Objective To investigate the disgnostic value of thoracic CT and X-ray plain film in giant bulla and its differential diagnosis with pneumothorax.Methods 6 patients with surgical proved giant pulmonary bulla were analyzed retrospectively,all patient underwent chest radiography and CT scan.Results The lesions occupied 30%~80% of thoracic cavity,4 cases lied left thoracic cavity and 2 cases lied right thoracic cavity.3 cases were misdiagnosed as pneumothorax by X-ray plain film.CT could show the wall and internal condition of bulla as well as compression of the normal pulmonary tissue.Conclusion Giant pulmonary bulla is easily misdiagnosed,thoracic CT is of significant value in diagnosis of giant bulla.

Objective To observe the effect of carvedilol and motoprolol on the weight of cardiac muscle in patients with hypertensive heart disease.Methods One hundred patients with hypertensive heart disease were divided into two groups by random number and treated with carvedilol(n =54) and motoprolol(n =46) respectively for 12 months.Data were recorded (left ventricular end-diastolic dimension (LVEDD),left ventricular end-systolic dimension (LVESD),left ventricular ejection fraction (LVEF),interventricular septal thickness (IVST),and left ventricular posterior wall thickness (LVPWT)) before and after the treatment.Left ventricular muscle mass(LVW),left ventricular mass index(LVWI) were calculated.Results After treatment,both groups improved with declined LVEDD (motoprolol group:(43.60 ± 4.43) mm vs.(46.70 ± 3.21) mm,t =18.143,P ＜ 0.01 ; carvedilol group:(42.50 ± 2.56) mm vs.(46.5 ± 3.18) mm,t =18.232,P ＜ 0.01),thinner IVST (motoprolol group:(9.68 ± 1.65) mm vs.(12.01 ± 1.56) mm,t =12.785,P ＜ 0.01 ; carvedilol group:(9.05 ± 1.04) mm vs.(11.59 ± 1.54) mm,t =7.865,P ＜ 0.01),and increased LVEF (motoprolol group:(52.89 ± 8.78)％ vs.(50.23 ± 7.88)％; carvedilol group:(54.65 ± 8.87％)％ vs.(50.22 ±7.89)％).In the carvedilol treatment group,LVMI significantly (t =4.987,P ＜ 0.01) declined from (133.75 ±25.89) g/m2 to (109.25 ± 22.53) g/m2.In the motoprolol treatment group,LVMI declined from (134.76 ± 25.87) g/m2 to (119.78 ± 23.65.53) g/m2 (t =5.689,P ＜ 0.01).After the treatment,each index of carvedilol group improved significantly compared with metoprolol group (P ＜ 0.05).Conclusion Both medications are able to reduce the weight of cardiac muscle of patients with hypertensive heart disease,and carvedilol is better than motoprolol.

Purpose To study the diagnostic value of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI),diffusion tensor imaging (DTI) and the combination of these two techniques in benign and malignant nodules of central prostate gland.Materials and Methods Forty-nine patients with nodular abnormal signal in central prostate gland who had accepted prostate MRI scan in Shihezi University Medical College First Affiliated Hospital from September 2015 to December 2016 were retrospectively analyzed,and all the subjects had accepted MRI T2WI,DTI and DCE scan.The pathological results confirmed that there were 21 prostate cancer (PCa) and 36 benign prostatic hyperplasia (BPH) among 57 central gland nodule samples.The fractional anisotropy value,apparent diffusion coefficient value,time to peak,SImax,the curve slope and timesignal intensity curve of nodules were respectively detected,the differences of the parameters between PCa group and BPH group were compared,and receiver operating characteristic (ROC) curve analysis was carried out.Results The difference of fractional anisotropy value,apparent diffusion coefficient value,time to peak and R between PCa group and BPH group was statistically significant (P＜0.05),and there was no significant difference in SImax (P＞0.05);the time-signal intensity curve type of PCa group was mainly in the form of fast-increasing and then decreasing,while the curve type of BPH group was mainly in the form of platform.The area under the ROC curve of DCE,DTI and the combination of DEC and DTI were 0.87 (95％ CI:0.751-0.942),0.85 (95％ CI:0.734-0.933) and 0.94 (95％ CI:0.837-0.983),respectively.Conclusion In the cases of diagnosing benign and malignant nodules of central prostate gland,the accuracy of DTI and DCE was lower than that of the combination of these two techniques,and the diagnostic effect of DTI combined with DCE was better.

