drivers and Its Relationship to Depression in Korea.

Workplace violence is related to various health effects including mental illness such as anxiety or depression. In this study, the relationship between the experience of workplace violence and depression in substitute drivers in Korea, namely, daeri drivers, was investigated. To assess workplace violence, questions regarding types and frequency of the experience of violence over the past year were asked to the daeri drivers. In order to assess the risk of depression, the Center for Epidemiological Studies Depression Scale was used. Odds ratios with 95% confidence intervals of depression were estimated using multiple logistic regression analysis. All of the daeri drivers had experienced instance of verbal violence while driving and 66 of the drivers (34.1%) had been in such a situation more than once in the past quarter of a year. Sixty-eight daeri drivers (42.2%) had experienced certain type of physical violence over the past year. Compared to daeri drivers who had experienced workplace verbal violence less than 4 times and who had not experienced workplace physical violence over the past year, higher odds ratio was observed in daeri drivers who had experienced workplace verbal violence or physical violence, more than 4 times and more than one time respectively, after adjustment. Experience of verbal or physical type of workplace violence over the past year increased the risk of depression in the daeri drivers. Because violence against drivers can compromise the safety of the driver, the customer, and all the passengers, it is imperative that the safety and health of daeri drivers be highlighted.

8.Lifelog Agent for Human Activity Pattern Analysis on Health Avatar Platform.

OBJECTIVES: To provide accurate personalized medical care, it is necessary to gather individual-related data or contextual information regarding the target person. Nowadays a large number of people possess smartphones, which enables sensors in the smartphones to be used for lifelogging. The objective of the study is to analyze human activity pattern by using lifelog agent cooperating with the Health Avatar platform. METHODS: Using the lifelog measured by accelerometer and gyroscope in a smartphone at a 50 Hz rate, the agent reveals how long the user walks, runs, sits, stands, and lies down, and this information is summarized by hours. The summaries are sent to the Health Avatar platform and finally are written in the Continuity of Care Record (CCR) format. RESULTS: The lifelog agent is successfully operated with the Health Avatar platform. In addition, we implement an application that displays the user's activity patterns in a graph and calculates the metabolic equivalent of task based calorie burned by hour or by day using the lifelog of the CCR form to show that the lifelog can be used as medical records. CONCLUSIONS: The agent shows how lifelogs are analyzed and summarized to help activity recognition. We believe that our agent demonstrates a way of incorporating lifelogs into medical care and a way of exploiting lifelogs in a medical format.

9.Effectiveness of Cognitive Training to Instrumental Activities of Daily Living in Community-Dwelling Elderly.

OBJECTIVE: This study was aimed to investigate the effectiveness of cognitive training to Instrumental Activities of Daily Living (IADL) in community-dwelling elderly. METHODS: The participant were 786 elders who were aged over 59 years visiting local rest area from February 2015 to November 2015. The demographic data was collected. IADL were evaluated by Seoul-Instrumental Activities of Daily Living (S-IADL). Cognitive functions were evaluated by Korean Version of Mini Mental Status Examination for Dementia Screening (MMSE-DS). Before and after cognitive training, we analysed these data. RESULTS: In all 15 items of S-IADL, 4 items (using the telephone, grooming, managing belongings, talking recent events) were specially improved (p<0.05). CONCLUSION: After cognitive training, indoor activities of S-IADL were improved but some items, especially outside activities were not improved for community dwelling elderly. In future, other programs to increase outside activities or social activities should be included in cognitive training programs for community dwelling elderly.

10.Determinants of health-related quality of life among community dwelling elderly.

INTRODUCTIONThis study determines the associations between self-reported chronic conditions, limitations in activities of daily living and health-related quality of life (HRQoL) among community dwelling elderly in Singapore.
MATERIALS AND METHODSA population-based cross-sectional survey was conducted among a random sample of 4200 residents from 58 blocks of dwellings in Marine Parade housing estate between April and May 2011. A structured questionnaire was used to collect data on demographic characteristics; chronic disease profile, health screenings, healthcare utilisation, physical activity, activities of daily living (ADL) and functional ability and health related quality of life. Quality of life was assessed using European Quality of life 5 Domain (EQ-5D). Ordinary least squares (OLS) regression was used to identify independent predictors of health related quality of life.
RESULTSA total of 2454 respondents for included for analysis. Most of the respondents were females (57.2%) and aged between 65 and 74 years (48.5%). Among them, 79.1% of the respondents were Chinese. Approximately three-fourth (77.5%) of the survey respondents reported having at least one of the 13 chronic medical conditions; high blood pressure (57.7%), high blood cholesterol (51.6%), diabetes (22.9%) were the most commonly reported conditions. Independent predictors of HRQoL with greatest decrements in EQ-5D index and visual analog scores (VAS) were unemployment, self-reported depression, arthritis and osteoporosis and ADL limitations for activities such as "unable to shower", "unable to do housework" and elderly with depressive symptoms (GDS score≥5).
CONCLUSIONThe study had identified predictors of HRQoL in elderly Singapore residents and also provides community-based EQ-5D index and VAS scores associated with a wide variety of chronic conditions and ADL limitations.