Objective To probe the effect of nursing interventions on depression of patients with acute stroke.Methods 72 cases of stroke patients with depression screened by Hamilton depression scale (HAMD)were divided into the intervention group and the control group with 36 cases in each group.The control group received routine treatment.The intervention group wag given nursing intervention based on routine treatment.The effect was assessed before and after 3,6 and 12 months of intervention with Zung self-rating depression scale(SDS)and improved Rankin scale(MRS). Results The value of SDS after 3,6 or 12 months was significantly lower than that of the control group(P＜0.05),and the value of MRS was significantly higher than that of the control group(P＜0.05).Conclusion Nursing intervention can reduce depression in patients with post-stroke depression.

Objective To investigate the effect of prescription of dispersing stagnated liver and strengthening spleen combined with psychological care in treatment of functional dyspepsia.Methods 60 patients with functional dyspepsia were randomly divided into the nursing group and the control group with 30 patients in each group.The control group was treated with oral intake of non-boiled prescription of dispersing stagnated liver and strengthening spleen.The nursing group was treated with psychological care on the basis of the control group.Both groups were assessed with SCL-90,HAMD,HAMA and symptom scales before treatment and 4 weeks after treatment.Results HAMD,HAMA,SCL-90 scores and symptom score were improved after treatment in both groups,the treatment effect of the nursing group improved more significantly than the control group.Conclusions To give patients with functional dyspepsia the prescription of dispersing stagnated liver and strengthening spleen combined with psychological care can significantly improve symptoms of functional dyspepsia and psychological symptoms.

Objective To probe into the effect of community health education in comprehensive treatment of type 2 diabetes mellltus(T2DM)patients.Methods 120 cases of T2DM patients were claasified randomly into the intervention group(55 cases)and the control group (65 cases).After systematic diabetes knowledge education was given to the intervention group,individualized intensive education was accepted by each patient of the intervention group.General diabetic education was taken by the control group.The levels of plasma glucose,urinary glucoee,glycosylated hemoglobin and blood-fat were tested before and 3,6 months after education.Results Six months after education,the level of self care ability,self care technique,self care sense of responsibility and health knowledge were higher than those before education in both groups.And six months after education the levels of HbAlc,TC and TG in the intervention group and the control group improved obviously.All indicators in the intervention group were better than those in the control group.Conctusions Community health education can effectively improve self care ability,self care technique,self care sense of responsibility and health knowledge of diabetes patients,and can improve their glucolipide metabolic level as well.

AIM: To design a simple and effective auxiliary device for detection of the compound action poten-tial (CAP) and the force on the isolated sciatic nerve and gastrocnemius in toads, and to investigate its practicability. METHODS: A simple “L” shape device (L tube), which was composed of a nerve chamber and an organ bath, was made for fixing the isolated sciatic nerve and gastrocnemius.After fixing, the sciatic CAP and the gastrocnemius force were detected by BL-420S data acquisition and analysis system.The specimens were radomly divided into control group, and the lidocaine nerve and muscle groups.The sciatic nerve or gastrocnemius of each lidocaine group was firstly treated with the corresponding concentration of lidocaine, and then washed out with Ringer’s solution, and its reversible anesthetic action on nerve conduction and muscle force was analyzed to verify the practicability of the L tube device.RESULTS: The CAP and the force of the sciatic-gastrocnemius specimens were detected concurrently by fixing the specimens in the L tube, and the liquid in the nerve chamber and organ bath was changed easily.Compared with the control, lidocaine at 0.05 and 0.2 g/L significantly increased the sciatic threshold stimulus voltage and maximal stimulus voltage (P <0.05), prolonged the absolute refractory period (P <0.01), and slowed down the conduction velocity (P <0.01).Lidocaine at 1 g/L complete-
ly blocked the sciatic conduction.The rest tension of the gastrocnemius was increased, and the maximal twitch force was decreased significantly by 1 g/L lidocaine (P <0.01), but the threshold and maximal stimulus voltage did not show statis-tic difference.The parameters of the sicatic nerve and gastrocnemius completely or partially recovered to the control level after washing out.CONCLUSION: As an auxiliary device, L tube makes the detection of CAP and force on the isolated sciatic nerve and gastrocnemius in toads more conveniently.

[ ABSTRACT] AIM:To explore the inhibitory effects of pantoprazole sodium on epithelial-mesenchymal transition and cisplatin resistance in lung cancer cells and the underlying mechanism .METHODS: Using MTT method, wound healing assay , Transwell experiment , Western blot , the differences of morphology , invasion ability , migration ability , drug sensitivity and protein expression between A 549/DDP cells and A549 cells were determined .The effect of pantoprazole so-dium on morphology , invasion ability , migration ability , drug sensitivity and protein expression in A 549/DDP cells were al-so observed.RESULTS: Compared with A549 cells, A549/DDP cells had higher invasion and migration abilities , and lower drug sensitivity , exhibited mesenchymal phenotype and activated c-Met/AKT/mTOR pathway .Pantoprazole sodium inhibited the abilities of invasion and migration , and reversed the mesenchymal phenotype , drug resistance and the c-Met/AKT/mTOR pathway activation in A549/DDP cells.Treatment with c-Met inhibitor SU11274, PI3K inhibitor LY294002 and mTOR inhibitor rapamycin had the same effects on A 549/DDP cells as that of pantoprazole sodium .CONCLUSION:Pantoprazole sodium inhibits invasion , migration, epithelial-mesenchymal transition and cisplatin resistance in lung cancer cells by down-regulating c-Met/AKT/mTOR pathways .

OBJECTIVE:To improve the hospital workflow and efficiency in temporary drug purchase approval process. METHODS:The approval function for temporary drug purchase was introduced into office automation(OA)system in our hospi-tal,and the effects were evaluated. RESULTS:According to ensuring the administrative approval process,system function permis-sion assignment and approval process design in temporary drug purchase in our hospital,functions for approving temporary drug purchase were established in OA system. It achieved convenient,efficient,timely,networking and paperless approval work,as well as standardized record,checking out at any time and automatic statistics for drug purchase. CONCLUSIONS:Introducing tem-porary drug purchase approval function into hospital OA system can simplify workflow,provide better service for clinic,and pro-mote development of hospital pharmacy management.

AIM: To investigate whether Flk1~+ CD31~- CD34~- cells isolated from human adult adipose tissue have characteristics of hemangioblasts in vivo. METHODS: After sublethally irradiated (300cGy) with a caesium source, the female non-obese diabetic/severe combined immunodeficient (NOD/SCID) mice were injected with human adipose tissue-derived Flk1~+ CD31~- CD34~- cells (10~5 cells per mouse) via tail vain with 0.4 mL Roswell Park Memorial Institute medium (RPMI-1640). The control mice received the same volume of RPMI-1640 medium. All mice were killed 2 months after transplantation for further study. The differentiation potential of Flk1~+ CD31~- CD34~- cells was assessed in bone marrow and gastrointestinal tract by the methods of flow cytometry, RT-PCR, FISH, and triple-color immunofluorescence. RESULTS: Flk1~+ CD31~- CD34~- human adipose tissue-derived adult stem cells differentiated into endothelial cells and hematopoietic cells at the single-cell level in vivo. CONCLUSION: Human adult adipose tissue-derived Flk1~+ CD31~- CD34~- cells bear characteristics of hemangioblast in vivo and may have potential application for the treatment of hematopoietic and vascular diseases.
